The question who is sane and who is crazy was the theme of the novel One Flew Over The Cuckoo's Nest. The question, what is sanity? was clearly asked in the play Equus. The idea that much of what we do is insane and that if we want to be sane, we must let ourselves go crazy has been strongly advanced by R.D. Laing. In the preface to the Pelican edition of his book The Divided Self, Laing writes: "In the context of our present pervasive madness that we call normality, sanity, freedom, all of our frames of reference are ambiguous and equivocal." And in the same preface: "Thus I would wish to emphasize that our 'normal' 'adjusted' state is too often the abdication of ecstasy, the betrayal of our true potentialities; that many of us are only too successful in acquiring a false self to adapt to false realities." Wilhelm Reich had a somewhat similar view of present-day human behavior. Thus Reich says, "Homo normalis blocks off entirely the perception of basic orgonotic functioning by means of rigid armoring; in the schizophrenic, on the other hand, the armoring practically breaks down and thus the biosystem is flooded with deep experiences from the biophysical core with which it cannot cope." The "deep experiences" to which Reich refers are the pleasurable streaming sensations associated with intense excitation that is mainly sexual in nature. The schizophrenic cannot cope with these sensations because his body is too contracted to tolerate the charge. Unable to "block" the excitation or reduce it as a neurotic can, and unable to "stand" the charge, the schizophrenic is literally "driven crazy." But the neurotic does not escape so easily either. He avoids insanity by blocking the excitation, that is, by reducing it to a point where there is no danger of explosion, or bursting. In effect the neurotic undergoes a psychological castration. However, the potential for explosive release is still present in his body, although it is rigidly guarded as if it were a bomb. The neurotic is on guard against himself, terrified to let go of his defenses and allow his feelings free expression. Having become, as Reich calls him, "homo normalis," having bartered his freedom and ecstasy for the security of being "well adjusted," he sees the alternative as "crazy." And in a sense he is right. Without going "crazy," without becoming "mad," so mad that he could kill, it is impossible to give up the defenses that protect him in the same way that a mental institution protects its inmates from self-destruction and the destruction of others.

Usually, partners who are unable to respond compassionately when hearing us speak our pain, whether they understand it or not, are unable to listen because that expressed hurt triggers their own feelings of powerlessness and helplessness. Many men never want to feel helpless or vulnerable. They will, at times, choose to silence a partner with violence rather than witness emotional vulnerability. When a couple can identify this dynamic, they can work on the issue of caring, listening to each other's pain by engaging in short conversations at appropriate times (i.e., it's useless to try and speak your pain to someone who is bone weary, irritable, reoccupied, etc.). Setting a time when both individuals come together to engage in compassionate listening enhances communication and connection. When we are committed to doing the work of love we listen even when it hurts.

Physiological stress, then, is the link between personality traits and disease. Certain traits — otherwise known as coping styles — magnify the risk for illness by increasing the likelihood of chronic stress. Common to them all is a diminished capacity for emotional communication. Emotional experiences are translated into potentially damaging biological events when human beings are prevented from learning how to express their feelings effectively. That learning occurs — or fails to occur — during childhood. The way people grow up shapes their relationship with their own bodies and psyches. The emotional contexts of childhood interact with inborn temperament to give rise to personality traits. Much of what we call personality is not a fixed set of traits, only coping mechanisms a person acquired in childhood. There is an important distinction between an inherent characteristic, rooted in an individual without regard to his environment, and a response to the environment, a pattern of behaviours developed to ensure survival. What we see as indelible traits may be no more than habitual defensive techniques, unconsciously adopted. People often identify with these habituated patterns, believing them to be an indispensable part of the self. They may even harbour self-loathing for certain traits — for example, when a person describes herself as “a control freak.” In reality, there is no innate human inclination to be controlling. What there is in a “controlling” personality is deep anxiety. The infant and child who perceives that his needs are unmet may develop an obsessive coping style, anxious about each detail. When such a person fears that he is unable to control events, he experiences great stress. Unconsciously he believes that only by controlling every aspect of his life and environment will he be able to ensure the satisfaction of his needs. As he grows older, others will resent him and he will come to dislike himself for what was originally a desperate response to emotional deprivation. The drive to control is not an innate trait but a coping style. Emotional repression is also a coping style rather than a personality trait set in stone. Not one of the many adults interviewed for this book could answer in the affirmative when asked the following: When, as a child, you felt sad, upset or angry, was there anyone you could talk to — even when he or she was the one who had triggered your negative emotions? In a quarter century of clinical practice, including a decade of palliative work, I have never heard anyone with cancer or with any chronic illness or condition say yes to that question. Many children are conditioned in this manner not because of any intended harm or abuse, but because the parents themselves are too threatened by the anxiety, anger or sadness they sense in their child — or are simply too busy or too harassed themselves to pay attention. “My mother or father needed me to be happy” is the simple formula that trained many a child — later a stressed and depressed or physically ill adult — into lifelong patterns of repression.

