Nurses and technicians do not feel safe bringing negative information to doctors or correcting a doctor who is about to make a mistake. Doctors will argue that if the others were “professionals” they would speak up, but in many a hospital the nurses will tell you that doctors feel free to yell at nurses in a punishing way, which creates a climate where nurses will certainly not speak up. Doctors engage patients in one-way conversations in which they ask only enough questions to make a diagnosis and sometimes make misdiagnoses because they don’t ask enough questions before they begin to tell patients what they should do.

These people, he felt, had chosen not to know. Another survivor of the Nazis, the social psychologist Ervin Staub, was saved from a certain death by Raoul Wallenberg, the diplomat who made Swedish passports for thousands of desperate Hungarians, keeping them safe from the Nazis. Staub studied cruelty and hatred, and he found one of the roots of such evil to be the turning away, choosing not to see or know, of bystanders. That not-knowing was read by perpetrators as a tacit approval. But if instead witnesses spoke up in protest of evil, Staub saw, it made such acts more difficult for the evildoers. For Frankl, the “not-knowing” he encountered in postwar Vienna was regarding the Nazi death camps scattered throughout that short-lived empire, and the obliviousness of Viennese citizens to the fate of their own neighbors who were imprisoned and died in those camps. The underlying motive for not-knowing, he points out, is to escape any sense of responsibility or guilt for those crimes. People in general, he saw, had been encouraged by their authoritarian rulers not to know—a fact of life today as well. That same plea of innocence, I had no idea, has contemporary resonance in the emergence of an intergenerational tension. Young people around the world are angry at older generations for leaving as a legacy to them a ruined planet, one where the momentum of environmental destruction will go on for decades, if not centuries. This environmental not-knowing has gone on for centuries, since the Industrial Revolution. Since then we have seen the invention of countless manufacturing platforms and processes, most all of which came to be in an era when we had no idea of their ecological impacts. Advances in science and technology are making ecological impacts more transparent, and so creating options that address the climate crisis and, hopefully, will be pursued across the globe and over generations. Such disruptive, truly “green” alternatives are one way to lessen the bleakness of Earth 2.0—the planet in future decades—a compelling fact of life for today’s young. Were Frankl with us today (he died in 1997), he would no doubt be pleased that so many of today’s younger people are choosing to know and are finding purpose and meaning in surfacing environmental facts and acting on them.

Sankai’s company, Cyberdyne, has also designed a more robust version of the exoskeleton for use by workers cleaning up the Fukushima Daiichi nuclear plant in the wake of the 2011 disaster. The company says the suit will almost completely offset the burden of over 130 pounds of tungsten radiation shielding worn by workers.* HAL is the first elder-care robotic device to be certified by Japan’s Ministry of Economy, Trade, and Industry. The suits lease for just under $2,000 per year and are already in use at over three hundred Japanese hospitals and nursing homes.21

Charging for admission, membership, subscriptions and, for some, earning profits are similarities these institutions have with businesses. Still, that doesn’t make them industries, which turn things into commodities and sell them for gain.

The land, thinly populated and producing in wasteful, haphazard way enormous surpluses of every kind, is deemed by its owners, a mere handful of men, unable to accommodate not only the starving millions of Europe but our own starving hordes. A country which makes itself ridiculous by sending out missionaries to the most remote parts of the globe, asking for pennies of the poor in order to maintain the Christian work of deluded devils who no more represent Christ than I do the Pope, and yet unable through its churches and missions at home to rescue the weak and defeated, the miserable and the oppressed. The hospitals, the insane asylums, the prisons filled to overflowing. Counties, some of them big as a European country, practically uninhabited, owned by an intangible corporation whose tentacles reach everywhere and whose responsibilities nobody can formulate or clarify. A man seated in a comfortable chair in New York, Chicago or San Francisco, a man surrounded by every luxury and yet paralyzed with fear and anxiety, controls the lives and destinies of thousands of men and women whom he has never seen, whom he never wishes to see and whose fate he is thoroughly uninterested in.

