Conditions that appeared to increase the risk and severity of COVID-19 included type 2 diabetes, obesity, high blood pressure and other heart conditions, chronic obstructive pulmonary disease (COPD), chronic kidney disease and cancer. The heart is essentially a muscular pump connected to an elaborate network of branching tubes. Although there are several kinds of cardiovascular disease, almost all arise from something going wrong in either the tubes or the pump. Most problems start with the tubes, primarily the arteries that carry blood from the heart to every nook and cranny of the body. Like the pipes in a building, arteries are vulnerable to getting clogged with unwanted deposits. This hardening of the arteries, termed atherosclerosis, starts with the buildup of plaque—a gloppy mixture of fat, cholesterol, and calcium—within the walls of arteries. Plaques, however, don’t simply accumulate in arteries like crud settling in a pipe. Instead, they are dynamic, changing, growing, shifting, and sometimes breaking. They develop when white blood cells in arteries trigger inflammation by reacting to damage usually caused by a combination of high blood pressure and so-called bad cholesterol that irritates the walls of the artery. In an effort to repair the damage, white blood cells produce a foamy mixture that incorporates cholesterol and other stuff and then hardens. As plaque accumulates, arteries stiffen and narrow, sometimes preventing enough blood from flowing to the tissues and organs that need it and further driving up blood pressure. One potentially lethal scenario is when plaques block an artery completely or detach and obstruct a smaller artery elsewhere. When this happens, tissues are starved of blood (also called ischemia) and die. Plaques can also cause the artery wall to dilate, weaken, and bulge (an aneurysm) or to tear apart (a rupture), which can lead to massive bleeding (a hemorrhage). Blocked and ruptured arteries create trouble anywhere in the body, but the most vulnerable locations are the narrow coronary arteries that supply the heart muscle itself. Heart attacks, caused by blocked coronary arteries, may damage the heart’s muscle, leading to less effective pumping of blood or triggering an electrical disturbance that can stop the heart altogether. Other highly vulnerable arteries are in the brain, which cause strokes when blocked by blood clots or when they rupture and bleed. To this list of more susceptible locations we should also add the retinas, kidneys, stomach, and intestines. The most extreme consequence of coronary artery disease is a heart attack, which, if one survives, leaves behind a weakened heart unable to pump blood as effectively as before, leading to heart failure.

The Look AHEAD subgroup analyses looked at only 3 possible variables and only one at a time. In the final analysis, Baum et al. discovered that intensive lifestyle modification averted cardiovascular events for two subgroups, patients with HbA1c 6.8% or higher (poorly managed diabetes) and patients with well-controlled diabetes (Hba1c < 6.8%) and good self-reported health. That finding applied to 85% of the entire patient population studied. On the other hand, the remaining 15% who had controlled diabetes but poor self-reported general health responded negatively to the lifestyle modification regimen. The negative and positive responders cancelled each other out in the initial statistical analysis, falsely concluding that lifestyle modification was useless. The Baum et al. re-analysis lends further support to the belief that a one-size-fits-all approach to medicine is inadequate to address all the individualistic responses that patients have to treatment.

One hundred percent of the nearly 1 million pounds of chloroform released in Mobile County in 1988 was released in Africatown. Chloroform is classified as a probable carcinogen by the EPA. Chronic long-term exposure, such as one would experience living in the shadow of two paper mills, can cause liver disease and affect the central nervous system. One hundred percent of the chlorine dioxide in the county was released in Africatown. Nearly all of the 1.8 million pounds of hydrochloric acid released in the county in 1988 was released in Africatown. Nearly all of the acetone, methanol, xylene, chlorine, methyl ethyl ketone, and toluene released in the county were released in Africatown. The chemicals are linked to cancer, birth defects, fertility problems, kidney and liver damage, nose and throat irritation, asthma, and loss of hearing and color vision.

As strange as it sounds, it is no longer possible to determine how many human genomes have been sequenced. At present the strategy of choice is whole-genome re-sequencing (Chapter 3) whereby next-generation sequence data are mapped onto a reference genome. The results have been breathtaking. The recently concluded (and aptly named) 1000 Genomes Project Consortium catalogued ~85 million SNPs, 3.6 million short insertions/deletions, and 60,000 larger structural variants in a global sampling of human genetic diversity. These data are catalysing research in expected and unexpected ways. Beyond providing a rich source of data for GWA-type studies focused on disease, scientists are also using the 1000 Genomes Project data to learn about our basic biology, something that proved surprisingly difficult when only a pair of genomes was available. For example, a recent GWAS taking advantage of the 1000 Genomes Project data identified ten genes associated with kidney development and function, genes that had previously not been linked to this critical aspect of human physiology. In 2016, Craig Venter’s team reported the sequencing of 10,545 human genomes. Beyond the impressively low cost (US$1,000–2,000 per genome) and high quality (30–40× coverage), the study was significant in hinting at the depths of human genome diversity yet to be discovered. More than 150 million genetic variants were identified in both coding and non-coding regions of the genome; each sequenced genome had on average ~8,600 novel variants. Furthermore, each new genome was found to contain 0.7 Mbp of sequence that is not contained in the reference genome. This underscores the need for methods development in the area of structure variation detection in personal genome data. Overall, however, the authors concluded that ‘the data generated by deep genome sequencing is of the quality necessary for clinical use’.

A Protein Is Not a Protein Companies are touting protein as a cure-all and for weight loss/muscle gain. They’re selling protein shakes, protein cookies, protein snack bars, even protein coffee. It’s true that protein is neither carbohydrate nor sugar nor fat, and you need it to maintain normal growth. However, your kidneys have a limited capacity to excrete the metabolic by-products of protein metabolism, and overexcretion can cause kidney damage. Therefore, protein quality is as important as protein quantity. For example, eggs and beans both contain protein, but are very different in quality. Dietary protein is made up of twenty separate amino acids strung together in different combinations and amounts. One of those amino acids, tryptophan, is rarer and therefore more important than others, because it’s the precursor of serotonin, an important brain neurotransmitter (see Chapter 19). Eggs, poultry, and fish are the best sources of this amino acid, while beans have very little. On the other hand, additional protein is needed if you’re building muscle, especially branched-chain amino acids (BCAAs; leucine, isoleucine, valine), which are 20 percent of muscle (see Chapter 18). BCAAs are in high concentration in corn products, and are what’s in those tubs of protein powder at the health food store. If you’re a bodybuilder, you need them; if you’re not a gym rat and consume excess BCAAs, your liver will take the amino groups off and turn them into organic acids, which will either be diverted into liver fat (through DNL) or into excess glucose, either of which can generate hyperinsulinemia and drive chronic disease. The goal is to get more tryptophan and less BCAAs in the protein you consume.
