A little way into it, he gets a bit tearful and distressed and, when discussing it with the filmmaker on camera in the next scene, reveals that he found the scene upsetting. The offcamera director asks the poignant question, “What do you think your victims must’ve felt like?” and Anwar initially almost fails to see the connection. Eventually, when the bloody obvious correlation hits him, he thinks it unlikely that his victims were as upset as he was, because he was “really” upset. The director, pressing the film’s point home, says, “Yeah but it must’ve been worse for them, because we were just pretending; for them it was real.” Evidently at this point the reality of the cruelty he has inflicted hits Anwar, because when they return to the concrete garden where the executions had taken place years before, he, on camera, begins to violently gag. This makes incredible viewing, as this literally visceral ejection of his self and sickness at his previous actions is a vivid catharsis. He gagged at what he’d done. After watching the film, I thought—as did probably everyone who saw it—how can people carry out violent murders by the thousand without it ever occurring to them that it is causing suffering? Surely someone with piano wire round their neck, being asphyxiated, must give off some recognizable signs? Like going “ouch” or “stop” or having blood come out of their throats while twitching and spluttering into perpetual slumber? What it must be is that in order to carry out that kind of brutal murder, you have to disengage with the empathetic aspect of your nature and cultivate an idea of the victim as different, inferior, and subhuman. The only way to understand how such inhumane behavior could be unthinkingly conducted is to look for comparable examples from our own lives. Our attitude to homelessness is apposite here. It was as if ordinary human life were a mobile machine full of holes, crannies, spaces, apertures, fissures, cavities, lairs, into one of which Bellamy was required to (and indeed desired to) fit himself. The machine moved slowly, resembling a train, or sometimes a merry-go-round. But as soon as Bellamy got on (or got in), the machine would soon eject him, sending him spinning back to a place where he was once more forced to be a spectator. Perhaps, that was in some mysterious sense his place, his destiny. But Bellamy did not want to be a spectator, nor could he (having no money of his own) afford to be one. Moreover he had never really mastered the art, apparently so simple for others, of passing the time. His failure to find a métier, to find a task which was his task, caused him continuous anxiety, nor did it occur to him to emulate the majority of mankind who positively resigned themselves, seeing no alternative, to alien and unsatisfying work. At one time he had suffered from depression, and was nearer to despair than his friends realised.

What a dreadful experience for a young boy who just wanted the company of his dad, who just wanted to whack at baseballs in the backyard with the old man, who just wanted to be taught to use a circular saw, who just wanted to learn the rudiments of five-card stud or blackjack, who just wanted to understand the precise location of the clitoris or how to pronounce clitoris, or who wanted to learn how to order meat from a waiter, or who wanted to say the word meat with great gusto, or who wanted to learn the proper way to mix and shake a martini, or who wanted to learn to say good little piece of tail, or who wanted to contemplate the necessity of moving on, or who wanted to neglect to shave, or who wanted to learn the specifics of firearms, wanted to be able to eject a used shell, to drive with one hand and dangle the other out the window, to belch without shame, who wanted to drink in the morning, who wanted not to bother flushing the toilet, who wanted to learn to walk naked from the bathroom without worrying about who saw him, and who wanted to cut down his colleagues, his personal friends, in midsentence when he had to. Who would not want his dad when his dad was gone?

When it begins it is like a light in a tunnel, a rush of steel and steam across a torn up life. It is a low rumble, an earthquake in the back of the mind. My spine is a track with cold black steel racing on it, a trail of steam and dust following behind, ghost like. It feels like my whole life is holding its breath. By the time she leaves the room I am surprised that she can’t see the train. It has jumped the track of my spine and landed in my mothers’ living room. A cold dark thing, black steel and redwood paneling. It is the old type, from the western movies I loved as a kid. He throws open the doors to the outside world, to the dark ocean. I feel a breeze tugging at me, a slender finger of wind that catches at my shirt. Pulling. Grabbing. I can feel the panic build in me, the need to scream or cry rising in my throat. And then I am out the door, running, tumbling down the steps falling out into the darkened world, falling out into the lifeless ocean. Out into the blackness. Out among the stars and shadows. And underneath my skin, in the back of my head and down the back of my spine I can feel the desperation and I can feel the noise. I can feel the deep and ancient ache of loudness that litters across my bones. It’s like an old lover, comfortable and well known, but unwelcome and inappropriate with her stories of our frolicking. And then she’s gone and the Conductor is closing the door. The darkness swells around us, enveloping us in a cocoon, pressing flat against the train like a storm. I wonder, what is this place? Those had been heady days, full and intense. It’s funny. I remember the problems, the confusions and the fears of life we all dealt with. But, that all seems to fade. It all seems to be replaced by images of the days when it was all just okay. We all had plans back then, patterns in which we expected the world to fit, how it was to be deciphered. Eventually you just can’t carry yourself any longer, can’t keep your eyelids open, and can’t focus on anything but the flickering light of the stars. Hours pass, at first slowly like a river and then all in a rush, a climax and I am home in the dorm, waking up to the ringing of the telephone. When she is gone the apartment is silent, empty, almost like a person sleeping, waiting to wake up. When she is gone, and I am alone, I curl up on the bed, wait for the house to eject me from its dying corpse. Crazy thoughts cross through my head, like slants of light in an attic. The Boston 395 rocks a bit, a creaking noise spilling in from the undercarriage. I have decided that whatever this place is, all these noises, sensations - all the train-ness of this place - is a fabrication. It lulls you into a sense of security, allows you to feel as if it’s a familiar place. But whatever it is, it’s not a train, or at least not just a train. The air, heightened, tense against the glass. I can hear the squeak of shoes on linoleum, I can hear the soft rattle of a dying man’s breathing. Men in white uniforms, sharp pressed lines, run past, rolling gurneys down florescent hallways.

