They replaced the emergency cots with beds and put better furniture in the enlarged building. Fewer members were now interested in going to sea on life-saving missions, so they hired lifeboat crews to do this work. The life-saving motif still prevailed in this club`s decoration, and there was a liturgical lifeboat in the room where the club initiations were held. About this time a large ship was wrecked off the coast, and the hired crews brought in boatloads of cold, wet and half-drowned people. They were dirty and sick and some had black skin and some had yellow skin. The beautiful new club was in chaos. So the property committee immediately had a shower house built outside the club where victims of shipwrecks could be cleaned up before coming inside. At the next meeting, there was a split in the club membership. Most of the members wanted to stop the club`s life-saving activities as being unpleasant and a hindrance to the normal social life of the club. Some members insisted upon life-saving as their primary purpose and pointed out that they were still called a life-saving station. But they were finally voted down and told that if they wanted to save lives of all the various kinds of people who were shipwrecked in those waters, they could begin their own life-saving station down the coast. So they did just that. As the years went by, the new station experienced the same changes that had occurred in the old. It evolved into a club, and yet another `spin-off` life saving station was founded. History continued to repeat itself, and if you visit the sea coast today, you will find a number of exclusive clubs along the shore. Or an All About Me would ring a member of the Woo-Woo crew and sob and sob about how selfish I was being and the Woo-Woo crew person would agree because I had refused to cough up two grand for a sweat lodge in Wicklow. Or one of the Runaways would tiptoe back for a sneaky look at me, then commandeer a Denier into launching a two-pronged attack, telling me how well I seemed. And actually that was the worst thing anyone could have done to me, because you can only sound like a self-pitying malingerer if you protest, ‘But I don’t feel well. I feel wretched beyond description.’ Not one person who loved me understood how I’d felt. They hadn’t a clue and I didn’t blame them, because, until it had happened to me, I hadn’t a clue either.

So we watched Worldhoppers. It didn’t complain about the lack of realism. After three episodes, it got agitated whenever a minor character was killed. When a major character died in the twentieth episode I had to pause seven minutes while it sat there in the feed doing the bot equivalent of staring at a wall, pretending that it had to run diagnostics. Then four episodes later the character came back to life and it was so relieved we had to watch that episode three times before it would go on. At the climax of one of the main story lines, the plot suggested the ship might be catastrophically damaged and members of the crew killed or injured, and the transport was afraid to watch it. (That’s obviously not how it phrased it, but yeah, it was afraid to watch it.) I was feeling a lot more charitable toward it by that point so was willing to let it ease into the episode by watching one to two minutes at a time. After it was over, it just sat there, not even pretending to do diagnostics. It sat there for a full ten minutes, which is a lot of processing time for a bot that sophisticated. Then it said, Again, please. So I started the first episode again.

The top turret gunner obeyed the order, but the ball turret gunner discovered that a piece of flak had jammed his turret and he could not get out. The other three men in his pact could have bailed out with parachutes, but they stayed with him until the plane hit the ground and exploded. They all died.

We carried it in a low-slung position as we ran up and down the Coronado sand dunes. We paddled the boat endlessly from north to south along the coastline and through the pounding surf, seven men, all working together to get the rubber boat to its final destination. But we learned something else on our journey with the raft. Occasionally, one of the boat crew members was sick or injured, unable to give it 100 percent. I often found myself exhausted from the training day, or down with a cold or the flu. On those days, the other members picked up the slack. They paddled harder. They dug deeper. They gave me their rations for extra strength. And when the time came, later in training, I returned the favor. The small rubber boat made us realize that no man could make it through training alone. No SEAL could make it through combat alone and by extension you needed people in your life to help you through the difficult times.

