In 1994, Friedman wrote a memo marked “Very Confidential” to Raymond, Mortimer, and Richard Sackler. The market for cancer pain was significant, Friedman pointed out: four million prescriptions a year. In fact, there were three-quarters of a million prescriptions just for MS Contin. “We believe that the FDA will restrict our initial launch of OxyContin to the Cancer pain market,” Friedman wrote. But what if, over time, the drug extended beyond that? There was a much greater market for other types of pain: back pain, neck pain, arthritis, fibromyalgia. According to the wrestler turned pain doctor John Bonica, one in three Americans was suffering from untreated chronic pain. If that was even somewhat true, it represented an enormous untapped market. What if you could figure out a way to market this new drug, OxyContin, to all those patients? The plan would have to remain secret for the time being, but in his memo to the Sacklers, Friedman confirmed that the intention was “to expand the use of OxyContin beyond Cancer patients to chronic non-malignant pain.” This was a hugely audacious scheme. In the 1940s, Arthur Sackler had watched the introduction of Thorazine. It was a “major” tranquilizer that worked wonders on patients who were psychotic. But the way the Sackler family made its first great fortune was with Arthur’s involvement in marketing the “minor” tranquilizers Librium and Valium. Thorazine was perceived as a heavy-duty solution for a heavy-duty problem, but the market for the drug was naturally limited to people suffering from severe enough conditions to warrant a major tranquilizer. The beauty of the minor tranquilizers was that they were for everyone. The reason those drugs were such a success was that they were pills that you could pop to relieve an extraordinary range of common psychological and emotional ailments. Now Arthur’s brothers and his nephew Richard would make the same pivot with a painkiller: they had enjoyed great success with MS Contin, but it was perceived as a heavy-duty drug for cancer. And cancer was a limited market. If you could figure out a way to market OxyContin not just for cancer but for any sort of pain, the profits would be astronomical. It was “imperative,” Friedman told the Sacklers, “that we establish a literature” to support this kind of positioning. They would suggest OxyContin for “the broadest range of use.” Still, they faced one significant hurdle. Oxycodone is roughly twice as potent as morphine, and as a consequence OxyContin would be a much stronger drug than MS Contin. American doctors still tended to take great care in administering strong opioids because of long-established concerns about the addictiveness of these drugs. For years, proponents of MS Contin had argued that in an end-of-life situation, when someone is in a mortal fight with cancer, it was a bit silly to worry about the patient’s getting hooked on morphine. But if Purdue wanted to market a powerful opioid like OxyContin for less acute, more persistent types of pain, one challenge would be the perception, among physicians, that opioids could be very addictive. If OxyContin was going to achieve its full commercial potential, the Sacklers and Purdue would have to undo that perception.

Edwards painted a portrait of the Trinity in which the love and joy of the three divine persons was so full and intense, it simply could not be contained. God’s fullness thus yearned to be expressed and replicated by sharing it with others. So this fullness overflowed, as it were, as God brought forth a creation that mirrored his triune beauty. And the pinnacle of this creation is created beings whose yearning for God mirrors, in a small way, his yearning for them. But whereas God’s yearning comes out of his fullness, our yearning comes out of emptiness. It’s a beautiful arrangement. The God of overflowing love longs to pour his love into others, so he creates beings that long for his love to be poured into them. But in my opinion (not Edwards’s), it wasn’t God’s original intention for us to ever go a moment with this longing unsatisfied. Living without the fullness of God’s love is a reality we have brought on ourselves through our rebellion, and it’s completely unnatural to us. And try as we may to run from it or numb it, the pain of our unnatural emptiness is acute and incurable. The profundity of our emptiness is the negative reflection of the profundity of the fullness of the One we long for.

Mr. Lecky's maniacs lay in wait to slash a man's head half off, to perform some erotic atrocity of disembowelment on a woman. Here, they fed thoughtlessly on human flesh; there, wishing to play with him, they plucked the mangled Tybalt from his shroud. The beastly cunning of their approach, the fantastic capriciousness of their intention could not be very well met or provided for. In his makeshift fort everywhere encircled by darkness, Mr. Lecky did not care to meditate further on the subject.

At the same time that he was devising a response to the Afghanistan incursion, Carter had to confront a much more acute crisis in Iran, where he had brought the greatest disaster of his presidency down upon himself. In November 1977, he welcomed the shah of Iran to the White House, and on New Year’s Eve in Tehran, raising his glass, he toasted the ruler. Though the shah was sustained in power by a vicious secret police force, Carter praised him as a champion of “the cause of human rights” who had earned “the admiration and love” of the Iranian people. Little more than a year later, his subjects, no longer willing to be governed by a monarch imposed on them by the CIA, drove the shah into exile. Critically ill, he sought medical treatment in the United States. Secretary of State Cyrus Vance warned that admitting him could have repercussions in Iran, and Carter hesitated. But under pressure from David Rockefeller, Henry Kissinger, and the head of the National Security Council, Zbigniew Brzezinski, he caved in. Shortly after the deposed shah entered the Mayo Clinic, three thousand Islamic militants stormed the US embassy compound in Tehran and seized more than fifty diplomats and soldiers. They paraded blindfolded US Marine guards, hands tied behind their backs, through the streets of Tehran while mobs chanted, “Death to Carter, Death to the Shah,” as they spat upon the American flag and burned effigies of the president—scenes recorded on camera that Americans found painful to witness.