Vulnerability is usually attacked, not with fists but with shaming. Many children learn quickly to cover up any signs of weakness, sensitivity, and fragility, as well as alarm, fear, eagerness, neediness, or even curiosity. Above all, they must never disclose that the teasing has hit its mark. Carl Jung explained that we tend to attack in others what we are most uncomfortable with in ourselves. When vulnerability is the enemy, it is attacked wherever it is perceived, even in a best friend. Signs of alarm may provoke verbal taunts such as “fraidy cat” or “chicken.” Tears evoke ridicule. Expressions of curiosity can precipitate the rolling of eyes and accusations of being weird or nerdy. Manifestations of tenderness can result in incessant teasing. Revealing that something caused hurt or really caring about something is risky around someone uncomfortable with his vulnerability. In the company of the desensitized, any show of emotional openness is likely to be targeted. The vulnerability engendered by peer orientation can be overwhelming even when children are not hurting one another. This vulnerability is built into the highly insecure nature of peer-oriented relationships. Vulnerability does not have to do only with what is happening but with what could happen — with the inherent insecurity of attachment. What we have, we can lose, and the greater the value of what we have, the greater the potential loss. We may be able to achieve closeness in a relationship, but we cannot secure it in the sense of holding on to it — not like securing a rope or a boat or a fixed interest-bearing government bond. One has very little control over what happens in a relationship, whether we will still be wanted and loved tomorrow. Although the possibility of loss is present in any relationship, we parents strive to give our children what they are constitutionally unable to give to one another: a connection that is not based on their pleasing us, making us feel good, or reciprocating in any way. In other words, we offer our children precisely what is missing in peer attachments: unconditional acceptance.

The shocked tears finally caught up to Christy's eyes, and she blinked to keep Todd in focus. "It is," she agreed. "God brought us back together, didn't He?" A giggle of joy and delight danced from her lips. "Do you remember what I said when you gave me back your bracelet?" Todd asked. "I said that if God ever brought us back together, I would put that bracelet back on your wrist, and that time, it would stay on forever." Christy nodded. She had replayed the memory of that day a thousand times in her mind. It had seemed impossible that God would bring them back together. Christy's heart pounded as she realized that God, in His weird way, had done the impossible. Todd reached into his pocket and pulled out the "Forever" ID bracelet. He tenderly held Christy's wrist, and circling it with the gold chain, he secured the clasp. Above their heads a fresh ocean wind blew through the palm trees. I do believe that we (autistic individuals such as myself) are very susceptible to suicidal thinking for multiple reasons that include: chronic high levels of anxiety, tendency to fixate on or get stuck on negative disturbing thoughts, low self-worth, inability to have significant or intimate relationships with others, replaying over and over again negative statements that others have said to us, feeling unable to be understood, lack [of] a solid self-identity, difficulty with expressing self to others, feelings of great isolation, feeling that you are or may be a burden to others, feeling unable to contribute to society or the greater good, etc […] I do believe that the most important thing that someone else can do for a struggling autistic individual is to affirm their self-worth, recognise and validate their struggles and affirm the things that they do that are greatly valued by others. The worst thing to do for an autistic individual, or any struggling individual for that matter, is to not believe them or to deny the validity of their struggles. My greatest and deepest hurt is that doctors, family members and important others did not believe me in my struggles, particularly when I was younger, before my diagnosis at the age of 35 years. Then we may wish to reflect back, paraphrasing what we have understood. We stay with empathy and allow others the opportunity to fully express themselves before we turn our attention to solutions or requests for relief. We need empathy to give empathy. When we sense ourselves being defensive or unable to empathize, we need to (1) stop, breathe, give ourselves empathy; (2) scream nonviolently; or (3) take time out.

