Much of clinician burnout is due to spending time writing notes, placing orders, generating referrals, writing prior authorization letters, and creating patient communication. In other words, burnout is caused by physicians having to generate output! With the emergence of large language models that are used to train generative AI solutions, these use cases will be at the frontier of AI’s applications in healthcare.
Ronald M. Razmi (AI Doctor: The Rise of Artificial Intelligence in Healthcare - A Guide for Users, Buyers, Builders, and Investors)
Seeing modern health care from the other side, I can say that it is clearly not set up for the patient. It is frequently a poor arrangement for doctors as well, but that does not mitigate how little the system accounts for the patient's best interest. Just when you are at your weakest and least able to make all the phone calls, traverse the maze of insurance, and plead for health-care referrals is that one time when you have to — your life may depend on it.
Ross I. Donaldson (The Lassa Ward: One Man's Fight Against One of the World's Deadliest Diseases)
I mean, how many people have jobs where they can say “I didn’t sleep last night because the mice wouldn’t stop talking” and get sympathy rather than a referral to a psychiatrist?

John Green (Turtles All the Way Down)
The farther right you go on the curve, the more you will encounter the clients and customers who may need what you have, but don't necessarily believe what you believe. As clients, they are the ones for whom, no matter how hard you work, it's never enough. Everything usually boils down to price with them. They are rarely loyal. They rarely give referrals and sometimes you may even wonder out loud why you still do business with them. "They just don't get it," our gut tells us. The importance of identifying this group is so that you can avoid doing business with them.

The referral is the easiest prospect in the world to sell. Ask any professional who hates selling (accountants, architects, lawyers) -- they'll tell you that 100% of their new business comes from referrals. That's because they're not capable of making sales calls and rely on the fall-in-your-lap method of selling.

Conviction rates in the military are pathetic, with most offenders going free AND THERE IS NO RECOURSE FOR APPEAL! The military believes the Emperor has his clothes on, even when they are down around his ankles and he is coming in the woman's window with a knife! Military juries give low sentences or clear offender's altogether. Women can be heard to say “it's not just me” over and over. Men may get an Article 15, which is just a slap on the wrist, and doesn't even follow them in their career. This is hardly a deterrent. The perpetrator frequently stays in place to continue to intimidate their female victims, who are then treated like mental cases, who need to be discharged. Women find the tables turned, letters in their files, trumped up Women find the tables turned, letters in their files, trumped up charges; isolation and transfer are common, as are court ordered psychiatric referrals that label the women as lying or incompatible with military service because they are “Borderline Personality Disorders” or mentally unbalanced. I attended many of these women, after they were discharged, or were wives of abusers, from xxx Air Force Base, when I was a psychotherapist working in the private sector. That was always their diagnosis, yet retesting tended to show something different after stabilization, like PTSD.

Don’t be afraid to say no to projects. Prove that you’re serious about specialization by turning down work that falls outside your area of expertise. The more people you say no to, the more referrals you’ll get to people who need your product or service.

The very best people aren’t out there looking for work. Great-performing people are happy and being amply rewarded where they are today. They don’t occur to people as referrals, because why would you bother referring someone who is happy at their current job?

You can't be responsible for anyone but you, Cole. We all make our own decisions, and pay our own consequences. You can love someone, but you can't fix them. You deserve me because I say you do, not because other people think so or don't.
Lyla Payne (By Referral Only (Whitman University, #2))
The rite, the becoming-animal of the scapegoat clearly illustrates this: a first expiatory animal is sacrificed, but a second is driven away, sent out into the desert wilderness. In the signifying regime, the scapegoat represents a new form of increasing entropy in the system of signs: it is charged with everything that was "bad" in a given period, that is, everything that resisted signifying signs, everything that eluded the referral from sign to sign through the different circles; it also assumes everything that was unable to recharge the signifier as its center and carries off everything that spills beyond the outermost circle.

Probably no other meeting we hold in the Church has the high referral and future baptismal harvest that a baptismal service does. Many of the investigators who attend a baptismal service (that is, the service of someone else being baptized) will go on to their own baptisms. That is more likely to occur if this service is a spiritual, strong teaching moment in which it is clear to participants and visitors alike that this is a sacred act of faith centered on the Lord Jesus Christ, that it is an act of repentance claiming the cleansing power of Christ, that through His majesty and Atonement it brings a remission of sins as well as, with confirmation, membership in His Church.

