We go through the futile process of asking for opinions and fish for compliments because we crave approval. We want to believe that the support and sign-off of someone we respect means our venture will succeed. But really, that person’s opinion doesn’t matter. They have no idea if the business is going to work. Only the market knows. You’re searching for the truth, not trying to be right. And you want to do it as quickly and cheaply as possible. Peter Navarro never hid his antagonism toward me. If you are wrong, thousands of people die.” The truth was the exact opposite. By that time, the FDA, which had given hydroxychloroquine emergency approval early in the pandemic, had revoked it on June 15, after it was found to cause heart problems and even death, not to mention proving ineffective against COVID. Virtually every newspaper in America has a daily column on astrology; there are hardly any that have even a weekly column on astronomy. There are ten times more astrologers in the United States than astronomers. At parties, when I meet people that do not know I’m a scientist, I am sometimes asked “Are you a Gemini?” (chances of success, one in twelve), or “What sign are you?” Much more rarely am I asked “Have you heard that gold is made in supernova explosions?” or “When do you think Congress will approve a Mars Rover?” (...) And personal astrology is with us still: consider two different newspaper astrology columns published in the same city on the same day. For example, we can examine The New York Post and the New York Daily News on September 21, 1979. Suppose you are a Libra - that is, born between September 23 and October 22. According to the astrologer for the Post, ‘a compromise will help ease tension’; useful, perhaps, but somewhat vague. According to the Daily News’ astrologer, you must ‘demand more of yourself’, an admonition that is also vague but also different. These ‘predictions’ are not predictions; rather they are pieces of advice - they tell you what to do, not what will happen. Deliberately, they are phrased so generally that they could apply to anyone. And they display major mutual inconsistencies. Why are they published as unapologetically as sport statistics and stock market reports? Astrology can be tested by the lives of twins. There are many cases in which one twin is killed in childhood, in a riding accident, say, or is struck by lightning, while the other lives to a prosperous old age. Each was born in precisely the same place and within minutes of the other. Exactly the same planets were rising at their births. If astrology were valid, how could two such twins have such profoundly different fates? It also turns out that astrologers cannot even agree among themselves on what a given horoscope means. In careful tests, they are unable to predict the character and future of people they knew nothing about except their time and place of birth.

Another example is tamoxifen, which is used for treatment of endocrine responsive breast cancer. Tamoxifen is given to patients postsurgery and dramatically reduces the rate of cancer recurrence. This drug is metabolized by cytochrome P450 2D6, the product of the CYP2D6 gene. Based on their DNA, there are patients with little CYP2D6 activity who are poor metabolizers and others with high activity who are extensive metabolizers. An FDA-approved genetic test exists for finding the variants of the CYP2D6 gene to help guide tamoxifen administration, but the lack of study data demonstrating its role in improving patient outcomes has, to date, led insurance companies to refuse to cover the test. Beyond having ramifications for drug efficacy, genetics also may play a role in the side effects of drugs.

The talk then turned to the topic of risk. The dozen or so regulatory agencies that had to approve the flight test did not share Musk’s love of it. The test would involve inserting all 211 control rods part-way, creating a power level low enough to resemble a blackout while continuing to cool the reactor to compensate for fission products. Use the residual steam in the system to drive a turbine, then isolate it and allow it to run down, generating electricity through its own inertia. The electrical output would be measured, allowing engineers to determine whether it was sufficient to power the water pumps in an emergency. Because the deliberately low power levels would appear to be a power failure to the control computer, which would then automatically activate the safety systems, these systems, including the backup diesel generators and Emergency Core Cooling System (ECCS), were disconnected in order to re-attempt the test straight away if it proved unsuccessful. Otherwise, the ECCS would automatically shutdown the reactor, preventing a repeat of the test for another year. Astonishingly, these measures were not in violation of safety procedures when approved by a Deputy Chief Engineer, despite many subsequent reports to the contrary.96

