Freud was fascinated with depression and focused on the issue that we began with—why is it that most of us can have occasional terrible experiences, feel depressed, and then recover, while a few of us collapse into major depression (melancholia)? In his classic essay “Mourning and Melancholia” (1917), Freud began with what the two have in common. In both cases, he felt, there is the loss of a love object. (In Freudian terms, such an “object” is usually a person, but can also be a goal or an ideal.) In Freud’s formulation, in every loving relationship there is ambivalence, mixed feelings—elements of hatred as well as love. In the case of a small, reactive depression—mourning—you are able to deal with those mixed feelings in a healthy manner: you lose, you grieve, and then you recover. In the case of a major melancholic depression, you have become obsessed with the ambivalence—the simultaneity, the irreconcilable nature of the intense love alongside the intense hatred. Melancholia—a major depression—Freud theorized, is the internal conflict generated by this ambivalence. This can begin to explain the intensity of grief experienced in a major depression. If you are obsessed with the intensely mixed feelings, you grieve doubly after a loss—for your loss of the loved individual and for the loss of any chance now to ever resolve the difficulties. “If only I had said the things I needed to, if only we could have worked things out”—for all of time, you have lost the chance to purge yourself of the ambivalence. For the rest of your life, you will be reaching for the door to let you into a place of pure, unsullied love, and you can never reach that door. It also explains the intensity of the guilt often experienced in major depression. If you truly harbored intense anger toward the person along with love, in the aftermath of your loss there must be some facet of you that is celebrating, alongside the grieving. “He’s gone; that’s terrible but…thank god, I can finally live, I can finally grow up, no more of this or that.” Inevitably, a metaphorical instant later, there must come a paralyzing belief that you have become a horrible monster to feel any sense of relief or pleasure at a time like this. Incapacitating guilt. This theory also explains the tendency of major depressives in such circumstances to, oddly, begin to take on some of the traits of the lost loved/hated one—and not just any traits, but invariably the ones that the survivor found most irritating. Psychodynamically, this is wonderfully logical. By taking on a trait, you are being loyal to your lost, beloved opponent. By picking an irritating trait, you are still trying to convince the world you were right to be irritated—you see how you hate it when I do it; can you imagine what it was like to have to put up with that for years? And by picking a trait that, most of all, you find irritating, you are not only still trying to score points in your argument with the departed, but you are punishing yourself for arguing as well. Out of the Freudian school of thought has come one of the more apt descriptions of depression—“aggression turned inward.” Suddenly the loss of pleasure, the psychomotor retardation, the impulse to suicide all make sense. As do the elevated glucocorticoid levels. This does not describe someone too lethargic to function; it is more like the actual state of a patient in depression, exhausted from the most draining emotional conflict of his or her life—one going on entirely within. If that doesn’t count as psychologically stressful, I don’t know what does.

I was diagnosed with ADHD in my mid fifties and I was given Ritalin and Dexedrine. These are stimulant medications. They elevate the level of a chemical called dopamine in the brain. And dopamine is the motivation chemical, so when you are more motivated you pay attention. Your mind won't be all over the place. So we elevate dopamine levels with stimulant drugs like Ritalin, Aderall, Dexedrine and so on. But what else elevates Dopamine levels? Well, all other stimulants do. What other stimulants? Cocaine, crystal meth, caffeine, nicotine, which is to say that a significant minority of people that use stimulants, illicit stimulants, you know what they are actually doing? They're self-medicating their ADHD or their depression or their anxiety. So on one level (and we have to go deeper that that), but on one level addictions are about self-medications. If you look at alcoholics in one study, 40% of male adult alcoholics met the diagnostic criteria for ADHD? Why? Because alcohol soothes the hyperactive brain. Cannabis does the same thing. And in studies of stimulant addicts, about 30% had ADHD prior to their drug use. What else do people self-medicate? Someone mentioned depression. So, if you have been treated for depression, as I have been, and you were given a SSRI medication, these medications elevate the level of another brain chemical called serotonin, which is implicated in mood regulation. What else elevates serotonin levels temporarily in the brain? Cocaine does. People use cocaine to self-medicate depression. People use alcohol, cannabis and opiates to self-medicate anxiety. Incidentally people also use gambling or shopping to self-medicate because these activities also elevate dopamine levels in the brain. There is no difference between one addiction and the other. They're just different targets, but the brain systems that are involved and the target chemicals are the same, no matter what the addiction. So people self-medicate anxiety, depression. People self-medicate bipolar disorder with alcohol. People self-medicate Post-Traumatic-Stress-Disorder. So, one way to understand addictions is that they're self-medicating. And that's important to understand because if you are working with people who are addicted it is really important to know what's going on in their lives and why are they doing this. So apart from the level of comfort and pain relief, there's usually something diagnosible that's there at the same time. And you have to pay attention to that. At least you have to talk about it.

The next significant incident would change Steven forever: an apartment building fire. The weather changes, sometimes without warning. Sometimes you can see it coming, much like the triggers a depressed person avoids, and you try to protect the plants before the storm. The intensity of the labor can get frustrating, especially if there is no relief in sight. One day, a tornado or hurricane passes through. Even though you see it on the horizon, you can’t stop it and you may not be able to seek shelter soon enough. The plants are torn from their roots, the garden completely destroyed. You may have thought you could protect it yourself, that the storm wouldn’t be that bad, or you simply didn’t know how or were afraid to ask for help. Your neighbors and family couldn’t help or didn’t know you needed help. The garden is gone. This is the way of depression; if you don’t have it, it’s very difficult to understand this cycle.

Recipients were given bags of coal and clothing, food tickets, and rent vouchers, and storekeepers were forbidden to give them change in cash.

Borderlines experience intense mood swings, ranging from sadness at the loss of the relationship to anger against the other person. The fear of abandonment becomes a reality and it fuels your emotional lability. There may be attempts by them to resolve things but if the relationship is really over, then we’re at stage 6, where the Borderline might spiral downward and experience a bout of severe depression. They may give into their thoughts of low self-worth and even resort to reckless behaviors and self-harming to seek distraction and relief. If the relationship hasn’t ended, the cycle may start all over again. The occurrence of this cycle and its intensity depends on whether or not you are managing your illness by seeking professional help, and if you have other sources of emotional support. I no longer have to worry myself to death about what I did right or wrong to cause a good or bad experience-because there really is no way of knowing. I don't have to look for spiritual lessons in every trouble that comes along. There have been many spiritual lessons to be sure, but they've given to me in the course of life; I haven't had to figure out a single one.

Whether we’re focusing on a task using the TPN or listening to the rambling of the demon, the engine is running at 2,000 RPM. There’s no easy way of shutting off our thoughts, of getting outside the self. In his book The Curse of Self, psychologist Mark Leary of Duke University shows the many downsides of this perpetual self-awareness. He shows that it leads to many forms of suffering, including “depression, anxiety, anger, jealousy, and other negative emotions.” He concludes that self-awareness is “single-handedly responsible for many, if not most of the problems that human beings face as individuals and as a species.” We can summarize this state in a single word: “selfing.” Meditation quiets self-awareness and gives us relief from selfing. In experienced meditators, the “self” parts of the prefrontal cortex go offline. The jargon for this is “hypofrontality.” Hypo is the opposite of hyper, and hypofrontality means the shutting down of the brain’s frontal lobes. The inner critic shuts up. The negative self-talk about “who I am” and “what I do” and “what other people think of me” ceases. We quit selfing. This gives us a sense of identity beyond the suffering self and all the roles it plays. Psychologist Robert Kegan is the former head of adult psychology at Harvard University. He calls the transcendence of selfing the “subject-object shift.” In altered states, we get out of the subjective selves we normally think we are. To be objective, you can’t be the object you’re contemplating. So when the brain enters a state of hypofrontality and we’re no longer enmeshed in the local self, we gain perspective on it. We realize we’re more than that. To realize it’s an object we’re observing, we have to step out of the suffering self. We see the demon from a distance as we step into an identity that is vastly greater than the one we previously inhabited. 2.8. When we make the subject-object shift we escape the limitations of the finite self. Kegan believes that making this jump is the most powerful way to facilitate personal transformation. He says that after it makes the subject-object shift, “the self is more about movement through different states of consciousness than about defending and identifying with any one form.

Maggie, too, was 'difficult': 'her friendships were seldom leisurely or refreshing things,' commented Arthur; Nellie more acerbic, added, 'If Maggie would only have an intimate relationship even with a cat, it would be a relief.' Her Oxford tutors found her 'remorseless.' At age twenty-five, still single, she did not know the facts of life. Over the years, her jealousy of her mother's companion Lucy Tait became more and more pronounced, as did her adoption of her father's expressions of strict disapproval. Her depressions turned to madness and violence, leading to her eventual hospitalization. There is another dramatic narrative, then, of the six children, all differently and profoundly scarred by their home life, which they wrote about and thought about repeatedly. Cross-currents of competition between the children, marked by a desperate need for intimacy, in tension with a restraint born of fear of violent emotion and profound distrust (at best) of sexual feeling, produced a fervid and damaging family dynamic. To put it concisely, we suffer when we resist the noble and irrefutable truth of impermanence and death. We suffer, not because we are basically bad or deserve to be punished, but because of three tragic misunderstandings. First, we expect that what is always changing should be graspable and predictable. We are born with a craving for resolution and security that governs our thoughts, words, and actions. We are like people in a boat that is falling apart, trying to hold on to the water. The dynamic, energetic, and natural flow of the universe is not acceptable to conventional mind. Our prejudices and addictions are patterns that arise from the fear of a fluid world. Because we mistakenly take what is always changing to be permanent, we suffer. Second, we proceed as if we were separate from everything else, as if we were a fixed identity, when our true situation is egoless. We insist on being Someone, with a capital S. We get security from defining ourselves as worthless or worthy, superior or inferior. We waste precious time exaggerating or romanticizing or belittling ourselves with a complacent surety that yes, that’s who we are. We mistake the openness of our being—the inherent wonder and surprise of each moment—for a solid, irrefutable self. Because of this misunderstanding, we suffer. Third, we look for happiness in all the wrong places. The Buddha called this habit “mistaking suffering for happiness,” like a moth flying into the flame. As we know, moths are not the only ones who will destroy themselves in order to find temporary relief. In terms of how we seek happiness, we are all like the alcoholic who drinks to stop the depression that escalates with every drink, or the junkie who shoots up in order to get relief from the suffering that increases with every fix.