Permanent Revolution THE INDUSTRIAL REVOLUTION OPENED up new ways to convert energy and to produce goods, largely liberating humankind from its dependence on the surrounding ecosystem. Humans cut down forests, drained swamps, dammed rivers, flooded plains, laid down hundreds of thousands of miles of railroad tracks, and built skyscraping metropolises. As the world was moulded to fit the needs of Homo sapiens, habitats were destroyed and species went extinct. Our once green and blue planet is becoming a concrete and plastic shopping centre. Today, the earth’s continents are home to billions of Sapiens. If you took all these people and put them on a large set of scales, their combined mass would be about 300 million tons. If you then took all our domesticated farmyard animals – cows, pigs, sheep and chickens – and placed them on an even larger set of scales, their mass would amount to about 700 million tons. In contrast, the combined mass of all surviving large wild animals – from porcupines and penguins to elephants and whales – is less than 100 million tons. Our children’s books, our iconography and our TV screens are still full of giraffes, wolves and chimpanzees, but the real world has very few of them left. There are about 80,000 giraffes in the world, compared to 1.5 billion cattle; only 200,000 wolves, compared to 400 million domesticated dogs; only 250,000 chimpanzees – in contrast to billions of humans. Humankind really has taken over the world.1 Ecological degradation is not the same as resource scarcity. As we saw in the previous chapter, the resources available to humankind are constantly increasing, and are likely to continue to do so. That’s why doomsday prophesies of resource scarcity are probably misplaced. In contrast, the fear of ecological degradation is only too well founded. The future may see Sapiens gaining control of a cornucopia of new materials and energy sources, while simultaneously destroying what remains of the natural habitat and driving most other species to extinction. In fact, ecological turmoil might endanger the survival of Homo sapiens itself. Global warming, rising oceans and widespread pollution could make the earth less hospitable to our kind, and the future might consequently see a spiralling race between human power and human-induced natural disasters. As humans use their power to counter the forces of nature and subjugate the ecosystem to their needs and whims, they might cause more and more unanticipated and dangerous side effects. These are likely to be controllable only by even more drastic manipulations of the ecosystem, which would result in even worse chaos. Many call this process ‘the destruction of nature’. But it’s not really destruction, it’s change. Nature cannot be destroyed. Sixty-five million years ago, an asteroid wiped out the dinosaurs, but in so doing opened the way forward for mammals. Today, humankind is driving many species into extinction and might even annihilate itself. But other organisms are doing quite well. Rats and cockroaches, for example, are in their heyday. These tenacious creatures would probably creep out from beneath the smoking rubble of a nuclear Armageddon, ready and able to spread their DNA. Perhaps 65 million years from now, intelligent rats will look back gratefully on the decimation wrought by humankind, just as we today can thank that dinosaur-busting asteroid. Still, the rumours of our own extinction are premature. Since the Industrial Revolution, the world’s human population has burgeoned as never before. In 1700 the world was home to some 700 million humans. In 1800 there were 950 million of us. By 1900 we almost doubled our numbers to 1.6 billion. And by 2000 that quadrupled to 6 billion. Today there are just shy of 7 billion Sapiens.

They’ll shrivel the Flats into a toothless granny. They’ll drain the swamp into a scab. The Old Ones will have nowhere to live. And if that doesn’t kill them, industry will. The factories and hospitals and shipyards that are sure to come. The Old Ones can’t survive a world filled with metal. They can’t survive the clatter and growl of machinery. I leave the Flats. The fields are not too far now. Just down the road. But the road looks long and I feel the prickle of tears again. It’s because I’ve been ill, I know. That’s all it is. And when the bog-holes are puckered shut, where will the Boggy Mun go? Will he go to the sea? And if he does, what then? Is the sea too big to drain? Probably not. Look what mankind can create. Now you can photograph a person moving, and when you look at the photograph, you’ll actually see him moving, which is why it’s called a moving picture. This is hard to believe, I know, but still, we humans are inventing such astonishing things. I shouldn’t be surprised if, in time, we’ll be able to drain the sea. And what of the Old Ones? The answers are perhaps as varied as the questions one asks, but a common theme that comes through in discussions with caregivers on the front lines and those who think a great deal about patient safety, is our failure to change our culture. What we have not done, they say, is create a “culture of safety,” as has been done so impressively in other industries, such as commercial aviation, nuclear power and chemical manufacturing. These “high-reliability organizations” are intrinsically hazardous enterprises that have succeeded in becoming (amazingly!) safe. Worse, the culture of health care is not only unsafe, it is incredibly dysfunctional. Though the culture of each health care organization is unique, they all suffer many of the same disabilities that have, so far, effectively stymied progress: An authoritarian structure that devalues many workers, lack of a sense of personal accountability, autonomous functioning and major barriers to effective communication. What is a culture of safety? Pretty much the opposite! Books have been written on the subject, and every expert has his or her own specific definition. But an underlying theme, a common denominator, is teamwork, founded on an open, supportive, mutually reinforcing, dedicated relationship among all participants. Much more is required, of course: Sensitivity to hazard, sense of personal responsibility, attitudes of awareness and risk, sense of personal responsibility and more. But those attitudes, that type of teamwork and those types of relationships are rarely found in health care organizations.

