The process of letting go and achieving relaxation can sometimes feel uncomfortable. But it is this uncomfortable feeling that has to be worked through to achieve success. 3. Learn diaphragmatic breathing. Diaphragmatic breathing is the basis of all relaxation and internal self-regulation. Often, breathing exercises of this type are in and of themselves a good means of stress management. Start breathing deeply to slow your body and mind down in preparation for relaxation. Conscious breathing is an essential part of this exercise. Inhale through nose, draw slowly into stomach (diaphragmatic region) and exhale through your mouth. This process should be done slowly and rhythmically. 4. Learn muscle relaxation. This is fairly easy to learn. The first step is to become aware of the difference between tense muscles and relaxed muscles. Then, learn to make your muscles feel limp and heavy. 5. Cultivate warm, dry hands. As you relax, your blood vessels dilate and the peripheral blood flow (at the skin’s surface) increases, resulting in warm hands. Anxiety is related to the fight-or-flight response. When confronted with stress, the body naturally sends blood away from extremities toward the torso in preparation for escape. While normal body temperature is 98.6 degrees, hand temperature is slightly cooler, and varies considerably depending on the degree of stress or relaxation. Don’t confuse the two—extremities are always cooler. Remember the mood rings of the 1970s? True, they were a gimmick, but they relied on stress-related surface temperature changes to create the desired effect. Bio-dots and stress cards available today work the same way, and can be a useful tool in learning to bring yourself down from an anxiety state. Still, you may not need a machine or other equipment to tell you how cold your hands are. If your hands feel cold to you, they are responding to stress. If your hands are warm and dry, you’ve achieved relaxation.

We also removed protected health information from the discharge summaries, diagnostic reports, and the approximately 700,000 free-text nursing and respiratory notes in MIMIC-II using an automated algorithm that has been shown to have superior performance in comparison to clinicians in detecting protected health information [4]. This algorithm accommodates the broad spectrum of writing styles in our data set, including personal variations in syntax, abbreviations, and spelling. We have posted the algorithm in open-source form as a general tool to be used by others for de-identification of free-text notes [5].

People reared in modern scientific societies may consider it only natural to ponder some feature of the world—the weather, say—and try to come up with a mechanistic explanation couched in the abstract language of natural law. But evolutionary psychology suggests that a much more natural way to explain anything is to attribute it to a humanlike agent. This is the way we’re “designed” by natural selection to explain things. Our brain’s capacity to think about causality—to ask why something happened and come up with theories that help us predict what will happen in the future—evolved in a specific context: other brains. When our distant ancestors first asked “Why,” they weren’t asking about the behavior of water or weather or illness; they were asking about the behavior of their peers. That’s a somewhat speculative (and, yes, hard-to-test!) claim. We have no way of observing our prehuman ancestors one or two or three million years ago, when the capacity to think explicitly about causality was evolving by natural selection. But there are ways to shed light on the process. For starters, we can observe our nearest nonhuman relatives, chimpanzees. We didn’t evolve from chimps, but chimps and humans do share a common ancestor in the not-too-distant past (4 to 7 million years ago). And chimps are probably a lot more like that common ancestor than humans are. Chimps aren’t examples of our ancestors circa 5 million BCE but they’re close enough to be illuminating. As the primatologist Frans de Waal has shown, chimpanzee society shows some clear parallels with human society. One of them is in the title of his book Chimpanzee Politics. Groups of chimps form coalitions—alliances—and the most powerful alliance gets preferred access to resources (notably a resource that in Darwinian terms is important: sex partners). Natural selection has equipped chimps with emotional and cognitive tools for playing this political game. One such tool is anticipation of a given chimp’s future behavior based on past behavior. De Waal writes of a reigning alpha male, Yeroen, who faced growing hostility from a former ally named Luit: “He already sensed that Luit’s attitude was changing and he knew that his position was threatened.” 8 One could argue about whether Yeroen was actually pondering the situation in as clear and conscious a way as de Waal suggests. But even if chimps aren’t quite up to explicit inference, they do seem close. If you imagine their politics getting more complex (more like, say, human politics), and them getting smarter (more like humans), you’re imagining an organism evolving toward conscious thought about causality. And the causal agents about which these organisms will think are other such organisms, because the arena of causality is the social arena. In this realm, when a bad thing happens (like a challenge for Yeroen’s alpha spot) or a good thing happens (like an ally coming to Yeroen’s aid), it is another organism that is making the bad or good thing happen.

Years ago, I represented a client, a firefighter/paramedic, in an administrative trial after he had been terminated for allegedly providing patient care that was below the department’s established standards. One central issue was the ongoing, on-the-job training firefighters/paramedics receive. Throughout the trial, senior officers of the department, including the Chief himself, preached and bloviated on and on about how the department is committed to providing only the best patient care and how their paramedics are held to a higher standard; how they are committed to serving the community with the highest level of blah, blah, blah. On cross examination, however, I asked each of them about how many hours a day each provider spends drilling or practicing firefighting technique and equipment. Each of them answered proudly that every firefighter/EMT and firefighter/paramedic, regardless of assignment, spends at least three hours each day practicing firefighting skills and/or rehearsing the use of various firefighting equipment; hoses, ladders, saws, and other firefighter equipment. Ok, that’s great. Through testimony, we determined that, based on a 10-shift work month, each firefighter/paramedic, regardless of assignment, spends at least 30 hours per month drilling, practicing, and/or rehearsing firefighting skills & equipment. That’s at a minimum of 360 hours per year of ongoing, on-the-job firefighter training. Outstanding. When the smoke is showing and the flames are roiling, they will be ready. They all displayed the same proud grin at how well trained their people are. For each of them, however, that smug grin quickly turned when I then asked about the number of hours per day each firefighter/paramedic spends drilling on or practicing patient care related techniques, skills, and tools. Every one of them squirmed as they responded with the truth that the department only offers three hours of patient care related education per month. That’s roughly a maximum of 36 hours of paramedic training for the entire year. It got worse when further testimony showed that patient care related calls account for more than 80 percent of their call volume and fire related calls less than 20 percent, I could see each of them deflate on the witness stand when I asked how they could truthfully say they were committed to providing the best patient care when barely 10 percent of their training addresses patient care, which constitutes over 80 percent of your department’s calls. The answers were more disjointed and nonsensical than a White House press briefing. Of course, across America the 10:1 ratio of ongoing firefighting training to EMS training is pretty consistent, which begs the question: Don’t they get it? Excellence is the product of practice. How can any rational person look at a 10:1 training ratio and declare themselves committed to the highest level of care? How can an agency neglect training on the most significant aspect of the business and then be surprised when issues of negligence and liability arise? Once again, it seems that old-school culture leaves EMS stuck in the mud and the law is not going to wait for agencies to figure out that living in the past compromises the future.
