Outlawing drugs in order to solve drug problems is much like outlawing sex in order to win the war against AIDS. We recognize that people will continue to have sex for nonreproductive reasons despite the laws and mores. Therefore, we try to make sexual practices as safe as possible in order to minimize the spread of the AIDS viruses. In a similar way, we continually try to make our drinking water, foods, and even our pharmaceutical medicines safer. The ubiquity of chemical intoxicants in our lives is undeniable evidence of the continuing universal need for safer medicines with such applications. While use may not always be for an approved medical purpose, or prudent, or even legal, it is fulfilling the relentless drive we all have to change the way we feel, to alter our behavior and consciousness, and, yes, to intoxicate ourselves. We must recognize that intoxicants are medicines, treatments for the human condition. Then we must make them as safe and risk free and as healthy as possible. Dream with me for a moment. What would be wrong if we had perfectly safe intoxicants? I mean drugs that delivered the same effects as our most popular ones but never caused dependency, disease, dysfunction, or death. Imagine an alcohol-type substance that never caused addiction, liver disease, hangovers, impaired driving, or workplace problems. Would you care to inhale a perfumed mist that is as enjoyable as marijuana or tobacco but as harmless as clean air? How would you like a pain-killer as effective as morphine but safer than aspirin, a mood enhancer that dissolves on your tongue and is more appealing than cocaine and less harmful than caffeine, a tranquilizer less addicting than Valium and more relaxing than a martini, or a safe sleeping pill that allows you to choose to dream or not? Perhaps you would like to munch on a user friendly hallucinogen that is as brief and benign as a good movie? This is not science fiction. As described in the following pages, there are such intoxicants available right now that are far safer than the ones we currently use. If smokers can switch from tobacco cigarettes to nicotine gum, why can’t crack users chew a cocaine gum that has already been tested on animals and found to be relatively safe? Even safer substances may be just around the corner. But we must begin by recognizing that there is a legitimate place in our society for intoxication. Then we must join together in building new, perfectly safe intoxicants for a world that will be ready to discard the old ones like the junk they really are. This book is your guide to that future. Cannan sat back, assessing his opposition. “I don’t know with certainty any more than you do. In the absence of definitive proof of guilt on behalf of my son and his friends, I suggest you and your fellows develop a sense of humor.” Then the captain’s tone changed, becoming more forbidding. “I can prevent an uprising, Narian. This, you’ll have to get used to.”
Not wanting to be in the dark, I snatched up the parchment in question. My mouth opened in shock and dismay as I silently read its contents, the men waiting for me to finish.
On this Thirtieth Day of May in the First Year of Cokyrian dominance over the Province of Hytanica, the following regulations shall be put into practice in order to assist our gracious Grand Provost in her effort to welcome Cokyri into our lands--and to help ensure the enemy does not bungle the first victory it has managed in over a century.

Regulation Six. When at long last foreign trade is allowed to resume, Hytanicans should strive to empathize with the reluctance of neighboring kingdoms to enter our lands, for Cokyri’s stench is sure to deter even the migrating birds.

