How come South Africa and Botswana, which have the highest female literacy and per capita incomes in Africa, are awash with HIV, while countries that score low on both - such as Guinea, Somalia, Mali, and Sierra Leone - have epidemics that are negligible by comparison? How come in country after country across Africa itself, from Cameroon to Uganda to Zimbabwe and in a dozen other countries as well, HIV is lowest in the poorest households, and highest in the richest households? And how is it that in many countries, more educated women are more likely to be infested with HIV than women with no schooling? Given the central place that technology holds in our lives, it is astonishing that technology companies have not put more resources into fixing this global problem. Advanced computer systems and artificial intelligence (AI) could play a much bigger role in shaping diagnosis and prescription. While the up-front costs of using such technology may be sizeable, the long-term benefits to the health-care system need to be factored into value assessments. We believe that AI platforms could improve on the empirical prescription approach. Physicians work long hours under stressful conditions and have to keep up to date on the latest medical research. To make this work more manageable, the health-care system encourages doctors to specialize. However, the vast majority of antibiotics are prescribed either by generalists (e.g., general practitioners or emergency physicians) or by specialists in fields other than infectious disease, largely because of the need to treat infections quickly. An AI system can process far more information than a single human, and, even more important, it can remember everything with perfect accuracy. Such a system could theoretically enable a generalist doctor to be as effective as, or even superior to, a specialist at prescribing. The system would guide doctors and patients to different treatment options, assigning each a probability of success based on real-world data. The physician could then consider which treatment was most appropriate.

The scariest thing is that nobody seems to be considering the impact on those wild fish of fish farming on the scale that is now being proposed on the coast of Norway or in the open ocean off the United States. Fish farming, even with conventional techniques, changes fish within a few generations from an animal like a wild buffalo or a wildebeest to the equivalent of a domestic cow. Domesticated salmon, after several generations, are fat, listless things that are good at putting on weight, not swimming up fast-moving rivers. When they get into a river and breed with wild fish, they can damage the wild fish's prospects of surviving to reproduce. When domesticated fish breed with wild fish, studies indicate the breeding success initially goes up, then slumps as the genetically different offspring are far less successful at returning to the river. Many of the salmon in Norwegian rivers, which used to have fine runs of unusually large fish, are now of farmed origin. Domesticated salmon are also prone to potentially lethal diseases, such as infectious salmon anemia, which has meant many thousands have had to be quarantined or killed. They are also prone to the parasite Gyrodactylus salaris, which has meant that whole river systems in Norway have had to be poisoned with the insecticide rotenone and restocked.

Whenever they decided to do a bit of extra work – say, to hoe the fields instead of scattering seeds on the surface – people thought, ‘Yes, we will have to work harder. But the harvest will be so bountiful! We won’t have to worry any more about lean years. Our children will never go to sleep hungry.’ It made sense. If you worked harder, you would have a better life. That was the plan. The first part of the plan went smoothly. People indeed worked harder. But people did not foresee that the number of children would increase, meaning that the extra wheat would have to be shared between more children. Neither did the early farmers understand that feeding children with more porridge and less breast milk would weaken their immune system, and that permanent settlements would be hotbeds for infectious diseases. They did not foresee that by increasing their dependence on a single source of food, they were actually exposing themselves even more to the depredations of drought. Nor did the farmers foresee that in good years their bulging granaries would tempt thieves and enemies, compelling them to start building walls and doing guard duty.

Governing bodies such as the CDC and the WHO can control the number of cases by simply advising the medical industry to increase or decrease the cycle threshold (CT).” In August of 2020, the New York Times reported that “a CT beyond 34 revolutions very rarely detect live virus, but most often, dead nucleotides that are not even contagious. In compliance with guidance from the CDC and the WHO, many top US labs have been conducting tests at cycle thresholds of 40 or more. NYT examined data from Massachusetts, New York, and Nevada and determined that up to 90 percent of the individuals who tested positive carried barely any virus.”17 90 percent! In May of 2021, CDC changed the PCR cycle threshold from 40 to 28 or lower for those who have been vaccinated. This one adjustment of the numbers allowed the vaccine pushers to praise the vaccines as a big success.

The downside is that he or she also should expect to die more slowly. The two most common causes of death in 1935 America were respiratory diseases (pneumonia and influenza) and infectious diarrhea, both of which kill rapidly. In contrast, the two most common causes of death in 2007 America were heart disease and cancer (each accounted for about 25 percent of total deaths). Some heart attack victims die within minutes or hours, but most elderly people with heart disease survive for years while coping with complications such as high blood pressure, congestive heart failure, general weakness, and peripheral vascular disease. Many cancer patients also remain alive for several years following their diagnosis because of chemo-therapy, radiation, surgery, and other treatments. In addition, many of the other leading causes of death today are chronic illnesses such as asthma, Alzheimer’s, type 2 diabetes, and kidney disease, and there has been an upsurge in the occurrence of nonfatal but chronic illnesses such as osteoarthritis, gout, dementia, and hearing loss.63 Altogether, the growing prevalence of chronic illness among middle-aged and elderly individuals is contributing to a health-care crisis because the children born during the post–World War II baby boom are now entering old age, and an unprecedented percentage of them are suffering from lingering, disabling, and costly diseases. Why? For one thing, psychotherapy consists of a gradual unfolding process wherein the therapist attempts to know the patient as fully as possible. A diagnosis limits vision; it diminishes ability to relate to the other as a person. Once we make a diagnosis, we tend to selectively inattend to aspects of the patient that do not fit into that particular diagnosis, and correspondingly overattend to subtle features that appear to confirm an initial diagnosis. What’s more, a diagnosis may act as a self-fulfilling prophecy. Relating to a patient as a “borderline” or a “hysteric” may serve to stimulate and perpetuate those very traits. Indeed, there is a long history of iatrogenic influence on the shape of clinical entities, including the current controversy about multiple-personality disorder and repressed memories of sexual abuse. And keep in mind, too, the low reliability of the DSM personality disorder category (the very patients often engaging in longer-term psychotherapy).

He stopped me one day in the Eisenhower Executive Office Building, where we were tested routinely for COVID, and again blasted my failure to encourage people to take hydroxychloroquine, the lack of which he said was causing people to die. He would not let it go. Perhaps he just had a thing about me. To give him the benefit of the doubt, I arranged with Cliff Lane to have Navarro present via Zoom his case on hydroxychloroquine’s effectiveness to the entire NIH guidelines panel cochaired by Cliff in early August. This group was thirty-five of the top experts in infectious disease, public health, and epidemiology from all over the country. Navarro made his presentation, and uniformly they politely said, “Mr. Navarro, there’s nothing there. These are anecdotes, and all the evidence indicates hydroxychloroquine doesn’t work and can even cause harm.” Navarro’s answer was that he valued his reading of the existing medical literature on hydroxychloroquine as much as or more than theirs. “If I am wrong, no one is harmed. If you are wrong, thousands of people die.” The truth was the exact opposite. By that time, the FDA, which had given hydroxychloroquine emergency approval early in the pandemic, had revoked it on June 15, after it was found to cause heart problems and even death, not to mention proving ineffective against COVID. I had given Navarro one last chance, but he still could not accept reality.

The poor may not always be with us.