Eton’s great strength is that it does encourage interests--however wacky. From stamp collecting to a cheese-and-wine club, mountaineering to juggling, if the will is there than the school will help you. Eton was only ever intolerant of two things: laziness and a lack of enthusiasm. As long as you got “into something,” then most other misdemeanors were forgivable. I liked that: it didn’t only celebrate the cool and sporty, but encouraged the individual, which, in the game of life, matters much more. Hence Eton helped me to go for the Potential Royal Marines Officer Selection Course, age only sixteen. This was a pretty grueling three-day course of endless runs, marches, mud yomps, assault courses, high-wire confidence tests (I’m good at those!), and leadership tasks. At the end I narrowly passed as one of only three out of twenty-five, with the report saying: “Approved for Officer Selection: Grylls is fit, enthusiastic, but needs to watch out that he isn’t too happy-go-lucky.” (Fortunately for my future life, I discarded the last part of that advice.) But passing this course gave me great confidence that if I wanted to, after school, I could at least follow my father into the commandos.

Outlawing drugs in order to solve drug problems is much like outlawing sex in order to win the war against AIDS. We recognize that people will continue to have sex for nonreproductive reasons despite the laws and mores. Therefore, we try to make sexual practices as safe as possible in order to minimize the spread of the AIDS viruses. In a similar way, we continually try to make our drinking water, foods, and even our pharmaceutical medicines safer. The ubiquity of chemical intoxicants in our lives is undeniable evidence of the continuing universal need for safer medicines with such applications. While use may not always be for an approved medical purpose, or prudent, or even legal, it is fulfilling the relentless drive we all have to change the way we feel, to alter our behavior and consciousness, and, yes, to intoxicate ourselves. We must recognize that intoxicants are medicines, treatments for the human condition. Then we must make them as safe and risk free and as healthy as possible. Dream with me for a moment. What would be wrong if we had perfectly safe intoxicants? I mean drugs that delivered the same effects as our most popular ones but never caused dependency, disease, dysfunction, or death. Imagine an alcohol-type substance that never caused addiction, liver disease, hangovers, impaired driving, or workplace problems. Would you care to inhale a perfumed mist that is as enjoyable as marijuana or tobacco but as harmless as clean air? How would you like a pain-killer as effective as morphine but safer than aspirin, a mood enhancer that dissolves on your tongue and is more appealing than cocaine and less harmful than caffeine, a tranquilizer less addicting than Valium and more relaxing than a martini, or a safe sleeping pill that allows you to choose to dream or not? Perhaps you would like to munch on a user friendly hallucinogen that is as brief and benign as a good movie? This is not science fiction. As described in the following pages, there are such intoxicants available right now that are far safer than the ones we currently use. If smokers can switch from tobacco cigarettes to nicotine gum, why can’t crack users chew a cocaine gum that has already been tested on animals and found to be relatively safe? Even safer substances may be just around the corner. But we must begin by recognizing that there is a legitimate place in our society for intoxication. Then we must join together in building new, perfectly safe intoxicants for a world that will be ready to discard the old ones like the junk they really are. This book is your guide to that future. In July 2018, a safety crisis rocked the global drug supply—and seemed to prove Baker’s point. Regulators in Europe announced a harrowing discovery: the widely used active ingredient for valsartan, a generic version of the blood pressure drug Diovan, contained a cancer-causing toxin known as NDMA (once used in liquid rocket fuel). The drug had been made by the Chinese company Zhejiang Huahai Pharmaceuticals, the world’s largest manufacturer of valsartan active ingredients. In the United States, over a dozen drug manufacturers, all of which used the Chinese ingredient, recalled their products, as did dozens more manufacturers around the world. The Chinese company tried to defend itself by explaining that it had altered its production process in 2012 to increase yields of the drug, a change that had been approved by regulators. In short, the change had been made to maximize profit. Some patients had been consuming the toxin for six years. As the FDA tried to reassure consumers that the risk of developing cancer, even from daily exposure to the toxin, was extremely low, a second cancer-causing impurity was detected in the ingredients. Though the valsartan catastrophe seemed to take the FDA by surprise, it shouldn’t have. In May 2017, an FDA investigator had found evidence at the plant in Linhai, China, that the company was failing to investigate potential impurities in its own drugs, which showed up as aberrant peaks in its test results. The investigator designated the plant as Official Action Indicated, but the agency downgraded that to VAI. In short, the company was let off the hook—only to wind up in the middle of a worldwide quality scandal less than a year later. By

