In airplane crashes and chemical industry accidents, in the infrequent but serious nuclear plant accidents, in the NASA Challenger and Columbia disasters, and in the British Petroleum gulf spill, a common finding is that lower-ranking employees had information that would have prevented or lessened the consequences of the accident, but either it was not passed up to higher levels, or it was ignored, or it was overridden. When I talk to senior managers, they always assure me that they are open, that they want to hear from their subordinates, and that they take the information seriously. However, when I talk to the subordinates in those same organizations, they tell me either they do not feel safe bringing bad news to their bosses or they’ve tried but never got any response or even acknowledgment, so they concluded that their input wasn’t welcome and gave up. Shockingly often, they settled for risky alternatives rather than upset their bosses with potentially bad news. When I look at what goes on in hospitals, in operating rooms, and in the health care system generally, I find the same problems of communication exist and that patients frequently pay the price. Nurses and technicians do not feel safe bringing negative information to doctors or correcting a doctor who is about to make a mistake. Doctors will argue that if the others were “professionals” they would speak up, but in many a hospital the nurses will tell you that doctors feel free to yell at nurses in a punishing way, which creates a climate where nurses will certainly not speak up. Doctors engage patients in one-way conversations in which they ask only enough questions to make a diagnosis and sometimes make misdiagnoses because they don’t ask enough questions before they begin to tell patients what they should do.

When problems of transference are involved, as they usually are, psychotherapy is, among other things, a process of map-revising. Patients come to therapy because their maps are clearly not working. But how they may cling to them and fight the process every step of the way! Frequently their need to cling to their maps and fight against losing them is so great that therapy becomes impossible, as it did in the case of the computer technician. Initially he requested a Saturday appointment. After three sessions he stopped coming because he took a job doing lawn-maintenance work on Saturdays and Sundays. I offered him a Thursday-evening appointment. He came for two sessions and then stopped because he was doing overtime work at the plant. I then rearranged my schedule so I could see him on Monday evenings, when, he had said, overtime work was unlikely. After two more sessions, however, he stopped coming because Monday-night overtime work seemed to have picked up. I confronted him with the impossibility of doing therapy under these circumstances. He admitted that he was not required to accept overtime work. He stated, however, that he needed the money and that the work was more important to him than therapy. He stipulated that he could see me only on those Monday evenings when there was no overtime work to be done and that he would call me at four o’clock every Monday afternoon to tell me if he could keep his appointment that evening. I told him that these conditions were not acceptable to me, that I was unwilling to set aside my plans every Monday evening on the chance that he might be able to come to his sessions. He felt that I was being unreasonably rigid, that I had no concern for his needs, that I was interested only in my own time and clearly cared nothing for him, and that therefore I could not be trusted. We got pregnant with Angel almost by accident. I was thinking it was just about time to go on birth control and wham-it happened. We wanted two children, but were thinking of spacing them out a little more. God and Angel had other plans. I’m so glad. Bubba and Angel are so close in age and such good friends that I can’t imagine it any other way. But at the time, I was more than a little apprehensive about it. Once again, it worked out that Chris was preparing to leave just when I was due. They say God only gives you what you can handle. Chris didn’t cope with crying babies very well. So either he paid the military to deploy him with each baby, or God was looking out for him with well-timed, newborn-avoiding deployments. This time, the Team guy karma worked: the sonogram technician confirmed it was a girl several months into the pregnancy. She was going to be the first female born into the Kyle side of the family in eighty years. Which made her unique, and her grandparents particularly tickled. Chris couldn’t resist the opportunity to tease them with the news. “We’re having a boy,” he said when he called them back in Texas with the news. “Oh, how nice,” they said. “No, we’re having a girl.” “Whoo-hoo!” they shouted. “No, we’re having a boy.” “Chris! Which is it!?” “A girl!” If they could have gotten away to visit us that night, I doubt they would have needed an airplane to fly.

Because we don't know, do we? Everyone knows . . . How what happens the way it does? What underlies the anarchy of the train of events, the uncertainties, the mishaps, the disunity, the shocking irregularities that define human affairs? Nobody knows, Professor Roux. "Everyone knows" is the invocation of the cliche and the beginning of the banalization of experience, and it's the solemnity and the sense of authority that people have in voicing the cliché that's so insufferable. All that we don't know is astonishing. Even more astonishing is what passes for knowing. As the audience filed back in, I began, cartoonishly, to envisage the fatal malady that, without anyone's recognizing it, was working away inside us, within each and every one of us: to visualize the blood vessels occluding under the baseball caps, the malignancies growing beneath the permed white hair, the organs misfiring, atrophying, shutting down, the hundreds of billions of murderous cells surreptitiously marching this entire audience toward the improbable disaster ahead. I couldn't stop myself. The stupendous decimation that is death sweeping us all away. Orchestra, audience, conductor, technicians, swallows, wrens—think of the numbers for Tanglewood alone just between now and the year 4000. Then multiply that times everything. The ceaseless perishing. What an idea! What maniac conceived it? And yet what a lovely day it is today, a gift of a day, a perfect day lacking nothing in a Massachusetts vacation spot that is itself as harmless and pretty as any on earth.

The Memory Business Steven Sasson is a tall man with a lantern jaw. In 1973, he was a freshly minted graduate of the Rensselaer Polytechnic Institute. His degree in electrical engineering led to a job with Kodak’s Apparatus Division research lab, where, a few months into his employment, Sasson’s supervisor, Gareth Lloyd, approached him with a “small” request. Fairchild Semiconductor had just invented the first “charge-coupled device” (or CCD)—an easy way to move an electronic charge around a transistor—and Kodak needed to know if these devices could be used for imaging.4 Could they ever. By 1975, working with a small team of talented technicians, Sasson used CCDs to create the world’s first digital still camera and digital recording device. Looking, as Fast Company once explained, “like a ’70s Polaroid crossed with a Speak-and-Spell,”5 the camera was the size of a toaster, weighed in at 8.5 pounds, had a resolution of 0.01 megapixel, and took up to thirty black-and-white digital images—a number chosen because it fell between twenty-four and thirty-six and was thus in alignment with the exposures available in Kodak’s roll film. It also stored shots on the only permanent storage device available back then—a cassette tape. Still, it was an astounding achievement and an incredible learning experience. I thought people would ask me questions about the technology: How’d you do this? How’d you make that work? I didn’t get any of that. They asked me when it was going to be ready for prime time? When is it going to be realistic to use this? Why would anybody want to look at their pictures on an electronic screen?”6 In 1996, twenty years after this meeting took place, Kodak had 140,000 employees and a $28 billion market cap. They were effectively a category monopoly. In the United States, they controlled 90 percent of the film market and 85 percent of the camera market.7 But they had forgotten their business model. Kodak had started out in the chemistry and paper goods business, for sure, but they came to dominance by being in the convenience business. Even that doesn’t go far enough. There is still the question of what exactly Kodak was making more convenient. Was it just photography? Not even close. Photography was simply the medium of expression—but what was being expressed? The “Kodak Moment,” of course—our desire to document our lives, to capture the fleeting, to record the ephemeral. Kodak was in the business of recording memories. And what made recording memories more convenient than a digital camera? But that wasn’t how the Kodak Corporation of the late twentieth century saw it. They thought that the digital camera would undercut their chemical business and photographic paper business, essentially forcing the company into competing against itself. So they buried the technology. Nor did the executives understand how a low-resolution 0.01 megapixel image camera could hop on an exponential growth curve and eventually provide high-resolution images. So they ignored it. Instead of using their weighty position to corner the market, they were instead cornered by the market.