How often we see (in a period when we extol that Greek historian because he knows how to appropriate an unfamiliar style so delusively like the original author's, instead of censuring him, since the first prize always goes to an author for having his own style―that is, a mode of expression and presentation qualified by his own individuality)―how often we see people who either out of mental-spiritual laziness live on the crumbs that fall from another's table or for more egotistical reasons seek to identify themselves with others, until eventually they believe it all, just like the liar through frequent repetition of his stories.

To speak of a communication failure implies a breakdown of some sort. Yet this does not accurately portray what occurs. In truth, communication difficulties arise not from breakdown but from the characteristics of the system itself. Despite promising beginnings in our intimate relationships, we tend over time to evolve a system of communication that suppresses rather than reveals information. Life is complicated, and confirming or disconfirming the well-being of a relationship takes effort. Once we are comfortably coupled, the intense, energy-consuming monitoring of courtship days is replaced by a simpler, more efficient method. Unable to witness our partners’ every activity or verify every nuance of meaning, we evolve a communication system based on trust. We gradually cease our attentive probing, relying instead on familiar cues and signals to stand as testament to the strength of the bond: the words “I love you,” holidays with the family, good sex, special times with shared friends, the routine exchange, “How was your day?” We take these signals as representative of the relationship and turn our monitoring energies elsewhere. ... Not only do the initiator’s negative signals tend to become incorporated into the existing routine, but, paradoxically, the initiator actively contributes to the impression that life goes on as usual. Even as they express their unhappiness, initiators work at emphasizing and maintaining the routine aspects of life with the other person, simultaneously giving signals that all is well. Unwilling to leave the relationship yet, they need to privately explore and evaluate the situation. The initiator thus contrives an appearance of participation,7 creating a protective cover that allows them to “return” if their alternative resources do not work out. Our ability to do this—to perform a role we are no longer enthusiastically committed to—is one of our acquired talents. In all our encounters, we present ourselves to others in much the same way as actors do, tailoring our performance to the role we are assigned in a particular setting.8 Thus, communication is always distorted. We only give up fragments of what really occurs within us during that specific moment of communication.9 Such fragments are always selected and arranged so that there is seldom a faithful presentation of our inner reality. It is transformed, reduced, redirected, recomposed.10 Once we get the role perfected, we are able to play it whether we are in the mood to go on stage or not, simply by reproducing the signals. What is true of all our encounters is, of course, true of intimate relationships. The nature of the intimate bond is especially hard to confirm or disconfirm.11 The signals produced by each partner, while acting out the partner role, tend to be interpreted by the other as the relationship.12 Because the costs of constantly checking out what the other person is feeling and doing are high, each partner is in a position to be duped and misled by the other.13 Thus, the initiator is able to keep up appearances that all is well by falsifying, tailoring, and manipulating signals to that effect. The normal routine can be used to attest to the presence of something that is not there. For example, initiators can continue the habit of saying, “I love you,” though the passion is gone. They can say, “I love you” and cover the fact that they feel disappointment or anger, or that they feel nothing at all. Or, they can say, “I love you” and mean, “I like you,” or, “We have been through a lot together,” or even “Today was a good day.

