Fifteen years ago, I rarely appeared on the outside, though I had considerable influence on the inside; back then, I was what one might call a "recessive personality." My passage from "recessive" to "dominant" is a key part of our story; be patient, you'll learn lots more about me later on. Indeed, since you will meet all eleven personalities who once roamed about, it gets a bit complex in the first half of this book; but don't worry, you don't have to remember them all, and it gets sorted out in the last half of the book. You may be wondering -- if not "Robert," who, then, was the dominant MPD personality back in the 1980s and earlier? His name was "Bob," and his dominance amounted to a long reign, from the early 1960s to the early 1990s. Since "Robert B. Oxnam" was born in 1942, you can see that "Bob" was in command from early to middle adulthood. Although he was the dominant MPD personality for thirty years, Bob did not have a clue that he was afflicted by multiple personality disorder until 1990, the very last year of his dominance. That was the fateful moment when Bob first heard that he had an "angry boy named Tommy" inside of him. How, you might ask, can someone have MPD for half a lifetime without knowing it? And even if he didn't know it, didn't others around him spot it? To outsiders, this is one of the most perplexing aspects of MPD. Multiple personality is an extreme disorder, and yet it can go undetected for decades, by the patient, by family and close friends, even by trained therapists. Part of the explanation is the very nature of the disorder itself: MPD thrives on secrecy because the dissociative individual is repressing a terrible inner secret. The MPD individual becomes so skilled in hiding from himself that he becomes a specialist, often unknowingly, in hiding from others. Part of the explanation is rooted in outside observers: MPD often manifests itself in other behaviors, frequently addiction and emotional outbursts, which are wrongly seen as the "real problem." The fact of the matter is that Bob did not see himself as the dominant personality inside Robert B. Oxnam. Instead, he saw himself as a whole person. In his mind, Bob was merely a nickname for Bob Oxnam, Robert Oxnam, Dr. Robert B. Oxnam, PhD.

It led me back to the inner path, which I have already followed for many lives. I believe the perception of what people think about DID is I might be crazy, unstable, and low functioning. After my diagnosis, I took a risk by sharing my story with a few friends. It was quite upsetting to lose a long term relationship with a friend because she could not accept my diagnosis. But it spurred me to take action. I wanted people to be informed that anyone can have DID and achieve highly functioning lives. I was successful in a career, I was married with children, and very active in numerous activities. I was highly functioning because I could dissociate the trauma from my life through my alters. Essentially, I survived because of DID. That's not to say I didn't fall down along the way. There were long term therapy visits, and plenty of hospitalizations for depression, medication adjustments, and suicide attempts. After a year, it became evident I was truly a patient with the diagnosis of DID from my therapist and psychiatrist. I had two choices. First, I could accept it and make choices about how I was going to deal with it. My therapist told me when faced with DID, a patient can learn to live with the live with the alters and make them part of one's life. Or, perhaps, the patient would like to have the alters integrate into one person, the host, so there are no more alters. Everyone is different. The patient and the therapist need to decide which is best for the patient. Secondly, the other choice was to resist having alters all together and be miserable, stuck in an existence that would continue to be crippling. Most people with DID are cognizant something is not right with themselves even if they are not properly diagnosed. My therapist was trustworthy, honest, and compassionate. Never for a moment did I believe she would steer me in the wrong direction. With her help and guidance, I chose to learn and understand my disorder. It was a turning point.

When, as they inevitably will, these prove inadequate (as did the original environment), the person responds with 'narcissistic rage' and disappointment, which, in the absence of an adequate 'selfobject' cannot be dealt with in a productive way.

I will not be spoken to in a condescending or sarcastic way. Loving partners will not patronize me. I will not allow my partner to call me jealous, crazy, or any other form of projection. My relationships will be mutual and equal at all times. Love is not about control and power. You will heal and self-repair and sign up for another relationship full of its own ruptures and repairs. It’s not ideal, opening yourself up like this, putting your shield down, but if you want the rewards of an intimate relationship, there’s no way around it.

The case of a patient with dissociative identity disorder follows: Cindy, a 24-year-old woman, was transferred to the psychiatry service to facilitate community placement. Over the years, she had received many different diagnoses, including schizophrenia, borderline personality disorder, schizoaffective disorder, and bipolar disorder. Dissociative identity disorder was her current diagnosis. Cindy had been well until 3 years before admission, when she developed depression, "voices," multiple somatic complaints, periods of amnesia, and wrist cutting. Her family and friends considered her a pathological liar because she would do or say things that she would later deny. Chronic depression and recurrent suicidal behavior led to frequent hospitalizations. Cindy had trials of antipsychotics, antidepressants, mood stabilizers, and anxiolytics, all without benefit. Her condition continued to worsen. Cindy was a petite, neatly groomed woman who cooperated well with the treatment team. She reported having nine distinct alters that ranged in age from 2 to 48 years; two were masculine. Cindy’s main concern was her inability to control the switches among her alters, which made her feel out of control. She reported having been sexually abused by her father as a child and described visual hallucinations of him threatening her with a knife. We were unable to confirm the history of sexual abuse but thought it likely, based on what we knew of her chaotic early home life. Nursing staff observed several episodes in which Cindy switched to a troublesome alter. Her voice would change in inflection and tone, becoming childlike as ]oy, an 8-year-old alter, took control. Arrangements were made for individual psychotherapy and Cindy was discharged. At a follow-up 3 years later, Cindy still had many alters but was functioning better, had fewer switches, and lived independently. She continued to see a therapist weekly and hoped to one day integrate her many alters.

