Every group within white America has at one time or another advanced its particular and narrowly defined interests at the expense of black people as a race. That applies to labor unionists, ethnic groups, college students, schoolteachers, taxpayers, and white women. Instead the aberrant methyl patterns were imprinted early on, and as neurons kept dividing and the brain kept growing, these patterns perpetuated themselves. The events of September 11, 2001, might have scarred the brains of unborn humans in similar ways. Some pregnant women in Manhattan developed post-traumatic stress disorder, which can epigenetically activate and deactivate at least a dozen genes, including brain genes. These women, especially the ones affected during the third trimester, ended up having children who felt more anxiety and acute distress than other children when confronted with strange stimuli. Notice that these DNA changes aren’t genetic, because the A-C-G-T string remains the same throughout. But epigenetic changes are de facto mutations; genes might as well not function. I look at it this way; it is like telling a dead person he/she can start over. How so, when that person’s life no longer exists? I know my life isn’t over. However, I am lost in a maze my mom set up for herself—and she too is lost in her own maze. When a person gets lost in their own maze, they are really fucked up. However, this maze shouldn’t be left for me to figure out. Unfortunately, I am in it, and I have to find my way out one way or another. What’s the point of taking Kace from me? He was safe and in good hands. Now he is worse off with people who are abusing him. He didn’t ask for this—I didn’t either. He deserves so much better. Again, what is the point of it all? Chapter FEEDING YOUR ATTENTION HOG I was once at a New Age party and wanted to get the attention of some particularly lovely sari-wearing, belly-dancing women who were floating in and out of the various rooms. I had discovered that I could move past some of my fear and make a connection with people through singing. So I pulled out my guitar and started playing a song I had worked particularly hard to polish, Fleetwood Mac’s “A Crystalline Knowledge of You.” I was able to make it through without too many mistakes and was starting to feel the relief that comes from surviving traumatic experiences. Then one of the belly-dancing goddesses called to me from across the room, “You are some kind of attention hog, aren’t you!” As soon as she said it, my life passed before me. The room started to swirl, as a typhoon of shame began to suck me down the toilet of my soul. “Embarrassment” is an inadequate word, when someone pins the tail on the jackass of what seems to be your most central core defect. I am usually scrupulous about checking with people when I make requests for attention. But this time I was caught with my hand in the cookie jar up to the elbow. I remember slinking away in silent humiliation, putting my guitar back in its case and making a beeline for my car. I just wanted to get back to my lair to lick my wounds, and try to hold my self-hate demons at bay with a little help from my friend Jack Daniels. After that incident I quit playing music in public at all. Several years later I was attending a very intense, emotional workshop with Dr. Marshall Rosenberg. Our group of about twenty people had been baring and healing our souls for several days. The atmosphere of trust, safety and connectedness had dissolved my defenses and left me with a innocent, childlike need to contribute. And then the words popped out of my mouth, “I’d like to share a song with you all.” These words were followed by the thought: “Now I’ve gone and done it. When everyone turns on me and confirms that I have an incurable narcissistic personality disorder, it will be fifty years before I sing in public again.” Dr. Rosenberg responded in a cheerful, inviting voice. “Sure, go get your guitar!” he said, as though he were unaware that I was about to commit hara-kiri. The others in the group nodded agreement. I ran to my car to get my guitar, which I kept well hidden in the trunk. I was also hoping that I would not just jump in my car and leave. I brought the guitar in, sat down, and played my song. Sweating and relieved that I made it through the song, my first public performance in years, I felt relief as I packed my guitar in its case. Then Dr. Rosenberg said, “And now I would like to hear from each group member how they felt about Kelly playing his song.” “Oh my God!” my inner jackals began to howl, “It was a setup! They made me expose my most vulnerable part and now they are going to crucify me, or maybe just take me out to the rock quarry for a well-deserved stoning!

Going too fast is one of the mistakes all negotiators are prone to making. If we’re too much in a hurry, people can feel as if they’re not being heard. You risk undermining the rapport and trust you’ve built. Put a smile on your face. When people are in a positive frame of mind, they think more quickly, and are more likely to collaborate and problem-solve (instead of fight and resist). Positivity creates mental agility in both you and your counterpart. There are three voice tones available to negotiators: The late-night FM DJ voice: Use selectively to make a point. Inflect your voice downward, keeping it calm and slow. When done properly, you create an aura of authority and trustworthiness without triggering defensiveness. The positive/playful voice: Should be your default voice. It’s the voice of an easygoing, good-natured person. Your attitude is light and encouraging. The key here is to relax and smile while you’re talking. The direct or assertive voice: Used rarely. Will cause problems and create pushback. Mirrors work magic. Repeat the last three words (or the critical one to three words) of what someone has just said. We fear what’s different and are drawn to what’s similar. Mirroring is the art of insinuating similarity, which facilitates bonding. Use mirrors to encourage the other side to empathize and bond with you, keep people talking, buy your side time to regroup, and encourage your counterparts to reveal their strategy.

