BEFRIENDING THE BODY
Trauma victims cannot recover until they become familiar with and befriend the sensations in their bodies. Being frightened means that you live in a body that is always on guard. Angry people live in angry bodies. The bodies of child-abuse victims are tense and defensive until they find a way to relax and feel safe. In order to change, people need to become aware of their sensations and the way that their bodies interact with the world around them. Physical self-awareness is the first step in releasing the tyranny of the past.
In my practice I begin the process by helping my patients to first notice and then describe the feelings in their bodies—not emotions such as anger or anxiety or fear but the physical sensations beneath the emotions: pressure, heat, muscular tension, tingling, caving in, feeling hollow, and so on. I also work on identifying the sensations associated with relaxation or pleasure. I help them become aware of their breath, their gestures and movements.
All too often, however, drugs such as Abilify, Zyprexa, and Seroquel, are prescribed instead of teaching people the skills to deal with such distressing physical reactions. Of course, medications only blunt sensations and do nothing to resolve them or transform them from toxic agents into allies.
The mind needs to be reeducated to feel physical sensations, and the body needs to be helped to tolerate and enjoy the comforts of touch. Individuals who lack emotional awareness are able, with practice, to connect their physical sensations to psychological events. Then they can slowly reconnect with themselves.

But these sad, spindly sprouts would grow 2 or 3 feet in length as they reached toward the distant light of the window. The sprouts were, in their bizarre, futile growth, a sort of desperate expression of the directional tendency I have been describing. They would never become plants, never mature, never fulfill their real potential. But under the most adverse circumstances, they were striving to become. Life would not give up, even if it could not flourish. In dealing with clients whose lives have been terribly warped, in working with men and women on the back wards of state hospitals, I often think of those potato sprouts. So unfavorable have been the conditions in which these people have developed that their lives often seem abnormal, twisted, scarcely human. Yet, the directional tendency in them can be trusted. The clue to understanding their behavior is that they are striving, in the only ways that they perceive as available to them, to move toward growth, toward becoming. To healthy persons, the results may seem bizarre and futile, but they are life's desperate attempt to become itself. This potent constructive tendency is an underlying basis of the person-centered approach.

Nevertheless, the therapist had made it clear from the outset that she was in no way going to pressure, hector, cajole, argue, persuade, flummox, trick, harangue, shame, or manipulate the depressed person into letting go of her arrested or vestigial defenses before she (i.e., the depressed person) felt ready and able to risk taking the leap of faith in her own internal resources and self-esteem and personal growth and healing to do so (i.e., to leave the nest of her defenses and freely and joyfully fly).

Here is his opinion as quoted on the Virginia Woolf Web site: As a child she was sexually abused, but the extent and duration is difficult to establish. At worst she may have been sexually harassed and abused from the age of twelve to twenty-one by her [half-]brother George Duckworth, [fourteen] years her senior, and sexually exploited as early as six by her other [half-] brother… It is unlikely that the sexual abuse and her manic-depressive illness are related. However tempting it may be to relate the two, it must be more likely that, whatever her upbringing, her family history and genetic makeup were the determining factors in her mood swings rather than her unhappy childhood [italics added]. More relevant in her childhood experience is the long history of bereavements that punctuated her adolescence and precipitated her first depressions.3 Ingram’s text goes against my own interpretation and ignores a large volume of literature that deals with trauma and the effects of childhood abuse. Here we see how people minimize the importance of information that might cause pain or discomfort—such as childhood abuse—and blame psychiatric disorders on family history instead. Woolf must have felt keen frustration when seemingly intelligent and well-educated people attributed her condition to her mental history, denying the effects of significant childhood experiences. In the eyes of many she remained a woman possessed by “madness.” Nevertheless, the key to her condition lay tantalizingly close to the surface, so easily attainable, and yet neglected. I think that Woolf’s suicide could have been prevented if she had had an enlightened witness with whom she could have shared her feelings about the horrors inflicted on her at such an early age. But there was no one to turn to, and she considered Freud to be the expert on psychic disorders. Here she made a tragic mistake. His writings cast her into a state of severe uncertainty, and she preferred to despair of her own self rather than doubt the great father figure Sigmund Freud, who represented, as did her family, the system of values upheld by society, especially at the time. I believe the perception of what people think about DID is I might be crazy, unstable, and low functioning. After my diagnosis, I took a risk by sharing my story with a few friends. It was quite upsetting to lose a long term relationship with a friend because she could not accept my diagnosis. But it spurred me to take action. I wanted people to be informed that anyone can have DID and achieve highly functioning lives. I was successful in a career, I was married with children, and very active in numerous activities. I was highly functioning because I could dissociate the trauma from my life through my alters. Essentially, I survived because of DID. That's not to say I didn't fall down along the way. There were long term therapy visits, and plenty of hospitalizations for depression, medication adjustments, and suicide attempts. After a year, it became evident I was truly a patient with the diagnosis of DID from my therapist and psychiatrist. I had two choices.
First, I could accept it and make choices about how I was going to deal with it. My therapist told me when faced with DID, a patient can learn to live with the live with the alters and make them part of one's life. Or, perhaps, the patient would like to have the alters integrate into one person, the host, so there are no more alters. Everyone is different.
The patient and the therapist need to decide which is best for the patient. Secondly, the other choice was to resist having alters all together and be miserable, stuck in an existence that would continue to be crippling. Most people with DID are cognizant something is not right with themselves even if they are not properly diagnosed. My therapist was trustworthy, honest, and compassionate. Never for a moment did I believe she would steer me in the wrong direction. With her help and guidance, I chose to learn and understand my disorder. It was a turning point.

