She was surprised to find that the more psychological safety a team felt, the higher its error rates. It appeared that psychological safety could breed complacency. When trust runs deep in a team, people might not feel the need to question their colleagues or double-check their own work. But Edmondson soon recognized a major limitation of the data: the errors were all self-reported. To get an unbiased measure of mistakes, she sent a covert observer into the units. When she analyzed those data, the results flipped: psychologically safe teams reported more errors, but they actually made fewer errors. By freely admitting their mistakes, they were then able to learn what had caused them and eliminate them moving forward. In psychologically unsafe teams, people hid their mishaps to avoid penalties, which made it difficult for anyone to diagnose the root causes and prevent future problems. They kept repeating the same mistakes.

Sometimes it is easier to be angry than to express your own suffering. The Israelis think that they are not Arabs, but they are very similar to the Arabs. They are human beings. They don’t want to die, and they want to live in safety. They want brotherhood, sisterhood, and peace. We are separated by names like “Buddhist,” “Christian,” “Jew,” “Muslim.” When we hear one of these words, we see an image and we feel alienated, we don’t feel connected. We have set up many structures in order to be separated from each other and make each other suffer. That is why it is very important to discover the human being in the other person, and to help the other person discover the human being in us. As human beings we are exactly the same. If you have many layers of garments, you prevent other people from seeing you as a human being.

Psychological safety plays a powerful role in the science of failing well. It allows people to ask for help when they’re in over their heads, which helps eliminate preventable failures. It helps them report—and hence catch and correct—errors to avoid worse outcomes, and it makes it possible to experiment in thoughtful ways to generate new discoveries.

KEY PRINCIPLES Do not collapse on the box; it is only a focal point to reach for. Use the paradoxical breathing technique (inhale as you descend and exhale as you stand). As you become comfortable with the movement and feel your spinal stability improve, you can switch to anatomical breathing (exhale as you descend and inhale as you stand up). Maintaining a neutral spine (slightly arched lower back) along with creased hips is the key to both performance and injury prevention.

What went wrong? Lasko and his team spent ages thinking about risk, but they never shifted their perspective from thinking of the Great Chicago Fire Festival as a unique project to seeing it as “one of those”; that is, part of a wider class of projects. If they had, they would have spent time thinking about live events. How do they fail? One common way is equipment failure. Mics don’t work. Computers crash. How is that risk mitigated? Simple: Identify essential equipment, get backups, and make contingency plans. That kind of analysis is dead easy—but only after you have shifted to the outside view. Notice that risk mitigation does not require predicting the exact circumstances that lead to disaster. Jim Lasko didn’t need to identify when and how the ignition system would fail, only recognize that it could. And have a Plan B if it did. Recall what Benjamin Franklin wrote in 1758: “A little neglect may breed great mischief.” This is why high safety standards are an excellent form of risk mitigation and a must on all projects. They’re not just good for workers; they prevent little things from combining in unpredictable ways into project-smashing black swans.

The need to prevent ferredoxin reacting with oxygen might also explain the propensity of rubisco to fix O2 through the apparently futile process of photorespiration. Very little in evolution is genuinely futile; if it survives natural selection there is usually a reason. In the case of rubisco, think what happens if CO2 levels fall while O2 levels rise inside a leaf (when the stomatal pores are closed). Now rubisco is obliged to slow down because its substrate, CO2, is in short supply. This means that NADPH cannot pass on its electrons to regenerate NADP+. As a result, ferredoxin in turn is unable to pass on its electrons, and so it becomes reactive with oxygen, just when oxygen levels are rising. To stave off catastrophe, rubisco consumes oxygen instead. Photorespiration converts NADPH back to NADP+, allowing ferredoxin to offload its electrons again. So it could be that photorespiration acts as a safety valve, lowering the levels of reactive ferredoxin and oxygen simultaneously, staving off an impending catastrophe.

In the words of David Kilcullen—former special adviser for counterinsurgency in George W. Bush’s administration and chief counterterrorism strategist for the U.S. State Department—the most important thing governments can do is to “remedy grievances and fix problems of governance that create the conditions that extremists exploit.” If America does not change its current course, dangers loom. In the case of the United States, the federal government should renew its commitment to providing for its most vulnerable citizens, white, Black, or brown. We need to undo fifty years of declining social services, invest in safety nets and human capital across racial and religious lines, and prioritize high-quality early education, universal healthcare, and a higher minimum wage. Right now many working-class and middle-class Americans live their lives “one small step from catastrophe,” and that makes them ready recruits for militants. Investing in real political reform and economic security would make it much harder for white nationalists to gain sympathizers and would prevent the rise of a new generation of far-right extremists.

And yet, as we will see, the police don’t actually help. They don’t prevent violence, and they don’t make any measurable contribution to public safety. If anything, what the police are most adept at is eating up billions of dollars in resources that could be repurposed to build truly secure communities. The police have wormed their way into the very foundations of American society and work every day to make themselves—and their bloated budgets—seem indispensable.