In his book-length review of the executive functions, Dr. Russell Barkley (2012) explored the reasons that these skills evolved in humans in the first place. He makes the compelling case that it was the selection pressures associated with humans living in larger groups of genetically unrelated individuals, which made it selectively advantageous to have good self-regulation skills. That is, these abilities became more important to survival as humans became more interdependent with and reliant on dealings with people who were not family. Attention-Deficit/Hyperactivity Disorder (ADHD) and executive dysfunction continue to have effects on the myriad relationships and social interactions in daily life. These connections include romantic and committed relationships/marriage, relationships with parents, siblings, children, and other relatives, friendships, and interactions with employers, coworkers, and customers. The executive functions in relationships also figure in the capacity for empathy and tracking social debt, that is, the balance of favors you owe others and favors owed to you. The ability to effectively organize behavior across time in goal-directed activities gains you “social collateral.” That is, the more you deliver on promises and projects, the more that you will be sought out by others and maintain bonds with them. Some of the common manifestations of ADHD and executive dysfunction that may create problems in relationships include: • Distractibility during conversations • Forgetfulness about matters relevant to another person • Verbal impulsivity—talking over someone else • Verbal impulsivity—saying the “wrong thing” • Breaking promises (acts of commission, e.g., making an expensive purchase despite agreeing to stay within a household budget) • Poor follow-through on promises (acts of omission, e.g., forget to pick up dry cleaning) • Disregarding the effects of one’s behavior on others (e.g., building up excessive debt on a shared credit card account) • Poor frustration tolerance, anger (e.g., overreacting to children’s behavior) • Lying to cover up mistakes • Impulsive behaviors that reduce trust (e.g., romantic infidelity)

The second reason that the traditional moral and legal approach to violence actually stimulates violence, rather than preventing it, is because it is based on one huge mistake — namely, an unquestioned assumption, which is not merely a factual error, but actually reverses the truth. The criminal justice system, and all the other institutions in our society, to the extent that they are based on moral ways of thinking, have not recognized that this assumption is mistaken because it cannot be examined, criticized, or tested as long as one is thinking in moral terms — for it is not a moral assumption. Rather, it is an empirical assumption — namely, the assumption that punishing those who commit violent acts will deter them and others from committing such acts in the future; in other words, the assumption that punishment (the deliberate infliction of pain) prevents violence. As long as we restrict ourselves to thinking in the traditional terms about punishment as a response to crime and violence (including war crimes), we find that the only kinds of questions we can ask are those that employ moral categories rather than empirical ones, such as, "Did the person deserve that punishment," or, "Was it a just punishment?" However, when one tests against empirical data the assumption that punishment will prevent violence, by examining how people actually behave, one finds that far from preventing violence, punishment it the most powerful stimulus to violent behavior that we have yet discovered. Punishment does not prevent violence, it causes it, in addition to being a form of it. So it is not surprising that the moral and legal approach to violence has been followed by four millennia of constantly increasing levels of violence.

Groups are, in a sense, a microcosm of the real world. In all groups, there are leaders and followers . . . and many people who fall somewhere in between. Some groups are professionally led, and some are self- or volunteer-directed. In every group, there will be people you like and people you don’t, people who seek you out, and people who do not. Understanding and joining in the group process and making it work for you is what is important. Experiment with several groups, if you like, to find the ones that you enjoy the most. Strive to find a group in which you think you would feel comfortable expressing yourself or interacting with others and which has an appropriate meaning for you (a self-help group should address your particular issues; a hobby club should focus on something you enjoy). Attend the group a few times to get a sense of how members interact with each other. If the thought of doing so still causes you anxiety, continue working on stress management, and remain fairly passive in the group until you feel more comfortable. In my own social therapy group program, our purpose is to help individuals learn how to control social anxiety and refine their interactive skills. Social anxiety is a people-oriented problem, which makes group experience important both theoretically and practically. Some traditional therapists have called my program unorthodox because it encourages patients to talk to and learn from each other—as opposed to the isolation and protection offered by many of the more conservative therapies. But I say that social interaction is something you learn by doing. My groups are places to practice, make mistakes, and experience success in a supportive yet challenging environment. Of course, even in such a supportive setting, resistance still arises. In a “friendly” forum, stressors can be explored and confronted more easily, however, and I have found that the degree to which a person uses the group is often a good indicator of how well he or she is progressing therapeutically. Good attendance shows effort and commitment; poor attendance indicates that a person is giving in to anxiety. I’ve heard all the excuses and manipulations—canceling plans is typical of people with avoidance problems related to social anxiety. (I’m sometimes tempted to open a garage to repair all those cars that break down on group night!) Yet often, after overcoming the initial stage of anxiety, many participants enjoy the process. As you consider the option of incorporating various kinds of groups in your community into your self-help program, remember that groups can be a very important component of your map for change. Groups can provide you with the opportunity to practice the skills that are crucial to your success. Make sure that your expectations are realistic and that you understand the purpose and the limitations of whatever group you join.