This was classic Musk. The e-mail is rough in its tone and yet not really unwarranted for a guy who just wants things done as efficiently as possible. It obsesses over something that other people might find trivial and yet he has a definite point. It’s comical in that Musk wants all acronym approvals to run directly through him, but that’s entirely in keeping with the hands-on management style that has, mainly, worked well at both SpaceX and Tesla. Employees have since dubbed the acronym policy the ASS Rule.

As Graedon scrutinized the FDA’s standards for bioequivalence and the data that companies had to submit, he found that generics were much less equivalent than commonly assumed. The FDA’s statistical formula that defined bioequivalence as a range—a generic drug’s concentration in the blood could not fall below 80 percent or rise above 125 percent of the brand name’s concentration, using a 90 percent confidence interval—still allowed for a potential outside range of 45 percent among generics labeled as being the same. Patients getting switched from one generic to another might be on the low end one day, the high end the next. The FDA allowed drug companies to use different additional ingredients, known as excipients, that could be of lower quality. Those differences could affect a drug’s bioavailability, the amount of drug potentially absorbed into the bloodstream. But there was another problem that really drew Graedon’s attention. Generic drug companies submitted the results of patients’ blood tests in the form of bioequivalence curves. The graphs consisted of a vertical axis called Cmax, which mapped the maximum concentration of drug in the blood, and a horizontal axis called Tmax, the time to maximum concentration. The resulting curve looked like an upside-down U. The FDA was using the highest point on that curve, peak drug concentration, to assess the rate of absorption into the blood. But peak drug concentration, the point at which the blood had absorbed the largest amount of drug, was a single number at one point in time. The FDA was using that point as a stand-in for “rate of absorption.” So long as the generic hit a similar peak of drug concentration in the blood as the brand name, it could be deemed bioequivalent, even if the two curves reflecting the time to that peak looked totally different. Two different curves indicated two entirely different experiences in the body, Graedon realized. The measurement of time to maximum concentration, the horizontal axis, was crucial for time-release drugs, which had not been widely available when the FDA first created its bioequivalence standard in 1992. That standard had not been meaningfully updated since then. “The time to Tmax can vary all over the place and they don’t give a damn,” Graedon emailed a reporter. That “seems pretty bizarre to us.” Though the FDA asserted that it wouldn’t approve generics with “clinically significant” differences in release rates, the agency didn’t disclose data filed by the companies, so it was impossible to know how dramatic the differences were.

