I decided to ask eight Vietnam combat veterans if they would be willing to take a standard pain test while they watched scenes from a number of movies. The first clip we showed was from Oliver Stone’s graphically violent Platoon (1986), and while it ran we measured how long the veterans could keep their right hands in a bucket of ice water. We then repeated this process with a peaceful (and long-forgotten) movie clip. Seven of the eight veterans kept their hands in the painfully cold water 30 percent longer during Platoon. We then calculated that the amount of analgesia produced by watching fifteen minutes of a combat movie was equivalent to that produced by being injected with eight milligrams of morphine, about the same dose a person would receive in an emergency room for crushing chest pain.

There are two types of memory frequently experienced by individuals who have had overwhelming trauma that has been suppressed psychologically or chemically. In my new job [in 1982] I was confronted on an almost daily basis with issues I thought I had left behind at the VA. My experience with combat veterans had so sensitized me to the impact of trauma that I now listened with a very different ear when depressed and anxious patients told me stories of molestation and family violence. The textbook went on to practically endorse incest, explaining that 'such incestuous activity diminishes the subject’s chance of psychosis and allows for a better adjustment to the external world.' In fact, as it turned out, incest had devastating effects on women’s well-being.

The dominant literary mode of the twentieth century has been the fantastic. This may appear a surprising claim, which would not have seemed even remotely conceivable at the start of the century and which is bound to encounter fierce resistance even now. However, when the time comes to look back at the century, it seems very likely that future literary historians, detached from the squabbles of our present, will see as its most representative and distinctive works books like J.R.R. Tolkien’s The Lord of the Rings, and also George Orwell’s Nineteen Eighty-Four and Animal Farm, William Golding’s Lord of the Flies and The Inheritors, Kurt Vonnegut’s Slaughterhouse-Five and Cat’s Cradle, Ursula Le Guin’s The Left Hand of Darkness and The Dispossessed, Thomas Pynchon’s The Crying of Lot-49 and Gravity’s Rainbow. The list could readily be extended, back to the late nineteenth century with H.G. Wells’s The Island of Dr Moreau and The War of the Worlds, and up to writers currently active like Stephen R. Donaldson and George R.R. Martin. It could take in authors as different, not to say opposed, as Kingsley and Martin Amis, Anthony Burgess, Stephen King, Terry Pratchett, Don DeLillo, and Julian Barnes. By the end of the century, even authors deeply committed to the realist novel have often found themselves unable to resist the gravitational pull of the fantastic as a literary mode. This is not the same, one should note, as fantasy as a literary genre – of the authors listed above, only four besides Tolkien would find their works regularly placed on the ‘fantasy’ shelves of bookshops, and ‘the fantastic’ includes many genres besides fantasy: allegory and parable, fairy-tale, horror and science fiction, modern ghost-story and medieval romance. Nevertheless, the point remains. Those authors of the twentieth century who have spoken most powerfully to and for their contemporaries have for some reason found it necessary to use the metaphoric mode of fantasy, to write about worlds and creatures which we know do not exist, whether Tolkien’s ‘Middle-earth’, Orwell’s ‘Ingsoc’, the remote islands of Golding and Wells, or the Martians and Tralfa-madorians who burst into peaceful English or American suburbia in Wells and Vonnegut. A ready explanation for this phenomenon is of course that it represents a kind of literary disease, whose sufferers – the millions of readers of fantasy – should be scorned, pitied, or rehabilitated back to correct and proper taste. Commonly the disease is said to be ‘escapism’: readers and writers of fantasy are fleeing from reality. The problem with this is that so many of the originators of the later twentieth-century fantastic mode, including all four of those first mentioned above (Tolkien, Orwell, Golding, Vonnegut) are combat veterans, present at or at least deeply involved in the most traumatically significant events of the century, such as the Battle of the Somme (Tolkien), the bombing of Dresden (Vonnegut), the rise and early victory of fascism (Orwell). Nor can anyone say that they turned their backs on these events. Rather, they had to find some way of communicating and commenting on them. It is strange that this had, for some reason, in so many cases to involve fantasy as well as realism, but that is what has happened.

Being traumatized is not just an issue of being stuck in the past; it is just as much a problem of not being fully alive in the present. One form of exposure treatment is virtual-reality therapy in which veterans wear high-tech goggles that make it possible to refight the battle of Fallujah in lifelike detail. As far as I know, the US Marines performed very well in combat. The problem is that they cannot tolerate being home. Recent studies of Australian combat veterans show that their brains are rewired to be alert for emergencies, at the expense of being focused on the small details of everyday life.43 (We’ll learn more about this in chapter 19, on neurofeedback.) More than virtual-reality therapy, traumatized patients need “real world” therapy, which helps them to feel as alive when walking through the local supermarket or playing with their kids as they did in the streets of Baghdad.