As the pumping engines for the circulatory system, ventricles must have a particular ovoid, lemonlike shape for strong, swift ejection of blood. If the end of the left ventricle balloons out, as it does in takotsubo hearts, the firm, healthy contractions are reduced to inefficient spasms—floppy and unpredictable. But what’s remarkable about takotsubo is what causes the bulge. Seeing a loved one die. Being left at the altar or losing your life savings with a bad roll of the dice. Intense, painful emotions in the brain can set off alarming, life-threatening physical changes in the heart. This new diagnosis was proof of the powerful connection between heart and mind. Takotsubo cardiomyopathy confirmed a relationship many doctors had considered more metaphoric than diagnostic. As a clinical cardiologist, I needed to know how to recognize and treat takotsubo cardiomyopathy. But years before pursuing cardiology, I had completed a residency in psychiatry at the UCLA Neuropsychiatric Institute. Having also trained as a psychiatrist, I was captivated by this syndrome, which lay at the intersection of my two professional passions. That background put me in a unique position that day at the zoo. I reflexively placed the human phenomenon side by side with the animal one. Emotional trigger … surge of stress hormones … failing heart muscle … possible death. An unexpected “aha!” suddenly hit me. Takotsubo in humans and the heart effects of capture myopathy in animals were almost certainly related—perhaps even the same syndrome with different names.

As in other Buddhist Tantric techniques, recommended preliminaries for these practices include developing skill at both calm-abiding (zhi gnas; śamatha) and insight meditation (lhag mthong; vipaśyanā). As in earlier Buddhist teachings, many Chöd dehadāna practices emphasize renunciation, purification, and self-transformation through the accumulation of merit and the exhaustion of demerit. Rather than suggesting that one must wait to accumulate adequate merit before offering the gift of the body, however, Chöd provides the opportunity for immediately efficacious offering of the body through techniques of visualization. Using a technique which echoes the traditional Buddhist teaching of the of the mind-made body (manomayākāya), the practitioner engages in visualizations which allow her to experience the non-duality of agent and object as she offers her body. The process of giving the body as a means of attainment is commonly articulated in Chöd practice texts (sgrub pa; sādhana). These practice texts exhibit the framework of mature Tantra sādhana, including the stages of generating bodhicitta, going for refuge, meditating on the four immeasurables, and making the eight-limbed offering. Generally speaking, the main section of a developed Chöd sādhana has three components. The first two—a transference of consciousness (nam mkha’ sgo ‘byed) practice, and a body maṇḍala (lus dkyil) practice—have distinctly purifying purposes. The Chöd transference of consciousness practice has parallels with other Buddhist practices called "’pho ba." In this part of the visualization practice, the practitioner’s consciousness is "ejected" from one's body through the Brahma aperture at the crown of one's head. At this time, one's consciousness can be visualized as becoming identical with an enlightened consciousness, which is embodied in a figure such as Machik, Vajrayoginī (Rdo rje rnal byor ma) or Vajravārāhī (Rdo rje phag mo). [....] In th[e] first stage of this transformation, the practitioner identifies with an enlightened being, thus overcoming attachment to her own body-mind aggregates and purifying them through this non-attachment. In the second stage, the practitioner can extend this identification: the practitioner identifies the microcosm of her body with macrocosms of the mundane and supramundane worlds. The body maṇḍala (lus dkyil) stage also allows the practitioner to reconceptualize her body as expanding through space and time and becoming indistinguishable from the realm of the supramundane, or the Dharmadhātu (chos kyi dbyings). Through the process of reconstructing her identity, the practitioner is able to see herself as the ultimate source of offerings for all sentient beings.