Once I know what the words are, I can learn how to stop their effect on me. I did it already when I learned the control code. Standing in front of a mirror, I said the control code words over and over until I was completely desensitised to them. That's what I have to do for the activation code words... but I have not been able to recall all of them as yet.'

Sacrifice is a notoriously hard concept to understand. Indeed, it is not a univocal concept, but is a name used for a variety of actions that attempt communication between the human and the divine or transcendent spheres.7 Contemplation of the abyss reveals the enormity and complexity of the evil that has been perpetrated upon a society. What would it take to overcome it? The images of cross and blood figure prominently in the Pauline language of reconciliation (cf. Rom 5:9; Col 1:20; Eph 2:13-16). Both cross and blood have paradoxical meanings that allow them to bridge the distance between the divine and human worlds, between life and death. The cross was the ultimate sign of Roman power over a conquered and colonized people. To be crucified was the most dishonorable and humiliating of ways to die. The cross stood as a sign of reassertion of Roman power and the capacity to reject and exclude utterly. Yet it was through the crucified Christ that God chose to reconcile the world. The apparent triumph of worldly power is turned against itself and becomes “Christ the power of God and the wisdom of God” (1 Cor 1:24). For John, the cross is at once instrument of humiliation and Christ's throne of glory (Jn 12:32). Similarly, blood is a sign of the divine life that God has breathed into every living being, and its shedding is a sign of death. The blood of the cross (Col 1:20) becomes the means of reconciling all things to God. In its being shed, the symbol of violence and death becomes the symbol of reconciliation and peace. To understand sacrifice, one must be prepared to inhabit the space within these paradoxes. Sacrifice understood in this way is not about the abuse of power, but about a transformation of power. A spirituality of reconciliation can be deepened by a meditation on the stories of the women and the tomb. These stories invite us to place inside them our experience of marginalization, of being incapable of imagining a way out of a traumatic past, of dealing with the kinds of absence that traumas create. They invite us to let the light of the resurrection—a light that even the abyss cannot extinguish—penetrate those absences.

In The Body Keeps the Score, Bessel van der Kolk writes about a form of therapy called EMDR, or Eye Movement Desensitization and Reprocessing. It’s a strange process reminiscent of hypnosis, where a patient revisits past traumas while moving their eyes left and right. It seemed too simple, almost hokey, but van der Kolk passionately sang its praises. He told the story of a patient who came out of a single forty-five-minute session of EMDR, looked at him, and said that “he’d found dealing with me so unpleasant that he would never refer a patient to me. Otherwise, he remarked, the EMDR session had resolved the matter of his father’s abuse.” Resolved! Here was a form of therapy, van der Kolk said, that could help “even if the patient and the therapist do not have a trusting relationship.” Then again, he said that EMDR was far more effective for adult-onset trauma, and it cured only 9 percent of childhood trauma survivors.

