Antidepression medication is temperamental. Somewhere around fifty-nine or sixty I noticed the drug I’d been taking seemed to have stopped working. This is not unusual. The drugs interact with your body chemistry in different ways over time and often need to be tweaked. After the death of Dr. Myers, my therapist of twenty-five years, I’d been seeing a new doctor whom I’d been having great success with. Together we decided to stop the medication I’d been on for five years and see what would happen... DEATH TO MY HOMETOWN!! I nose-dived like the diving horse at the old Atlantic City steel pier into a sloshing tub of grief and tears the likes of which I’d never experienced before. Even when this happens to me, not wanting to look too needy, I can be pretty good at hiding the severity of my feelings from most of the folks around me, even my doctor. I was succeeding well with this for a while except for one strange thing: TEARS! Buckets of ’em, oceans of ’em, cold, black tears pouring down my face like tidewater rushing over Niagara during any and all hours of the day. What was this about? It was like somebody opened the floodgates and ran off with the key. There was NO stopping it. 'Bambi' tears... 'Old Yeller' tears... 'Fried Green Tomatoes' tears... rain... tears... sun... tears... I can’t find my keys... tears. Every mundane daily event, any bump in the sentimental road, became a cause to let it all hang out. It would’ve been funny except it wasn’t. Every meaningless thing became the subject of a world-shattering existential crisis filling me with an awful profound foreboding and sadness. All was lost. All... everything... the future was grim... and the only thing that would lift the burden was one-hundred-plus on two wheels or other distressing things. I would be reckless with myself. Extreme physical exertion was the order of the day and one of the few things that helped. I hit the weights harder than ever and paddleboarded the equivalent of the Atlantic, all for a few moments of respite. I would do anything to get Churchill’s black dog’s teeth out of my ass. Through much of this I wasn’t touring. I’d taken off the last year and a half of my youngest son’s high school years to stay close to family and home. It worked and we became closer than ever. But that meant my trustiest form of self-medication, touring, was not at hand. I remember one September day paddleboarding from Sea Bright to Long Branch and back in choppy Atlantic seas. I called Jon and said, “Mr. Landau, book me anywhere, please.” I then of course broke down in tears. Whaaaaaaaaaa. I’m surprised they didn’t hear me in lower Manhattan. A kindly elderly woman walking her dog along the beach on this beautiful fall day saw my distress and came up to see if there was anything she could do. Whaaaaaaaaaa. How kind. I offered her tickets to the show. I’d seen this symptom before in my father after he had a stroke. He’d often mist up. The old man was usually as cool as Robert Mitchum his whole life, so his crying was something I loved and welcomed. He’d cry when I’d arrive. He’d cry when I left. He’d cry when I mentioned our old dog. I thought, “Now it’s me.” I told my doc I could not live like this. I earned my living doing shows, giving interviews and being closely observed. And as soon as someone said “Clarence,” it was going to be all over. So, wisely, off to the psychopharmacologist he sent me. Patti and I walked in and met a vibrant, white-haired, welcoming but professional gentleman in his sixties or so. I sat down and of course, I broke into tears. I motioned to him with my hand; this is it. This is why I’m here. I can’t stop crying! He looked at me and said, “We can fix this.” Three days and a pill later the waterworks stopped, on a dime. Unbelievable. I returned to myself. I no longer needed to paddle, pump, play or challenge fate. I didn’t need to tour. I felt normal.

The next year, in March 1888, her parents sent her to Philadelphia, to be examined and cared for by Dr. Silas Weir Mitchell, a physician famous for treating patients, mainly women, suffering from neurasthenia, or nervous exhaustion. Mitchell’s solution for Theodate was his then-famous “Rest Cure,” a period of forced inactivity lasting up to two months. “At first, and in some cases for four or five weeks, I do not permit the patient to sit up or to sew or write or read,” Mitchell wrote, in his book Fat and Blood. “The only action allowed is that needed to clean the teeth.” He forbade some patients from rolling over on their own, insisting they do so only with the help of a nurse. “In such cases I arrange to have the bowels and water passed while lying down, and the patient is lifted on to a lounge at bedtime and sponged, and then lifted back again into the newly-made bed.” For stubborn cases, he reserved mild electrical shock, delivered while the patient was in a filled bathtub. His method reflected his own dim view of women. In his book Wear and Tear; or, Hints for the Overworked, he wrote that women “would do far better if the brain were very lightly tasked.