AIDS itself ends up looking like a side-effect of this demagogic virulence. `Tu me préserves actif, je te préservatif' ['You keep me active, I condom you']: this scabrous irony, heavy with blackmail, which is also that of Benetton, as it once was of the BNP, in fact conceals a technique of manipulation and dissolution of the social body by the stimulation of the vilest emotions: self-pity and self-disgust. Politicians and advertisers have understood that the key to democratic government -- perhaps even the essence of the political? -- is to take general stupidity for granted: `Your idiocy, your resentment, interest us!' Behind which lurks an even more suspect discourse: `Your rights, your destitution, your freedom, interest us!' A word of explanation about how the information in this book was obtained, evaluated and used. This book is designed to present, as best my reporting could determine, what really happened. The core of this book comes from the written record—National Security Council meeting notes, personal notes, memos, chronologies, letters, PowerPoint slides, e-mails, reports, government cables, calendars, transcripts, diaries and maps. Information in the book was supplied by more than 100 people involved in the Afghanistan War and national security during the first 18 months of President Barack Obama’s administration. Interviews were conducted on “background,” meaning the information could be used but the sources would not be identified by name. Many sources were interviewed five or more times. Most allowed me to record the interviews, which were then transcribed. For several sources, the combined interview transcripts run more than 300 pages. I have attempted to preserve the language of the main characters and sources as much as possible, using their words even when they are not directly quoted, reflecting the flavor of their speech and attitudes. Many key White House aides were interviewed in-depth. They shared meeting notes, important documents, recollections of what happened before, during and after meetings, and assisted extensively with their interpretations. Senior and well-placed military, intelligence and diplomatic officials also provided detailed recollections, read from notes or assisted with documents. Since the reporting was done over 18 months, many interviews were conducted within days or even hours after critical discussions. This often provided a fresher and less-calculated account. Dialogue comes mostly from the written record, but also from participants, usually more than one. Any attribution of thoughts, conclusions or feelings to a person was obtained directly from that person, from notes or from a colleague whom the person told. Occasionally, a source said mid-conversation that something was “off-the-record,” meaning it could not be used unless the information was obtained elsewhere. In many cases, I was able to get the information elsewhere so that it could be included in this book. Some people think they can lock up and prevent publication of information by declaring it “off-the-record” or that they don’t want to see it in the book. By day I mean night, and the tales are those we produce during REM sleep. Well over a century since Freud’s The Interpretation of Dreams, there is still no consensus on why we dream. I read Freud’s book for a junior-year high school class called Hygiene (yes, that’s really what it was called), a somewhat bizarre requirement taught by the school’s gym teachers and sports coaches that focused on first aid and common standards of cleanliness. Lacking material to fill an entire semester, the class was padded by mandatory student presentations on topics deemed loosely relevant. I chose sleep and dreams and probably took it all too seriously, reading Freud and spending after-school hours combing through research literature. The wow moment for me, and for the class too, was the work of Michel Jouvet, who in the late 1950s explored the dream world of cats.32 By impairing part of the cat brain (the locus coeruleus, if you like that sort of thing), Jouvet removed a neural block that ordinarily prevents dream thoughts from stimulating bodily action, resulting in sleeping cats who crouched and arched and hissed and pawed, presumably reacting to imaginary predators and prey. If you didn’t know the animals were asleep, you might think they were practicing a feline kata. More recently, studies on rats using more refined neurological probes have shown that their brain patterns when dreaming so closely match those recorded when awake and learning a new maze that researchers can track the progress of the dreaming rat mind as it retraces its earlier steps.33 When cats and rats dream it surely seems they’re rehearsing behaviors relevant to survival.
Our common ancestor with cats and rodents lived some seventy or eighty million years ago, so extrapolating a speculative conclusion across species separated by tens of thousands of millennia comes with ample warning labels. But one can imagine that our language-infused minds may produce dreams for a similar purpose: to provide cognitive and emotional workouts that enhance knowledge and exercise intuition—nocturnal sessions on the flight simulator of story. Perhaps that is why in a typical life span we each spend a solid seven years with eyes closed, body mostly paralyzed, consuming our self-authored tales.34
Intrinsically, though, storytelling is not a solitary medium. Storytelling is our most powerful means for inhabiting other minds. And as a deeply social species, the ability to momentarily move into the mind of another may have been essential to our survival and our dominance. This offers a related design rationale for coding story into the human behavioral repertoire—for identifying, that is, the adaptive utility of our storytelling instinct.

The RAND Corporation (Caulkins, et al., Mandatory Minimum Drug Sentences, 1997), for example, found that treatment of heavy users of drugs reduced serious crimes against both persons and property ten times as much as conventional law enforcement did, and fifteen times as much as mandatory minimum sentences. And, not surprisingly, treatment was also vastly more effective in reducing cocaine consumption than either conventional law enforcement or mandatory minimum sentences.