Making Requests Consciously Sometimes we may be able to communicate a clear request without putting it in words. Suppose you’re in the kitchen and your sister, who is watching television in the living room, calls out, “I’m thirsty.” In this case, it may be obvious that she is requesting you to bring her a glass of water from the kitchen. However, in other instances, we may express our discomfort and incorrectly assume that the listener has understood the underlying request. For example, a woman might say to her husband, “I’m annoyed you forgot the butter and onions I asked you to pick up for dinner.” While it may be obvious to her that she is asking him to go back to the store, the husband may think that her words were uttered solely to make him feel guilty. When we simply express our feelings, it may not be clear to the listener what we want them to do. Even more often, we are simply not conscious of what we are requesting when we speak. We talk to others or at them without knowing how to engage in a dialogue with them. We toss out words, using the presence of others as a wastebasket. In such situations, the listener, unable to discern a clear request in the speaker’s words, may experience the kind of distress illustrated in the following anecdote.

Because my people never expressed negative feelings directly (not a bequest of our totalitarian surroundings, but because they wished, above all, to show love, and what kind of love was it, they thought, if you disagreed openly?), I had to learn how to listen for what was meant rather than said, becoming acutely observant. That same love, however, meant I was never discouraged from speaking. A table of adults would fall silent so I could ask, or say. That last was the key: A fellow immigrant writer friend with a nearly identical background had only the first three, and had to work much harder to find the courage to put words on a page. I owed to my elders the career that hand given them such alarm.

You avoid discussing certain subjects because you’re afraid of your fiancé(e)’s reaction. Your fiancé(e) finds it extremely difficult to express emotions, or is prone to extreme emotions (such as out-of-control anger or exaggerated fear). Or he/she swings back and forth between emotional extremes (such as being very happy one minute, then suddenly exhibiting extreme sadness the next). Your fiancé(e) displays controlling behavior. This means more than a desire to be in charge—it means your fiancé(e) seems to want to control every aspect of your life: your appearance, your lifestyle, your interactions with friends or family, and so on. Your fiancé(e) seems to manipulate you into doing what he or she wants. You are continuing the relationship because of fear—of hurting your fiancé(e), or of what he or she might do if you ended the relationship. Your fiancé(e) does not treat you with respect. He or she constantly criticizes you or talks sarcastically to you, even in public. Your fiancé(e) is unable to hold down a job, doesn’t take personal responsibility for losing a job, or frequently borrows money from you or from friends. Your fiancé(e) often talks about aches and pains, and you suspect some of these are imagined. He or she goes from doctor to doctor until finding someone who will agree that there is some type of illness. Your fiancé(e) is unable to resolve conflict. He or she cannot deal with constructive criticism, or never admits a mistake, or never asks for forgiveness. Your fiancé(e) is overly dependant on parents for finances, decision-making or emotional security. Your fiancé(e) is consistently dishonest and tries to keep you from learning about certain aspects of his or her life. Your fiancé(e) does not appear to recognize right from wrong, and rationalizes questionable behavior. Your fiancé(e) consistently avoids responsibility. Your fiancé(e) exhibits patterns of physical, emotional or sexual abuse toward you or others. Your fiancé(e) displays signs of drug or alcohol abuse: unexplained absences of missed dates, frequent car accidents, the smell of alcohol or strong odor of mouthwash, erratic behavior or emotional swings, physical signs such as red eyes, unkempt look, unexplained nervousness, and so on. Your fiancé(e) has displayed a sudden, dramatic change in lifestyle after you began dating. (He or she may be changing just to win you and will revert back to old habits after marriage.) Your fiancé(e) has trouble controlling anger. He or she uses anger as a weapon or as a means of winning arguments. You have a difficult time trusting your fiancé(e)—to fulfill responsibilities, to be truthful, to help in times of need, to make ethical decisions, and so on. Your fiancé(e) has a history of multiple serious relationships that have failed—a pattern of knowing how to begin a relationship but not knowing how to keep one growing. Look over this list. Do any of these red flags apply to your relationship? If so, we recommend you talk about the situation as soon as possible with a pastor, counselor or mentor.