I was already an atheist, and by my senior year I had became obsessed with the question “What is the meaning of life?” I wrote my personal statement for college admissions on the meaninglessness of life. I spent the winter of my senior year in a kind of philosophical depression—not a clinical depression, just a pervasive sense that everything was pointless. In the grand scheme of things, I thought, it really didn’t matter whether I got into college, or whether the Earth was destroyed by an asteroid or by nuclear war. My despair was particularly strange because, for the first time since the age of four, my life was perfect. I had a wonderful girlfriend, great friends, and loving parents. I was captain of the track team, and, perhaps most important for a seventeen-year-old boy, I got to drive around in my father’s 1966 Thunderbird convertible. Yet I kept wondering why any of it mattered. Like the author of Ecclesiastes, I thought that “all is vanity and a chasing after wind” (ECCLESIASTES 1:14) . I finally escaped when, after a week of thinking about suicide (in the abstract, not as a plan), I turned the problem inside out. There is no God and no externally given meaning to life, I thought, so from one perspective it really wouldn’t matter if I killed myself tomorrow. Very well, then everything beyond tomorrow is a gift with no strings and no expectations. There is no test to hand in at the end of life, so there is no way to fail. If this really is all there is, why not embrace it, rather than throw it away? I don’t know whether this realization lifted my mood or whether an improving mood helped me to reframe the problem with hope; but my existential depression lifted and I enjoyed the last months of high school.

Charles’ wife, Susannah, said of Charles at that time, “My beloved’s anguish was so deep and violent, that reason seemed to totter in her throne, and we sometimes feared that he would never preach again.”5 Though it cannot be said for all of us or for every person that we have loved, it remains true that, in this cherished case, Charles Spurgeon did preach again. But sorrows of many kinds haunted and hounded him for the rest of his life. His depression came, not only from circumstances, or from questions about whether or not he was consecrated to God, but also from the chemistry of his body. God gave to us a preacher who knew firsthand what it felt like for his reason to totter, not just once, but many times during his life and ministry. And somehow this fellow sufferer named Charles and his dear wife Susannah (who also suffered physically most of her adult life) still made a go of it, insisting to each other and to their generation that the sorrowing have a Savior. On that November morning, in weakness, Charles did what some of us are not yet able to do in our sorrows; he read the Bible. Perhaps it will comfort you to learn that for a while “the very sight of the Bible” made Charles cry.6 Many of us know what this feels like. But this Scripture passage, Philippians 2:9-11, “had such a power of comfort upon [his] distressed spirit.” And being found in human form, he [Jesus] humbled himself by becoming obedient to the point of death, even death on a cross. Therefore God has highly exalted him and bestowed on him the name that is above every name (Phil. 2:8-9). From this Scripture, Charles set the larger story of his hope before us. The same Heavenly Father who picked up His son out of the muck, misery and mistreatment can do the same for us.

The World State that governs the entire globe is never threatened by wars, revolutions, strikes or demonstrations, because all people are supremely content with their current conditions, whatever they may be. Huxley’s vision of the future is far more troubling than George Orwell’s Nineteen Eighty-Four. Huxley’s world seems monstrous to most readers, but it is hard to explain why. Everybody is happy all the time – what could be wrong with that?