In Nigeria, the Global Fund to Fight AIDS, Tuberculosis and Malaria has granted US$74 million so far for HIV, all of it for work with the ‘general population’. Nigeria gets large slabs from PEPFAR, too, US$105 million in 2006 and rising; 90 per cent of the prevention money was going to ‘general population’ interventions.4 ‘Youth’ is an especially popular focus for prevention efforts in Nigeria, even though HIV tests in several thousand recent graduates from technical college showed that just 1.2 per cent were infected–hardly a sign of an epidemic that is out of control among young people in the general population. Meanwhile, Nigeria has a vibrant sex industry. I can’t say how vibrant because the national programme has until now more or less ignored commercial sex. In a national survey in 2003, 3 per cent of men said they visited a prostitute in the last year, so that would be 1.2 million clients right there, and the probable total is a lot higher.5 There are no estimates of how many women sell sex, and there’s no routine HIV surveillance among sex workers. Sporadic studies are not encouraging. In 2003, 21 per cent of sex workers in the western city of Ibadan and 48 per cent in nearby Saki were infected with HIV.6 Of course, we don’t have a clue how much HIV is spread in sex between men in Nigeria, because no one has asked–the first studies got underway only in 2007. General Wound Cleaning
Note: A rescuer should wash his or her hands and put on protective gloves and protective eyewear before cleaning an open wound.
All wounds acquired in a wilderness environment should be regarded as contaminated and, therefore, require cleansing to prevent infection and promote healing. There are three effective methods of wound cleaning available to the WFR: You can scrub, irrigate, and debride.
Scrubbing: Disinfectants (such as isopropyl alcohol, povidone-iodine, and hydrogen peroxide) and soaps and detergents should not be put directly into wounds because they can damage viable tissue and may actually increase the incidence of wound infection. These substances may be used to scrub around a wound prior to wound cleaning, with soap and water working as well as anything else.
Irrigating: The most effective and practical method of removing bacteria and debris from a wound involves using a high-pressure irrigation syringe. Irrigation syringes that supply adequate pressure are available commercially in quality first-aid kits. Without an irrigation syringe, you can put water in a plastic bag, punch a pinhole in the bag, and squeeze the water out forcefully, or you can melt a pinhole in the center of the lid of a water bottle with a hot needle and squeeze the water out forcefully. These and other improvised methods are not nearly as effective as an irrigation syringe, but they may be the best you can do. Simply rinsing or soaking a wound is inadequate to remove bacteria. The cleanest water available, most preferably water disinfected for drinking, should be used for irrigating. The tip of the irrigating device should be held 1 to 2 inches above the wound surface, and the plunger of the syringe forcefully depressed. Be sure to tilt the wound to irrigate contaminants out and away from the wound. The volume of irrigation fluid required varies with the size of the wound and the degree of contamination, but plan on using at least a half liter of water.
Note: Wound irrigation is the single most important factor in preventing infection.

Contusion
Bruises seldom require emergency care, but large bruises benefit from cold, compression, and/or elevation. Substantial bruises should cause you to assess the patient for damage to underlying structures, such as bones and organs. Large bruises should be protected from freezing in extremes of cold because a bruised area will freeze sooner than normal skin.
Abrasions
Abrasions are the exception to the rule of wound cleaning: You need to scrub within the wound to achieve adequate cleaning. A sterile gauze pad is adequate for scrubbing. Scrubbing may be enhanced by using any soap, but all soap should be carefully rinsed and then irrigated from the wound after scrubbing. Green Soap Sponges are packaged with soap and water already in the sponge, making them useful additions to first-aid kits. It is important to remove all embedded debris not only to reduce the risk of infection but also to prevent subsequent “tattooing” (scarring) of the skin. With a deep abrasion, self-scrubbing is seldom successful due to the high level of pain associated with the exposed nerves.
After cleansing, abrasions can be kept moist to avoid desiccation and speed healing with microthin film dressings that can be left in place until healing occurs. Without microthin film dressings, a topical agent, such as an antibiotic ointment, can be applied, followed by a dressing of a sterile gauze pad or a roll of sterile gauze to keep the ointment in place. Tape, an elastic wrap, or some other holder may be used to hold a sterile gauze pad in place. Ideally, gauze dressings should be changed twice a day, or at least once a day, as well as any time the gauze gets wet.

