I recently realized that pain is one of the few things that seems to really get my attention and that I have spent a lot of my time just coasting down life’s path. Perhaps this is the reason why I seem to grow the most during the hard times, even though it often takes all the energy I can muster just to get through them. Quite a few years ago, while I was visiting a friend who was dying from AIDS, I saw a tapestry on the hospital wall that read: The Chinese word for “crisis” has two characters. One stands for danger; the other for opportunity. The times in my life that have been the most difficult have quite often proven to be my best opportunities for growth; to get closer to becoming the person I am meant to be. There are also those who will simply never be able to understand my truth no matter what anyone says to them. However, others will hear the truth very clearly, understand it completely, and even care greatly. Moreover, I can hear, I understand, and I care. I have also learned that there are times when it is better to be silent. Sometimes words are just not necessary… Like when I am sharing with someone who already knows my heart. And then there are times when words are pointless… like when I have already spoken my truth to someone, yet they are simply not capable of hearing what it is that I am saying. Just as there are batterers who will victimize partner after partner, so are there serial victims, women who will select more than one violent man. Given that violence is often the result of an inability to influence events in any other way, and that this is often the result of an inability or unwillingness to effectively communicate, it is interesting to consider the wide appeal of the so-called strong and silent type. The reason often cited by women for the attraction is that the silent man is mysterious, and it may be that physical strength, which in evolutionary terms brought security, now adds an element of danger. The combination means that one cannot be completely certain what this man is feeling or thinking (because he is silent), and there might be fairly high stakes (because he is strong and potentially dangerous). I asked a friend who has often followed her attraction to the strong and silent type how long she likes men to remain silent. “About two or three weeks,” she answered, “Just long enough to get me interested. I like to be intrigued, not tricked. The tough part is finding someone who is mysterious but not secretive, strong but not scary.” One of the most common errors in selecting a boyfriend or spouse is basing the prediction on potential. This is actually predicting what certain elements might add up to in some different context: He isn’t working now, but he could be really successful. He’s going to be a great artist—of course he can’t paint under present circumstances. He’s a little edgy and aggressive these days, but that’s just until he gets settled. Listen to the words: isn’t working; can’t paint; is aggressive. What a person is doing now is the context for successful predictions, and marrying a man on the basis of potential, or for that matter hiring an employee solely on the basis of potential, is a sure way to interfere with intuition. That’s because the focus on potential carries our imagination to how things might be or could be and away from how they are now. Spousal abuse is committed by people who are with remarkable frequency described by their victims as having been “the sweetest, the gentlest, the kindest, the most attentive,” etc. Indeed, many were all of these things during the selection process and often still are—between violent incidents. But even though these men are frequently kind and gentle in the beginning, there are always warning signs. Victims, however, may not always choose to detect them.

First, I am thrilled that paramedics are finally getting the respect they deserve for being the professionals they can be. The scope of practice is expanding, and patient care modalities are improving, seemingly by the minute. Patient outcomes are also improving as a result, and EMS is passing through puberty and forging into adulthood. On the other hand, autonomy in the hands of the “lesser-motivated,” can be a very dangerous thing. You know as well as I do that there are still plenty of providers who operate from a subjective, complacent, and downright lazy place. Combined with the ever-expanding autonomy, that provider just became more dangerous than he or she ever has been – to the patients and to you. Autonomy in patient care places more pressure for excellence on the provider charged with delivering it, and also on the partner and crew members on scene. Since the base hospital is not involved like it once was, they are likewise less responsible for the errors and omissions of the medics on the scene. Now more than ever, crew members are being held to answer for the mistakes and follies of their coworkers; now more than ever, EMS providers are working without a net. What’s next? I predict (and hope) emergency medical Darwinism is going to force some painful and necessary changes. First, increasing autonomy is going to result in the better and best providing superior patient care. More personal ownership of the results is going to manifest in outcomes such as increased cardiac arrest survival rates, faster and more complete stroke recovery, and significantly better outcomes for STEMI patients, all leading to the brass ring: EMS as a profession, not just a job. On the flip side of that coin, you will see consequences for the not-so-good and completely awful providers. There will be higher instances of licensure action, internal discipline, and wash-out. Unfortunately, all those things will stem from generally preventable negative patient outcomes. The danger for the better provider will be in the penumbra; the murky, gray area of time when providers are self-categorizing. Specifically, the better provider who is aware of the dangerously poor provider but does nothing to fix or flush him or her, is almost certain to be caught up in a bad situation caused by sloppy, complacent, or ultimately negligent patient care that should have been corrected or stopped. The answer is as simple as it is difficult. If you are reading this, it is more likely because you are one of the better, more committed, more professional providers. This transition is up to you. You must dig deep and find the strength necessary to face the issue and force the change; you have to demand more from yourself and from those around you. You must have the willingness to help those providers who want it – and respond to those who need it, but don’t want it – with tough love by showing them the door. In the end, EMS will only ever be as good as you make it. If you lay silent through its evolution, you forfeit the right to complain when it crumbles around you.