In 2021 the respected journal Nature Medicine published a peer-reviewed, placebo-controlled trial on psychedelic assisted treatment of trauma. The results were impressive. With just three, medically supervised sessions using MDMA, 67 per cent no longer had PTSD – more than double the placebo group. There was no increased risk of abuse and, crucially, those with dissociation responded as well as those without.3 Given the special skills otherwise required to navigate dissociation, this latter finding was a big deal. There are currently over a hundred psychedelic-assisted therapy trials being conducted worldwide. It would appear that these drugs allow a resetting of a part of the brain known as the ‘Default Mode Network’ (DMN) that otherwise holds on to recurring, distressing thoughts – especially around guilt and shame. During REM/dreaming sleep the DMN fires up, but the normal resetting process fails with overwhelming trauma.

Eagle allows us to rise above the mundane battles that occupy our lives and consume our energy and attention. Eagle gives us wings to soar above trivial day-to-day struggles into the high peaks close to Heaven. Eagle and condor represent the self-transcending principle in nature. Biologists have identified the self-transcending principle as one of the prime agendas of evolution. Living molecules seek to transcend their selfhood to become cells, then simple organisms, which then form tissues, then organs, and then evolve into complex beings such as humans and whales. Every transcending jump is inclusive of all of the levels beneath it. Cells are inclusive of molecules, yet transcend them; organs are inclusive of cells, yet go far beyond them; whales are inclusive of organs yet cannot be described by them, as the whole transcends the sum of its parts. The transcending principle represented by eagle states that problems at a certain level are best solved by going up one step. The problems of cells are best resolved by organs, while the needs of organs are best addressed by an organism such as a butterfly or a human. The same principle operates in our lives. Think of nested Russian dolls. Material needs are the tiny doll in the center. The larger emotional doll encompasses them, and both are contained within the outermost spiritual doll. In this way, we cannot satisfy emotional needs with material things, but we can satisfy them spiritually. When we go one step up, our emotional needs are addressed in the solution. We rise above our life dilemmas on the wings of eagle and see our lives in perspective.

I had always thought that having a flashback meant fully hallucinating your past. In the movies, soldiers would be transported back to Afghanistan—they’d see desert sand and automatic rifles in a waking nightmare. But even when I remembered moments of abuse, I knew where I was. I knew I was on the couch. I knew I was not going to die.
But I soon learned that in trauma lingo, people often aren’t talking about the movie version of flashbacks. They’re talking about emotional flashbacks.
For example, before I quit my job, my boss often came into my office to tell me I’d made some minor mistake. If my body and brain were totally in the present, I would have felt embarrassed for messing up but would recognize that it wasn’t a huge deal, acknowledge my faults, and get back to it. Instead, after my boss left, I always felt guilt and anxiety and shame and terror. I’d run downstairs to have a cigarette, text a friend about how I was a moron, and spend half an hour freaking out about how nobody respected me and I’d probably end up fired. Even though consciously I was completely in the present, my emotions were back in 1997, back when I was a little kid and making a mistake on a spelling test could literally be a matter of life and death. This return was an emotional flashback.

The approach adopted by Daniel Stern and the followers of John Bowlby still appears to gain only peripheral attention in psychoanalytic circles, perhaps because by his theory of initial attachment Bowlby exploded a taboo. By linking the causes of antisocial behavior with the absence of a resilient attachment to the mother, he was flying in the face of Freud’s drive theory. But my conviction is that we have to go a step further than Bowlby went. We are dealing here not just with antisocial behavior and so-called narcissistic disorders but with the inescapable realization that denying and repressing our childhood traumas means reducing our capacity to think and conspiring to erect barriers in our minds. Brain research has succeeded in uncovering the biological foundations of the denial phenomenon. But the consequences, the impact on our mentality, have not yet been adequately contemplated. No one appears to be interested in examining how insensitivity to the suffering of children–a phenomenon found the world over–is bound up with a form of mental paralysis that has its roots in childhood. As children, we learn to suppress and deny natural feelings and to believe sincerely that the cuffs and blows we receive are for our own good and do us no lasting injury. Our brains, furnished with this false information, then instruct us to raise our own children by the same methods, telling them that it is good for them just as it was good for us.

