College students were instructed to sit by themselves for up to fifteen minutes in a sparsely furnished, unadorned room and “entertain themselves with their thoughts.” They were allowed to think about whatever they liked, the only rules being that they should remain in their seat and stay awake. Before they entered the room they were obliged to surrender any means of distraction they had about their person, such as cell phones, books, or writing materials. Afterward, they were asked to rate the experience on various scales. Unsurprisingly, a majority reported that they found it difficult to concentrate and their minds had wandered, with around half saying they didn’t enjoy the experience. A subsequent experiment, however, revealed that many found being left alone in an empty room with nothing to occupy their minds so unpleasant (this is, after all, what makes solitary confinement such a harsh punishment in prisons) that they would rather give themselves electric shocks. In the first part of this experiment, the volunteers were asked to rate the unpleasantness of a shock delivered via electrodes attached to their ankle and say whether they would pay a small amount of money to avoid having to experience it again. In the second part, during which they were left alone with their thoughts for fifteen minutes, they were presented with the opportunity to zap themselves once again. Amazingly, among those who had said they would pay to avoid a repeat experience, 67 percent of the men (12 out of 18) and 25 percent of the women (6 out of 24) opted to shock themselves at least once. One of the women gave herself nine electric shocks. One of the men subjected himself to no fewer than 190 shocks, though he was considered exceptional—a statistical “outlier”—and his results were excluded from the final analysis. In their report for the journal Science, the researchers write, “What is striking is that simply being alone with their own thoughts for 15 minutes was apparently so aversive that it drove many participants to self-administer an electric shock that they had earlier said they would pay to avoid.” This goes a long way toward explaining why many people initially find it so hard to meditate, because to sit quietly with your eyes closed is to invite the mind to wander here, there, and everywhere. In a sense, that is the whole point: we are simply learning to notice when this has happened. So the frustrating realization that your thoughts have been straying—yet again—is a sign of progress rather than failure. Only by noticing the way thoughts ricochet about inside our heads like ball bearings in a pinball machine can we learn to observe them dispassionately and simply let them come to rest, resisting the urge to pull back the mental plunger and fire off more of them. All forms of complex causation, and especially nonlinear transformations, admittedly stack the deck against prediction. Linear describes an outcome produced by one or more variables where the effect is additive. Any other interaction is nonlinear. This would include outcomes that involve step functions or phase transitions. The hard sciences routinely describe nonlinear phenomena. Making predictions about them becomes increasingly problematic when multiple variables are involved that have complex interactions. Some simple nonlinear systems can quickly become unpredictable when small variations in their inputs are introduced.23 As so much of the social world is nonlinear, fifty plus years of behavioral research and theory building have not led to any noticeable improvement in our ability to predict events. This is most evident in the case of transformative events like the social-political revolution of the 1960s, the end of the Cold War, and the rise and growing political influence of fundamentalist religious groups. Radical skepticism about prediction of any but the most short-term outcomes is fully warranted. This does not mean that we can throw our hands up in the face of uncertainty, contingency, and unpredictability. In a complex society, individuals, organizations, and states require a high degree of confidence—even if it is misplaced—in the short-term future and a reasonable degree of confidence about the longer term. In its absence they could not commit themselves to decisions, investments, and policies. Like nudging the frame of a pinball machine to influence the path of the ball, we cope with the dilemma of uncertainty by doing what we can to make our expectations of the future self-fulfilling. We seek to control the social and physical worlds not only to make them more predictable but to reduce the likelihood of disruptive and damaging shocks (e.g., floods, epidemics, stock market crashes, foreign attacks). Our fallback strategy is denial.

After any highly stressful event, such as an automobile accident, it is normal for memories, emotions, and sensations associated with the trauma to flood involuntarily into consciousness. In most cases, people replay these memories over and over again, and this "replay" mechanism actually helps defuse their emotional content and allows people to put the experience behind them. This kind of mental processing is healthy and does not lead to long-term problems. But events that are extremely traumatic—being caught in a hurricane, attacked in a war, being the victim of an assault or a rape, or having suffered severe abuse as a child—are not effectively processed by some people. When images or memories of the event return, they are not able to think about them analytically or dispassionately, but instead they reexperience the terror all over again. These intrusive thoughts do not fade with time but are persistent, and each time they occur they are newly traumatizing. Such people are haunted by nightmares, flashbacks, and feelings of anxiety, fear, and foreboding that make them experience the trauma not as a painful event of the past but as a real, in-the-present, on-going threat. As a result, their entire stress-response system, in body and mind, becomes stuck in a state of constant alert, but the state tends to be unstable. Their emotions tend to swing from one extreme to its opposite. To cope with such emotional overload, these people organize their lives around avoiding any reminder of the trauma and the feelings it invokes. It is ultimately a futile struggle, however—like fighting an invisible enemy. The battle for control sets off a vicious cycle of intrusive thoughts that produce fear and anxiety followed by desperate attempts to achieve psychological numbing to reduce the anxiety. They progressively lose the ability to control or modulate their physiological response to any kind of stressor, and stimuli completely unrelated to the trauma may trigger intrusive memories. Lit up like a pinball machine, all their internal bells and whistles blaring, they cannot articulate how they feel because they cannot decipher the messages that their nervous system is sending them. Eventually, just having a feeling, any feeling, can seem enormously threatening.

The ability to understand from your observation and listening what is appropriate behavior or an appropriate response in wildly divergent environments and circumstances. Judgment. The ability to discern between fighting for a principle and surrendering cordially to a matter of taste, and acting appropriately on all occasions.

We make a plan. And so it goes, on into the evening. On days like this, doctoring feels like pinball: nonstop random events—intercepted here, altered there, prolonged or postponed by this or that, the bells and boinks sounding all around—and sometimes you can’t be sure whether you’re the guy pushing the buttons, manipulating the levers, and bumping the machine, or whether you’re inside the machine, whether you’re the pinball itself.
