Once irrigation has been accomplished, eyewear is typically not necessary, but the rescuer should still be wearing protective gloves. Remember to wash your hands before and after donning the gloves. Soap, water, and gloves are a tough trio on germs. In the absence of protective gloves, the rescuer may improvise with clean plastic bags over her or his hands. With relatively minor wounds, to prevent sharing germs, the patient may be directed in the management of his or her own wound (including control of blood loss). Contusion Bruises seldom require emergency care, but large bruises benefit from cold, compression, and/or elevation. Substantial bruises should cause you to assess the patient for damage to underlying structures, such as bones and organs. Large bruises should be protected from freezing in extremes of cold because a bruised area will freeze sooner than normal skin. Abrasions Abrasions are the exception to the rule of wound cleaning: You need to scrub within the wound to achieve adequate cleaning. A sterile gauze pad is adequate for scrubbing. Scrubbing may be enhanced by using any soap, but all soap should be carefully rinsed and then irrigated from the wound after scrubbing. Green Soap Sponges are packaged with soap and water already in the sponge, making them useful additions to first-aid kits. It is important to remove all embedded debris not only to reduce the risk of infection but also to prevent subsequent “tattooing” (scarring) of the skin. With a deep abrasion, self-scrubbing is seldom successful due to the high level of pain associated with the exposed nerves. After cleansing, abrasions can be kept moist to avoid desiccation and speed healing with microthin film dressings that can be left in place until healing occurs. Without microthin film dressings, a topical agent, such as an antibiotic ointment, can be applied, followed by a dressing of a sterile gauze pad or a roll of sterile gauze to keep the ointment in place. Tape, an elastic wrap, or some other holder may be used to hold a sterile gauze pad in place. Ideally, gauze dressings should be changed twice a day, or at least once a day, as well as any time the gauze gets wet.

The spinothalamic system, on the other hand, runs through the “grey matter” of the cord, so named because it has no white fatty insulation sheaths around its axons. Their spatial orientation is not nearly so carefully preserved at all levels, and they make many more internuncial synaptic junctions on their way up the cord. Their transmission speed is roughly one-fifth of that of the dorsal tract. This system carries impulses which announce pain; thermal sensations, both hot and cold; crude touch sensations that are not acutely localized; pressure sensations that do not rely upon fine distinctions; kinesthetic sensations having to do with chronic conditions, or the body at rest; tickles and itches; and sexual sensations. It is a fact of considerable significance to our reflex responses that pain sensations are carried exclusively by the slower spinothalamic pathway. This means that more neutral and at the same time more detailed sensory information will always reach the spinal circuits and the cortex slightly before the stab of pain arrives. This gives us a brief moment to assess the location and the cause of the pain before we react, so that our reflex withdrawal can be more appropriately tailored to the actual source of the pain and more effectively directed; that is, so that we will be able to assess the intensity of the burn, and will be sure to jerk away from the flame rather than towards it, and will arrest our jerk before we crash into the wall. This time lag gives a special role to general tactile sensations—including body work—when we are in pain. It means that it is possible to bombard the consciousness with more rapidly transmitted and more detailed touch sensations which tend to displace the pain response from the foreground. This is why rubbing the spot that hurts, or jumping up and down, or shaking the injured hand are often effective for alleviating pain. This is the principle behind the mother’s instinctual rocking and stroking of her hurt child, and it is a principle that can be turned to great advantage in bodywork. If the rest of the body can be inundated with touch sensations, particularly pleasurable ones, the part that is in pain can be shifted away from the mind’s central focus. On the other hand, this very same mechanism presents a danger: By keeping ourselves busy, and by forcing our attention onto other matters, it is possible to suppress pain signals which may be very important, possible to bury our awareness of threatening conditions beneath a layer of faster, more acute, but more trivial sensations. The mind’s mechanisms of selection and focus can play tricks that are nasty as well as ones that are helpful. One of the principal strengths of bodywork is that it can generate the sensory information—the self awareness—that is necessary for the individual to identify and gain control over conflicting tendencies of this kind.

The first is the belief that the universe is essentially harmonious, and that the source of that concord lies in mathematical proportions which can be directly related to musical harmonies. Pythagoras, in an oft-repeated legend, was said to have meditated on the sound of smiths beating hammers upon anvils, and to have argued that a hammer half as heavy produced a note an octave above its full-sized fellow.3 More important were the experiments with a single string, or monochord, attributed to him by his successors. If a stretched string is divided exactly into two it produces a sound an octave higher than the fundamental pitch (the ratio 2:1), the intervals of the fourth and fifth can similarly be expressed as the ratios 4:3 and 3:2 respectively, and all other intervals can be described in mathematical terms.4 These numerical proportions were then extended to describe the relationships of the planetary spheres, both in their relative distance one from another, and in the speed of their movement. The ideas were given influential (if obscure) expression in Plato’s Timaeus, and endlessly elaborated in succeeding centuries up to the Renaissance. From that angle, the annoyed, hustling and bustling, highly important people angling their way through the obstacle course of onlookers seem insignificant. Our sun and moon and eight planets are just one little neighborhood among an estimated 200 billion neighborhoods that make up our universe.19 If we think of the Milky Way galaxy as being the size of the entire continent of North America, our solar system would fit into a coffee cup.20 Two Voyager spacecrafts are cruising toward the edge of the solar system at a rate of more than 35,000 miles per hour. They’ve been doing that for more than forty years and have traveled more than 11 billion miles, with no end in sight.21 When NASA sends communication to one of those Voyagers traveling at that velocity, it takes about seventeen hours to get there.22 That data has led scientists to estimate that to send a “speed of light” message to the edge of the universe would take more than 15 billion years to arrive.23 “So, yes, Chelsea art dealer, you are very important. But when we think about what we’re all gazing at while you make your agitation known through grunts and mumbles, you’re also impossibly young, urgently expiring, and unbelievably small.” You and I see the world with our own two eyes, and from that minuscule perspective, we tend to convince ourselves that we are (or at least should be) in control, directing our own lives, and scripting our future. We come back again to the truth that Philip Yancey reminded us of earlier in the chapter: “Prayer is the act of seeing reality from God’s point of view.” God is the one who calls us to “be still, and know that I am God.” Psalm 8 marvels at this very wonder:
