Cocaine blasts the reward circuitry so that it pumps out massive amounts of exciting dopamine. This accounts for the high. Then two things happen simultaneously. First, the high begins to fade as the brain disposes of the extra dopamine. Second, because so much excess dopamine can damage or kill nerve cells, the cells protect themselves by reducing the number of dopamine receptors (little “hands”) on their surfaces. If a thunderstorm rolls in, you close all the windows and wait for it to pass. That’s what the cells do, except they assume that another storm is on the way, and stay closed up for a while. The addict has lowered her sensitivity to dopamine—a substance that helped give her the high. Now our addict feels rotten. She has two choices: Take more cocaine to jack up her mood artificially by saturating the remaining dopamine receptors, or suffer withdrawal symptoms. Withdrawal symptoms arise when the reward circuitry is starving for dopamine. Whether you have too few receptors for dopamine, or too little dopamine circulating around the nerve cells, you get the same result. Your reward circuitry batteries are low, leaving you with an acute desire to feel normal again.

They were older. “And I said, ‘This doesn’t make any sense to me, because I’m very proud of them, and we have a good relationship.’ ” But then he started to look at what, specifically, he was doing with his sons that made his feelings so negative. “And what was I doing?” he asked. “I was saying, ‘It’s time to get up, or you will be late for school.’ Or, ‘You haven’t put away your cereal dish from breakfast.’ ” He was nagging, in other words, and nagging is not a flow activity. “I realized,” he said, “that being a parent consists, in large part, of correcting the growth pattern of a person who is not necessarily ready to live in a civilized society.” I asked if, in that same data set, he had any numbers about flow in family life. None were in his book. He said he did. “They were low. Family life is organized in a way that flow is very difficult to achieve, because we assume that family life is supposed to relax us and to make us happy. This allows them to come gently and gracefully out of negative feeling states into more positive states of mind. One of the happiest people I know is someone who also gets quite low from time to time. The difference, it seems, is that he has become comfortable with his low moods. It’s almost as though he doesn’t really care because he knows that, in due time, he will be happy again. To him, it’s no big deal. The next time you’re feeling bad, rather than fight it, try to relax. See if, instead of panicking, you can be graceful and calm. How come in country after country across Africa itself, from Cameroon to Uganda to Zimbabwe and in a dozen other countries as well, HIV is lowest in the poorest households, and highest in the richest households? And how is it that in many countries, more educated women are more likely to be infested with HIV than women with no schooling? Myth #3: Fasting Causes Low Blood Sugar Sometimes people worry that blood sugar will fall very low during fasting and they will become shaky and sweaty. Luckily, this does not actually happen. Blood sugar level is tightly monitored by the body, and there are multiple mechanisms to keep it in the proper range. During fasting, our body begins by breaking down glycogen (remember, that’s the glucose in short-term storage) in the liver to provide glucose. This happens every night as you sleep to keep blood sugars normal as you fast overnight. Some even feel a sense of euphoria. They usually attribute this to achieving some kind of spiritual enlightenment, but the truth is much more down-to-earth and scientific than that: it’s the ketones! Ketones are a “superfood” for the brain. When the body and brain are fueled primarily by fatty acids and ketones, respectively, the “brain fog,” mood swings, and emotional instability that are caused by wild fluctuations in blood sugar become a thing of the past and clear thinking is the new normal. If you fast for longer than twenty-four to thirty-six hours, glycogen stores become depleted. The liver now can manufacture new glucose in a process called gluconeogenesis, using the glycerol that’s a by-product of the breakdown of fat. This means that we do not need to eat glucose for our blood glucose levels to remain normal. A related myth is that brain cells can only use glucose for energy. This is incorrect. Human brains, unique amongst animals, can also use ketone bodies—particles that are produced when fat is metabolized—as a fuel source. This allows us to function optimally even when food is not readily available. Ketones provide the majority of the energy we need. Consider the consequences if glucose were absolutely necessary for brain function. After twenty-four hours without food, glucose stored in our bodies in the form of glycogen is depleted. At that point, we’d become blubbering idiots as our brains shut down. In the Paleolithic era, our intellect was our only advantage against wild animals with their sharp claws, sharp fangs, and bulging muscles. Without it, humans would have become extinct long ago. When glucose is not available, the body begins to burn fat and produce ketone bodies, which are able to cross the blood-brain barrier to feed the brain cells. Up to 75 percent of the brain’s energy requirements can be met by ketones. Of course, that means that glucose still provides 25 percent of the brain’s energy requirements. So does this mean that we have to eat for our brains to function?

FIFTH UPDATE: Ro and Keefe insisted on being at the next bandage change (or “the Great Fitzphie Ooze Fest”), and it was even stinkier than the last one. But Sophie’s hand was back to normal size! Time to focus on the nerve damage and rebuilding strength. SIXTH UPDATE: After a special mineral soak and a little more strength training, Sophie regained feeling in her hand and could FINALLY get out of bed! (!!!) SEVENTH UPDATE: Fitz went home yesterday, and Sophie’s mood seemed a little low.