Psychoanalysis has suffered the accusation of being “unscientific” from its very beginnings (Schwartz, 1999). In recent years, the Berkeley literary critic Frederick Crews has renewed the assault on the talking cure in verbose, unreadable articles in the New York Review of Books (Crews, 1990), inevitably concluding, because nothing else really persuades, that psychoanalysis fails because it is unscientific. The chorus was joined by philosopher of science, Adolf Grunbaum (1985), who played both ends against the middle: to the philosophers he professed specialist knowledge of psychoanalysis; to the psychoanalysts he professed specialist knowledge of science, particularly physics. Neither was true (Schwartz, 1995a,b, 1996a,b, 2000).
The problem that mental health clinicians always face is that we deal with human subjectivity in a culture that is deeply invested in denying the importance of human subjectivity. Freud’s great invention of the analytic hour allows us to explore, with our clients, their inner worlds. Can such a subjective instrument be trusted? Not by very many. It is so dangerously close to women’s intuition. Socalled objectivity is the name of the game in our culture. Nevertheless, 100 years of clinical practice have shown psychoanalysis and psychotherapy not only to be effective, but to yield real understandings of the dynamics of human relationships, particularly the reality of transference–countertransference re-enactments now reformulated by our neuroscientists as right brain to right brain communication (Schore, 1999).

FEW: Well, there is a hierarchy, isn’t there? OM: I don’t think so. FEW: If you didn’t think there was, you wouldn’t have used that example. You would have said, “Suppose someone has been molested by a neighbor” or “neglected by their mother” or “bullied all their lives.” But there is a hierarchy. Trauma people don’t like to hear that. To them, all trauma matters. OM: Where would you place your trauma on the hierarchy? FEW: All I’m saying is that trauma doesn’t get you a lifelong get-out-of-jail-free card. It also doesn’t necessarily confer wisdom, or the right to pontificate, which I realize I’m doing right now. OM: Well. I’m willing to concede that life handles some people more roughly than it does others, and that you do have a choice in how you deal with it. You can decide what you want to do with it, but not until after you address it, which—I’m sorry to say—involves talking about it, for as long as it takes, identifying fears and triggers— FEW: Triggers. God. This is why I’m not crazy about therapy. I really hate the language.

true—helping a hurting person is a bit scary. We want to do the right thing, not the wrong thing—say what will help, not what will hurt. To add to our confusion, our friend is “not quite herself.” She’s different. We want our friend fixed and back to normal. All you have to do is care. Harold Ivan Smith described the process so well: Grief sharers always look for an opportunity to actively care. You can never “fix” an individual’s grief, but you can wash the sink full of dishes, listen to him or her talk, take his or her kids to the park. You can never “fix” an individual’s grief but you can visit the cemetery with him or her. Grief sharing is not about fixing—it’s about showing up. Coming alongside. Being interruptible. “Hanging out” with the bereaving. In the words of World War II veterans, “present and reporting for duty.” The grief path is not a brief path. It’s a marathon, not a sprint.[1] What can you expect from a friend who is hurting? Actually, not very much. And the more her experience moves beyond a loss and closer to a crisis or trauma, the more this is true. Sometimes you’ll see a friend experiencing a case of the “crazies.” Her response seems irrational. She’s not herself. Her behavior is different from or even abnormal compared to the person not going through a major loss. Just remember, she’s reacting to an out-of-the-ordinary event. What she experienced is abnormal, so her response is actually quite normal. If what the person has experienced is traumatic she may even seem to exhibit some of the symptoms of ADD (Attention Deficit Disorder). And because your friend is this way, she is not to be avoided. Others are needed at this time in her life. These are responses you can expect. Your friend is no longer functioning as she once did—and probably won’t for a while. You Are Needed You are needed when a person experiences a sudden intrusion or disruption in her life. If you (or another friend) aren’t available, the only person she has to talk with for guidance, support, and direction is herself. And who wants support from someone struggling with a case of the “crazies”? But a problem may arise when your friend doesn’t realize that she needs you, at least at that particular time. Your sensitivity is needed at this point. Remember, when your friend is hurting and facing a loss, you are dealing with a loss as well, because the relationship you had with your friend has changed. It’s not the same.
