Be assertive. This is something that depends on confidence, and will take practice, but it’s worth it. Being assertive means standing up for yourself and exuding pride in who you are. Put your strategies into play. After the information you’ve absorbed so far, you have an advantage in that you are aware of your wants, what the narcissist demands, what you are able to do and those secret tiny areas you may have power over. Tap into these areas to put together your own strategies. Re-set your boundaries. A boundary is an unseen line in the sand. It determines the point you won’t allow others to cross over or they’ll hurt you. These are non-negotiable and others must be aware of them and respect them. But you have to know what those lines are before making them clear to others. Have consequences. As an extension of the above point, if a person tries ignoring your boundaries, make sure you give a consequence. There doesn't need to be a threat, but more saying, “If you ________, we can’t hang out/date/talk/etc.” You’re just saying that crossing the boundary hurts you so if they choose to disregard it, you choose not to accept that treatment. The narcissist will not tolerate you standing up for yourself, but it’s still important. The act of advocating for yourself will increase your self-confidence, self-esteem and self-worth. Then you’ll be ready to recover and heal.

When you find flow, you get “maybe the most potent dose of reward chemistry” your brain can give you—which is the reason he believes flow is the most addictive state on Earth. Once we start to feel flow in an experience, we are motivated to do what it takes to get more. But it’s a circular relationship—if you have motivation to accomplish a task but you have no flow, you will eventually burn out. Motivation and flow need to work together, and they must be coupled with a solid recovery protocol, like good sleep and nutrition.

First, I am thrilled that paramedics are finally getting the respect they deserve for being the professionals they can be. The scope of practice is expanding, and patient care modalities are improving, seemingly by the minute. Patient outcomes are also improving as a result, and EMS is passing through puberty and forging into adulthood. On the other hand, autonomy in the hands of the “lesser-motivated,” can be a very dangerous thing. You know as well as I do that there are still plenty of providers who operate from a subjective, complacent, and downright lazy place. Combined with the ever-expanding autonomy, that provider just became more dangerous than he or she ever has been – to the patients and to you. Autonomy in patient care places more pressure for excellence on the provider charged with delivering it, and also on the partner and crew members on scene. Since the base hospital is not involved like it once was, they are likewise less responsible for the errors and omissions of the medics on the scene. Now more than ever, crew members are being held to answer for the mistakes and follies of their coworkers; now more than ever, EMS providers are working without a net. What’s next? I predict (and hope) emergency medical Darwinism is going to force some painful and necessary changes. First, increasing autonomy is going to result in the better and best providing superior patient care. More personal ownership of the results is going to manifest in outcomes such as increased cardiac arrest survival rates, faster and more complete stroke recovery, and significantly better outcomes for STEMI patients, all leading to the brass ring: EMS as a profession, not just a job. On the flip side of that coin, you will see consequences for the not-so-good and completely awful providers. There will be higher instances of licensure action, internal discipline, and wash-out. Unfortunately, all those things will stem from generally preventable negative patient outcomes. The danger for the better provider will be in the penumbra; the murky, gray area of time when providers are self-categorizing. Specifically, the better provider who is aware of the dangerously poor provider but does nothing to fix or flush him or her, is almost certain to be caught up in a bad situation caused by sloppy, complacent, or ultimately negligent patient care that should have been corrected or stopped. The answer is as simple as it is difficult. If you are reading this, it is more likely because you are one of the better, more committed, more professional providers. This transition is up to you. You must dig deep and find the strength necessary to face the issue and force the change; you have to demand more from yourself and from those around you. You must have the willingness to help those providers who want it – and respond to those who need it, but don’t want it – with tough love by showing them the door. In the end, EMS will only ever be as good as you make it. If you lay silent through its evolution, you forfeit the right to complain when it crumbles around you.

Family Mobbing encompasses varied acts of aggression that cannot be understood by examining one motivation or cause. The pattern of behavior always isolates one family member and inflicts as much emotional pain as possible. Unlike sibling rivalry, the intention is to establish superiority or to provoke fear and distress. Factors to consider include the motives, the degree of severity, a power of imbalance, victimization element, physical injuries, and trauma.

