My wife coughs and coughs, and then when her throat is clear, she moans. The nurse’s aide moves back and forth downstairs. The hospital bed jingles. I wrote that it was a spouse coughing because that seemed something a reader could identify with, while a brother would be too specific to me. I lie here, listening to my wife cough, and it is hard to believe that she is dying. It was strange to write something down and for that thing to come into existence. The fact that the sentence existed made Birju’s coughing somehow less awful. As I sat on my bed, I thought about how I could end my story. I held my pencil above the sheet of paper. According to the essays I had read on Hemingway, all I needed to do was attach something to the end of the story that was both unexpected and natural. I imagined Birju dying; this had to be what would eventually happen. As soon as I imagined this, I did not want him gone. I felt a surge of love for Birju. Even though he was sick and swollen, I did not want him gone. I wrote: I lie in my bed and listen to her cough and am glad she is coughing because this means she is alive. Soon she will die, and I will no longer be among the lucky people whose wives are sick. Fortunate are the men whose wives cough. The lack of differentiation in the unconscious arises in the first place from the almost direct association of all the brain centres with each other, and in the second from the relatively weak energie value of the unconscious elements.83 That they possess relatively little energy is clear from the fact that an unconscious element at once ceases to be subliminal as soon as it acquires a stronger accent of value; it then rises above the threshold of consciousness, and it can do this only by virtue of the energy accruing to it. It becomes a “lucky idea” or “hunch,” or, as Herbart calls it, a “spontaneously arising presentation.” The strong energic value of the conscious contents has the effect of intense illumination, whereby their differences become clearly perceptible and any confusion between them is ruled out. In the unconscious, on the contrary, the most heterogeneous elements possessing only a vague analogy can be substituted for one another, just because of their low luminosity and weak energic value. Even heterogeneous sense-impressions coalesce, as we see in “photisms” (Bleuler) or in colour hearing. Language, too, contains plenty of these unconscious contaminations, as I have shown in the case of sound, light, and emotional states.84

Missouri had 18 heroin overdose deaths in 2001; ten years later, there were 245.26 Heroin deaths in Minnesota shot from 3 to 98 between 1999 and 2013.27 Michigan saw fatal heroin overdoses surge from a few dozen a year in 2002 to more than 100 a year starting in 2009.28 In just one year, heroin-related fatalities in Connecticut nearly doubled, to 257 in 2013.29 Between 2007 and 2012, heroin use in the United States is estimated to have increased by almost 80 percent.30 And that’s just heroin. More than 40,000 Americans were killed from all illegal drug use in 2010, surpassing car accidents and shootings as a cause of death.31 The addicts who die may be the lucky ones. In 2001, a seventeen-year-old boy in New Jersey who scored 700 on the math SAT took a heroin overdose that left him unable to stand, walk, or bathe himself. Furthermore, by not being dependent on any single kind of food, they were less liable to suffer when one particular food source failed. Agricultural societies are ravaged by famine when drought, fire or earthquake devastates the annual rice or potato crop. Forager societies were hardly immune to natural disasters, and suffered from periods of want and hunger, but they were usually able to deal with such calamities more easily. If they lost some of their staple foodstuffs, they could gather or hunt other species, or move to a less affected area. Ancient foragers also suffered less from infectious diseases. Most of the infectious diseases that have plagued agricultural and industrial societies (such as smallpox, measles and tuberculosis) originated in domesticated animals and were transferred to humans only after the Agricultural Revolution. Ancient foragers, who had domesticated only dogs, were free of these scourges. Moreover, most people in agricultural and industrial societies lived in dense, unhygienic permanent settlements – ideal hotbeds for disease. Foragers roamed the land in small bands that could not sustain epidemics. The wholesome and varied diet, the relatively short working week, and the rarity of infectious diseases have led many experts to define pre-agricultural forager societies as ‘the original affluent societies’.

Thinking that negative actions or words of others are related to you, or assuming that you are the cause of a negative event when you actually had no connection with it. Overgeneralizations. Seeing one negative situation as representative of all similar events. Labeling. Attaching negative labels to ourselves or others. Rather than focusing on a particular thing that you didn’t like and want to change, you might label yourself a loser or a failure. Magnification/minimization. Emphasizing bad things and deemphasizing good in a situation, such as making a big deal about making a mistake, and ignoring achievements. Emotional reasoning. Letting your feelings about something guide your conclusions about how things really are, as in “I feel hopeless, so my situation really must be hopeless.” Discounting positives. Disqualifying positive experiences as evidence that your negative beliefs are false—for example, by saying that you got lucky, something good happened accidentally, or someone was lying when giving you a compliment. Negativity bias. Seeing only the bad aspects of a situation and dwelling on them, in the process viewing the situation as completely bad even though there may have been positives. Should/must statements. Setting up expectations for yourself based on what you think you “should” do. These usually come from perceptions of what others think, and may be totally unrealistic. You might feel guilty for failing or not wanting these standards and feel frustration and resentment. Buddhism sets this in context. When the word “should” is used, it leaves no leeway for flexibility of self-acceptance. It is fine to have wise, loving, self-identified guidelines for behavior, but remember that the same response or action to all situations is neither productive nor ideal. One size never fits all.  Jumping to conclusions. Making negative predictions about the outcome of a situation without definite facts or evidence. This includes predicting a bad future event and acting as if it were already fact, or concluding that others reacted negatively to you without asking them. ​Dysfunctional automatic thoughts like these are common. If you think that they are causing suffering in your life, make sure you address them as a part of your CBT focus.

If instead we consider a drop in all the Earth’s oceans, then we have a factor of 1024. That is a lot bigger than a bucket but nowhere close to how insignificant the mass of the drop of water is when compared to zero-point energy. To continue, what if the ocean was the size of the sun? That gives us a ratio on the order of 1041, which is still a long way off. If the ocean was the size of the solar system we get a ratio on the order of 1050. Now if we expand the ocean to the size of the galaxy we get ~1076 and we are still not anywhere close. What if the ocean is the size of the known visible universe? Assuming a radius of 7.4 x 1026 meters the mass ratio is 5 x 1095. There we go. So, the density of water compared to the energy of the vacuum is equivalent to five 1 ml drops of water in an ocean the size of the visible universe. Since we are mostly water and have a similar density to water, the vacuum fluctuations inside our body are like having all the mass-energy of an ocean of water the size of the universe inside each little part of us. Wow, we are pretty insignificant in the big scheme of things and so is any other body of solid matter or any amount of energy associated with it. This zero-point energy is all around us and all throughout us. We are lucky that zero-point energy is not detectable or anything we did would be undetectable noise to any sensor we could possibly make. Even worse, if we could absorb even a small fraction of that energy, we would be vaporized in an instant. Or, if all that energy participated in a gravitational force, the universe would be crushed to a speck.

In capitalism, the normal people think the special people are free because they can control people, and the special think the normals are free because there's no pressure. What I learned is that too far over the top edge is madness or evil: Max swimming toward his death, Parker jumping off the roof, Lucky Mike raping a little girl. But too far over the bottom edge is destitution and isolation: the man with the elephant trunk for a forehead, the breastfeeding women crying "help me" from the doorways in Phnom Penh, Rocky Balboa cut in half by a power-mad tourist. The two poles are related. The calamities I saw were not separate from the freedom of the special people, they were the result of their freedom. Lucky Mike's ultimate freedom depended on the ultimate slavery of another. Those at the top are not free either, because their freedom is tied inexorably to the sacrifice of those at the bottom.

As she looked down at us in our off-duty moments the Princess represented youth, femininity, and a glamour beyond our gray steel world. She embodied the innocent vulnerability we were in extremis employed to defend. Also, being royal, she commanded the tribal loyalty our profession had valued above all else for more than a thousand years, since the days of King Alfred. In addition, as a matter of simple fact, this tasty-looking bird was our future Queen. Later, when that day in Loch Ewe felt like a relic from another lifetime, I often marveled at the Princess’s effect on military people. That unabashed loyalty symbolized by Arethusa’s portrait was typical of reactions in messhalls and barracks worldwide. Sometimes the men gave the impression that they would have died for her not because it was their duty, but because they wanted to. She really seemed worth it.

We wanted to have the television and washing machines and dishwashers they have; we do. Yet we laughed at their simple-minded glitz, their ignorance of the world - all the while heading in droves for Las Vegas and Los Angeles. We wanted the American Dream - without the name and without the responsibilities; we have it, to a large extent - and it is this that allows us to caress our little sense of moral superiority. The number of Canadians who expressed sympathy for the victim while blaming him (and watching his movies and his TV sitcoms, listening to his music, eating his food and dreaming of Florida) attained, in a time of grave crisis, a level of self-satisfied hypocrisy that is usually found only in the NDP, those paragons of democratic values who have few good words for the Americans but much mindless applause for Castro. We're lucky in this country to have none of the international responsibilities the Americans do, because then we wouldn't be able to lord it morally over them - and then where would we be? Canadians have no problems anywhere in the world, we like to boast. What we don't realize is, it's not because we're likeable, it's because we're inoffensive. We're welcome by default.

