I was diagnosed with ADHD in my mid fifties and I was given Ritalin and Dexedrine. These are stimulant medications. They elevate the level of a chemical called dopamine in the brain. And dopamine is the motivation chemical, so when you are more motivated you pay attention. Your mind won't be all over the place. So we elevate dopamine levels with stimulant drugs like Ritalin, Aderall, Dexedrine and so on. But what else elevates Dopamine levels? Well, all other stimulants do. What other stimulants? Cocaine, crystal meth, caffeine, nicotine, which is to say that a significant minority of people that use stimulants, illicit stimulants, you know what they are actually doing? They're self-medicating their ADHD or their depression or their anxiety. So on one level (and we have to go deeper that that), but on one level addictions are about self-medications. If you look at alcoholics in one study, 40% of male adult alcoholics met the diagnostic criteria for ADHD? Why? Because alcohol soothes the hyperactive brain. Cannabis does the same thing. And in studies of stimulant addicts, about 30% had ADHD prior to their drug use. What else do people self-medicate? Someone mentioned depression. So, if you have been treated for depression, as I have been, and you were given a SSRI medication, these medications elevate the level of another brain chemical called serotonin, which is implicated in mood regulation. What else elevates serotonin levels temporarily in the brain? Cocaine does. People use cocaine to self-medicate depression. People use alcohol, cannabis and opiates to self-medicate anxiety. Incidentally people also use gambling or shopping to self-medicate because these activities also elevate dopamine levels in the brain. There is no difference between one addiction and the other. They're just different targets, but the brain systems that are involved and the target chemicals are the same, no matter what the addiction. So people self-medicate anxiety, depression. People self-medicate bipolar disorder with alcohol. People self-medicate Post-Traumatic-Stress-Disorder. So, one way to understand addictions is that they're self-medicating. And that's important to understand because if you are working with people who are addicted it is really important to know what's going on in their lives and why are they doing this. So apart from the level of comfort and pain relief, there's usually something diagnosible that's there at the same time. And you have to pay attention to that. At least you have to talk about it.

I did not doubt she would give me the aspirin; but I knew she would also do something else. I knew she would take me to the dentist next morning. I could not get what I wanted out of her without getting something more, which I did not want. I wanted immediate relief from pain; but I could not get it without having my teeth set permanently right. We want them to be like us, but they are not. The homeless do not want our pity, nor do they deserve our scorn. Our judgments about them, for good or for ill, negate their right to live as they please. Both the urge to rescue and the need to condemn fail to take into account the concept of their personal liberty, which they may exercise as they see fit as long as their actions fall within the law. The homeless are not lesser mortals. For Terrence and Felix, their battles were within and their victories hard-won. I think of these two men as soldiers of the poor, part of an army of the disaffiliated. The homeless have established a nation within a nation, but we are not at war. Why should we not coexist in peace when we may be in greater need of salvation than they? This is what the homeless long for: respect, freedom from hunger, shelter from the elements, safety, the companionship of the like-minded. They want to live without fear. They want to enjoy the probity of the open air without the risk of bodily harm. They want to be warm. They want the comfort of a clean bed when they are ill, relief from pain, a hand offered in friendship. Ordinary conversation. Simple needs. Why are their choices so hard for us to accept? What you see before you is their home. This is their dwelling place. This grass, this sunlight, these palms, this mighty ocean, the moon, the stars, the clouds overhead though they sometimes harbor rain. Under this canopy they have staked out a life for themselves. For Terrence and for Felix, this is also the wide bridge over which they passed from life into death. Their graves will be unmarked but that does not mean they are forgotten. She said Dad was as silent, those next moments, as he was incandescent; he made no sound except a strange whistling, which turned out, of course, to be the broom handle, en route to any number of painful destinations. What was odd, she said, was how the boys weren’t even up to the job of running away—Tommy went screeching to his knees before the first blow landed, and Israel prostrated himself and moaned as though the devil had hold of his liver. The two of them just lost their minds, Dolly said, while her own reaction was nearly as insensible; she suddenly could not stop laughing. Here was Dad, his face still lit though now even the flashlight had gone out, smiling (Dolly said) though his eyes looked terribly melancholy, whacking Finch and Basca every second or two while the pair of them shrieked in no English you’d recognize—Dolly said the laughter just flooded through her and came not only from relief, as you might surmise, but from a reckless and holy sort of joy she had never felt before, not even while cheerleading.