The result is that in many housing courts around the country, 90 percent of landlords are represented by attorneys, and 90 percent of tenants are not. Low-income families on the edge of eviction have no right to counsel. But when tenants have lawyers, their chances of keeping their homes increase dramatically. In the 1963 landmark case Gideon v. Wainwright, the Supreme Court unanimously established the right to counsel for indigent defendants in criminal cases on the grounds that a fair trial was impossible without a lawyer. Eighteen years later, the court heard the case of Abby Gail Lassiter, a poor black North Carolinian, who appeared without counsel at a civil trial that resulted in her parental rights being terminated. This time, a divided court ruled that defendants had a right to counsel only when they risked losing their physical liberty. Incarceration is a misery, but the outcomes of civil cases also can be devastating. Just ask Ms. Lassiter.

General Guidelines for Wilderness Medical Kits
1. Accept the fact that there is no such thing as the perfect wilderness medical kit. Many factors should determine your choices of specific contents. No matter how much you plan and prepare, someday you will want something that is not there and/or discover you’ve carried an item for years and never used it. When considering the contents of a kit, take into account (1) the environmental extremes you will face (altitude, cold, heat, endemic diseases), (2) the number of people that may require care; (3) the number of days the kit will be in use; (4) the distance from definitive medical care; (5) the availability of rescue services; (6) your medical expertise and/or the expertise of other group members; and (7) preexisting problems of group members, such as individuals with diabetes.
2. Evaluate and repack your wilderness medical kit before every trip. Renew medications that have reached expiration dates. Replace items that have been damaged by heat, cold, or moisture. Remove items that are unnecessary for the proposed trip, such as insect repellent on winter trips, and add items that may be useful on the upcoming adventure.
3. Do not fill your kit with items you do not know how to use. Maintain a high level of familiarity with the proper uses of all the items in your wilderness medical kit.
4. Choose specific items for the wilderness medical kit, whenever possible, that are versatile rather than particular. For example, a wide variety of sizes and shapes of Band-Aids is nice, but wound coverings can be created from pads of gauze and strips of tape. Triangular bandages are useful, but safety pins and T-shirts can be used to make slings. Medical adhesive tape has limited usefulness compared with duct tape.
5. Encourage each group member to pack and carry a personal first-aid kit to reduce the size and weight of the general wilderness medical kit.

Storm’s Fast-Flux and Conficker’s Domain-Flux In 2007, security researchers identified a new technique used by the infamous Storm botnet (Higgins, 2007). The technique, named fast-flux, used domain name service (DNS) records to hide the command and control servers that controlled the Storm botnet. DNS records typically translate a domain name to an IP address. When a DNS server returns a result, it also specifies the TTL that the IP address remains valid for before the host should check again. The attackers behind the Storm botnet changed the DNS records for the command-and-control server rather frequently. In fact, they used 2,000 redundant hosts spread amongst 384 providers in more than 50 countries (Lemos, 2007). The attackers swapped the IP addresses for the command-and-control server frequently and ensured the DNS results returned with a very short TTL. This fast-flux of IP addresses made it difficult for security researchers to identify the command-and-control servers for the botnet and even more difficult to take the servers offline. While fast-flux proved difficult in the takedown of the Storm botnet, a similar technique used the following year aided in the infection of seven million computers in over two hundred countries (Binde et al., 2011). Conficker, the most successful computer worm to date, spread by attacking a vulnerability in the Windows Service Message Block (SMB) protocol. Once infected, the vulnerable machines contacted a command-and-control server for further instructions. Identifying and preventing communication with the command-and-control server proved absolutely necessary for those involved with stopping the attack. However, Conficker generated different domain names every three hours, using the current date and time at UTC. For the third iteration of Conficker, this meant 50,000 domains were generated every three hours. Attackers registered only a handful of these domains to actual IP addresses for the command-and-control servers. This made intercepting and preventing traffic with the command-and-control server very difficult. Because the technique rotated domain names, researchers named it domain-flux. In the following section, we will write some Python scripts to detect fast-flux and domain-flux in the wild to identify attacks.