We're constantly reminded that this precious life is what you make of it. But what if you're not sure of what you want to make it into? On the one hand there are those resolute in their life's agenda and objectives, often set by the scriptural society they choose to adhere to, or one passed down from parents and family. They know what they want because they allow themselves to be told what is important, to be guided by those who have gone before. A proven formula maybe, or an unrealistic dream. Is true success in ones life fairly measured against someone else's achievements, should we use those achievements of others as our own check list? Surely we will find happiness just as they have, or not, at the end of it. The opposite end of the spectrum sees the tragic dreamers, unable to answer the question of why they're even here, the absence of knowing what their true calling is drives them close to insanity, desperate to live a meaningful life but haunted by the inability to see what constitutes as such. Often turning to artistic release to try and express themselves, their own high standards against which they measure themselves tragically, often fatally high. I find myself somewhere in the middle. I know what society expects but I don't agree with all of it. Much I have to adhere to simply to exist. Fortunately an education grants me a career not a job, that in the current world gives me choices that others do not and I am thankful. But I'm concious that the well beaten paths lead to the same final destination that others have arrived at and been disappointed in themselves, for not aiming higher or being brave enough to be different. Life is what we make of it, but regardless of how lofty or how humble our desired achievements are we should never lose sight of the fact that it is our life to live. We should all feel comfortable enough to make our own mistakes, to make deviations from the main path, to explore with our own eyes and minds. We should ignore those who tell us our dreams are too big, or to lowly or just plain wrong. Deciding whose own advice and guidance to follow, or ignore is often the hardest thing.

No sound strategy for studying fascism can fail to examine the entire context in which it was formed and grew. Some approaches to fascism start with the crisis to which fascism was a response, at the risk of making the crisis into a cause. A crisis of capitalism, according to Marxists, gave birth to fascism. Unable to assure ever-expanding markets, ever-widening access to raw materials, and ever-willing cheap labor through the normal operation of constitutional regimes and free markets, capitalists were obliged, Marxists say, to find some new way to attain these ends by force. Others perceive the founding crisis as the inadequacy of liberal state and society (in the laissez-faire meaning of liberalism current at that time) to deal with the challenges of the post-1914 world. Wars and revolutions produced problems that parliament and the market—the main liberal solutions—appeared incapable of handling: the distortions of wartime command economies and the mass unemployment attendant upon demobilization; runaway inflation; increased social tensions and a rush toward social revolution; extension of the vote to masses of poorly educated citizens with no experience of civic responsibility; passions heightened by wartime propaganda; distortions of international trade and exchange by war debts and currency fluctuations. Fascism came forward with new solutions for these challenges. Fascists hated liberals as much as they hated socialists, but for different reasons. For fascists, the internationalist, socialist Left was the enemy and the liberals were the enemies’ accomplices. With their hands-off government, their trust in open discussion, their weak hold over mass opinion, and their reluctance to use force, liberals were, in fascist eyes, culpably incompetent guardians of the nation against the class warfare waged by the socialists. As for beleaguered middle-class liberals themselves, fearful of a rising Left, lacking the secret of mass appeal, facing the unpalatable choices offered them by the twentieth century, they have sometimes been as ready as conservatives to cooperate with fascists. Every strategy for understanding fascism must come to terms with the wide diversity of its national cases. The major question here is whether fascisms are more disparate than the other “isms.” This book takes the position that they are, because they reject any universal value other than the success of chosen peoples in a Darwinian struggle for primacy. The community comes before humankind in fascist values, and respecting individual rights or due process gave way to serving the destiny of the Volk or razza. Therefore each individual national fascist movement gives full expression to its own cultural particularism. Fascism, unlike the other “isms,” is not for export: each movement jealously guards its own recipe for national revival, and fascist leaders seem to feel little or no kinship with their foreign cousins. It has proved impossible to make any fascist “international” work.

