Two decades ago the federal government invited 150,000 men and women to participate in an experiment of screening for cancer in four organs: prostate, lung, colon, and ovary. The volunteers were less likely to smoke, more likely to exercise, had higher socioeconomic status, and fewer medical problems than members of the general population. Those are the kinds of people who seek preventive intervention. Of course, they are going to do better. Had the study not been randomized, the investigators might have concluded that screening was the best thing since sliced bread. Regardless of which group they were randomly assigned to, the participants had substantially lower death rates than the general population—for all cancers (even those other than prostate, lung, colon, and ovary), for heart disease, and for injury. In other words, the volunteers were healthier than average. With randomization, the study showed that only one of the four screenings (for colon cancer) was beneficial. Without it, the study might have concluded that prostate cancer screening not only lowered the risk of death from prostate cancer but also deaths from leukemia, heart attack, and car accidents (although you would hope someone would raise the biological plausibility criterion here).

Obesity is suicide on lay-away: It has plenty of time to degrade quality of life before finally ending life prematurely.

After its exit from a tiny hole in the base of the skull, it descends in the neck tucked between the carotid artery and jugular vein. If you were to slice it and look at its cut end head-on, you’d see fibers called efferents, because they carry outbound signals to your chest cavity. These are half of the nerve fibers that your brain uses to control your heart rate in times of stress or rest; specifically, these are the fibers which are activated in rest. They are also, unsurprisingly, the pathway that Buddhist monks and others, through years of training, use to lower their heart rate with thought alone. Less well known is that the wandering nerve is a two-way street. It also carries signals (via afferent fibers) back into the skull from your heart and lungs to shower the brain with ascending information signaling the brain to enter a more calm and restful state. And these fibers can be manipulated or even hijacked. How? You can do it yourself through the intense practice of mindful breathing. Meditative breathing can calm the electrical oscillations and stress responses in your mind by resetting your vagal tone. The neuroscience term is “network reset.” So, despite the increasing popularity of inserting catheters and wires into brains, the basic ability to modify your thoughts and feelings is actually already a built-in technology in each of us. I’m not saying that mindful, meditative breathing alone would have necessarily relieved Raymond’s compulsive use of eyedrops; certainly, it would not have helped Nathan Copeland regain feeling in his fingers. But I do say this: never underestimate the power of the human brain.

Suppose a young man is more interested in using his lungs to get high by sniffing glue. What would you think of me if I said, “That's interesting—I guess the purpose of my lungs is to oxygenate my blood, but the purpose of his lungs is to get high?” You'd think me a fool, and rightly so. By sniffing glue, he doesn't change the purpose built into his lungs, he only violates it. We can ascertain the purposes of the other features of our design in the same way. The purpose of the eyes is to see, the purpose of the heart is to pump blood, the purpose of the thumb is to oppose the fingers so as to grasp, the purpose of the capacity for anger is to protect endangered goods, and so on. If we can ascertain the meanings and purposes of all those other powers, there is no reason to think that we cannot ascertain the meanings and purposes of the sexual powers. Natural function and personal meaning are not alien to each other. They are connected. In a rightly ordered way of thinking, they turn out to be different angles of vision on the same thing. The second objection is that it doesn't make any difference even if we can ascertain the meanings or purposes of the sexual powers, because an is does not imply an ought. This dogma too is false. If the purpose of eyes is to see, then eyes that see well are good eyes, and eyes that see poorly are poor ones. Given their purpose, this is what it means for eyes to be good. Moreover, good is to be pursued; the appropriateness of pursuing it is what it means for anything to be good. Therefore, the appropriate thing to do with poor eyes is try to turn them into good ones. If it really were impossible to derive an ought from the is of the human design, then the practice of medicine would make no sense. Neither would the practice of health education. Consider the young glue-sniffer again. How should we advise him? Is the purpose of his lungs irrelevant? Should we say to him, “Sniff all you want, because an is does not imply an ought”? Of course not; we should advise him to kick the habit. We ought to respect the is of our design. Nothing in us should be put into action in a way that flouts its inbuilt meanings and purposes.

Biomedicine locates sickness in a specific place in an individual body: a headache, a stomachache a torn knee, lung cancer. Medical anthropologists instead locate sickness and health in three interconnected bodies: the political, the social, and the physical. The prevailing political economy impacts the distribution of sickness and health in a society and the means available to heal those who are sick. For example, poor individuals worldwide are more exposed to toxins that make them sick, while the rich stay healthier. The social body constructs the meanings and experiences surrounding particular physical states. It determines the ideal physical body, legitimizing biomedical practices like plastic surgery to attain it. The social body also determines the boundaries of the physical body. Some cultures locate sickness not in individuals but instead in families or communities. As any caregiver knows, we live the sickness too. And while biomedicine can cure diseases it flounders with permanent hurts, troubles of the mind, states present from birth or that are incurable or progressive. In biomedicine, these states are stigmatized and feared. We medical anthropologists have a term for this: social death.