The Agency boys knew that Sukarno routinely engaged in extramarital affairs. But everyone in Indonesia also knew it. Indonesian elites didn’t shy away from Sukarno’s activities the way the Washington press corps protected philanderers like JFK. Some of Sukarno’s supporters viewed his promiscuity as a sign of his power and masculinity. Others, like Sumiyati and members of the Gerwani Women’s Movement, viewed it as an embarrassing defect. But the CIA thought this was their big chance to expose him. So they got a Hollywood film crew together.35 They wanted to spread the rumor that Sukarno had slept with a beautiful blond flight attendant who worked for the KGB, and was therefore both immoral and compromised. To play the president, the filmmakers (that is, Bing Crosby and his brother Larry) hired a “Hispanic-looking” actor, and put him in heavy makeup to make him look a little more Indonesian. They also wanted him bald, since exposing Sukarno—who always wore a hat—as such might further embarrass him. The idea was to destroy the genuine affection that young Sakono, and Francisca, and millions of other Indonesians, felt for the Founding Father of their country. The original flagship for the company was the MS City of New York, commanded by Captain George T. Sullivan, On March 29, 1942, she was attacked off the coast of Cape Hatteras, North Carolina, by the German submarine U-160. The torpedo struck the MS City of New York at the waterline under the ship’s bridge, instantly disabling her. After allowing the survivors to get into lifeboats the submarine sunk the ship. Almost two days after the attack, a destroyer, the USS Roper, rescued 70 survivors, of which 69 survived. An additional 29 others were picked up by USS Acushnet, formerly a seagoing tugboat and revenue cutter, operated by the U.S. Coast Guard. All these survivors were taken to the Naval Base in Norfolk, Virginia. Almost two weeks later, on April 11, 1942, a U.S. Army bomber on its way to Europe spotted a lifeboat drifting in the Gulf Stream. The boat contained six passengers: four women, one man and a young girl plus thirteen crew members. Tragically two of the women died of exposure. The eleven survivors picked up by the U.S. Coast Guard Cutter CG-455 and were brought to Lewes, Delaware. The final count showed that seven passengers died as well as one armed guard and sixteen crewmen. First, I am thrilled that paramedics are finally getting the respect they deserve for being the professionals they can be. The scope of practice is expanding, and patient care modalities are improving, seemingly by the minute. Patient outcomes are also improving as a result, and EMS is passing through puberty and forging into adulthood. On the other hand, autonomy in the hands of the “lesser-motivated,” can be a very dangerous thing. You know as well as I do that there are still plenty of providers who operate from a subjective, complacent, and downright lazy place. Combined with the ever-expanding autonomy, that provider just became more dangerous than he or she ever has been – to the patients and to you. Autonomy in patient care places more pressure for excellence on the provider charged with delivering it, and also on the partner and crew members on scene. Since the base hospital is not involved like it once was, they are likewise less responsible for the errors and omissions of the medics on the scene. Now more than ever, crew members are being held to answer for the mistakes and follies of their coworkers; now more than ever, EMS providers are working without a net. What’s next? I predict (and hope) emergency medical Darwinism is going to force some painful and necessary changes. First, increasing autonomy is going to result in the better and best providing superior patient care. More personal ownership of the results is going to manifest in outcomes such as increased cardiac arrest survival rates, faster and more complete stroke recovery, and significantly better outcomes for STEMI patients, all leading to the brass ring: EMS as a profession, not just a job. On the flip side of that coin, you will see consequences for the not-so-good and completely awful providers. There will be higher instances of licensure action, internal discipline, and wash-out. Unfortunately, all those things will stem from generally preventable negative patient outcomes. The danger for the better provider will be in the penumbra; the murky, gray area of time when providers are self-categorizing. Specifically, the better provider who is aware of the dangerously poor provider but does nothing to fix or flush him or her, is almost certain to be caught up in a bad situation caused by sloppy, complacent, or ultimately negligent patient care that should have been corrected or stopped. The answer is as simple as it is difficult. If you are reading this, it is more likely because you are one of the better, more committed, more professional providers. This transition is up to you. You must dig deep and find the strength necessary to face the issue and force the change; you have to demand more from yourself and from those around you. You must have the willingness to help those providers who want it – and respond to those who need it, but don’t want it – with tough love by showing them the door. In the end, EMS will only ever be as good as you make it. If you lay silent through its evolution, you forfeit the right to complain when it crumbles around you.

The Coast Guard patrol boat watched the City of Savannah steaming off toward New York. The Cahoone’s captain believed this, coupled with the general view of the situation, conveyed the impression that all passengers had been rescued. It was an unhappy mistake. Another followed. The Cahoone called up the Monarch of Bermuda. The Cahoone’s log recorded: “Monarch of Bermuda so busy handling press radio traffic that we cannot break in with a call.” The Monarch of Bermuda later denied the charge; its radio operators insisted they were only transmitting names of survivors and dead. Next the Cahoone approached the Morro Castle. The patrol boat’s log documents another curious incident: “Held verbal conversation with the crew of the Morro Castle, grouped on forecastle deck. When asked if they wanted to be taken off, some member of the crew, apparently an officer, replied they were going to stand by for a tow to port.” The official Coast Guard report on the Cahoone’s role makes equally strange reading: “Had the Morro Castle or the Monarch of Bermuda given the Cahoone any information that lifeboats had gone ashore or that passengers had jumped over the side, the Cahoone could have gone inshore to search, and possibly some lives might have been saved by that vessel.” (Author’s italics) In all, the Cahoone spent ninety minutes floundering around the Morro Castlebefore going off to search for swimmers. In the end it recovered two bodies.

On December 3, 1934, the grand jury handed out indictments. Accused of willful negligence were Acting Captain William Warms, Chief Engineer Eban Abbott, and Ward Line vice-president Henry E. Cabaud. In the preamble to the charges against Warms, the indictment declared: “Members of the crew were without discipline and did not know what to do, and the passengers were left to help themselves; the passengers in large numbers were pushed into the water or jumped in the water or perished in the fire.” Warms was accused specifically of failing to observe the law in ten matters: 1. To divide the sailors in equal watches. 2. To keep himself advised of the extent of the fire. 3. To maneuver, slow down, or stop the vessel. 4. To have the passengers aroused. 5. To provide the passengers with life preservers. 6. To take steps for the protection of lives. 7. To organize the crew to fight the fire properly. 8. To send distress signals promptly. 9. To see that the passengers were put in lifeboats and that the lifeboats were lowered. 10. To control and direct the crew in the lifeboats after the lifeboats had been lowered.