Bipolar II disorder is a highly misunderstood form of bipolar illness. By its very designation as type II, clinicians, patients, and the public often assume it is less impairing than bipolar I, “the real thing.” When we examine the diagnostic criteria for bipolar II, they sound very mild. Who doesn’t get sad and happy? Who doesn’t have mood swings? Why would a four-day period of excess energy, which does not affect the ability to function, be of any clinical importance? Several longitudinal studies have found that bipolar II is far more impairing than we once thought. It is characterized by lengthy and recurrent periods of depression, comorbid anxiety disorders, and high rates of substance and alcohol misuse. The occasional hypomanias of bipolar II—in which people experience elation and irritability, exuberance, increased energy, and reduced need to sleep—are not as impairing as the full manic episodes of bipolar I, but they can certainly have a negative impact on family members and friends. Moreover, for the person with the disorder, these high periods are often short-lived, and they do little to alleviate the suffering caused by depressive phases. The hypomanic periods may even overlap with the low phases, resulting in an agitated, anxiety-ridden, and highly distressing period of depression. People with bipolar II often have difficulty maintaining jobs and relationships, and, like people with bipolar I, they are at high risk for suicide.

You see, I was adopted into what many consider the perfect family, and while I can say that I was raised in a loving home, there still wasn't enough love in the world that could’ve convinced me that I was enough. There wasn’t enough love in the world to make me believe I was loveable. Although my adoptive parents gave me all of their love, there wasn’t enough love in the world that could make me stop craving the love of my birth mother. It's taken me a very long time to accept myself. It’s taken years to win the war between who I am versus the crippling insecurities that made me hate myself. I’d love to be the perfect woman without flaws or insecurities, but this isn’t Barbie’s Dreamhouse. So, I apologize in advance for my inconsistency, at times. I apologize in advance for my mood swings. I apologize in advance for my immaturity. I apologize for my stupidity. I apologize for my moments of low self-esteem. I apologize for my lingering self-doubt. And I apologize for believing that I wasn’t good enough. I’m still a work in progress, and one day, I’ll even be confident enough to stop apologizing, but in the meantime, please bear with me. A man in a small Wisconsin city had been on the program for about three years and had enjoyed contented sobriety through that period. Then bad luck began to hit him in bunches. The firm for which he had worked for some fifteen years was sold; his particular job was phased out of existence, and the plant moved to another city. For several months, he struggled along at odd jobs while looking for a company that needed his specialized experience. Then another blow hit him. His wife was forced to enter a hospital for major surgery, and his company insurance had expired. At this point he cracked, and decided to go on an all-out binge. He didn’t want to stage this in the small city, where everyone knew his sobriety record. So he went to Chicago, checked in at a North Side hotel, and set forth on his project. It was Friday night, and the bars were filled with a swinging crowd. But he was in no mood for swinging—he just wanted to get quietly, miserably drunk. Finally, he found a basement bar on a quiet side street, practically deserted. He sat down on a bar stool and ordered a double bourbon on the rocks. The bartender said, “Yes, sir,” and reached for a bottle. Then the bartender stopped in his tracks, took a long, hard look at the customer, leaned over the bar, and said in a low tone, “I was in Milwaukee about four months ago, and one night I attended an open meeting. You were on the speaking platform, and you gave one of the finest A.A. talks I ever heard.” The bartender turned and walked to the end of the bar. For a few minutes, the customer sat there—probably in a state of shock. Then he picked his money off the bar with trembling hands and walked out, all desire for a drink drained out of him. It is estimated that there are about 8,000 saloons in Chicago, employing some 25,000 bartenders. The most visible feature of self-oriented perfectionism is this hypercompetitive streak fused to a sense of never being good enough. Hypercompetitiveness reflects a paradox because people high in self-oriented perfectionism can recoil from competition due to fear of failure and fear of losing other people's approval. Socially-prescribed perfectionism makes for a hugely pressured life, spent at the whim of everyone else's opinions, trying desperately to be somebody else, somebody perfect. Perfectionism lurks beneath the surface of mental distress. Someone who scores high on perfectionism also scores high on anxiety. The ill-effects of self-oriented perfectionism correlate with anxiety and it predicts increases in depression over time. There are links between other-oriented perfectionism and higher vindictiveness, a grandiose desire for admiration and hostility toward others, as well as lower altruism, compliance with social norms and trust. People with high levels of socially-prescribed perfectionism typically report elevated loneliness, worry about the future, need for approval, poor-quality relationships, rumination and brooding, fears of revealing imperfections to others, self-harm, worse physical health, lower life satisfaction and chronically low self-esteem. Perfectionism makes people extremely insecure, self-conscious and vulnerable to even the smallest hassles. Perfection is man's ultimate illusion. It simply doesn't exist in the universe. If you are a perfectionist, you are guaranteed to be a loser in whatever you do. Socially-prescribed perfectionism has an astonishingly strong link with burnout. What I don't have - or how perfectionism grows in the soil of our manufactured discontent. No matter what the advertisement says, you will go on with your imperfect existence whether you make that purchase or not. And that existence is - can only ever be - enough. Make a promise to be kind to yourself, taking ownership of your imperfections, recognizing your shared humanity and understanding that no matter how hard your culture works to teach you otherwise, no one is perfect and everyone has an imperfect life. Socially-prescribed perfectionism is the emblem of consumer culture. Research shows that roaming outside, especially in new places, contributes to enhanced well-being. Other benefits of getting out there in nature include improved attention, lower stress, better mood, reduced risk of psychiatric disorders and even upticks in empathy and cooperation. Perfection is not necessary to live an active and fulfilling life.