Democratic process embodies the apparent contradiction of safe struggle. Combat veterans with unhealed PTSD have the greatest difficulty conceiving of any struggle apart from killing and dying. Passionate struggle conducted within rules of safety and fairness simply doesn't make sense to them or seems a hollow charade. For them it is psychologically impossible to win a struggle without killing or to lose without dying, and they do not want to do either. Many veterans' response is to withdraw and not participate. Democracy embodies safe struggle over the shape and implementation of a future. An unhealed combat veteran cannot think in terms of a future. Democratic political activity presupposes that the future exists and that it is meaningful. Combat taught the survivor of prolonged combat not to imagine a future or to want anything. Prior to seeing the point of one's voluntary participation in a social process, one must feel that it is safe to want something.

Forgetting combat trauma is not a legitimate goal of treatment. Veterans find it morally degrading to forget the dead. To know why this is so, we need only recall what we have seen in the earliest chapters on the existential functions of guilt and rage. The task is to remember -- rather than relive and reenact -- and to grieve. For combat veterans this means grieving not only the dead but also their own lost innocence in both its meanings, as blamelessness and as unawareness of evil. Also, many prewar relationships with parents, friends, siblings, and spouses are now gone forever. A secure sense of the goodness of the social order is irretrievably lost and must be mourned. One veteran said, You're afraid that once you start to cry you'll never stop. And once you do start, it seems like it will never stop. I cried for a whole year. We must all strive to be a trustworthy audience for victims of abuse of power. I like to think that Aristotle had something like this in mind when he made tragedy the centerpiece of education for citizens in a democracy.

America’s last step into the Vietnam quagmire came on November 22, 1963, when Lyndon Baines Johnson was sworn in as the thirty-sixth president of the United States. Unlike Kennedy, Johnson was no real veteran. During World War II he used his influence as a congressman to become a naval officer, and, despite an utter lack of military training, he arranged a direct commission as a lieutenant commander. Fully aware that “combat” exposure would make him more electable, the ambitious Johnson managed an appointment to an observation team that was traveling to the Pacific. Once there, he was able to get a seat on a B-26 combat mission near New Guinea. The bomber had to turn back due to mechanical problems and briefly came under attack from Japanese fighters. The pilot got the damaged plane safely back to its base and Johnson left the very next day. This nonevent, which LBJ had absolutely no active part of, turned into his war story. The engine had been “knocked out” by enemy fighters, not simply a routine malfunction; he, LBJ, had been part of a “suicide mission,” not just riding along as baggage. The fabrication grew over time, including, according to LBJ, the nickname of “Raider” Johnson given to him by the awestruck 22nd Bomber Group.

I recall canoeing with veterans along tranquil meanders of the Saco River in Maine, when one of them, who had served on riverboats in the Mekong Delta, pointed out the tan mudbank on the outside of a curve. He said that such an innocent riverbank would be riddled with tunnels and invisible machine-gun and rock-propelled-grenade positions. The cumulative effect of prolonged attacks on mental function is to undermine the soldier's trust in his own perceptions. Another veteran said: Nothing is what it seems. That mountain there--maybe it wasn't there yesterday, and won't be there tomorrow. You get to the point where you're not even sure it is a mountain.

Anthropologists like Kohrt, Hoffman, and Abramowitz have identified three factors that seem to crucially affect a combatant's transition back into civilian life. The United States seems to rank low on all three. First, cohesive and egalitarian tribal societies do a very good job at mitigating effects of trauma, but by their very nature, many modern societies are exactly the opposite: hierarchical and alienating. America's great wealth, although a blessing in many ways, has allowed for the growth of an individualistic society that suffers high rates of depression and anxiety. Both are correlated with chronic PTSD. Secondly, ex-combatants shouldn't be seen -or be encouraged to see themselves - as victims... Lifelong disability payments for a disorder like PTSD, which is both treatable and usually not chronic, risks turning veterans into a victim class that is entirely dependent on the government for their livelihood... Perhaps most important, veterans need to feel that they're just as necessary and productive back in society as they were on the battlefield... Recent studies of something called 'social resilience' have identified resource sharing and egalitarian wealth distribution as major components of a society's ability to recover from hardship. And societies that rank high on social resilience...provide soldiers with a significantly stronger buffer against PTSD than low-resilience societies. In fact, social resilience is an even better predictor of trauma recovery than the level of resilience of the person himself.

Support on the home front for the soldier, regardless of ethical and political disagreements over the war itself. is essential. This is never easy in the emotionally polarized climate of a war. However, when facing individual soldiers, we must remember that all modern soldiers serve under constraint. The justice of overall war aims and of operational theories -- "strate-gic" bombing of civilians to weaken the industrial capacity to wage war is an example of such theory -- is not within the individual soldier's scope of moral choice, unless he or she is willing to face imprisonment or death by refusing to fight. I cannot hold soldiers to an ethical standard that requires martyrdom in order simply to be blameless. I am not arguing against the Nuremberg principles, which say that no person is absolved of responsibility for horrible acts by the fact that he or she was legally ordered to do them. I am speaking from the pain that I feel when I witness in our veterans the ruin of moral life by the overwhelming coercive social power of military institutions and of war itself.

After we had been back on Pavuvu about a week, I had one of the most heartwarming and rewarding experiences of my entire enlistment in the Marine Corps. It was after taps, all the flambeaus were out, and all of my tent mates were in their sacks with mosquito nets in place. We were all very tired, still trying to unwind from the tension and ordeal of Peleliu. All was quiet except for someone who had begun snoring softly when one of the men, a Gloucester veteran who had been wounded on Peleliu, said in steady measured tones, “You know something, Sledgehammer?” “What?” I answered. “I kinda had my doubts about you,” he continued, “and how you’d act when we got into combat, and the stuff hit the fan. I mean, your ole man bein’ a doctor and you havin’ been to college and bein’ sort of a rich kid compared to some guys. But I kept my eye on you on Peleliu, and by God you did OK; you did OK.” “Thanks, ole buddy,” I replied, nearly bursting with pride. Many men were decorated with medals they richly earned for their brave actions in combat, medals to wear on their blouses for everyone to see. I was never awarded an individual decoration, but the simple, sincere personal remarks of approval by my veteran comrade that night after Peleliu were like a medal to me. I have carried them in my heart with great pride and satisfaction ever since.

Hitler initially served in the List Regiment engaged in a violent four-day battle near Ypres, in Belgian Flanders, with elite British professional soldiers of the initial elements of the British Expeditionary Force. Hitler thereby served as a combat infantryman in one of the most intense engagements of the opening phase of World War I. The List Regiment was temporarily destroyed as an offensive force by suffering such severe casualty rates (killed, wounded, missing, and captured) that it lost approximately 70 percent of its initial strength of around 3,600 men. A bullet tore off Hitler’s right sleeve in the first day of combat, and in the “batch” of men with which he originally advanced, every one fell dead or wounded, leaving him to survive as if through a miracle. On November 9, 1914, about a week after the ending of the great battle, Hitler was reassigned as a dispatch runner to regimental headquarters. Shortly thereafter, he was awarded the Iron Cross Second Class. On about November 14, 1914, the new regimental commander, Lieutenant Colonel Philipp Engelhardt, accompanied by Hitler and another dispatch runner, moved forward into terrain of uncertain ownership. Engelhardt hoped to see for himself the regiment’s tactical situation. When Engelhardt came under aimed enemy smallarms fire, Hitler and the unnamed comrade placed their bodies between their commander and the enemy fire, determined to keep him alive. The two enlisted men, who were veterans of the earlier great four-day battle around Ypres, were doubtlessly affected by the death of the regiment’s first commander in that fight and were dedicated to keeping his replacement alive. Engelhardt was suitably impressed and proposed Hitler for the Iron Cross Second Class, which he was awarded on December 2. Hitler’s performance was exemplary, and he began to fit into the world around him and establish the image of a combat soldier tough enough to demand the respect of anyone in right wing, Freikorps-style politics after the war. -- Hitler: Beyond Evil and Tyranny, p. 88

The most alarming rhetoric comes out of the dispute between liberals and conservatives, and it’s a dangerous waste of time because they’re both right. The perennial conservative concern about high taxes supporting a nonworking “underclass” has entirely legitimate roots in our evolutionary past and shouldn’t be dismissed out of hand. Early hominids lived a precarious existence where freeloaders were a direct threat to survival, and so they developed an exceedingly acute sense of whether they were being taken advantage of by members of their own group. But by the same token, one of the hallmarks of early human society was the emergence of a culture of compassion that cared for the ill, the elderly, the wounded, and the unlucky. In today’s terms, that is a common liberal concern that also has to be taken into account. Those two driving forces have coexisted for hundreds of thousands of years in human society and have been duly codified in this country as a two-party political system. The eternal argument over so-called entitlement programs—and, more broadly, over liberal and conservative thought—will never be resolved because each side represents an ancient and absolutely essential component of our evolutionary past. So how do you unify a secure, wealthy country that has sunk into a zero-sum political game with itself? How do you make veterans feel that they are returning to a cohesive society that was worth fighting for in the first place? I put that question to Rachel Yehuda of Mount Sinai Hospital in New York City. Yehuda has seen, up close, the effect of such antisocial divisions on traumatized vets. “If you want to make a society work, then you don’t keep underscoring the places where you’re different—you underscore your shared humanity,” she told me. “I’m appalled by how much people focus on differences. Why are you focusing on how different you are from one another, and not on the things that unite us?” The United States is so powerful that the only country capable of destroying her might be the United States herself, which means that the ultimate terrorist strategy would be to just leave the country alone. That way, America’s ugliest partisan tendencies could emerge unimpeded by the unifying effects of war. The ultimate betrayal of tribe isn’t acting competitively—that should be encouraged—but predicating your power on the excommunication of others from the group. That is exactly what politicians of both parties try to do when they spew venomous rhetoric about their rivals. That is exactly what media figures do when they go beyond criticism of their fellow citizens and openly revile them. Reviling people you share a combat outpost with is an incredibly stupid thing to do, and public figures who imagine their nation isn’t, potentially, one huge combat outpost are deluding themselves.

In short, the Arabs were different from us, so different that ‘they have no objection to being killed’, as Hugh Trenchard, head of the RAF general staff, explained to the sensitive souls of the British Parliament.49 Arabs loved war precisely because it involved a confrontation with death, and as opposed to the effeminate Europeans, they did not make the flabby distinction between combatants and non-combatants. If you thought about it properly, not bombing them would almost amount to insulting their values.

Freud continued to be perplexed by traumatized patients behaving in compulsive and seemingly very unpleasurable ways, almost as if they wanted to relive the event that had traumatized them rather than forget it. On the one hand, conflicts between our conscious aims and our unconscious desires could go some of the way toward explaining this. People’s reactions to traumatic events may be much more complex than we like to imagine, for one thing—we harbor unconscious death wishes for those close to us, as a result of sibling rivalries and the like. But ambivalence could not be the whole story. Why do war veterans obsessively relive objectively horrifying combat situations in their dreams? Why do neurotics find it so hard to “let go” of real or imagined traumas and end up staging situations that essentially repeat and reinforce them, almost as though they are trying to (re)create those traumas instead of move on? For a doctor whose whole theory of human motivation rested on the individual’s pursuit of his or her wishes in dreams and of pleasure in daily life, this compulsive returning to past traumas was a conundrum

Author’s Note Writing about a suicidal character is one of the most challenging things I’ve ever done, but also one of the most important. Suicide is always tragic, but it has become an epidemic among American active-duty service members and veterans alike. The statistics are staggering and heart-wrenching. In the U.S. Army, which has the highest suicide rate among the branches (48.7 percent of all military suicides in 2012), the suicide rate in 2012 was thirty per hundred thousand, compared with fourteen per hundred thousand among civilians and eighteen per hundred thousand in 2008. In 2012, 841 active-duty service members attempted or committed suicide. Among veterans, as of November 2013, twenty-two committed suicide every day. Every. Day. A frightening 30 percent of veterans say they’ve considered suicide, and 45 percent say they know an Iraq or Afghanistan veteran who has attempted or committed suicide. In a study of veterans, combat-related guilt was the most significant predictor of suicide attempts and of preoccupation with suicide after discharge. Veterans’ suicidal thoughts are also related to feelings that one does not belong with other people or has become a burden. Couple these sad realities with the fact that veterans are less likely to seek care than active-duty military or civilians, and you begin to understand why statistics like these exist. Suicide is a process that begins with ideas and thoughts, followed by planning, and finally followed by a suicidal act. A common feature of many theories of trauma is the idea that the causative—the wounding—event is not remembered but relived, as it is in the flashbacks of combat veterans, experienced anew with a visceral immediacy that affords no critical distance. To remember something, you have to consign it to the past—put it behind you—but trauma remains in the present; it fills that present entirely. You are inside it. Your mouth is always filled with the taste of blood. The killers are always crashing through the brush behind you. Some researchers believe that trauma bypasses the normal mechanisms of memory and engraves itself directly on some portion of the brain, like a brand. Cattle are branded to signify that they are someone’s property, and so, too, were slaves. The brand of trauma signifies that henceforth you yourself are property, the property of that which has injured you. The psychoanalyst Sándor Ferenczi believed that trauma is characterized by the victim’s helpless identification with the perpetrator, and elsewhere in the literature one often comes across the word “possession.” The moment of trauma marks an event horizon after which memory ceases. Or else memory breaks down, so that the victim can reconstruct the event but not the feeling that accompanied it, or alternatively only the feeling.