The Last Words of Jesus Some of the most profound lessons of the scriptures are contained in the brief but poignant words Jesus uttered from the cross. There is a sermon in each, but let us focus on Jesus’ words when he said, “It is finished” (John 19:30). I see a number of meanings in these words. He had finished the bitter cup, the cup of trembling, right down to the last taste of vinegar on his lips, offered mockingly in response to his request “I thirst” (John 19:28). He had completed his mission, his Father’s will; the grand moment in the Father’s plan of mercy and happiness had been accomplished. But that grand moment also included his suffering. No other throughout the long ages of man has ever—nor will ever—suffer to the degree he did, yet there came a time in that suffering when he could say, “It is finished.” It was finished, and that will likewise be true of each human soul. No matter what we have suffered, are now suffering, or may yet suffer, there will come a time when we will all echo his words: “It is finished!” Our tears will be changed, and though they will continue to flow, they will be tears of relief, tears of gratitude, tears of joy. The former things will have passed away. That passing away will be so complete and the rejoicing so full that we will not remember, save for the lessons we have learned, our past pain. Isaiah, who was specifically enjoined to speak words of comfort to the people, recorded the Lord’s words: “For, behold, I create new heavens and a new earth: and the former shall not be remembered, nor come into mind. But be ye glad and rejoice” (Isaiah 65:17–18).

Doing the Substance now is like attending Black Mass but you still can’t stop, even though the Substance no longer gets you high. You are, as they say, Finished. You cannot get drunk and you cannot get sober; you cannot get high and you cannot get straight. You are behind bars; you are in a cage and can see only bars in every direction. You are in the kind of a hell of a mess that either ends lives or turns them around. Unfortunately, Evie had been conditioned by too many encounters with Uncle Peregrine to discern between angry gestures and the beginnings of a physical attack. She flinched instinctively, her own arms flying up to shield her head. When the expected pain of a blow did not come, she let out a breath and tentatively lowered her arms to find Sebastian staring at her with blank astonishment. Then his face went dark. “Evie,” he said, his voice containing a bladelike ferocity that frightened her. “Did you think I was about to…Christ. Someone hit you. Someone hit you in the past—who the hell was it?” He reached for her suddenly—too suddenly—and she stumbled backward, coming up hard against the wall. I would never harm you. You know that, don’t you?” Transfixed by the light, glittering eyes that held hers with such intensity, Evie couldn’t move or make a sound. She started as he approached her slowly. “It’s all right,” he murmured. “Let me come to you. It’s all right. Easy.” One of his arms slid around her, while he used his free hand to smooth her hair, and then she was breathing, sighing, as relief flowed through her. Sebastian brought her closer against him, his mouth brushing her temple. “Who was it?” he asked. “M-my uncle,” she managed to say. The motion of his hand on her back paused as he heard her stammer. “Maybrick?” he asked patiently. “No, th-the other one.” “Stubbins.” “Yes.” Evie closed her eyes in pleasure as his other arm slid around her. Clasped against Sebastian’s hard chest, with her cheek tucked against his shoulder, she inhaled the scent of clean male skin, and the subtle touch of sandalwood cologne. “How often?” she heard him ask. “More than once?” “I…i-it’s not important now.” “How often, Evie?” Realizing that he was going to persist until she answered, Evie muttered, “Not t-terribly often, but…sometimes when I displeased him, or Aunt Fl-Florence, he would lose his temper. The l-last time I tr-tried to run away, he blackened my eye and spl-split my lip.” “Did he?” Sebastian was silent for a long moment, and then he spoke with chilling softness. “I’m going to tear him limb from limb.” “I don’t want that,” Evie said earnestly. “I-I just want to be safe from him. From all of them.” Sebastian drew his head back to look down into her flushed face. “You are safe,” he said in a low voice. He lifted one of his hands to her face, caressing the plane of her cheekbone, letting his fingertip follow the trail of pale golden freckles across the bridge of her nose. As her lashes fluttered downward, he stroked the slender arcs of her brows, and cradled the side of her face in his palm. “Evie,” he murmured. “I swear on my life, you will never feel pain from my hands. I may prove a devil of a husband in every other regard…but I wouldn’t hurt you that way. You must believe that.

