If my own child were turning tricks on the streets, enslaved not only by the drug but also criminal dealers and pimps, I would want her to have the benefit of maintenance drugs, even if she sometimes misused them or otherwise figured out how to glean a subtle high from the experience. If my child's fear of dopesickness was so outsized that she refused even MAT, I would want her to have access to clean needles that prevented her from getting HIV and/or hepatitis C and potentially spreading them to others.

Fatty liver is a completely reversible process. Emptying the liver of its surplus glucose and dropping insulin levels returns the liver to normal. Hyperinsulinemia drives DNL, which is the primary determinant of fatty liver disease. Normalizing insulin levels reverses the fatty liver. Refined carbohydrates, which cause large increases in insulin, are far more sinister than dietary fat. High carbohydrate intake can increase DNL tenfold, whereas high fat consumption, with correspondingly low carbohydrate intake, does not change hepatic fat production

As a result of its investigation, the NIH said that to qualify for funding, all proposals for research on human subjects had to be approved by review boards—independent bodies made up of professionals and laypeople of diverse races, classes, and backgrounds—to ensure that they met the NIH’s ethics requirements, including detailed informed consent. Scientists said medical research was doomed. In a letter to the editor of Science, one of them warned, “When we are prevented from attempting seemingly innocuous studies of cancer behavior in humans … we may mark 1966 as the year in which all medical progress ceased.” Later that year, a Harvard anesthesiologist named Henry Beecher published a study in the New England Journal of Medicine showing that Southam’s research was only one of hundreds of similarly unethical studies. Beecher published a detailed list of the twenty-two worst offenders, including researchers who’d injected children with hepatitis and others who’d poisoned patients under anesthesia using carbon dioxide. Southam’s study was included as example number 17. Despite scientists’ fears, the ethical crackdown didn’t slow scientific progress. In fact, research flourished. And much of it involved HeLa. 18

DENGUE FEVER (BREAKBONE FEVER) Dengue fever is a viral infection found throughout Central America. In Costa Rica outbreaks involving thousands of people occur every year. Dengue is transmitted by aedes mosquitoes, which often bite during the daytime and are usually found close to human habitations, often indoors. They breed primarily in artificial water containers such as jars, barrels, cans, plastic containers and discarded tires. Dengue is especially common in densely populated, urban environments. Dengue usually causes flulike symptoms including fever, muscle aches, joint pains, headaches, nausea and vomiting, often followed by a rash. Most cases resolve uneventfully in a few days. Severe cases usually occur in children under the age of 15 who are experiencing their second dengue infection. There is no treatment for dengue fever except taking analgesics such as acetaminophen/paracetamol (Tylenol) and drinking plenty of fluids. Severe cases may require hospitalization for intravenous fluids and supportive care. There is no vaccine. The key to prevention is taking insect-protection measures. HEPATITIS A Hepatitis A is the second-most-common travel-related infection (after traveler’s diarrhea). It’s a viral infection of the liver that is usually acquired by ingestion of contaminated water, food or ice, though it may also be acquired by direct contact with infected persons. Symptoms may include fever, malaise, jaundice, nausea, vomiting and abdominal pain. Most cases resolve without complications, though hepatitis A occasionally causes severe liver damage. There is no treatment. The vaccine for hepatitis A is extremely safe and highly effective. You should get vaccinated before you go to Costa Rica. Because the safety of hepatitis A vaccine has not been established for pregnant women or children under the age of two, they should instead be given a gammaglobulin injection. LEISHMANIASIS Leishmaniasis occurs in the mountains and jungles of all Central American countries. The infection is transmitted by sand flies, which are about one-third the size of mosquitoes. Most cases occur in newly cleared forest or areas of secondary growth. The highest incidence is in Puerto Viejo de Talamanca. It causes slow-growing ulcers over exposed parts of the body There is no vaccine. RABIES Rabies is a viral infection of the brain and spinal cord that is almost always fatal. The rabies virus is carried in the saliva of infected animals and is typically transmitted through an animal bite, though contamination of any break in the skin with infected saliva may result in rabies. Rabies occurs in all Central American countries. However, in Costa Rica only two cases have been reported over the last 30 years. TYPHOID Typhoid fever is caused by ingestion of food or water contaminated by a species of salmonella known as Salmonella typhi . Fever occurs in virtually all cases. Other symptoms may include headache, malaise, muscle aches, dizziness, loss of appetite, nausea and abdominal pain. A pretrip vaccination for typoid is recommended, but not required. It’s usually given orally, and is also available as an injection. TRAVELER’S DIARRHEA Tap water is safe and of a high quality in Costa Rica, but when you’re far off the beaten path it’s best to avoid tap water unless it has been boiled, filtered or chemically disinfected (iodine tablets). To prevent diarrhea, be wary of dairy products that might contain unpasteurized milk; and be highly selective when eating food from street vendors.