Johnson’s carefully arranged ceremony in which he insisted that Jackie Kennedy be present was to put his imprimatur and that of the Kennedys, on his presidency. Additionally, Judge Sarah T. Hughes, who administered the oath, had recently been blocked from elevation on the federal bench by Attorney General Robert Kennedy. This impediment would be removed under President Lyndon Johnson. Robert Kennedy knew his brother was murdered by a domestic conspiracy and, at a minimum, suspected that Lyndon Johnson was complicit. Kennedy would tell his aide Richard Goodwin, “there’s nothing I can do about it. Not now.”86 In essence, Kennedy understood that with both the FBI and the Justice Department under the control of Lyndon Johnson and Kennedy nemesis J. Edgar Hoover, there was, indeed, nothing he could do immediately. While numerous biographers describe RFK as being shattered by the murder of his brother, Robert Kennedy was not so bereaved that it prevented him from seeking to maneuver his way onto the 1964 ticket as vice president. Indeed, RFK had Jackie Kennedy call Johnson to lobby for Bobby’s selection. Johnson declined, far too cunning to put Bobby in the exact position that he had maneuvered John Kennedy into three years previous. Robert Kennedy knew that only by becoming president could he avenge his brother’s death. After lukewarm endorsements of the Warren Commission’s conclusions between 1963 and 1968, while campaigning in the California primary, RFK would be asked about his brother’s murder. In the morning, he mumbled half-hearted support for the Warren Commission conclusions but asked the same question that afternoon he would tell a student audience in Northern California that if elected he would reopen the investigation into his brother’s murder. Kennedy’s highly regarded press secretary Frank Mankiewicz would say he was “shocked” by RFK’s comment because he had never said anything like it publicly before. Mankiewicz and Robert Kennedy aide Adam Walinsky would ultimately conclude that JFK had been murdered by a conspiracy, but to my knowledge, neither understood the full involvement of LBJ. Only days after Robert Kennedy said he would release all the records of the Kennedy assassination, the New York Senator would be killed in an assassination eerily similar to his brother’s, in which there are disputes, even today, about the number of shooters and the number of shots. During digestion, glucose is absorbed into your bloodstream. Normally, it then enters your body’s cells, aided by the action of insulin. The hormone insulin comes from your pancreas. When you eat, your pancreas secretes insulin into your bloodstream. As insulin circulates, it acts like a key, unlocking microscopic doors that allow glucose to enter your cells. In this way, insulin lowers the amount of glucose in your bloodstream and prevents it from reaching high levels. As your blood glucose level drops, so does the secretion of insulin from your pancreas. Your liver acts as a glucose storage and manufacturing center. When the level of insulin in your blood is high, such as after a meal, your liver stores extra glucose as glycogen in case your cells need it later. When your insulin levels are low, for example, when you haven’t eaten in a while, your liver releases the stored glucose into your bloodstream to keep your blood sugar level within a normal range. When you have diabetes If you have diabetes, this process doesn’t work properly. Instead of being transported into your cells, excess glucose builds up in your bloodstream, and eventually some of it is excreted in your urine. This usually occurs when your pancreas produces little or no insulin, or your cells don’t respond properly to insulin, or for both reasons. The medical term for this condition is diabetes mellitus (MEL-lih-tuhs). Mellitus is a Latin word meaning “honey sweet,” referring to the excess sugar in your blood and urine.