The detainee wills nothing, attempts nothing, merely stares at blankness as his mind opens toward such signs as might appear. Something, nothing. However little, however great, whatever God vouchsafes is sufficient. The least sign is enough. A crease in the plastic. A shadow cast against its insensate skin, then fleeing, gone. A raindrop: trickling through the roof, one small drop might touch the wall, leave a transparent streak, a tear without sorrow to confirm his understanding of what is and must be. Recognition. Acceptance. By such a sign he will know he is not forsaken. That God notices and prepares a place. He will not serve in the harvest. He will eat the food, drink the water, ride the bus. He will not pick the berries so prized by his captors. Droids will cajole and threaten; perhaps they will beat him. If so, they incriminate themselves. He relishes their degradation together with God’s tasking, this new test of will and faith. To suffer in silence, as meek as a lamb. Ibn al Mohammed will remove himself from himself. Self fading into background, his presence will diminish. His body will persist; corporeally, he must endure. But his self will become absent. Mind and its thought, heart and all emotion will disperse smoke-like into nothingness and in its vanishing forestall injury, indignity, all pain. Does God approve? Does God see? A mere token will assure Ibn al Mohammed for a lifetime. Standing at the bars, he watches. Minutes pass. How long must he wait? God speaks at His leisure to those with patience to attend. What does it mean, to have enough patience to attend to God? It is a discipline to expect nothing because you deserve nothing and merit only death. Ibn al Mohammed has waited all his life. What has he seen? His father taken away. His mother and sisters scrounging in a desert. He himself is confined in-cage. Squats on a stool, shits in a pail. Rain rattles across sheet tin, pock-pock-pock-pock. Food is delivered on a tray. A damp bed beneath his body, a white wall before his eyes. What does Ibn al Mohammed see? He sees nothing. But as she rounded the last turn before the hall landing, she nearly collided with Sir Ian, carrying his mother’s shawl. “Oh!” Lina exclaimed, coming to an abrupt halt a step above his. “Rather careless of you to leave this behind,” he said. His eyes danced. “Mayhap I should demand a penance before returning it.” “You dare,” she said, stiffening and wishing he were not so fiendishly beguiling with that boyish gleam of mischief in his eyes. He looked up, as if to heaven, and murmured, “Just one wee ki—” “Shame on you, Sir Ian Colquhoun,” she interjected, thinking she sounded just like her mother. “Galbraith cannot know that you are on this stairway.” “Once again, you are wrong, lass,” he said, his eyes still alight. “He is still with Lizzie on the dais—giving her a well-deserved scolding, I trust. I saw that you had left the shawl and offered to find a maidservant to return it to you. But this is much better. I do think you should thank me prettily for taking so much trouble.” “I will thank you. After you have returned it to me.” Cocking his head, he held the shawl higher, so she’d have to reach for it. When she did, he moved it back out of her reach. Lina lowered her outstretched hand to her side and eyed him sternly from her slightly superior height. “I thought you sought my approval.” He stepped up to the stair below hers, putting the shawl out of reach again. His face was now inches higher than hers and his body again much too close for comfort. “I’d prefer something else just now,” he said softly, looking into her eyes. Reaching with his left hand for her right wrist, he held it firmly. Apparently oblivious of her attempt to snatch it free, he pressed the shawl into her hand and let go of her wrist, his gaze never leaving hers. She waited to see what he would do next. He smiled then, wryly, as if he dared her to walk away. His lips were tantalizingly close. Lina shut her eyes. “Coward,” Ian murmured, enjoying himself. Her eyes flew open. Then, to his astonishment, she learned forward, brushed her lips against his right cheek, and whirled, snatching up her skirts in her free hand as first her right foot and then her left blindly sought the next stair upward. Reaching out, he easily caught her arm. “Not so fast,” he said, turning her back to face him. “You must not kiss and run, lass. That’s against the rules.” “The lady makes the rules, sir. Let go of me.” She was two steps above his again, looking disdainfully down her nose at him. She did not try to pull away. She was testing him, he knew. But she was right about who made the rules. Even so, the urge was strong to seize her and teach her what kissing was all about. However, he also wanted to make her desire that kiss enough to abandon her disapproval. And that was the greater challenge. Sakes, if he were seeking a wife and had no royal duty commanding him . . . Shifting his grip to her hand, he drew it to his lips and slowly kissed each knuckle. Then he kissed the silky skin above them, turned her trembling hand palm up long enough to breathe gently into that tender palm . . . and released her. With a barely discernable gasp, she turned away, her dignity apparently still—or again—intact. He enjoyed watching her move, so he stood where he was to savor the sight. His reward came when she stopped before vanishing around the next curve and looked back. Her lips parted slowly, invitingly, in surprise. He bowed and had the delight of seeing her whirl again and hurry away. “I shall win this battle, I think,” he murmured to himself.