Unfortunately, sitting rests the parts of the body that don’t need much of it while working the parts that desperately do. Specifically, it disengages the lower extremities while utilizing the spine. (This is in sharp contrast to squatting, which disengages the spine while utilizing the lower extremities.) Because sitting positions the spine vertically, it provides no rest or relief from the gravitational forces that compress it. Without a periodic therapeutic reprieve through the day, the relentless load overwhelms the entire structure, joints and muscles alike. To maintain an erect seated posture, some muscle groups in the back have to continually contract. Since this requires a great deal of energy, the muscles quickly become fatigued. (That is why slumping is more comfortable: It takes less energy to maintain.) When the muscles tire, you rely on the backrest more and your muscles less. The less you rely on your muscles, the weaker and more dysfunctional they become. The weaker and more dysfunctional they become, the more you rely on the backrest. The more you rely on the backrest, the more you tend to slump. The more you slump, the more pronounced the debilitating C-shaped curvature becomes. This weakens the muscles in your back even further, which causes them to overload the joints they serve. Sitting in chairs affects even the areas seemingly at rest (particularly the hips and knees). Because sitting keeps the joints static for long periods, the muscles that serve them become fixed in a short, tight position. When at last you do get up and move, the muscles impose more stress on these joints, thereby increasing their susceptibility to wear and tear. The prolonged stasis also prevents the joints from being lubricated with nourishing synovial fluid. Once depleted, the hips and knees, like the spine, deteriorate and erode. Is it any wonder that the areas most traumatized by sitting, namely, the lower back, hips, and knees, are also the most arthritic and disabled areas of the body in the world today? The real mystery is why so few people have made the connection between prolonged sitting and the epidemic of chronic pain. In fact, they need only look to their own bodies for an abundance of evidence.

Maybe she wasn’t a nice girl but she was smart. That was easy to see. She was watching Lockie with his dad and Pete could see her body sag with relief. She was relieved to get him home. It must have been a promise she had made the boy. Pete had no idea how she’d got him home. She didn’t look like she had a cent to her name. He sighed. So many questions to answer and the worst part was that some of the answers would be things he did not want to hear. Some of the answers would keep him up at night for the rest of his life. He wished he didn’t have to know, but he figured that if Lockie had been through it his family should know about it. If Lockie had been one of the small skeletons buried in the yard in Sydney they would have only been able to imagine what he had suffered. Now they would know. Which way was better? Pete thought about all the other parents who were waiting for the results of tests from the police. For a moment he let go of what needed to be done and what was to come and he offered up a prayer of thanks. Then he offered up a prayer for strength for all those other parents who would never again get to feel their kid’s arms around their neck. And then he wiped his eyes because he was a grown man and a cop and he really shouldn’t be standing in the driveway crying.