Piotr had a book of logic—I think it was Wittgenstein translated into Polish—and for reasons best known to himself, he kept it in the freezer. This book was his favorite thing in the world. And every morning he would wake up with this imbecilic smile on his face, take his book out of the freezer, wait patiently until the page he wanted to read unfroze, read to us from it in Polish, then turn the page and put the book back in the freezer for the next day. Brigitte’s father had started the pornography industry in Sweden—a very big deal; the porn revolution really began there—and she hated her father; she hated everybody. She was a deeply depressed person: she literally never spoke a word. All of us in the flat ate all our meals together, and she would just sit there, completely silent. Then in the middle of the night one night, Edmondo knocked on our door. I opened it and said, “What’s wrong?” “She talks, she talks!” he said. “What did she say?” I asked. “She said, ‘Boo,’ ” he said. “That’s not much,” I said. The next morning, she packed and left. And then you fall asleep, rolled up in a sleeping bag against the considerable fall in temperature as the night goes on, perhaps waking an hour or two before dawn for just long enough to notice that those little stars are still there—as bright as ever—and do not even look as though they are getting ready to be extinguished by the advent of another day. It is a lovely, comforting feeling when the world around you is quite still; and there is no sound anywhere to penetrate the delightful peace that surrounds you. When the dawn comes, and the stars have all gone away, there is something sharp and exhilarating about the smell in the air. It is fresh and clean and tantalisingly different to the atmosphere which will pervade the day once the sun has come up over the distant horizon. Then there will be no escape from its merciless and desiccating heat, which drains you of energy and leaves you burned and incapable of any prolonged activity. And the bright reflection of the sun off the light-coloured sand can be piercing and painful to the eyes. There is probably not even a tiny breeze to move that sullen, sultry air, and there can be no relief from its effects until once more, and inevitably, the great ball of fire that is the sun will slide slowly below the land and allow it to grow cool. It would be foolish to pretend that all of those who served with the LRDG saw the desert in the way that I have described it, all or even much of the time. But I am quite sure that when their minds were not diverted by rather more pressing considerations concerning the enemy, there were few who were not moved by the beauty of the sky at night. They all spent quite a number of hours on sentry duty, when, alone with his thoughts and in such surroundings, no man can be oblivious of such a miraculous revelation.

Mulling it over, I wasn’t really listening to Ephraim as he talked on the radio upstairs. But at some point I became aware that his voice was raised. ‘They were expected days ago, you know that. It was always going to be tough. With such a small window of time they’d have to be incredibly quick,’ he was saying. ‘No, I’ve not had any contact… no… not a word.’ I moved to the bottom of the stairs to listen properly. ‘The weather was set fair so that shouldn’t have been… She had the co-ordiantes… Yes, I know the whole north coast is German-occupied, that’s whny they had to act fast.
