He can clear his mask of water, by blowing valuable air into it through his nose, or he can continue to swim with his mask full of water blurring his vision. The swimmer makes the right decision and retrieves the snorkel. All this time both trainees are holding their breath, battling the urge to surface and suck in a lung full of sweet fresh air. With lungs burning and vision dimming, the swimmer hands the snorkel to his buddy. After taking only two breaths, his buddy returns the snorkel and, finally the instructor allows the swimmer to breathe his two breaths. While the trainees try to breathe, instructors splash water into foam around them while screaming insults. Despite the distractions, the snorkel travels back and forth between the trainees until once again, an instructor snatches it, tosses it across the pool, and floods both students’ masks. This harassment continues until the instructor is satisfied with the trainees’ performance.

We’re both better people for it. I collect penguins as a way to remind myself to notice the good things about the people in my life a lot more than the bad things. This has been so helpful for me as well as for many of my patients. It is often necessary to have something that reminds us of this prescription. It’s not natural for most of us to notice what we like about our life or what we like about others, especially if we unconsciously use turmoil to stimulate our prefrontal cortex. Focusing on the negative aspects of others or of your own life makes you more vulnerable to depression and can damage your relationships.

The lumbering plane that Clemente was on nose-dived into heavy seas shortly after takeoff from San Juan. Clemente was thirty-eight years old and had been a perennial All-Star, four-time winner of the National League batting championship, defensive genius, and MVP in 1966. He led the Pirates to two world championships, one in 1960 and the other a decade later in 1971. “Mr. Clemente was the leader of Puerto Rican efforts to aid the Nicaraguan victims and was aboard the plane because he suspected that relief supplies were falling into the hands of profiteers,” the New York Times reported after his death was presumed.3 Clemente was scheduled to meet Anastasio Somoza, the military dictator of Nicaragua, at the airport, one of the very grafters he was attempting to circumvent with his personal mission. Clemente’s body was never recovered. It was a bad omen for the start of 1973.

The term “alternobaric vertigo” was coined by Lundgren in 1965 to describe vertigo in deep-sea divers, but also referred to aircraft pilots in 1966. It occurs during ascent and rarely descent and is a result of asymmetrical middle-ear pressures. Classically the vertigo due to this pathogenesis is transient but may last for several minutes. It is frequently associated with nausea and vomiting. It has been reproduced in pressure chamber experiments with some divers and fliers, but has not been reported spontaneously at ground level (Figure F–1). FIGURE F–1. We encountered a 15-year-old female with bilateral tympanostomy tubes who manifested persistent severe vertigo, at ground level, secondary to a unilateral middle-ear pressure of +200 mm H2O elicited by an obstructed tympanostomy tube in the presence of chronic nasal obstruction. She had had long-term tubes placed due to recurrent and chronic otitis media. Physical examination revealed achondroplasia, which is an autosomal dominate disorder characterized by abnormal bone growth, short arms and legs, short stature and a large head, which is associated with otitis media. The pathogenesis of otitis media in these individuals may be related to abnormal anatomy causing Eustachian tube dysfunction. Balance testing was abnormal, Eustachian tube function tests revealed dysfunction of tube. Surgery was performed to replace the obstructed tube with a patent one, and an adenoidectomy and bilateral inferior turbinate reduction to relieve the chronic nasal obstruction. Postoperatively balance testing was normal, Eustachian tube function remained dysfunctional, but she had complete resolution of her vertigo following the surgery. FIGURE F–2. You inherit a very sick goose, a machine that, by this time, is rusted and starts to break down. You have to invest heavily in downtime and maintenance. Costs skyrocket; profits nose-dive.