Peace and happiness which it could be said are not only the goal, but the baseline, default state of being that we naturally return to—once whatever led to anxiety, anger, or sadness stepping in has been resolved. You may well have experienced the relief, and lightness of being, that’s the result of instigating a difficult yet necessary conversation or quitting a job or relationship that’s been crushing your spirit. Could it be that joy was there all along, like a balloon held underwater always trying to bob to the surface? People always feel sorry for you if you’re physically sick. It doesn’t matter if you have cancer or a cold. People always feel sorry for you and ask you if you’re okay. You need money? You got it! You want to meet a celebrity? Of course you can! You want to go to a convention, ComiCon, Disney World, anywhere in the world? You’re going to go there. That doesn’t happen when you’re mentally ill. If you’re mentally ill, people look at you differently. People roll their eyes when you talk about how sad you are. People won’t lift a finger to help you. “Get a job,” they’ll tell you. “Stop being so lazy. Be grateful you don’t have cancer. Get over it. It’s in the past. You have no reason to be sad.” And that isn’t how it works. But, of course, they wouldn’t know that. They’ve never been mentally ill, they don’t know how you can be so permanently damaged by your past that your present is painful and your future looks bleak. They don’t understand that most days getting out of bed is a chore. They don’t get that sometimes getting a job is out of the question because you’re just too damn afraid to even speak to anyone. That isn’t something you can just get over. But no one knows that because mental illnesses aren’t a real problem apparently. Apparently, the fact that over 800,000 million people die from suicide each year isn’t a real problem. Apparently, the fact that 15% of the adolescent population self-harms isn’t a real problem either. And, apparently, it isn’t a cause to worry that one in 200 American women suffer from an eating disorder. I had already gone through a period of buying everything there was to buy at the Pottery Barn and decorating my apartment like a Pottery Barn outlet, and then getting rid of it all during a massive upgrade. Now everything at the Pottery Barn looked ersatz and mass-produced. To buy any of it now would be to regress in aspiration and selfhood. I didn’t want to buy anything at the Pottery Barn so much as I wanted to recapture the feeling of wanting to buy everything from the Pottery Barn. Something similar happened at the music store. I should try to find some new music, I thought, because there was a time when new music could lift me out of a funk like nothing else. But I wasn’t past the Bs when I saw the only thing I really cared to buy. It was the Beatles’ Rubber Soul, which had been released in 1965. I already owned Rubber Soul. I had owned Rubber Soul on vinyl, then on cassette, and now on CD, and of course on my iPod, iPod mini, and iPhone. If I wanted to, I could have pulled out my iPhone and played Rubber Soul from start to finish right there, on speaker, for the sake of the whole store. But that wasn’t what I wanted. I wanted to buy Rubber Soul for the first time all over again. I wanted to return the needle from the run-out groove to the opening chords of “Drive My Car” and make everything new again. That wasn’t going to happen. But, I thought, I could buy it for somebody else. I could buy somebody else the new experience of listening to Rubber Soul for the first time. So I took the CD up to the register and paid for it and, walking out, felt renewed and excited. But the first kid I offered it to, a rotund teenager in a wheelchair looking longingly into a GameStop window, declined on the principle that he would rather have cash. A couple of other kids didn’t have CD players. I ended up leaving Rubber Soul on a bench beside a decommissioned ashtray where someone had discarded an unhealthy gob of human hair. I wandered, as everyone in the mall sooner or later does, into the Best Friends Pet Store. Many best friends—impossibly small beagles and corgis and German shepherds—were locked away for display in white cages where they spent their days dozing with depression, stirring only long enough to ponder the psychic hurdles of licking their paws. Could there be anything better to lift your spirits than a new puppy?

Sometimes when we’re being tested by discouragement, it seems God is silent. We pray and we don’t hear anything. We read the Scripture and still come away feeling like God is a million miles away. But remember, this is a test. When you’re in school, teachers never talk during tests. They stand up at the front of the room very quietly just watching all of the students taking the exam. The teachers have been preparing you in the days and weeks prior to the test. Often, they’ve put in extra hours making sure everyone has the opportunity to succeed. On test day, they want to see if you’ve learned the lessons. They know that you have the information you need. They know you’re prepared. You’re ready. Now all you’ve got to do is put into practice what you’ve learned. God works the same way as your teachers here on earth. When He is silent, don’t assume He has left you. He is right there with you during the test. The silence means only that God has prepared you, and now He is watching to see if you have learned. He would not give you the test unless He knew you were ready. God is not mad at you when He is silent. He has not forsaken you. His silence is a sign that He has great confidence in you. He knows you have what it takes. He knows you will come through the test victoriously or He would not have permitted you to be tested. The key is to remain upbeat and not be discouraged or bitter. Put into practice what you’ve learned. Stay in faith. Hang on to your happiness. Treat others kindly. Be a blessing. If you do that, you will pass the test and flourish in a new season. God will bring things out of you that you didn’t even know were in you. Understand, if you don’t allow the enemy to discourage you, one of his greatest weapons has been lost. Today is a new day. God is breathing new hope into your heart and new vision into your spirit. He is the Glory and the lifter of our heads. Look up with a fresh vision, and God will do for you what He promised David. He will lift you out of the pit. He will set your feet on a rock. He will put a new song in your heart. You won’t drag through life defeated and depressed. You will soar through life full of joy, full of faith, full of victory.