So, if I’d believed in Reiki, then I may have experienced some benefit from it, but I don’t, so I didn’t get anything out of the treatment. Mistakes to Avoid Even though Joseph was the instrument of God who brought deliverance from the famine, we need not think that God approved of everything that Joseph did or that we should imitate him as a biblical model. His policies for Egypt did not focus on sharing; they focused on redistribution. They certainly should not be considered a biblical model for economic policies today. One might also question Joseph’s strategy as he interacted with his brothers. He was not showing love to them; he was testing them. The text does not seek to approve or condemn—it simply reports. We cannot derive authoritative guidance from the text about how families are supposed to interact or how past wrongs should be confronted. God’s actions through Joseph are much more important than Joseph’s actions themselves. And since the agency hadn’t yet reviewed and approved it for commercial use, it could only be tested on human subjects under strict conditions set by an institutional review board. One of those conditions was that the test subjects give their informed consent—something that was notoriously hard to obtain in a war zone. Mattis was reluctant to give up. He wanted to know if they could suggest a way forward. As he’d put it to Elizabeth in an email a few months earlier, he was convinced her invention would be “a game-changer” for his men. Gutierrez and Shoemaker proposed a solution: a “limited objective experiment” using leftover de-identified blood samples from soldiers. It would obviate the need to obtain informed consent and it was the only type of study that could be put together as quickly as Mattis seemed to want to proceed. They agreed to pursue that course of action. Fifteen minutes after they’d walked in, Shoemaker and Gutierrez shook Mattis’s hand and walked out. Shoemaker was immensely relieved. All in all, Mattis had been gruff but reasonable and a workable compromise had been reached. The limited experiment agreed upon fell short of the more ambitious live field trial Mattis had had in mind. Theranos’s blood tests would not be used to inform the treatment of wounded soldiers. They would only be performed on leftover samples after the fact to see if their results matched the army’s regular testing methods. But it was something. Earlier in his career, Shoemaker had spent five years overseeing the development of diagnostic tests for biological threat agents and he would have given his left arm to get access to anonymized samples from service members in theater. The data generated from such testing could be very useful in supporting applications to the FDA. Yet, over the ensuing months, Theranos inexplicably failed to take advantage of the opportunity it was given. When General Mattis retired from the military in March 2013, the study using leftover de-identified samples hadn’t begun. When Colonel Edgar took on a new assignment as commander of the Army Medical Research Institute of Infectious Diseases a few months later, it still hadn’t started. Theranos just couldn’t seem to get its act together. In July 2013, Lieutenant Colonel Shoemaker retired from the army. At his farewell ceremony, his Fort Detrick colleagues presented him with a “certificate of survival” for having the courage to stand up to Mattis in person and emerging from the encounter alive. They also gave him a T-shirt with the question, “What do you do after surviving a briefing with a 4 star?” written on the front. The answer could be found on the back: “Retire and sail off into the sunset.

Nine years later, with Republicans now in control of Congress and the White House, it’s not hard to imagine this test case for privatized disaster response being adopted on a national scale.

Which leaves many towns in southern Florida no choice but to dig their sand from inland quarries and haul it to the coast one roaring, diesel-spewing truck at a time. Tourists and locals hate the noise and traffic, and county officials hate the extra cost, which can be easily double that of dredged sand. But it does have some advantages. The inland mines, with their elaborate sorting and washing machines, can deliver sand of a precise spec—the exact size, shape, and color county officials deem appropriate for the beach. Beach town residents and tourists alike are very particular about the color and consistency of their beaches. The sugary white-sand beach has become the global standard of perfection, and any resort falling short of it loses points. (That’s nothing compared to the fussiness of Olympic beach volleyball players. To make sure their bare feet come into contact only with grains of just the right size and shape, sand was brought in from Hainan Island for the 2008 Beijing Games, and from a quarry in Belgium for the 2004 Athens Games.)15 “You pump sand from the ocean floor, you don’t know what you’re getting,” said Eastman. That’s not exactly true; sea sand is examined closely to make sure it is suitable for a given beach before the regulatory agencies will allow it to be dredged for nourishment. But land-mined sand can be sorted, sifted, and cleaned to a uniform standard. The grains that Eastman was emplacing were all about the size of a salt grain, all the same silver gray, unadulterated with stones or shell fragments. Their color was approved using the Munsell color order system, a visual index of hues created in 1915. Therefore we currently have an incentive system in place to generate fake creativity: an incentive system in which there are un-creative people who dishonestly strive to be regarded as original because they want to appropriate the label of creative and usurp the title of ‘genius’. In sum, under modernity creativity has been reduced to novelty – and novelty can be simulated. It is trivially easy for clever and well-trained people to generate mere novelty, so there is an excess of it (we call it ‘fashion’). Therefore the discriminative test applied to novelties is whether they are approved by the social systems that allocate high status. When novelty is socially approved, then the person who generated it gets to be called creative – maybe even a creative genius. Thus: Novelty of outcome + Social Approval of that outcome = Fake creativity And fake creativity is an attribute bestowed upon an outcome or person; bestowed by the social systems for generating status – in other words the mass media (primarily), politics, civil administration, the legal system, education... in a nutshell the Leftist Establishment.