Then she wrote him a referral for using foul language, and I heard her mutter under her breath (because I was sitting right next to her desk when this all went down), "... and if I could give him one for being a fucking idiot, I'd do that too." I don't know if anyone else heard her, but if they did, no one said anything. I so did not realize that teachers cussed or that they didn't like some of their students. At that moment, Ms. Abernard became my new hero.

I recognized this mania for referrals from people our parents’ age: the way they thought that the most difficult problems could only be solved by special insider information that you had coaxed out of some guy with a name like Chuck.

Another significant factor in the problem that blacks faced in getting over-the-road truck driving positions was the refusal of white truck drivers to ride with them. In 1966, The Wall Street Journal reported that one Teamster official asked, “Would you like to climb in a bunk bed that a nigger just got out of?” Another said, “To my knowledge no law has been written yet that says a white man has to bed down with Negroes.”[102] Teamster officials protected union men who were discharged by a company for refusing to ride with a black driver.[103] Seniority rules, the refusal of white drivers to ride with black drivers, and the Teamsters’ highly discriminatory job-referral practices contributed to reducing black opportunities for jobs in the trucking industry.[104]

As a result of the Clinton team’s tenacious pushback, the Times appended two separate corrections to its original article—first claiming that Mrs. Clinton herself was not the focus of any investigation and then, a day later, changing the description of the inspector general’s transmission to the FBI from “criminal referral” to “security referral.” Though the Times may have thought those clarifications were necessary, their original story was much closer to the mark. It was true that the transmission to the FBI from the inspector general did not use the word “criminal,” but by the time of the news story we had a full criminal investigation open, focused on the secretary’s conduct. We didn’t correct the Times and contradict the Clinton campaign because—consistent with our practice—we were not yet to a point where it was appropriate to confirm an investigation.

We live in a culture that teaches us that "men" are the sexual aggressors and pursuers. We also live in a world where most women, trans, and non-binary folks have had negative experiences with men who are hitting on them. These factors tend to lead to some big gender differences for those exploring non-monogamy. Cisgender men often struggle when they first enter the world of non-monogamy. Within consensual non-monogamy (CNM) communities, most folks who sleep with cis men choose their partners based on referrals and endorsements. As in the world of business, it truly is who you know. Cis men who have been in the communities longer have dated and interacted with more people, and, therefore, have more word of mouth. It is an unfortunate reality that many, especially cisgender women, will not date men they don't already know about through their friends and communities. So, if you're a cis man exploring CNM, expect that it may take a while before you start seeing the kind of attention that others get. Focus on being kind, respectful, and honest. Respect the needs and boundaries of everyone with whom you interact. Spend lots of time getting to know other people simply as people - especially of your preferred gender to date - and form genuine friendships and connections with them free from any pressure to become sexual.

According to the Department of Justice’s investigation of the Missoula County Attorney’s Office, from January 2008 through April 2012 the Missoula Police Department referred 114 reports of sexual assault of adult women to the MCAO for prosecution. A “referral” indicated that the police department had completed its investigation of the case in question, determined that there was probable cause to charge the individual accused of sexual assault, and recommended that the case be prosecuted. Of the 114 sexual assaults referred for prosecution, however, the MCAO filed charges in only 14 of those cases. The reasons most often given for declining to prosecute were “insufficient evidence” or “insufficient corroboration”—that is, lack of probable cause. Kirsten Pabst was in charge of sexual assault cases for all but the final two months of the fifty-two-month period investigated by the DOJ.

His direction may not be a booming thunder or a voice in your ear. It may be a high-risk loan application going through, a referral seemingly out of nowhere or a scholarship award for something you were not qualified for. If these sorts of things are happening, God is creating a path for you. He is providing confirmation that tells you, “This is right!” Isaiah, 30:21 proves this to be true, “Whether you turn to the right or to the left, your ears will hear a voice behind you, saying,“This is the way; walk in it.

in sales, activity drives everything—that’s the law. Activity is the hard work of sales and it is the price you pay for your commission checks. Activities include: Cold calling, first-time visits, follow-up calls, product demonstrations, walk-throughs, test drives, site visits, open houses, tours, presentations, proposals, referrals, direct mail, and so forth. The fact is, if you have enough activity, you will at least sell something, even if you do everything else wrong. If you have no activity, but do everything else right, you will sell nothing.