A study in the Journal of Neuroscience tested pain sensitivity in women at different times during their menstrual cycle—first during their period when estradiol is at its lowest and then when their estradiol levels were at their highest. The women in the study were subjected to a controlled amount of pain and asked to rate the level of their discomfort. At low levels of estradiol, the women reported feeling much more pain than when the hormone was at its highest. The implication is that when your estrogen levels are low, such as during menopause or during the premenstrual or menstrual phase of your cycle, you are likely to feel pain more acutely, which is also likely true for emotional pain. Just one more reason a smart man is especially sensitive at this time!

But I don't know anyone who has an easy life forever. Everyone I know gets their heart broken sometime, by something. The question is not, will my life be easy or will my heart break? But rather, when my heart breaks, will I choose to grow? Sometimes in the moments of the most searing pain, we think we don't have a choice. But we do. It's in those moments that we make the most important choice: grow or give up. It's easy to want to give up under the weight of what we're carrying. It seems sometimes like the only possible choice. But there's always, always, always another choice, and transformation is waiting for us just beyond that choice. This is what I know: God can make something beautiful out of anything, out of darkness and trash and broken bones. He can shine light into even the blackest night, and he leaves glimpses of hope all around us. An oyster, a sliver of moon, one new bud on a black branch, a perfect tender shoot of asparagus, fighting up through the dirt for the spring sun. New life and new beauty are all around us, waiting to be discovered, waiting to be seen. I'm coming to think there are at least two kinds of pain. There's the anxiety and fear I felt when we couldn't sell our house. And then there's the sadness I felt when I lost the baby or when my grandma passed away. Very different kinds of pain. The first kind, I think, is the king that invites us to grow. The second kind is the kind that invites us to mourn. God's not trying to teach me a lesson through my grandma's death. I wasn't supposed to love her less so the loss hurt less acutely, I'm not supposed to feel less strongly about the horror of death and dying. When we lose someone we love, when a dear friend moves away, when illness invades, it's right to mourn. It's right to feel deep, wrenching sadness. But then there's the other kind of pain, that first kind. My friend Brian says that the heart of all human conflict is the phrase "I'm not getting what I want." When you're totally honest about the pain, what's at the center? Could it be that you're not getting what you want? You're getting an invitation to grow, I think, as unwelcome as it may be. It's sloppy theology to think that all suffering is good for us, or that it's a result of sin. All suffering can be used for good, over time, after mourning and healing, by God's graciousness. But sometimes it's just plain loss, not because you needed to grow, not because life or God or anything is teaching you any kind of lesson. The trick is knowing the difference between the two.

Crying is therapeutic Most people can relate to the calming and stress reducing effect of a “good cry.” Grieving children should be supported in their need to cry. Unfortunately, children sometimes suppress their tears, thinking that they can control their pain if they control their crying. Parents may find their child’s pain very stressful or threatening and may therefore knowingly or unknowingly suppress natural expressions of grief. They may try to distract the child by promising a treat if he stops crying; cutting the feelings short (“Hush, hush”); minimizing the feelings (“You’re OK now”); contradicting his reality (“You’re going to love it here”); criticizing (“Stop making such a fuss”); embarrassing (“You’re too big to act like such a baby”); or threatening (“Stop it right now or I’ll give you something to cry about”). Crying should be supported with empathy and nurturing. It might be helpful to say something like, “I can tell that you are feeling very bad. Maybe it is because we were just looking at pictures of Nana, and you’re thinking about her now and missing her. Let’s sit here together for a while and I’ll rub your back.” Don’t rush the toddler’s grief before she is ready to let go of it. When the crying has subsided, offer a cold glass of juice or a walk outside. Often, children are more receptive to being cuddled, making eye contact, and other attachment strategies after an episode of acute sadness.

So first Diana, then William gave her a kiss and an affectionate hug. The way the Princess introduced her boys to her dying friend, Adrian Ward-Jackson, was a practical lesson in seeing the reality of life and death. When Diana told her eldest son that Adrian had died, his instinctive response revealed his maturity. “Now he’s out of pain at last and really happy.” At the same time the Princess is acutely aware of the added burdens of rearing two boys who are popularly known as “the heir and the spare.” Self-discipline is part of the training. Every night at six o’clock the boys sit down and write thank-you notes or letters to friends and family. It is a discipline which Diana’s father instilled in her, so much so that if she returns from a dinner party at midnight she will not sleep easily unless she has penned a letter of thanks. Sometimes our heart is moved to tenderness when we consider God's great wisdom with regard to each of us. At other times we can be softened only through suffering. We may then feel utterly destroyed to the point of despairing of life itself. But if, with help from on high, we gather the strength to turn to God at that point, He will find an opening so as to enter our heart with His grace. Nowadays, people are familiar with acute suffering of one kind or another. Pain and suffering have become the common language of humankind. God may well be using this to break through into our hardened and loveless world with His grace and the result would be a spiritual renaissance. He has already changed the lives of so many people, and He can easily change many more, and even extend it to the whole world. We might also be moved by the thought that our life has been nothing but a chain of errors, a chain of betrayals, a long series of failures. We see that nothing in our life is worthy of God and nothing in us is fit to look upon the Face of God. Neither can He look upon us, nor can we stand in His presence. If we turn to God with such awareness He will unfailingly find and opening for His grace. In this way, we can make a new start at any moment of our life, for we know that our God will respond. But if He is to visit man with His grace, He will first require our cooperation. He requires the presence of our human factor, little and weak though it is. He created us out of nothing, but He does not create us anew unless we consent to work with Him. Our tiny human factor is absolutely necessary to Him, so we need to align it with His infinitely great and divine factor, and the miracle of the union of our heart with His grace will then take place.
