Robert
This is Robert speaking. Today I'm the only personality who is strongly visible inside and outside. My own term for such an MPD role is dominant personality. Fifteen years ago, I rarely appeared on the outside, though I had considerable influence on the inside; back then, I was what one might call a "recessive personality." My passage from "recessive" to "dominant" is a key part of our story; be patient, you'll learn lots more about me later on. Indeed, since you will meet all eleven personalities who once roamed about, it gets a bit complex in the first half of this book; but don't worry, you don't have to remember them all, and it gets sorted out in the last half of the book. You may be wondering -- if not "Robert," who, then, was the dominant MPD personality back in the 1980s and earlier? His name was "Bob," and his dominance amounted to a long reign, from the early 1960s to the early 1990s. Since "Robert B. Oxnam" was born in 1942, you can see that "Bob" was in command from early to middle adulthood.
Although he was the dominant MPD personality for thirty years, Bob did not have a clue that he was afflicted by multiple personality disorder until 1990, the very last year of his dominance. That was the fateful moment when Bob first heard that he had an "angry boy named Tommy" inside of him. How, you might ask, can someone have MPD for half a lifetime without knowing it? And even if he didn't know it, didn't others around him spot it?
To outsiders, this is one of the most perplexing aspects of MPD. Multiple personality is an extreme disorder, and yet it can go undetected for decades, by the patient, by family and close friends, even by trained therapists. Part of the explanation is the very nature of the disorder itself: MPD thrives on secrecy because the dissociative individual is repressing a terrible inner secret. The MPD individual becomes so skilled in hiding from himself that he becomes a specialist, often unknowingly, in hiding from others. Part of the explanation is rooted in outside observers: MPD often manifests itself in other behaviors, frequently addiction and emotional outbursts, which are wrongly seen as the "real problem."
The fact of the matter is that Bob did not see himself as the dominant personality inside Robert B. Oxnam. Instead, he saw himself as a whole person. In his mind, Bob was merely a nickname for Bob Oxnam, Robert Oxnam, Dr. Robert B. Oxnam, PhD.

In her repeated search for a rescuer, she instead finds those who chronically diminish her just like her earliest abusers. Of course, her resilience, adept skill set in adapting to chaotic environments and ability to “bounce back” was also birthed in early childhood. This is also seen as an “asset” to toxic partners because it means she will be more likely to stay within the abuse cycle in order to attempt to make things “work.” She then suffers not just from early childhood trauma, but from multiple re-victimizations in adulthood until, with the right support, she addresses her core wounds and begins to break the cycle step by step. Before she can break the cycle, she must first give herself the space and time to recover. A break from establishing new relationships is often essential during this time; No Contact (or Low Contact from her abusers in more complicated situations such as co-parenting) is also vital to the healing journey, to prevent compounding any existing traumas.

Researchers who study peer relationships have found that there are actually two different kinds of peer popularity. Sociometric popularity is the term used to describe well-liked teens with reputations for being kind and fun, while perceived popularity describes teens who hold a lot of social power but are disliked by many classmates. These two distinct groups emerge in studies that employ a simple peer-nomination method to examine social dynamics in school settings. Girls are given lists naming all the girls in their class (and boys are given lists naming all the boys) and asked to circle the names of the three girls they like the most, the three girls they like the least, and the girls who are considered to be popular. With this technique, researchers have found that many well-liked girls aren’t considered to be popular, and that many girls who are considered to be popular aren’t actually well liked. In fact, the disliked-but-popular girls are described by their classmates as domineering, aggressive, and stuck up, while the liked-but-unpopular girls are described as kind and trustworthy. A third group also emerges: well-liked girls who are identified by peers as being popular. They are amiable and faithful but differ from their liked-but-unpopular peers in that they aren’t easy to push around. In other words, the girls in the liked-and-popular group have found the relational sweet spot of being both friendly and assertive—a skill set girls often struggle to master and to which we’ll return soon. So we know from the research that when teens use the term popular, they’re likely to be describing girls with perceived popularity—girls who use cruelty to gain social power. What enables us to put fantasy behind us and grow to maturity is the capacity to doubt. When a child of six or seven begins to doubt Saint Nick’s ability to get down the chimney or to be in so many different places at once, then he or she begins to doubt the objective reality of this mysterious person. The same capacity to doubt emerges during the often turbulent period of adolescence. We first doubt and then challenge the validity of our parent’s authority. We come to recognise that these once authoritative and almost divine figures are quite human and fallible after all. The perplexing process of alternating between doubt and trust, rebellion and obedience, is essential for our growth to mature adulthood. Persons of fifty who still rely on their parents for guidance in everyday matters are clearly suffering from stunted growth.
And so it is with the evolution of culturally defined opinions. Without the capacity to doubt, we cannot grow from childish beliefs to the maturity of faith. Doubt is not the enemy of faith, but of false beliefs. Indeed, our entire catalogue of assumptions and beliefs should be continually subjected to critical examination, and those found to be false or inadequate should be replaced by those we find convincing within our cultural context. Yet expressing or even entertaining doubt sometimes takes so much courage that we may say it takes real faith to doubt.
Thirty years ago an anonymous well-wisher sent me through the post a little book entitled The Faith to Doubt by the American scholar Homes Hartshorne. I found it an exciting text and have treasured it ever since. Among other things it says, “People today are not in need of assurances about the truth of doubtful beliefs. They need the faith to doubt. They need the faith by which to reject idols. The churches cannot preach to this age if they stand outside of it, living in the illusory security of yesterday’s beliefs. These [already] lie about us broken, and we cannot by taking thought raise them from the dead”.
Far from demonstrating a lack of faith, the very act of discarding outworn beliefs may in fact do just the opposite by opening the door for genuine faith to operate again. Indeed the assertion that one needs to believe a particular creed or set of doctrines in order to have faith is an invitation to credulity rather than to faith— and childlike faith is vastly different from childish credulity