I don’t think I’m too wrapped up in these identities until someone gets it wrong. I know it sounds pathetic, and believe me I am embarrassed to admit it, but I sometimes—more often than I wish—find myself wanting to be identified by something I’ve done or accomplished. Most of us grew up being taught that our identity as a person is based on our accomplishments. Your identity is closely tied to the points you score, the trophies you win, the grades you make, the diplomas you earn, the jobs you get, the promotions you receive, the portfolio you build. We build our resumés, display our achievements, and frame our accomplishments. In Philippians 3 Paul talks about how his identity used to be wrapped up in these things. He had some pretty impressive credentials. He was born into the right family, attended notable schools, received impressive degrees, landed in a powerful position. If he was introduced by someone who was identifying him, everyone would have been impressed. But here’s Paul’s conclusion about all of that. I once thought these things were valuable, but now I consider them worthless because of what Christ has done. Yes, everything else is worthless when compared with the infinite value of knowing Christ Jesus my Lord. For his sake I have discarded everything else, counting it all as garbage, so that I could gain Christ. (Philippians 3:7–8 NLT) I am a follower of Jesus. No mistake I have ever made and no success I have ever had says as much about me as that. There are books for when you’re bored. Plenty of them. There are books for when you’re calm. The best kind, in my opinion. There are also books for when you’re sad. And there are books for when you’re happy. There are books for when you’re thirsty for knowledge. And there are books for when you’re desperate. The latter are the kind of books Ulises Lima and Belano wanted to write. A serious mistake, as we’ll soon see. Let’s take, for example, an average reader, a cool-headed, mature, educated man leading a more or less healthy life. A man who buys books and literary magazines. So there you have him. This man can read things that are written for when you’re calm, but he can also read any other kind of book with a critical eye, dispassionately, without absurd or regrettable complicity. That’s how I see it. I hope I’m not offending anyone. Now let’s take the desperate reader, who is presumably the audience for the literature of desperation. What do we see? First: the reader is an adolescent or an immature adult, insecure, all nerves. He’s the kind of fucking idiot (pardon my language) who committed suicide after reading Werther. Second: he’s a limited reader. Why limited? That’s easy: because he can only read the literature of desperation, or books for the desperate, which amounts to the same thing, the kind of person or freak who’s unable to read all the way through In Search of Lost Time, for example, or The Magic Mountain (a paradigm of calm, serene, complete literature, in my humble opinion), or for that matter, Les Misérables or War and Peace. Am I making myself clear? Good. So I talked to them, told them, warned them, alerted them to the dangers they were facing. It was like talking to a wall. Furthermore: desperate readers are like the California gold mines. Sooner or later they’re exhausted! Why? It’s obvious! One can’t live one’s whole life in desperation. In the end the body rebels, the pain becomes unbearable, lucidity gushes out in great cold spurts. The desperate reader (and especially the desperate poetry reader, who is insufferable, believe me) ends up by turning away from books. Inevitably he ends up becoming just plain desperate. Or he’s cured! And then, as part of the regenerative process, he returns slowly—as if wrapped in swaddling clothes, as if under a rain of dissolved sedatives—he returns, as I was saying, to a literature written for cool, serene readers, with their heads set firmly on their shoulders. This is what’s called (by me, if nobody else) the passage from adolescence to adulthood.

As Dorothy believes that the Wizard is the only one who can help her, she tries harder and harder to please him. Similarly, your involvement with the NPD individual is characterized by an ever-increasing effort to please and gain approval. However, like the Wizard, the narcissist's approval is rarely given. Instead, you are more likely to see the unpredictable anger and rage over the smallest infraction or mistake. Great sensitivity to criticism, or intolerance of anything perceived as less than a perfect performance, can cause the NPD individual to unleash an outburst of sharp and hurtful rage. At times these experiences leave you feeling helpless, unable to do anything but crawl off to a corner to figure out what happened. Over time, these behaviors insidiously lower your self-esteem and set you on a path of consistent and increasing self-doubt. The sheer intensity of the narcissist causes you to wonder what transgressions you committed to provoke such an outpouring of anger, disdain, or criticism. Sometimes a cold, unmoving stare from him communicates a chilling absence of all human feeling and a reflexive desire to run for cover. As your self-esteem withers and your confidence in knowing your reality diminishes, you gradually concede more power and control to the NPD person.