It’s almost as naïve and problematic as believing that one action in Genesis 3 is supposed to upend God’s entire created order. These assumptions seem to reject the very sovereignty of God the book of Genesis is trying to show us. In contrast to the other Mesopotamian gods, the God of the Israelites is loving and good and intentional. All of creation finds its meaning within its relation to God.

So much has been lost that will never come back again, that the loss of the joy of the relatively few humans who were lucky enough to live on the strand, who combed the beaches, and fished, and rode the waves, and lay in the sun – that’s nothing much to grieve for, given everything else that has been lost, all the suffering, all the hunger, all the death, all the extinctions. Most of the mammal species are gone. Still, it was a way of life much beloved, and still remembered in art and song, image and story – still legendary, still a lost golden age, vibrating at some level below thought, there in their salty blood and tears, in the long, curled waves of DNA that still break inside them all.

As we walked through this typical small town market, they picked up many more items, “just in case we get snowed in.” I expressed my regret for not being able to help in defraying the ever-increasing cost of the groceries, but it didn’t seem to bother them. “We picked you up and it’s our treat,” Rita explained. “Come on, let’s get going before we get stuck here,” Connie said, with a sound of urgency, to her mother who was still looking around. Picking up two economy-sized bags of potato chips along with some pretzels didn’t impress me as being staples, but to be fair, she did also pick up bacon, eggs, English muffins and a container of milk. Getting back into the car, we turned north again, past where they first picked me up, and then left onto Mountain Street. I knew from the many times that I had come through Camden that Mount Battie was back up here somewhere, but after a short distance of about a mile or so, we turned left again and pulled into the driveway of a big old farmhouse connected to a barn, which looked very much like many other houses in Maine. By this time the snow was coming down in big wet flakes, accumulating fast. It wouldn’t take long before the roads would become totally impassable. I knew that this could become a worse mess than I had anticipated, especially on the back roads. The coastal towns in Maine don’t usually get as cold as the towns in the interior, thus allowing the air to hold more moisture. In turn, they are apt to get more big wet snowflakes that accumulate faster. However, the salt air also melts the snow more rapidly. I seldom had to worry about the weather, but this time I was lucky to have been picked up by these “Oh So Fine Ladies” and was glad that I decided to accept their offer to stay with them.

Finding a person to declare your craziest, most profound insecurities to is not exactly a picnic. But the bureaucratic idiocy of America’s healthcare system turns what should be a chore into torture. If you’re a middle-class person in America, the dance goes like this: You call your insurance provider to find a meager list of therapists who take your insurance. Most of the people on the list are licensed clinical social workers or licensed mental health counselors. They can be wonderful and very helpful, but they often have less schooling and experience than, say, psychologists and PhDs. After digging deeper, you find that some of these therapists don’t take your insurance after all; others have full client lists. And even if they do have space in the day to treat someone, they might not be interested in treating you. According to one study, a low-income Black person had up to an 80 percent lower chance of receiving a callback for an appointment than a middle-class white person. And even though intellectually, therapists tell you that anger can be a helpful and legitimate emotion in processing trauma, God forbid you actually seem angry on the phone. Several mental health professionals have told me that therapists often avoid rageful clients because they seem threatening or scary. Therapists instead prefer to take on YAVIS—Young, Attractive, Verbal, Intelligent, and Successful clients. They love an amenable type, someone who is curious about their internal workings and eager to plumb them, someone who’s already read articles in The New Yorker about psychology to familiarize them with the language of metacognition and congruence. Good luck if you’re a regular-ass Joe who’d rather watch It’s Always Sunny in Philadelphia. But say you get lucky and find a licensed clinical psychologist with an open slot. The psychologist is white, of course (86 percent of psychologists in the United States are), which isn’t ideal if you are a person of color. But, fine, whatever: You just need to receive an official diagnosis for your insurance. You are certain you have complex PTSD, but he can’t diagnose you with that because it’s not in the Diagnostic and Statistical Manual of Mental Disorders. Your insurance only covers treatment for conditions listed in the DSM in order to assign a number of sessions to you. Most forms of insurance will pay for, say, only six months of therapy relating to anxiety, ten for depression, as if you should be better by then. Another consequence of C-PTSD not being in the DSM: This psychologist hasn’t been trained in treating it. He says he doesn’t believe that it’s a real diagnosis. He’d like to provide you with some questionnaires to see if you have something he can actually handle—bipolar disorder, maybe, or manic depression. This does not inspire confidence, so you leave. After some internet sleuthing, you find a woman of color who seems really cool. She’s specifically trained in the treatment of complex trauma. She has blurbs on her website that resonate with you—it seems as if she might truly understand you. But she doesn’t take insurance. (Psychologists are the least likely of any medical provider to take insurance—only about 45 percent of them do. And most of the time, the ones who don’t are the most qualified practitioners.) You can’t exactly blame her. You learn on the internet that insurance companies haven’t updated reimbursement rates for therapists in up to twenty years, despite rising rates for office rent and other administrative costs. If therapists were to rely on reimbursement rates from insurance alone, they’d wind up making about $50,000 a year on average, which is fine, but like, not great if you’re an actual doctor.