I am Doug Hammer, U.S. Army Green Beret, retired. I served my country in two wars, in combat and received the Purple Heart for my war wounds. Until recently, I was willing to put up with my country imprisoning conservatives who did not agree with the President. I was willing to accept the imprisoning of pastors and others who were not favored by the White House. My thought was that America elected the President, so we get what we deserve. However, I have concluded that the President is not legitimately in office, that he has the worst interests of the country at heart and that he is attempting to destroy the capitalistic system and install a socialist, even Communist, system in its place. I finally concluded that rumors of Russian troops, stationed at this base, being used to execute Americans were true. Our band of veterans, which we have named the American Resistance, has taken out those foreign troops. We take full responsibility for doing so. We are now in control of Fort Carson, along with the command structure of the active duty troops stationed at this base. We call on all American military personnel, wherever located in the world, to join with us in resisting this illegitimate administration, rebuking them and removing them from office, by force of arms, if necessary. In his recent guest editorial, Richard McNally voices skepticism about the National Vietnam Veteran’s Readjustment Study (NVVRS) data reporting that over one-half of those who served in the Vietnam War have posttraumatic stress disorder (PTSD) or subclinical PTSD. Dr McNally is particularly skeptical because only 15% of soldiers served in combat units (1). He writes, “the mystery behind the discrepancy in numbers of those with the disease and of those in combat remains unsolved today” (4, p 815). He talks about bizarre facts and implies many, if not most, cases of PTSD are malingered or iatrogenic. Dr McNally ignores the obvious reality that when people are deployed to a war zone, exposure to trauma is not limited to members of combat units (2,3). At the Operational Trauma and Stress Support Centre of the Canadian Forces in Ottawa, we have assessed over 100 Canadian soldiers, many of whom have never been in combat units, who have experienced a range of horrific traumas and threats in places like Rwanda, Somalia, Bosnia, and Afghanistan. We must inform Dr McNally that, in real world practice, even cooks and clerks are affected when faced with death, genocide, ethnic cleansing, bombs, landmines, snipers, and suicide bombers ... One theory suggests that there is a conscious decision on the part of some individuals to deny trauma and its impact. The combat during the war was only a part of the horror. When soldiers came home, they were faced with new challenges, new fights to be won. When the anti-war public sentiment was strong, as it was during the Vietnam war, our brave soldiers came home to expressions of disdain and revulsion instead of the respect and honor they deserved. But perhaps the ultimate betrayal for veterans, who willingly risked their lives when their government asked, was making them fight to prove their sicknesses and disabilities were caused by the war in order to receive the free medical treatment they needed, or to be compensated. These were the worst indignities of war.

But a bureaucracy was nothing more than a machine for communal work, after all, and in those grueling and gorgeous weeks of the spring of 1961 the men and women of NASA’s Space Task Group for Project Mercury knew that bureaucracy, when coupled with a spiritual motivation, in this case true patriotism and profound concern for the life of the single-combat warrior himself—bureaucracy, poor gross hideously ridiculed twentieth-century bureaucracy, could take on the aura, even the ecstasy, of communion. The grisliest down-home alligator-poaching crackers manning the gasoline pumps on Route A1A would say to the tourists, as the No-Knock flowed, “Well, that Atlas vehicle’s given us more fits than a June bug on a porch bulb, but we got real confidence in that Redstone, and I think we’re gonna make it.” Everyone who felt the spirit of NASA at that time wanted to be part of it. It took on a religious dimension that engineers, no less than pilots, would resist putting into words. But all felt it.

In 1974 Freedman and Kaplan’s Comprehensive Textbook of Psychiatry stated that “incest is extremely rare, and does not occur in more than 1 out of 1.1 million people.”16 As we have seen in chapter 2 this authoritative textbook then went on to extol the possible benefits of incest: “Such incestuous activity diminishes the subject’s chance of psychosis and allows for a better adjustment to the external world. . . . The vast majority of them were none the worse for the experience.” How misguided those statements were became obvious when the ascendant feminist movement, combined with awareness of trauma in returning combat veterans, emboldened tens of thousands of survivors of childhood sexual abuse, domestic abuse, and rape to come forward. Consciousness-raising groups and survivor groups were formed, and numerous popular books, including The Courage to Heal (1988), a best-selling self-help book for survivors of incest, and Judith Herman’s book Trauma and Recovery (1992), discussed the stages of treatment and recovery in great detail.