The cruelty of unrequited love isn’t really that we haven’t been loved back, rather that our hopes have been aroused by someone who can never disappoint us, someone whom we will have to keep believing in because we lack the knowledge that would set us free. In a position of longing for a new person when we are constrained within an existing relationship, we must beware too of the ‘incumbent problem’: the vast but often overlooked and unfair advantage that all new people, and also cities and jobs, have over existing – or, as we put it, incumbent – ones. The beautiful person glimpsed briefly in the street, the city visited for a few days, the job we read about in a couple of tantalizing paragraphs in a magazine all tend to seem immediately and definitively superior to our current partner, our long-established home and our committed workplace and can inspire us to sudden and (in retrospect sometimes) regrettable divorces, relocations and resignations. When we spot apparent perfection, we tend to blame our spectacular bad luck for the mediocrity of our lives, without realizing that we are mistaking an asymmetry of knowledge for an asymmetry of quality: we are failing to see that our partner, home and job are not especially awful, but rather that we know them especially well. The corrective to insufficient knowledge is experience. We need to mine the secret reality of other people and places and so learn that, beneath their charms, they will almost invariably be essentially ‘normal’ in nature: that is, no worse yet no better than the incumbents we already understand.

First, I am thrilled that paramedics are finally getting the respect they deserve for being the professionals they can be. The scope of practice is expanding, and patient care modalities are improving, seemingly by the minute. Patient outcomes are also improving as a result, and EMS is passing through puberty and forging into adulthood. On the other hand, autonomy in the hands of the “lesser-motivated,” can be a very dangerous thing. You know as well as I do that there are still plenty of providers who operate from a subjective, complacent, and downright lazy place. Combined with the ever-expanding autonomy, that provider just became more dangerous than he or she ever has been – to the patients and to you. Autonomy in patient care places more pressure for excellence on the provider charged with delivering it, and also on the partner and crew members on scene. Since the base hospital is not involved like it once was, they are likewise less responsible for the errors and omissions of the medics on the scene. Now more than ever, crew members are being held to answer for the mistakes and follies of their coworkers; now more than ever, EMS providers are working without a net. What’s next? I predict (and hope) emergency medical Darwinism is going to force some painful and necessary changes. First, increasing autonomy is going to result in the better and best providing superior patient care. More personal ownership of the results is going to manifest in outcomes such as increased cardiac arrest survival rates, faster and more complete stroke recovery, and significantly better outcomes for STEMI patients, all leading to the brass ring: EMS as a profession, not just a job. On the flip side of that coin, you will see consequences for the not-so-good and completely awful providers. There will be higher instances of licensure action, internal discipline, and wash-out. Unfortunately, all those things will stem from generally preventable negative patient outcomes. The danger for the better provider will be in the penumbra; the murky, gray area of time when providers are self-categorizing. Specifically, the better provider who is aware of the dangerously poor provider but does nothing to fix or flush him or her, is almost certain to be caught up in a bad situation caused by sloppy, complacent, or ultimately negligent patient care that should have been corrected or stopped. The answer is as simple as it is difficult. If you are reading this, it is more likely because you are one of the better, more committed, more professional providers. This transition is up to you. You must dig deep and find the strength necessary to face the issue and force the change; you have to demand more from yourself and from those around you. You must have the willingness to help those providers who want it – and respond to those who need it, but don’t want it – with tough love by showing them the door. In the end, EMS will only ever be as good as you make it. If you lay silent through its evolution, you forfeit the right to complain when it crumbles around you.

I know from experience, no one wants to spend time with “drunk Annie,” who can’t stop talking or laughing loudly at her own jokes. You may feel that a little alcohol is good for your conversation skills or your golf game. The problem with alcohol is that once you start drinking you can’t judge the point where a little is good and a lot becomes a disaster. When you are making a fool of yourself, or when your conversation skills wane, you remain unaware. Even if you could gauge the exact amount to drink, booze doesn’t make you cleverer, funnier, more creative, or more interesting. There is nothing inherent in alcohol that can do this. More often when a shy person gets drunk, they end up emotional, weepy, and repetitive. We don’t realize how bad we look when drinking because we are drunk and so is everyone else. It’s the old question: If everyone jumped off a cliff, would you? With alcohol, as a culture, our answer is disturbing—yes.
