It was Freud's ambition to discover the cause of hysteria, the archetypal female neurosis of his time. I've known other people with the same aversion, or awareness, and they weren't even famous, fame is not a deciding factor, there are many individuals who experience their life as if it were the material of some detailed report, and they inhabit that life pending its hypothetical or future plot. They don't give it much thought, it's just a way of experiencing things, companionable, in a way, as if there were always spectators or permanent witnesses, even of their most trivial goings-on and in the dullest of times. Perhaps it's a substitute for the old idea of the omnipresence of God, who saw every second of each of our lives, it was very flattering in a way, very comforting despite the implicit threat and punishment, and three or four generations aren't enough for Man to accept that his gruelling existence goes on without anyone ever observing or watching it, without anyone judging it or disapproving of it. And in truth there is always someone: a listener, a reader, a spectator, a witness, who can also double up as simultaneous narrator and actor: the individuals tell their stories to themselves, to each his own, they are the ones who peer in and look at and notice things on a daily basis, from the outside in a way; or, rather, from a false outside, from a generalised narcissism, sometimes known as "consciousness". That's why so few people can withstand mockery, humiliation, ridicule, the rush of blood to the face, a snub, that least of all ... I've known men like that, men who were nobody yet who had that same immense fear of their own history, of what might be told and what, therefore, they might tell too. Of their blotted, ugly history. But, I insist, the determining factor always comes from outside, from something external: all this has little to do with shame, regret, remorse, self-hatred although these might make a fleeting appearance at some point. These individuals only feel obliged to give a true account of their acts or omissions, good or bad, brave, contemptible, cowardly or generous, if other people (the majority, that is) know about them, and those acts or omissions are thus encorporated into what is known about them, that is, into their official portraits. It isn't really a matter of conscience, but of performance, of mirrors. One can easily cast doubt on what is reflected in mirrors, and believe that it was all illusory, wrap it up in a mist of diffuse or faulty memory and decide finally that it didn't happen and that there is no memory of it, because there is no memory of what did not take place. Then it will no longer torment them: some people have an extraordinary ability to convince themselves that what happened didn't happen and what didn't exist did.

How much can the disapproval or neglect of public opinion be offset by the memory of the steady attention of significant people in the past? In relationships, do we have enough self-love to leave an abusive union? Or are we so down on ourselves that we carry an implicit belief that harm is all we deserve? In a different vein, how good are we at apologizing to a lover for things that may be our fault? How rigidly self-righteous do we need to be? Can we dare to admit mistakes or does an admission of guilt or error bring us too close to our background sense of nullity? In the bedroom, how clean and natural or alternatively disgusting and unacceptable do our desires feel? Might they be a little odd, but not for that matter bad or dark, since they emanate from within us and we are not wretches? At work, do we have a reasonable, well-grounded sense of our worth and so feel able to ask for (and properly expect to get) the rewards we are due? Can we resist the need to please others indiscriminately? Are we sufficiently aware of our genuine contribution to be able to say no when we need to?

I begin this chapter with President Ronald Reagan’s Farewell Speech on January 11, 1989. President Reagan encouraged the rising generation to “let ’em know and nail ’em on it”—that is, to push back against teachers, professors, journalists, politicians, and others in the governing generation who manipulate and deceive them: An informed patriotism is what we want. And are we doing a good enough job teaching our children what America is and what she represents in the long history of the world? Those of us who are over 35 or so years of age grew up in a different America. We were taught, very directly, what it means to be an American. And we absorbed, almost in the air, a love of country and an appreciation of its institutions. If you didn’t get these things from your family, you got them from the neighborhood, from the father down the street who fought in Korea or the family who lost someone at Anzio. Or you could get a sense of patriotism from school. And if all else failed, you could get a sense of patriotism from the popular culture. The movies celebrated democratic values and implicitly reinforced the idea that America was special. TV was like that, too, through the mid-sixties. But now, we’re about to enter the nineties, and some things have changed. Younger parents aren’t sure that an unambivalent appreciation of America is the right thing to teach modern children. And as for those who create the popular culture, well-grounded patriotism is no longer the style. Our spirit is back, but we haven’t reinstitutionalized it. We’ve got to do a better job of getting across that America is freedom—freedom of speech, freedom of religion, freedom of enterprise. And freedom is special and rare. It’s fragile; it needs [protection]. So, we’ve got to teach history based not on what’s in fashion but what’s important—why the Pilgrims came here, who Jimmy Doolittle was, and what those 30 seconds over Tokyo meant. You know, 4 years ago on the 40th anniversary of D-Day, I read a letter from a young woman writing to her late father, who’d fought on Omaha Beach. Her name was Lisa Zanatta Henn, and she said, “We will always remember, we will never forget what the boys of Normandy did.” Well, let’s help her keep her word. If we forget what we did, we won’t know who we are. I’m warning of an eradication of the American memory that could result, ultimately, in an erosion of the American spirit. Let’s start with some basics: more attention to American history and a greater emphasis on civic ritual. And let me offer lesson number one about America: All great change in America begins at the dinner table. So, tomorrow night in the kitchen, I hope the talking begins. And children, if your parents haven’t been teaching you what it means to be an American, let ’em know and nail ’em on it. That would be a very American thing to do.1