On the other hand, autonomy in the hands of the “lesser-motivated,” can be a very dangerous thing. You know as well as I do that there are still plenty of providers who operate from a subjective, complacent, and downright lazy place. Combined with the ever-expanding autonomy, that provider just became more dangerous than he or she ever has been – to the patients and to you. Autonomy in patient care places more pressure for excellence on the provider charged with delivering it, and also on the partner and crew members on scene. Since the base hospital is not involved like it once was, they are likewise less responsible for the errors and omissions of the medics on the scene. Now more than ever, crew members are being held to answer for the mistakes and follies of their coworkers; now more than ever, EMS providers are working without a net. What’s next? I predict (and hope) emergency medical Darwinism is going to force some painful and necessary changes. First, increasing autonomy is going to result in the better and best providing superior patient care. More personal ownership of the results is going to manifest in outcomes such as increased cardiac arrest survival rates, faster and more complete stroke recovery, and significantly better outcomes for STEMI patients, all leading to the brass ring: EMS as a profession, not just a job. On the flip side of that coin, you will see consequences for the not-so-good and completely awful providers. There will be higher instances of licensure action, internal discipline, and wash-out. Unfortunately, all those things will stem from generally preventable negative patient outcomes. The danger for the better provider will be in the penumbra; the murky, gray area of time when providers are self-categorizing. Specifically, the better provider who is aware of the dangerously poor provider but does nothing to fix or flush him or her, is almost certain to be caught up in a bad situation caused by sloppy, complacent, or ultimately negligent patient care that should have been corrected or stopped. The answer is as simple as it is difficult. If you are reading this, it is more likely because you are one of the better, more committed, more professional providers. This transition is up to you. You must dig deep and find the strength necessary to face the issue and force the change; you have to demand more from yourself and from those around you. You must have the willingness to help those providers who want it – and respond to those who need it, but don’t want it – with tough love by showing them the door. In the end, EMS will only ever be as good as you make it. If you lay silent through its evolution, you forfeit the right to complain when it crumbles around you.

Once a kid is born you’ve been seized by a commitment, the strength of which you couldn’t even have imagined beforehand. It brings you to the doorstep of disciplined service. When a parent falls in love with a child, the love arouses amazing energy levels; we lose sleep caring for the infant. The love impels us to make vows to the thing we love; parents vow to always be there for their kid. Fulfilling those vows requires us to perform specific self-sacrificial practices; we push the baby in a stroller when maybe we’d rather go out alone for a run. Over time those practices become habits, and those habits engrave a certain disposition; by the time the kid is three, the habit of putting the child’s needs first has become second nature to most parents. Conclusion: Adulthood at Last, Ready or Not We have seen in this chapter that the feeling of being in-between is a common part of being an emerging adult. Entering adulthood is no longer as definite and clear-cut as getting married. Young people reach adulthood not because of a single event, but as a consequence of the gradual process of becoming self-sufficient and learning to stand alone. The independence of emerging adulthood is welcome, and they take pride in being able to take care of themselves without relying on their parents’ assistance. Nevertheless, the responsibilities of adulthood can be onerous and stressful, and emerging adults sometimes look back with nostalgia on a childhood and adolescence that seem easier in some ways than their lives now. Claims that most emerging adults experience a “quarterlife crisis”35 in their twenties may be exaggerated; life satisfaction and well-being go up from adolescence to emerging adulthood, for most people. But even if it is not exactly a “crisis,” emerging adulthood is experienced as a time of new and not always welcome responsibilities, a time of not just exhilarating independence and exploration but stress and anxiety as well. Despite the difficulties that come along with managing their own lives, most emerging adults look forward to a future they believe is filled with promise. Whether their lives now are moving along nicely or appear to be going nowhere, they almost unanimously believe that eventually they will be able to create for themselves the kind of life they want. They will find their soul mate, or at least a loving and compatible marriage partner. They will find that dream job, or at least a job that will be enjoyable and meaningful. Eventually this happy vision of the future will be tested against reality, and for many of them the result will be a jarring collision that will force them to readjust their expectations. But during emerging adulthood everything still seems possible. Nearly everyone still believes their dreams will prevail, whatever perils the world may hold for others. Are they too optimistic? Oh yes, at least from the perspective of their elders, who know all too well the likely fate of youthful dreams. Yet is important to understand their optimism as a source of strength, as a psychological resource they will need to draw upon during a stage of life that is often difficult. Given their high expectations for life, they are almost certain to fall short, but it is their self-belief that allows them to get up again after they have been knocked down, even multiple times. They may be optimistic, but the belief that they will ultimately succeed in their pursuit of happiness gives them the confidence and energy to make it through the stresses and uncertainty of the emerging adult years.