When someone is judging you, it's unlikely that their judgment is actually about you. As I see it, we're all carrying around a bunch of suitcases. We have our insecurities suitcase. We have our stress suitcase. We have our guilt and our worries suitcases. Some suitcases we might have been carrying since our childhood, stories we were told about who we are that aren't even true. They're fiction that we were handed, picked up, believed, and still carry. Sometimes a person comes along with one of their suitcases, with their issues all packed up and ready to go, and they try to hand it to us. Do not pick up that suitcase! Do not pick it up! Because if you pick up their suitcase, you will be up all night, worrying if what they said about you is true, stressing yourself out, questioning yourself, getting bitter, and feeding your insecurities. Over a suitcase that never belonged to you in the first place. So if people keep trying to hand off their suitcases to you like you're a bellhop, you might need to break up with them the same way you would break off an unhealthy relationship with an emotionally abusive boyfriend. And as you go through life, trying to figure out how to ferry around those suitcases that do belong to you (and we all have our own stuff . . . the stuffiest of stuffs!), don't try to hand those off to someone else as a way to try to get rid of your pain. Instead, sit down with a friend or a great therapist and have a big, nonjudgmental "let's unpack these suitcases together" session.

I was headed into the final fitting of my leg. I’d gone through the test socket phase and my leg was finally ready. I was so excited! My right leg was still pretty weak and by all normal standards, I should not be able to walk right away. But then, of course, I never like to be like everyone else. Most guys can use the parallel bars for support. I only have one arm so that only helped me so much. Good thing I didn’t really need them. I started walking without faltering right away. “Yeah, this feels good. I feel good. You can back up,” I told them. They backed up and I started walking by myself, holding on with one hand. Then, feeling bolder, I lifted my hand off the bar. I took a step. And then another step. I was walking without any help. I walked up and down those parallel bars the very first day I put on my leg. I did all this with an audience. Dad and Uncle Johnny were right there with me, watching and cheering me on. They were so excited. Uncle Johnny snapped a picture and sent it to my mom back home in Alabama. And as any proud mom would do, she sent that picture to everyone she knew. That picture went the pre-viral version of viral! Intelligence aids achievement and problem-solving, and even leadership. Successful people are generally brimming with confidence. And, as Aristotle said, “beauty is a greater recommendation than any letter of introduction.” So, YAVIS clients are well received nearly everywhere they go, and many therapists light up when one comes walking in the door. Still, there are two paths to being smart and charming when you are young: Life has been good or life has been bad. When life has been good, maybe someone goes to see a therapist for a while because some isolated thing is not currently going well. Most likely, the difficulty will be resolved quickly and the client will be on his way. When life has been bad, someone goes to see a therapist because even though things look pretty on the outside the person feels horrible on the inside, and this is a discrepancy that even many therapists cannot hold. Sometimes it is just too jarring to imagine that someone who seems so perfect has lived a life that has been so imperfect. What results is a therapy where the client’s image gets in the way of the help that he or she needs. The client has come to focus on what has not gone well, but the therapist is blinded by what has. Too often, being successful when you are young is about survival. Some people are good at hiding their troubles. They are good at “falling up.

Hypnotherapy You may have seen scenes on television in which hypnotists make people act like chickens or take off their clothes. In reality, hypnotherapy is nothing like that. During hypnosis you are highly focused and are not distracted by random thoughts. At the same time, you are aware of outside events, such as the telephone ringing or a door slamming. Or, he or she might ask you to focus on an object in your line of vision until your eyes become heavy. Once you are in the hypnotized state, it is easier to focus on your anxiety. You can talk about past experiences, can work on your self-esteem, and can prepare for upcoming social events. You won’t have distracting thoughts or be monitoring everything you say. You may remember events you had forgotten, or may come up with new ways to help yourself cope with the symptoms of anxiety. Adriana was really nervous when her therapist suggested they use hypnosis to work on her fear of meeting new people, but she decided to try it. First, the therapist asked her to visualize a quiet place where she felt completely relaxed and comfortable. When Adriana’s body felt heavy and warm, the therapist asked her to describe how she feels when she speaks with strangers. Adriana discussed how she feels embarrassed and worried, how her face gets red and hot, and how her mind is distracted by negative thoughts. Next, the therapist asked Adriana to visualize being introduced to a stranger. She imagined herself feeling calm and relaxed and looking the person in the eyes. She rehearsed what she would say about herself and said it over and over, sounding more confident each time. The therapist then asked her to think of three things that could help her in those situations. Adriana decided to try relaxing, making sure she is breathing properly, and focusing on the other person instead of on her negative thoughts. Later that week, she dined with a friend and his cousin, whom she had never met before. She was able to take deep breaths and remain relaxed. Once initial introductions went well, Adriana felt more confident and was able to maintain conversations for the entire evening.

People with borderline personality disorder are terrified of abandonment, but for some that might mean feeling anxious when their partners don’t respond to texts right away; for others that may mean choosing to stay in volatile, dysfunctional relationships rather than being alone. • In therapy we aim for self compassion (am I a human?) versus self esteem (Am I good or bad: a judgment) • The techniques we use are a bit like the type of brain surgery in which the patient remains awake throughout the procedure, as the surgeons operate, they keep checking in with the patient: can you feel this? can you say this words? They are constantly calibrating how close they are to sensitive regions of the brain, and if they hit one, they back up so as not to damage it.