That being an attentive and generous friend and citizen will somehow matter in the social fabric, save a thread or two from unraveling. And you must do all of this with the sure and certain knowledge that you will never get proper credit for it, at all, one bit, and in fact the vast majority of the things you do right will go utterly unremarked; except, perhaps, in ways we will never know or understand, by the Arab Jew who once shouted about his cloak, and may have been somehow also the One who invented and infuses this universe and probably a million others—not to put a hard number on it or anything. Humility, the final frontier, as my late brother Kevin used to say. When we are young we build a self, a persona, a story in which to reside, or several selves in succession, or several at once, sometimes; when we are older we take on other roles and personas, other masks and duties; and you and I both know men and women who become trapped in the selves they worked so hard to build, so desperately imprisoned that sometimes they smash their lives simply to escape who they no longer wish to be; but finally, I think, if we are lucky, if we read the book of pain and loss with humility, we realize that we are all broken and small and brief, that none among us is actually rich or famous or more beautiful than another; and then, perhaps, we begin to understand something deep and true finally about humility. This is what I know: that the small is huge, that the tiny is vast, that pain is part and parcel of the gift of joy, and that there is love, and then there is everything else. That’s a lot of Wembley Stadiums full of people, and even more double-decker buses (apparently, these are the standard British measurements for size). And we live on a planet that is spinning at 67,000 miles per hour through space, around a sun that is the centre of our solar system (and our solar system is spinning around the centre of the Milky Way at 530,000 miles per hour). Just our solar system (which is a tiny speck within the entire universe) is very big indeed. If Earth was a peppercorn and Jupiter was a chestnut (the standard American measurements), you’d have to place them 100 metres apart to get a sense of the real distance between us. And this universe is only one of many. In fact, the chances are that there are many, many more populated Earths – just like ours – in other universes. And that’s just space. Have a look at time, too. If you’re lucky, you may spend 85 years on this Earth. Man has been around for 100,000 years, so you’re going to spend just 0.00085 per cent of man’s history living on this Earth. And man’s stay on Earth has been very short in the context of the age of the Earth (which is 4.5 billion years old): if the Earth had been around for the equivalent of a day (with the Big Bang kicking it all off at midnight), humans didn’t turn up until 11.59.58 p.m. That means we’ve only been around for the last two seconds. A lifetime is gone in a flash. There are relatively few people on this Earth that were here 100 years ago. Just as you’ll be gone (relatively) soon.

In 2010, a cognitive neuroscientist named Reza Habib asked twenty-two people to lie inside an MRI and watch a slot machine spin around and around. Half of the participants were “pathological gamblers”—people who had lied to their families about their gambling, missed work to gamble, or had bounced checks at a casino— while the other half were people who gambled socially but didn’t exhibit any problematic behaviors. Everyone was placed on their backs inside a narrow tube and told to watch wheels of lucky 7s, apples, and gold bars spin across a video screen. The slot machine was programmed to deliver three outcomes: a win, a loss, and a “near miss,” in which the slots almost matched up but, at the last moment, failed to align. None of the participants won or lost any money. All they had to do was watch the screen as the MRI recorded their neurological activity. “We were particularly interested in looking at the brain systems involved in habits and addictions,” Habib told me. “What we found was that, neurologically speaking, pathological gamblers got more excited about winning. Through definitive actions, he pares the ür-configuration containing all his possible worlds to a stunted fraction of its former self. How many brilliant futures are ruled out with each step, with each decisive word? Billions, in a very real sense. The further he gets himself into any situation, the more severe the pruning of his possibility tree. Thus his inability to focus on any enthusiasm for too long, a metaphysical fickleness that functions as a defense mechanism against the death of possibility.