Sheriff Thorp intended to question LeRoy regarding a safe blown in the A.C. Gillespie & Son store in Brewster a few days earlier and two residential burglaries in Brewster. A mild mannered Iowa farmer, Thorp came to the Okanogan in 1900, carried mail between Chesaw and Loomis, ran for sheriff. Armed with a six-shooter, Thorp feared only that some day, he might have to kill someone, which would compel him to resign, and this might be the day. LeRoy sat very still, watching the frontier sheriff approach the card table. “I’ll have to take you in, partner.” said Thorp. There must have been an unearthly silence in the saloon as LeRoy rose. Thorp drew his revolver, “I’m going to search you.” LeRoy turned as if to throw off his coat, and then jerked a pistol from a shoulder holster. The two opened fire simultaneously LeRoy dancing about to present an elusive target. LeRoy got off four shots. Thorp emptied his revolver, striking LeRoy’s right hand, causing him to drop his gun, and hitting the suspect in the shoulder as he bolted out a rear door. LeRoy staggered a few yards up Salmon Creek before hiding in some brush. “Look out, he’s got another gun” someone yelled from across the creek. Having borrowed a second revolver, the sheriff pounced, kicking LeRoy’s gun from his hand. LeRoy was rolled onto a piece of barn board and carried into the Elliot Hotel. There his wounds, including a punctured lung were treated. In LeRoy’s hotel room Thorp found two more guns, wedges and drills, and a supply of nitroglycerine. Two days later, LeRoy broke out of the county jail. Wearing only his nightshirt, a blanket for trousers, shoes and an old mackinaw taken from an elderly trusty who served as jailer, the desperado flew through chilling weather to Okanogan. Three days later, Thorp caught up with him in a fleld of sagebrush below Malott. LeRoy came out with his hands up commenting mildly he wished he had a gun so the two could shoot it out again. In January, 1910, at Conconully LeRoy was convicted of burglarizing the William Plemmon’s home at Brewster. Since this was his third burglary conviction, he was sentenced to life imprisonment in the state penitentiary at Walla Walla as a habitual criminal. After serving nine years, LeRoy, in ill health, was released in 1919. He once met Fred Thorp on a street in Spokane. They chatted for a few minutes. While there were, in pioneer times, numerous other confrontations between armed men, the Thorp-LeRoy gun flght probably was the closest Okanogan County ever came to a HIGH NOON shootout.

Watch a person asleep and note his breathing: it is as regular and rhythmic as the swish of the waves on the shore, as regular as the ticking of the clock. That is the natural way; when consciousness stops controlling it, the breathing becomes natural. If, then, you are seeking health and vigor, imitate the natural. Breathing Exercise. Of marvelous value is the deep breathing exercise, which should be taken just as regular as the morning ablution. The best place is somewhere where one can get fresh air, preferably air that the sun has shone on. Then bending back the shoulders, throwing forward the chest and upward the chin, inhale slowly just as much air as the lungs will hold. Hold in this air while you count ten. Exhale it slowly Repeat this four or five times. Then, after a moment’s rest, empty the lungs to the utmost, then draw in all the air possible, and when the lungs seem full, draw in just a little more, pack the lungs, as it were, and hold the breath while you count twenty slowly. Like high blood pressure and diabetes, chronic inflammation has no visible symptoms (though it can be measured by a lab test known as high-sensitivity C-reactive protein [hs CRP]). But it damages the vascular system, the organs, the brain, and body tissues. It slowly erodes your health, gradually overwhelming the body’s anti-inflammatory defenses. It causes heart disease. It causes cognitive decline and memory loss. Even obesity and diabetes are linked to inflammation because fat cells are veritable factories for inflammatory chemicals. In fact, it’s likely that inflammation is the key link between obesity and all the diseases obesity puts you at risk for developing. When your joints are chronically inflamed, degenerative diseases like arthritis are right around the corner. Inflamed lungs cause asthma and other respiratory illnesses. Inflammation in the brain is linked to Alzheimer’s disease and other neurological conditions, including brain fog and everyday memory lapses that we write off as normal aging—except those memory lapses are not an inevitable consequence of aging at all. They are, however, an inevitable consequence of inflammation, because inflammation sets your brain on fire. Those “I forgot where I parked the car” moments start happening more frequently, and occurring prematurely. Inflamed arteries can signal the onset of heart disease. Chronic inflammation has also been linked to various forms of cancer; it triggers harmful changes on a molecular level that result in the growth of cancer cells. Inflammation is so central to the process of aging and breakdown at the cellular level that some health pundits have begun referring to the phenomena as “inflam-aging.” That’s because inflammation accelerates aging, including the visible signs of aging we all see in the skin. In addition to making us sick, chronic inflammation can make permanent weight loss fiendishly difficult. The fat cells keep churning out inflammatory proteins called cytokines, promoting even more inflammation. That inflammation in turn prevents the energy-making structures in the cells, called mitochondria, from doing their jobs efficiently, much like a heat wave would affect the output of a factory that lacks air-conditioning—productivity declines under extreme conditions. One of the duties of the mitochondria is burning fat; inflammation interferes with the job of the mitochondria, making fat burning more difficult and fat loss nearly impossible. While someone trying to lose weight may initially be successful, after a while, the number on the scale gets stuck. The much-discussed weight-loss “plateau” is often a result of this cycle of inflammation and fat storage. And here’s even more bad news: Adding more exercise or eating fewer calories in an attempt to break through the plateau will have some effect on weight loss, but not much. And continuing to lose weight becomes much harder to accomplish. Why? Because inflammation decreases our normal ability to burn calories.
