I so clearly remember what it was like, being young and always in the grip of some big fat daydream. I wanted to be a writer always, but more than that, I wanted to have an extraordinary life. I’m sure I dreamed it a million different ways, and that plenty of them were ridiculous, but I think the daydreams were training for writing, and I also think they spurred me to pursue my dreams for real.
Daydreaming, however awesome it is, is passive. It happens in your head. Learning to make dreams real is another matter, and I think it should be the work of your life. Everyone’s life, whatever their dream (unless their dream is to be an axe murderer or something.)
It took me a while to finish a book. Too long. And you know, it doesn’t matter how good a writer you are unless you finish what you start! I think this is the hardest part for most people who want to write. I was in my mid-30s before I figured it out. The brain plays tricks. You can be convinced you’re following your dream, or that you’re going to start tomorrow, and years can pass like that. Years.
The thing is, there will be pressure to adjust your expectations, always shrinking them, shrinking, shrinking, until they fit in your pocket like a folded slip of paper, and you know what happens to folded slips of paper in your pocket. They go through the wash and get ruined. Don’t ever put your dream in your pocket. If you have to put it somewhere, get one of those holsters for your belt, like my dad has for his phone, so you can whip it out at any moment.
Hello there, dream.
Also, don’t be realistic. The word “realistic” is poison. Who decides?
And “backup plan” is code for, “Give up on your dreams,” and everyone I know who put any energy into a backup plan is now living that backup plan instead of their dream. Put all your energy into your dream. That’s the only way it will ever become real.
The world at large has this attitude, “What makes you so special that you think you deserve an extraordinary life?”
Personally, I think the passion for an extraordinary life, and the courage to pursue it, is what makes us special. And I don’t even think of it as an “extraordinary life” anymore so much as simple happiness. It’s rarer than it should be, and I believe it comes from creating a life that fits you perfectly, not taking what’s already there, but making your own from scratch.
You can let life happen to you, or you can happen to life. It’s harder, but so much better.

And outside, behind the houses, all the space usually given over to front and back lawns, would be common too. And there could be vegetable gardens, and fields and woods for the kids to play in. There's be problems about somebody picking the tomatoes somebody else planted, or the roses, or the kids trampling through the pea patch, but the fifty groups or individuals who lived in the houses would have complete charge and complete responsibility for what went on in their little enclave. At the other side of the houses, facing the, would be a little community center. It would have a community laundry -- why does everybody have to own a washing machine?-- and some playrooms and a little cafe and a communal kitchen. All this while in most churches not being allowed to speak with any authority on the direction or doctrine of the church.

When they came out of the “hibiscus tangle” at the back of the beach and “caught sight of the debris washing down the slope,” he writes, “I nearly fell out of the saddle.” The bones that were scattered all along the slope and on the beach below were not pig bones but human bones! More personal ownership of the results is going to manifest in outcomes such as increased cardiac arrest survival rates, faster and more complete stroke recovery, and significantly better outcomes for STEMI patients, all leading to the brass ring: EMS as a profession, not just a job. On the flip side of that coin, you will see consequences for the not-so-good and completely awful providers. There will be higher instances of licensure action, internal discipline, and wash-out. Unfortunately, all those things will stem from generally preventable negative patient outcomes. The danger for the better provider will be in the penumbra; the murky, gray area of time when providers are self-categorizing. Specifically, the better provider who is aware of the dangerously poor provider but does nothing to fix or flush him or her, is almost certain to be caught up in a bad situation caused by sloppy, complacent, or ultimately negligent patient care that should have been corrected or stopped. The answer is as simple as it is difficult. If you are reading this, it is more likely because you are one of the better, more committed, more professional providers. This transition is up to you. You must dig deep and find the strength necessary to face the issue and force the change; you have to demand more from yourself and from those around you. You must have the willingness to help those providers who want it – and respond to those who need it, but don’t want it – with tough love by showing them the door. In the end, EMS will only ever be as good as you make it. If you lay silent through its evolution, you forfeit the right to complain when it crumbles around you.

General Wound Cleaning
Note: A rescuer should wash his or her hands and put on protective gloves and protective eyewear before cleaning an open wound.
All wounds acquired in a wilderness environment should be regarded as contaminated and, therefore, require cleansing to prevent infection and promote healing. There are three effective methods of wound cleaning available to the WFR: You can scrub, irrigate, and debride.
Scrubbing: Disinfectants (such as isopropyl alcohol, povidone-iodine, and hydrogen peroxide) and soaps and detergents should not be put directly into wounds because they can damage viable tissue and may actually increase the incidence of wound infection. These substances may be used to scrub around a wound prior to wound cleaning, with soap and water working as well as anything else.
Irrigating: The most effective and practical method of removing bacteria and debris from a wound involves using a high-pressure irrigation syringe. Irrigation syringes that supply adequate pressure are available commercially in quality first-aid kits. Without an irrigation syringe, you can put water in a plastic bag, punch a pinhole in the bag, and squeeze the water out forcefully, or you can melt a pinhole in the center of the lid of a water bottle with a hot needle and squeeze the water out forcefully. These and other improvised methods are not nearly as effective as an irrigation syringe, but they may be the best you can do. Simply rinsing or soaking a wound is inadequate to remove bacteria. The cleanest water available, most preferably water disinfected for drinking, should be used for irrigating. The tip of the irrigating device should be held 1 to 2 inches above the wound surface, and the plunger of the syringe forcefully depressed. Be sure to tilt the wound to irrigate contaminants out and away from the wound. The volume of irrigation fluid required varies with the size of the wound and the degree of contamination, but plan on using at least a half liter of water.
Note: Wound irrigation is the single most important factor in preventing infection.