For physical issues, we have an entire pharmacopoeia of pain medicine. For the actual pain of grief, we have . . . nothing. It’s always seemed so bizarre to me that we have an answer for almost every physical pain, but for this—some of the most intense pain we can experience—there is no medicine. You’re just supposed to feel it. And in a way, that’s true. The answer to pain is simply to feel it. Some traditions speak of practicing compassion in the face of pain, rather than trying to fix it. As I understand the Buddhist teaching, the fourth form of compassion in the Brahma Viharas, or the four immeasurables, describes an approach to the kinds of pain that cannot be fixed: upekkha, or equanimity. Upekkha is the practice of staying emotionally open and bearing witness to the pain while dwelling in equanimity around one’s limited ability to effect change. This form of compassion—for self, for others—is about remaining calm enough to feel everything, to remain calm while feeling everything, knowing that it can’t be changed. Equanimity (upekkha) is said to be the hardest form of compassion to teach, and the hardest to practice. It’s not, as is commonly understood, equanimity in the way of being unaffected by what’s happened, but more a quality of clear, calm attention in the face of immoveable truth. When something cannot be changed, the “enlightened” response is to pay attention. To feel it. To turn toward it and say, “I see you.” That’s the big secret of grief: the answer to the pain is in the pain. Or, as e. e. cummings wrote, healing of the wound is to be sought in the blood of the wound itself. It seems too intangible to be of use, but by allowing your pain to exist, you change it somehow. There’s power in witnessing your own pain. The challenge is to stay present in your heart, to your heart, to your own deep self, even, and especially, when that self is broken. Pain wants to be heard. It deserves to be heard. Denying or minimizing the reality of pain makes it worse. Telling the truth about the immensity of your pain—which is another way of paying attention—makes things different, if not better. It’s important to find those places where your grief gets to be as bad as it is, where it gets to suck as much as it does. Let your pain stretch out. Take up all the space it needs. When so many others tell you that your grief has to be cleaned up or contained, hearing that there is enough room for your pain to spread out, to unfurl—it’s healing. It’s a relief. The more you open to your pain, the more you can just be with it, the more you can give yourself the tenderness and care you need to survive this. Your pain needs space. Room to unfold. I think this is why we seek out natural landscapes that are larger than us. Not just in grief, but often in grief. The expanding horizon line, the sense of limitless space, a landscape wide and deep and vast enough to hold what is—we need those places. Sometimes grief like yours cannot be held by the universe itself. True. Sometimes grief needs more than an endless galaxy. Maybe your pain could wrap around the axle of the universe several times. Only the stars are large enough to take it on. With enough room to breathe, to expand, to be itself, pain softens. No longer confined and cramped, it can stop thrashing at the bars of its cage, can stop defending itself against its right to exist. There isn’t anything you need to do with your pain. Nothing you need to do about your pain. It simply is. Give it your attention, your care. Find ways to let it stretch out, let it exist. Tend to yourself inside it. That’s so different from trying to get yourself out of it. The way to come to pain is with open eyes, and an open heart, committed to bearing witness to your own broken place. It won’t fix anything. And it changes everything.

I want to end here with the most common and least understood sexual problem. So ordinary is this problem, so likely are you to suffer from it, that it usually goes unnoticed. It doesn't even have a name. The writer Robertson Davies dubs it acedia. “Acedia” used to be reckoned a sin, one of the seven deadly sins, in fact. Medieval theologians translated it as “sloth,” but it is not physical torpor that makes acedia so deadly. It is the torpor of the soul, the indifference that creeps up on us as we age and grow accustomed to those we love, that poisons so much of adult life. As we fight our way out of the problems of adolescence and early adulthood, we often notice that the defeats and setbacks that troubled us in our youth are no longer as agonizing. This comes as welcome relief, but it has a cost. Whatever buffers us from the turmoil and pain of loss also buffers us from feeling joy. It is easy to mistake the indifference that creeps over us with age and experience for the growth of wisdom. Indifference is not wisdom. It is acedia. The symptom of this condition that concerns me is the waning of sexual attraction that so commonly comes between lovers once they settle down with each other. The sad fact is that the passionate attraction that so consumed them when they first courted dies down as they get to know each other well. In time, it becomes an ember; often, an ash. Within a few years, the sexual passion goes out of most marriages, and many partners start to look elsewhere to rekindle this joyous side of life. This is easy to do with a new lover, but acedia will not be denied, and the whole cycle happens again. This is the stuff of much of modern divorce, and this is the sexual disorder you are most likely to experience call it a disorder because it meets the defining criterion of a disorder: like transsexuality or S-M or impotence, it grossly impairs sexual, affectionate relations between two people who used to have them. Researchers and therapists have not seen fit to mount an attack on acedia. You will find it in no one’s nosology, on no foundation's priority list of problems to solve, in no government mental health budget. It is consigned to the innards of women's magazines and to trashy “how to keep your man” paperbacks. Acedia is looked upon with acceptance and indifference by those who might actually discover how it works and how to cure it. It is acedia I wish to single out as the most painful, the most costly, the most mysterious, and the least understood of the sexual disorders. And therefore the most urgent.

Why are you in the hallway?” My bewildered gaze swept down his body to his bare feet. “They’re . . . they’re letting you walk around like this?” Jack shook his head. “They parked me in a room around the corner to wait for a couple more tests. I asked if anyone had told you I was okay, and nobody knew for sure.