And it does so at the cost of a simplification that no longer has anything to do with language or with the slightest ambivalence and which, therefore, puts an end to this subtle lack of distinction between the real and the virtual, as subtle as the lack of distinction between good and evil. Through special effects, everything acquires an operational self-evidence, a spectacular reality that is, properly speaking, the reign of simulation. What the directors of these films have not realized (any more than the simulationist artists of New York in the eighties) is that simulation is a hypothesis, a game that turns reality itself into one eventuality among others.

The biology of potential illness arises early in life. The brain’s stress-response mechanisms are programmed by experiences beginning in infancy, and so are the implicit, unconscious memories that govern our attitudes and behaviours toward ourselves, others and the world. Cancer, multiple sclerosis, rheumatoid arthritis and the other conditions we examined are not abrupt new developments in adult life, but culminations of lifelong processes. The human interactions and biological imprinting that shaped these processes took place in periods of our life for which we may have no conscious recall. Emotionally unsatisfying child-parent interaction is a theme running through the one hundred or so detailed interviews I conducted for this book. These patients suffer from a broadly disparate range of illnesses, but the common threads in their stories are early loss or early relationships that were profoundly unfulfilling emotionally. Early childhood emotional deprivation in the histories of adults with serious illness is also verified by an impressive number of investigations reported in the medical and psychological literature. In an Italian study, women with genital cancers were reported to have felt less close to their parents than healthy controls. They were also less demonstrative emotionally. A large European study compared 357 cancer patients with 330 controls. The women with cancer were much less likely than controls to recall their childhood homes with positive feelings. As many as 40 per cent of cancer patients had suffered the death of a parent before the age of seventeen—a ratio of parental loss two and a half times as great as had been suffered by the controls. The thirty-year follow-up of Johns Hopkins medical students was previously quoted. Those graduates whose initial interviews in medical school had revealed lower than normal childhood closeness with their parents were particularly at risk. By midlife they were more likely to commit suicide or develop mental illness, or to suffer from high blood pressure, coronary heart disease or cancer. In a similar study, Harvard undergraduates were interviewed about their perception of parental caring. Thirty-five years later these subjects’ health status was reviewed. By midlife only a quarter of the students who had reported highly positive perceptions of parental caring were sick. By comparison, almost 90 per cent of those who regarded their parental emotional nurturing negatively were ill. “Simple and straightforward ratings of feelings of being loved are significantly related to health status,” the researchers concluded.

Although out of awareness, they can plague him and his relationships for his entire life unless he finds a way to bring them into conscious awareness and work with them. Furthermore, when the caregiver is attuned in her early interactions, her more mature nervous system is regulating the infant’s neurochemistry and homeostasis. This, in turn, has a profound influence on the structural organization of the developing brain. Conversely emotional trauma will negatively impact the parts of the brain which are developing at the time of trauma. For example, if high levels of stress hormones are circulating in a pregnant mother, it up-regulates the fetus’ developing stress response – making the child, and future adult hypersensitive to stress. Relational trauma that occurs around the time of birth has a negative impact on both the developing micro-architecture of the amygdala itself, and the amygdala’s connection to the HPA axis, as well as to other parts of the limbic system. Thus high levels of early unrepaired interpersonal stress have a profoundly harmful effect on the ability to form social bonds, and on temperament. Suffering unrepaired and frequent emotional stress after about ten months interferes with the experience-dependent maturation of the highest level regulatory systems in the right orbifrontal cortex. Schore emphasized that when the caregiver is unable to help the child to regulate either a specific emotion or intense emotions in general, or – worse – that she exacerbates the dysregulation, the child will start to go into a state of hypoaroused dissociation as soon as a threat of dysregulation arises. This temporaily reduces conscious emotional pain in the child living with chronic trauma, but those who characterologically use the emotion-deadening defense of dissociation to cope with stressful interpersonal events subsequently dissociate to defend against both daily stresses, and the stress caused when implicitly held memories of trauma are triggered. In the developing brain, repeated neurological states become traits, so dissociative defense mechanisms are embedded into the core structure of the evolving personality, and become a part of who a person is, rather than what a person does. Dissociation, which appears in the first month of life, seems to be a last resort survival strategy. It represents detachment from an unbearable situation. The infant withdraws into an inner world, avoids eye contact and stares into space. Dissociation triggered by a hypoaroused state results in a constricted state of consciousness, and a void of subjectivity. Being cut off from our emotions impacts our sense of who we are as a person. Our subjective sense of self derives from our unconscious experience of bodily-based emotions and is neurologically constructed in the right brain. If we cannot connect to our bodily emotions then our sense of self is built on fragile foundations. Many who suffered early relational trauma have a disturbed sense of their bodies and of what is happening within them physiologically as well as emotionally. The interview moved along to the topic of how we can possibly master these adverse and potentially damaging relational experiences. Schore replied by explaining that the human brain remains plastic and capable of learning throughout the entire life span, and that with the right therapeutic help and intervention we can move beyond dissociation as our primary defense mechanism, and begin to regulate our emotions more appropriately. When the relationship between the therapist and the client develops enough safety, the therapeutic alliance can act as a growth-facilitating environment that offers a corrective emotional experience via “rewiring” the right brain and associated neurocircuits.