SEPTEMBER 11 Fueling Relief When we finally got the clearance to drive through the checkpoints, two weeks after the World Trade Center attacks, the street was lined with New Yorkers—New Yorkers!—waving banners with simple messages. “We love you. You’re our heroes. God bless you. Thank you.” The workers were running on that support as their vehicles ran on fuel. They had so little good news in a day. They faced a mountainously depressing task of removing tons and tons of twisted steel, compacted dirt, smashed equipment, broken glass. But every time they drove past the barricades, they faced a line of fans cheering them on, like the tunnel of cheerleaders that football players run through, reminding them that an entire nation appreciated their service. In a Salvation Army van with lights flashing, we attracted some of the loudest cheers of all. Moises Serrano, the Salvation Army officer leading us, was Incident Director for the city. He had been on the job barely a month when the planes hit. He worked thirty-six straight hours and slept four, forty hours and slept six, forty more hours and slept six. Then he took a day off. His assistant had an emotional breakdown early on, in the same van I was riding in, and may never recover. Many of the Salvationists I met hailed from Florida, the hurricane crews who keep fully stocked canteens and trucks full of basic supplies. When the Manhattan buildings fell, they mobilized all those trucks and drove them to New York. The crew director told me, “To tell you the truth, I came up here expecting to deal with Yankees, if you know what I mean. Instead, it’s all smiles and thank yous.” I came to appreciate the cheerful toughness of the Salvation Army. These soldiers worked in the morgue and served on the front lines. Over the years, though, they had developed an inner strength based on discipline, on community, and above all on a clear vision of whom they were serving. The Salvation Army may have a hierarchy of command, but every soldier knows he or she is performing for an audience of One. West Coast labor unions nervously eyed the tens of thousands of Filipinos who competed with whites for agricultural jobs—since Filipinos were U.S. nationals, no law stopped them from moving to the mainland. Then there was the military situation to consider. Japan had invaded Manchuria in 1931 and seemed poised to advance on Southeast Asia in pursuit of colonies. The Philippines and Guam stood right in its path. Would the United States really go to war over these faraway, barely known, and not-very-profitable possessions? Maybe it wouldn’t have to. Two years into the Depression, Calvin Coolidge noted a “reversal of opinion” about Philippine independence. A number of politicians, FDR included, were coming around on the issue. Rather than absorbing the Philippines’ trade and migrants and defending it against Japan, the new thinking went, why not just get rid of it? The same year that the third great Viking ship found in Norway was excavated, at Oseberg, the town of Ålesund burned. At that time the Viking ships were displayed in makeshift exhibition halls, and the great Ålesund fire hastened the process of building a separate museum for them. The architect Fritz Holland proposed building an enormous crypt for them beneath the royal palace in Oslo. It was to be 63 metres long and 15 metres wide, with a niche for each ship. The walls were to be covered with reliefs of Viking motifs. Drawings exist of this underground hall. It is full of arches and vaults, and everything is made of stone. The ships stand in a kind of depression in the floor. More than anything it resembles a burial chamber, and that is fitting, one might think, both because the three ships were originally graves and because placed in a subterranean crypt beneath the palace gardens they would appear as what they represented: an embodiment of a national myth, in reality relics of a bygone era, alive only in the symbolic realm. The crypt was never built, and the power of history over the construction of national identity has since faded away almost entirely. There is another unrealised drawing of Oslo, from the 1920s, with tall brick buildings like skyscrapers along the main thoroughfare, Karl Johans Gate, and Zeppelins sailing above the city. When I look at these drawings, of a reality that was never realised, and feel the enormous pull they exert, which I am unable to explain, I know that the people living in Kristiania in 1904, as Oslo was called then, would have stared open-mouthed at nearly everything that surrounds us today and which we hardly notice, unable to believe their eyes. What is a stone crypt compared to a telephone that shows living pictures? What is the writing down of Draumkvedet (The Dream Poem), a late-medieval Norwegian visionary ballad, compared to a robot lawnmower that cuts the grass automatically?

We come back to life thinner, paler, weaker … but as survivors. Survivors who don’t get pats on the back from coworkers who congratulate them on making it. A sense of futility and alienation has replaced the delight of simple joys and successes.