It impacts the gallbladder by causing contraction and release of stored bile.1 2 3 Milk Thistle (Silybum Marianum) has the ability to increase the solubility of bile and its use has been shown to significantly reduce biliary cholesterol concentrations and bile saturation indexes.4 It has potent antioxidant activity which supports phase I detoxification and prevents the depletion of hepatic glutathione which is important for phase II detoxification.5 6 7 Silybum marianum has anti-inflammatory chemical properties that are inhibitors of inflammatory prostaglandins and leukotriens as well as chemical properties that promote protein synthesis to replace damaged liver cells. 8 9 10 11 Ginger (Zingiber Officinale) contains chemical compounds that have been shown to increase bile secretion and to reduce hepatic cholesterol levels by up-regulating the enzyme cholesterol-7-alpha-hydorxylase which is the rate limiting enzyme in bile acid synthesis.12 13 14

across an unexpected finding: Alcohol consumption was associated with liver inflammation (no surprise there), but coffee consumption was associated with less liver inflammation.81 These results were replicated in subsequent studies performed around the world. In the United States, a study was done with people at high risk for liver disease—for example, those who were overweight or drank too much alcohol. Subjects who drank more than two cups of coffee a day appeared to have less than half the risk of developing chronic liver problems as those who drank less than one cup.82 What about liver cancer, one of the most feared complications of chronic liver inflammation? It is now the third-leading cause of cancer-related death, an upsurge driven largely by increases in hepatitis C infections and

nonalcoholic fatty liver disease.83 The news is good. A 2013 review of the best studies to date found that people who drank the most coffee had half the risk of liver cancer compared to those who drank the least.84 A subsequent study found the consumption of four or more cups of coffee a day was associated with 92 percent lower risk among smokers dying from chronic liver disease.85 Of course, quitting smoking would have helped as well; smoking may multiply by as much as tenfold the odds of those with hepatitis C dying from

Similarly, heavy alcohol drinkers who consume more than four cups of coffee per day appear to reduce their risk of liver inflammation—but not by nearly as much as people who cut down on alcohol.87 Liver cancers are among the most avoidable cancers, through hepatitis B vaccination, control of hepatitis C transmission, and reduction of alcohol consumption. These three measures could, in principle, wipe out 90 percent of liver cancers worldwide. It remains unclear whether coffee drinking has an additional role to

But what if you’re already infected with hepatitis C or are among the nearly one in three American adults89 with nonalcoholic fatty liver disease? Until relatively recently, no clinical trials had put coffee to the test. But in 2013, researchers published a study in which forty patients with chronic hepatitis C were placed into two groups: The first group consumed four cups of coffee daily for a month, while the second group drank no coffee at all. After

The ADA, for instance, calls it a “myth” that sugar causes type 2 diabetes, because that’s caused by “genetics and lifestyle factors” that make us fat—i.e., “calories from any source.” It then proceeds to recommend that we all avoid sugar-sweetened beverages to prevent diabetes, adding that we can “save money” by doing so. The organization accepts the role of fat accumulation in the liver as quite possibly a causal factor in the development of insulin resistance, diabetes, and obesity, but ignores the evidence building steadily since the 1980s that implicates sugars as the cause of that hepatic fat accumulation.

In 2009, a start-up Danish drug company called Santaris announced successful use of LNA’s remarkable properties against the hepatitis C virus. The LNA in the medication bonds with microRNA in the HepC virus, preventing the virus from duplicating. Essentially, the LNA “locks” the gene in the off position.