God is blessing the church in China with extraordinary growth. However, when Chinese churches and ministers who had experienced God’s blessing in their rural ministries entered the mushrooming cities of China and tried to minister and communicate the gospel in the same ways that had been blessed in the countryside, they saw less fruitfulness. Over a decade ago, several Dutch denominations approached us. While they were thriving outside of urban areas, they had not been able to start new, vital churches in Amsterdam in years — and most of the existing ones had died out. These leaders knew the gospel; they had financial resources; they had the desire for Christian mission. But they couldn’t get anything off the ground in the biggest city of their country.2 In both cases, ministry that was thriving in the heartland of the country was unable to make much of a dent in the city. It would have been easy to say, “The people of the city are too spiritually proud and hardened.” But the church leaders we met chose to respond humbly and took responsibility for the problem. They concluded that the gospel ministry that had fit nonurban areas well would need to be adapted to the culture of urban life. And they were right. This necessary adaptation to the culture is an example of what we call “contextualization.”3 SOUND CONTEXTUALIZATION Contextualization is not — as is often argued — “giving people what they want to hear.”4 Rather, it is giving people the Bible’s answers, which they may not at all want to hear, to questions about life that people in their particular time and place are asking, in language and forms they can comprehend, and through appeals and arguments with force they can feel, even if they reject them. Sound contextualization means translating and adapting the communication and ministry of the gospel to a particular culture without compromising the essence and particulars of the gospel itself. The great missionary task is to express the gospel message to a new culture in a way that avoids making the message unnecessarily alien to that culture, yet without removing or obscuring the scandal and offense of biblical truth. A contextualized gospel is marked by clarity and attractiveness, and yet it still challenges sinners’ self-sufficiency and calls them to repentance. It adapts and connects to the culture, yet at the same time challenges and confronts it. If we fail to adapt to the culture or if we fail to challenge the culture — if we under- or overcontextualize — our ministry will be unfruitful because we have failed to contextualize well.

To give simple instances: if the patient has a deep conviction that he is failing to make a fair contribution to society, he may be unable to digest and absorb foods that should normally be adequate. If he feels that society is not giving him proper appreciation he may find it impossible to be satisfied with normal feeding, and be plagued by cravings for all manner of things - even those that he consciously recognises as destructive.

Ultimately, attachment theory helps one understand the ways in which people function on an individual level and while interacting with one another. Although attachment theory has a variety of applications, it tends to be especially useful in couples’ therapy. Since each attachment style has generalized trends, understanding your or your partner’s coping mechanisms, subconscious beliefs, and perceptions can relieve substantial communication issues. For example, in a relationship, the Dismissive-Avoidant may be withdrawn, autonomous, and seemingly independent. To the Dismissive-Avoidant, they are functioning as they always have—on their own. To an Anxious Attachment, however, it may feel as though their partner is on the verge of abandoning them and may cause serious emotional distress. However, the Dismissive-Avoidant’s coping mechanisms don’t necessarily mean they are detaching from the relationship—they are actually just detaching from their own emotions. Now, although none of these behaviors are necessarily healthy in a relationship, understanding why they occur is the first step. Once partners understand each other’s coping mechanisms and vulnerabilities, they can begin to supply their partner with the things that they do need. For example, the Dismissive-Avoidant needs continuous and unwavering emotional support and validation. Since they were emotionally neglected as a child, they need to slowly learn that they can consistently and predictably rely on others. The Anxious Attachment individual needs reassurance and affection to understand that they are good enough and that they won’t be rejected. The simple knowledge of the pain points of your partner and the pain points that lie within yourself opens up a whole stream of communication that you previously were unable to tap into—because your conscious mind didn’t even know it was there. So, make space for yourself and take care of your own emotional needs. There’s no restriction on how you can or should express your feelings. Even crying out of anger is a reasonable solution.