The starting point to constructing your workout program is deciding how often you will exercise. The current general recommendations for physical activity are 150 minutes of aerobic activity week at moderate-intensity exercise. That equates to about half an hour a day, five days a week. That activity should include doing a range of physical activities that incorporate fitness, strength, balance and flexibility. However, such activities as gardening and playing sport should also be included in your total exercise count. Including these types of activities will help to make sure that you are getting the proper balance between exercise and recovery. A recovery friend of mine once belonged to an AA group called “What’s Your Motivation?” She said she’d always ask herself that in situations where she had to say or do something she might regret, and she’d ask others as well. She asked me that once or twice. So, you start out by asking yourself that question when the situation arises, and a lot of time you realize there is no good motive behind the thing you want to do or say, so you don’t say it. You don’t do it. After a while, it becomes second nature. Unfortunately, however, so many people out there are living their lives while untreated for their afflictions. Whether it’s addiction, including alcoholism, or a type of personality disorder, their behavior often stems from how they feel about themselves based on other people’s words and actions, things they had inadvertently taken on and clung to fiercely. They may have a desperate need for attention, validation, admiration, and respect. Maybe their delusions distort their perception of themselves and how others view them. They are so busy worrying about themselves that they are often oblivious to their motives and may not realize how little regard they have for others. In a genuine sense, they are fighting for themselves, but they’re not winning. Many of us have lived that way once upon a time and, because of it, spent a copious amount of energy on damage control. Knowing we said something we shouldn’t have said or did something we shouldn’t have done and going into this anxiety-ridden desperation to save our “image”—an image that likely isn’t real but a delusion. When we should be more concerned about apologizing or making amends, we’re more obsessed with not wanting to be seen in a negative light and having to act in order to change the negative perception. It takes recovery, healing, and time to learn that if you are intent on doing the right thing, doing right by people, and having everyone’s best interests at heart, you’ll know how to react and respond to things. And if you ever say or do something you regret, you simply say you were wrong and apologize. Empathy for others and for ourselves is what makes it possible. It makes us care about how we treat people and the effect it’s having on not only them but on our lives and the lives of anyone who cares about us. We eventually understand that how we treat people is just as important as catering to our own needs. I think it’s important to understand what made us a certain way in life and to acknowledge that, but then we have to fix it. It becomes our job and responsibility to heal that so that we grow and change. Too many people never get to a point where they can see it, let alone understand it, so those of us who do are quite fortunate.

IV. THE INNER SENSE OF LIFE While the law of life is working within us, we have a deeper consciousness of life. His intercession very effectively motivates our inner seed. How do we know that our inner seed has been motivated? By our inner sense, our inner consciousness. Although you may feel too tired to attend a particular church meeting, the seed within will not let you sleep. By the inward “pum, pum, pum,” the energizing of the heavenly intercession, you realize that you must go to that meeting. Sometimes, when we are attracted to the worldly things, the inward “pum, pum, pum” gives us no rest, and we find ourselves doing what the Lord desires. What a mercy this is! I have the full assurance that in the coming years the heavenly “pum, pum, pum” will accomplish a great deal in the Lord’s recovery. We will never be satisfied with what we have seen in the past. This ministry will be higher and richer. The Lord will use the churches and the messages to gather together His true seekers to be His testimony to the whole earth. Everything in the Bible concerning the church will be fulfilled before the Lord comes back. Deep within, we [433] all have the sense that nothing but this standard model can satisfy us. We cannot be happy unless we are saturated with Him. This is a strong proof that He is interceding for us to be glorified, to be brought into His perfection.

There is no evidence from anywhere in the world that harm reduction measures encourage drug use. Denying addicts humane assistance multiplies their miseries without bringing them one inch closer to recovery. There is also no contradiction between harm reduction and abstinence. The two objectives are incompatible only if we imagine that we can set the agenda for someone else’s life regardless of what he or she may choose. We cannot. Short of extreme coercion there is absolutely nothing anyone can do to induce another to give up addiction, except to provide the island of relief where contemplation and self-respect can, perhaps, take root. Those ready to choose abstinence should receive every possible support — much more support than we currently provide. But what of those who don’t choose that path? The impossibility of changing other people is not restricted to addictions. Try as we may to motivate another person to be different or to do this or not to do that, our attempts founder on a basic human trait: the drive for autonomy. “And one may choose what is contrary to one’s own interests and sometimes one positively ought,” wrote Fyodor Dostoevsky in Notes from the Underground. “What man wants is simply independent choice, whatever that independence may cost and wherever it may lead.” The issue is not whether the addict would be better off without his habit — of course he would — but whether we are going to abandon him if he is unable to give it up. Are we willing to care for human beings who suffer because of their own persistent behaviours, mindful that these behaviours stem from early life misfortunes they had no hand in creating? The harm reduction approach accepts that some people — many people — are too deeply enmeshed in substance dependence for any realistic “cure” under present circumstances. There is, for now, too much pain in their lives and too few internal and external resources available to them. In practising harm reduction we do not give up on abstinence — on the contrary, we may hope to encourage that possibility by helping people feel better, bringing them into therapeutic relationships with caregivers, offering them a sense of trust, removing judgment from our interactions with them and giving them a sense of acceptance. At the same time, we do not hold out abstinence as the Holy Grail and we do not make our valuation of addicts as worthwhile human beings dependent on their making choices that please us. Harm reduction is as much an attitude and way of being as it is a set of policies and methods.