What do you do when another people group with proven military strength dominates you? What do you do when its cultural hegemony erodes one’s own culture and values? What do you do when you are being drawn into the very systems and societal patterns that are also taxing, exploiting, humiliating, and executing you on a regular basis? The uncritical or despair–filled stance is to adopt an “if you can’t beat them, join them” mentality. Paul, however, takes a more subversive posture: “I exhort you, brothers and sisters, by the mercies of God, to present your bodies as a sacrifice—alive, holy, and pleasing to God—which is your reasonable service. Do not be conformed to this present world, but be transformed by the renewing of your mind, so that you may test and approve what is the will of God—what is good and well–pleasing and perfect” (Romans 12:1–2). These two verses challenge us with an embodied, decolonizing way of life that refuses to join the oppressive systems that manage and puppet most people’s lives. First, we are told that we must put our very bodies, through action, on the line. Our bodies must become living sacrifices. Our bodies, and what we do with them, actually matter. We are not disembodied souls, and God cares about more than our spiritual lives. God says, Put your body on the line! What kind of bodily life will you engage in? Will your body be aligned with the rituals of American civil religion? Or will you vulnerably place your body in confrontation with the establishment, as Jesus did with his own body when he flipped tables in judgment of the injustice and idolatry in the temple? Apparently such bodily involvement is our reasonable service to God.

Little by little, as the team members stitched together small pieces of information, they stumbled into Ranbaxy’s secret: the company manipulated almost every aspect of its manufacturing process to quickly produce impressive-looking data that would bolster its bottom line. Each member of Thakur’s team came back with similar examples. At the behest of managers, the company’s scientists substituted lower-purity ingredients for higher ones to reduce costs. They altered test parameters so that formulations with higher impurities could be approved. They faked dissolution studies. To generate optimal results, they crushed up brand-name drugs into capsules so that they could be tested in lieu of the company’s own drugs. They superimposed brand-name test results onto their own in applications. For some markets, the company fraudulently mixed and matched data streams, taking its best data from manufacturing in one market and presenting it to regulators elsewhere as data unique to the drugs in their markets. For other markets, the company simply invented data. Document forgery was pervasive. The company even forged its own standard operating procedures, which FDA investigators rely on to assess whether a company is following its own policies. In one instance, employees backdated documents and then artificially aged them in a steamy room overnight in an attempt to fool regulators during inspections.

The report noted the “non-availability” in India and Latin America of validation methods, stability data, and bioequivalence reports. In short, Ranbaxy had almost no method for confirming the content of drugs in those markets. For example, the data collected by Thakur’s team showed that of the 163 drug products approved in Brazil since 2000, almost all had been filed with phony batch records and stability data that did not exist. The report noted that in a majority of regulatory filings, Ranbaxy had “intentionally misrepresented” small research and development batches (some two thousand doses) as exhibit batches one hundred times the size, and then deceptively performed crucial tests for bioequivalence and stability on the smaller, easier-to-control batches. The result was that its commercial-sized batches had not actually been tested before being sold, putting millions of patients at risk.