Sand tray can be an embodied conversation between our inner world and outer awareness, held and witnessed by another. Because of the tactile experiences of the sand and minatures and the symbolic nature of the figures, we have the opportunity to make contact with implicit memories that have no words. Over the years, it has seemed to me that working with the children inside is only half the process. These imported others continue to add their pain, fear and associated capacity for wounding other people to the stream of implicit memory that can be touched and awakened. They are equal members of our inner community. This internalisation process may be a primary way that wounds get passed from generation to generation. My father’s harshly critical voice can be touched and awakened in me, then directed at my own child. At the same time, the ongoing wounds these internalized ones carry can mean that our protective parts need to keep vigilant so that the remaining distress doesn’t overwhelm our outside world all the time and to ensure the still wounded ones aren’t further heart. This continuing sense of danger requires us to devote resources that otherwise might flow towards warm relationships and creativity to containing the injury.

For me, hearing a relational history at the beginning of our work helps me form pictures of some encounters that bought pain and others that offered empathic support. Early in life, who comforted this person? Who kept her safe? Who was distant? Who needed her to regulate them? Who felt dangerous? Who bought confusion or chaos? Who criticized and who was accepting? We might quickly discover that one person brought contradictory experiences - the confusing one also comforted, or the dangerous one at home was a primary support of safety in the outside world. All this helps us begin to feel into the qualities of relatedness our person has taken in.

... when an experience is too strong for our current internal and external regulatory resource to manage ... [chemical changes activate to] tuck these pathways into our ... body. In this way, our ongoing lives are protected from the constant incursion of the raw pain and fear and the injured parts of ourselves are partly shielded from new injury. We might say they have been enwombed, awaiting the arrival of support. At the same time, the memories also remain malleable enough that they can be touched and awakened, which is essential for healing. However, we also remain vulnerable to them being brought into activity when support isn't available... a frowning face (man or woman), certain breathing patterns, and even sensory fragments (the color of a person's shirt or hair, the smell of alcohol on someone's breath) all have some probability of awakening the terror. The widely dispersed individual streams that make up these memories are all gathered into the neural net that formed at the time of the initial experience, and when our outer or inner world tugs on any strand, there is some probability that more of the neural net will open, bringing the rush of embodied feelings. Most often, the explicit memory does not arrive at the same time, so there is no context for the flood of sensations and emotions, which feels as if they are related to what is happening right now .... What can look like an out-of-proportion response to what is happening in the moment is exactly in proportion to what is unfolding internally. If we sense this so deeply that this knowing is viscerally available when our patients are having strong emotional experiences, we will be able to offer them acknowledgement of the validity of their experience rather than having to control or change it.

When we experience a break in connection followed by repeated attempts at repair until the bond is restored, we build implicit pathways of resilience. We come to know in a visceral way that when things break down interpersonally, someone will return to help us come back into relationship. That wired-in optimism and expectation makes it much more likely that we will form relationships that have this quality. Most of the people who come to us haven't had this experience consistently in their lives, so when they encounter it with us, it is often surprising to the point of tears. As we accept and then rejoice in our humanness, we offer this vital gift of rupture and repair to those around us.

Certainly Dian needed the gorillas. But perhaps the gorillas understood Dian better than any human ever did. Ian Redmond told a story at the National Geographic memorial benefit for Dian. He hadn’t planned to tell it; it was prompted by a question: how did the gorillas react to Dian’s death? “This goes beyond the bounds of strict science,” Ian said. “Just after Dian’s death, three gorilla groups who had been at some distance from Visoke suddenly homed in on the mountain. One group traveled almost continually for two days to arrive in the vicinity.” Ian is a scientist and would not want to volunteer the interpretation implicit in the gorillas’ sudden, purposeful movement toward the mountain that was Dian’s home: that they had come, in her hour of death, to be near to her.