In the early 1970s, racial and gender discrimination was still prevalent. The easy camaraderie prevailing in the operating room evaporated at the completion of surgical procedures. There was an unspoken pecking order of seating arrangements at lunch among my fellow physicians. At the top were the white male 'primary producers' in prestigious surgical specialties. They were followed by the internists. Next came the general practitioners. Last on the list were the hospital-based physicians: the radiologists, pathologists and anaesthesiologists - especially non-white, female ones like me. Apart from colour, we were shunned because we did not bring in patients ourselves but, like vultures, lived off the patients generated by other doctors. We were also resented because being hospital-based and not having to rent office space or hire nursing staff, we had low overheads. Since a physician's number of admissions to the hospital and referral pattern determined the degree of attention and regard accorded by colleagues, it was safe for our peers to ignore us and target those in position to send over income-producing referrals. This attitude was mirrored from the board of directors all the way down to the orderlies.

As John Jantsch says in his book, The Referral Engine: Teaching Your Business to Market Itself, “Being recognized as a source of good information, including referrals, is a great way to connect with others. Think about how eagerly you responded the last time someone asked you for directions, offering up your favorite shortcut and tips for avoiding traffic. We all do it. Making referrals is a deeply satisfying way to connect with others, and asking for referrals is just the other side of the same phenomenon. I think the growth of many popular social networks can be traced to the fact that people love to connect and form communities around shared ideas.”1
Stephen Wershing (Stop Asking for Referrals: A Revolutionary New Strategy for Building a Financial Service Business that Sells Itself)
While advertising was once used primarily to create a sale or enhance an image, it must now be used to create awareness about Web content. • While SEO was at one time primarily a function of optimizing a Web site, it must now be a function of optimizing brand assets across social media. • While lead generation used to consist of broadcasting messages, it must now rely heavily on being found in the right place at the right time. • While lead conversion in the past often consisted of multiple sales calls to supply information, it must now supplement Web information gathering with value delivery. • While referrals used to be a simple matter of passing a name, they now rely heavily on an organization’s online reputation, ratings, and reviews. • While physical store location has always mattered, online location for the local business has become a life-and-death matter.
John Jantsch (Duct Tape Marketing Revised and Updated: The World's Most Practical Small Business Marketing Guide)
The sex ratio of those being referred had shifted dramatically too. The number of girls (known at that time at GIDS as ‘natal females’, now ‘birth-assigned females’) seeking help had equalled the number of boys for the first time in 2011. Previously, GIDS’s caseload had been nearly three-quarters male for those referred in childhood, or two-thirds overall. At first, this change was understood to be positive – a sort of balancing-out – and attributed to the fact that the girls were perhaps being better supported to seek help. But by 2015 it was clear that, in fact, something bigger was happening. There had been a complete reversal. Referrals for natal girls made up 65 percent of the total. In 2019/20 girls outnumbered boys by a ratio of six to one in some age groups, most markedly between the ages of 12 and 14 … Moreover, the majority were girls whose gender-related distress had begun after the onset of puberty, during adolescence. They didn’t have a history of childhood dysphoria.

Several recent studies (Bliss, 1980; Boon & Draijer, 1993a; Coons & Milstein, 1986; Coons, Bowman, & Milstein, 1988; Putnam et al., 1986; Ross et al., 1989b) are largely consistent in terms of the general trends that they demonstrate. At the time of diagnosis (prior to exploration) approximately two to four personalities are in evidence. In the course of treatment an average of 13 to 15 are encountered, but this figure is deceptive. The mode in virtually all series is three, and median number of alters is eight to ten. Complex cases, with 26 or more alters (described in Kluft, 1988), constitute 15-25% of such series and unduly inflate the mean. Series currently being studied in tertiary referral centers appear to be more complex still (Kluft, Fink, Brenner, & Fine, unpublished data). This is subject to a number of interpretations. It is likely that the complexity of the more difficult and demanding cases treated in such settings may be one aspect of what makes them require such specialized care. It is also possible that the staff of such centers is differentially sensitive to the need to probe for previously undiscovered complexity in their efforts to treat patients who have failed to improve elsewhere. However, it is also possible that patients unduly interested in their disorders and who generate factitious complexity enter such series differently, or that some factor in these units or in those who refer to them encourages such complexity or at least the subjective report thereof.
Richard P. Kluft