Some areas in which you can set up your life to fit your temperament are: --Have the right level of busyness in your life. Note that being understimulated (for example, having too few enjoyable activities to look forward to) can be as much of a problem as being overstimulated. --Pick the physical activity level that’s right for you. Fine-tuning your physical activity level could be as simple as getting up from your desk and taking a walk periodically to keep yourself feeling calm and alert. Lifting things (such as carrying shopping bags up stairs) can also increase feelings of alertness and energy. Having pleasurable activities to look forward to and enough physical activity will help protect you against depression. --Have the right level of social contact in your life, and have routines that put this on autopilot. For example, a routine of having drinks after work on a Friday with friends, or attending a weekly class with your sister. Achieving the right level of social contact might also include putting mechanisms in place to avoid too much social interruption, like having office hours rather than an open-door policy. --Keep a balance of change and routine in your life. For example, alternate going somewhere new for your vacation vs. returning to somewhere you know you like. What the right balance of change and routine is for you will depend on your natural temperament and how much change vs. stability feels good to you. --Allow yourself the right amount of mental space to work up to doing something—enough time that you can do some mulling over the prospect of getting started but not so much time that it starts to feel like avoidance of getting started. --If coping with change sucks up a lot of energy for you, be patient with yourself, especially if you’re feeling stirred up by change or a disruption to your routines or plans. As mentioned in Chapter 2, keep some habits and relationships consistent when you’re exploring change in other areas. --Have self-knowledge of what types of stress you find most difficult to process. Don’t voluntarily expose yourself to those types without considering alternatives. For example, if you want a new house and you know you get stressed out by making lots of decisions, then you might choose to buy a house that’s already built, rather than building your own home. If you know making home-improvement decisions is anxiety provoking for you, you might choose to move to a house that’s new or recently renovated, rather than doing any major work on your current home or buying a fixer-upper. There’s always a balance with avoidance coping, where some avoidance of the types of stress that you find most taxing can be very helpful.

Here’s a classic case in point: Eva had been taking an antidepressant for two years but wanted to get off it because she was planning to get pregnant. Her doctor advised her not to stop taking the drug, which motivated her to see me. Eva explained that her saga had begun with PMS, featuring a week each month when she was irritable and prone to crying fits. Her doctor prescribed a birth control pill (a common treatment) and soon Eva was feeling even worse, with insomnia, fatigue, low libido, and a generally flat mood dogging her all month long. That’s when the doctor added the Wellbutrin to “pick her up,” as he said, and handle her presumed depression. From Eva’s perspective, she felt that the antidepressant helped her energy level, but it had limited benefits in terms of her mood and libido. And if she took it after midnight, her insomnia was exacerbated. She soon became accustomed to feeling stable but suboptimal, and she was convinced that the medication was keeping her afloat. The good news for Eva was that with careful preparation, she could leave medication behind—and restore her energy, her equilibrium, and her sense of control over her emotions. Step one consisted of some basic diet and exercise changes along with better stress response strategies. Step two involved stopping birth control pills and then testing her hormone levels. Just before her period, she had low cortisol and progesterone, which were likely the cause of the PMS that started her whole problem. Further testing revealed borderline low thyroid function, which may well have been the result of the contraceptives—and the cause of her increased depressive symptoms. When Eva was ready to begin tapering off her medication, she did so following my protocol. Even as her brain and body adjusted to not having the antidepressant surging through her system anymore, her energy levels improved, her sleep problems resolved, and her anxiety lifted.