The project was in the district of Democratic Congressman Rudolph Tenerowicz, who persuaded his colleagues that funding for the agency should be cut off unless Foreman was fired and the Sojourner Truth units were assigned only to whites. The director of the Federal Housing Administration supported Tenerowicz, stating that the presence of African Americans in the area would threaten property values of nearby residents. Foreman was forced to resign. The Federal Works Agency then proposed a different project for African Americans on a plot that the Detroit Housing Commission recommended, in an industrial area deemed unsuitable for whites. It soon became apparent that this site, too, would provoke protests because it was not far enough away from a white neighborhood. First Lady Eleanor Roosevelt protested to the president. The FWA again reversed course and assigned African Americans to the Sojourner Truth project. Whites in the neighborhood rioted, leading to one hundred arrests (all but three were African Americans) and thirty-eight hospitalizations (all but five were African Americans). Following the war, Detroit's politicians moblized white voters by stirring up fear of integration in public housing. Mayor Edward Jeffries's successful 1945 reelection campaign warned that projects with African Americans could be located in white neighborhoods if his opponent, Dick Frankensteen, won. Jeffries's literature proclaimed, 'Mayor Jeffries Is Against Mixed Housing.

The one thing that defined me—my love of cars—was absolutely killed by that job. Everyone who got in a car with me said I had the best job in the world, and for a while, I agreed with them. But after 30,000 laps on the same track, I can tell you I want nothing more to do with them. I did that job because I loved driving cars. I didn’t do it because I loved hospitality or the thrill customers received. I did it because I drove cars I couldn’t afford. I was in it for the wrong reasons. Don’t “do what you love,” because even if you are lucky to make a living doing it, you won’t love it for very long. You should love the value you create. When comparative price is taken into account, Home offered much more for the money in terms of current (or past) earnings and dividends. The very low book value of Home illustrates a basic ambiguity or contradiction in common-stock analysis. On the one hand, it means that the company is earning a high return on its capital—which in general is a sign of strength and prosperity. On the other, it means that the investor at the current price would be especially vulnerable to any important adverse change in the company’s earnings situation. Since Hospital was selling at over four times its book value in 1969, this cautionary remark must be applied to both companies. TABLE 18-3. Pair 3. CONCLUSIONS: Our clear-cut view would be that both companies were too “rich” at their current prices to be considered by the investor who decides to follow our ideas of conservative selection. As we’ve seen, up to 25 percent of employed seniors from our top universities are heading to financial services each year. Our financial services industry (and to a lesser extent its attendant legal industry) plays an equivalent role to the oil industry in Saudi Arabia in terms of talent attraction. You can see a similar dynamic at work in other fields with fixed slots. There were 682 orthopedic surgery residents in the United States in 2012. That number is set because there are only so many funded residency slots in teaching hospital programs throughout the country.4 If I were to kick butt in medical school and get one of these residencies, I would be on the way to becoming an orthopedic surgeon, probably the most coveted residency due to money, lifestyle, low morbidity of patients, gratification from restoring mobility, and other factors. But let’s say that I didn’t make it and fell short—there would still be 682 orthopedic surgeons five years from now because the next guy would have gotten that slot. We’re all competing to fit through the same finite gate. The value difference if I perform really strongly and get one of these coveted spots is not one more surgeon—it’s the gap between me and the 683rd person who didn’t get it (and perhaps went into a less prestigious or less lucrative specialty). From a value creation standpoint, it’s not ideal for a massive level of talent to be going to existing enterprises that have captured large economic rents or where people are fighting for a set of finite slots. The rents and slots will stay essentially constant. Contrast this with new business formation. If I were to say, “There are only going to be 682 new successful businesses started in the United States next year,” people would instantly regard that as ridiculous. It’s unknown and unknowable. But we all know that if another enterprising team comes along and starts a cool company, that number goes up by one.

There is only one way for women to reach full human potential—by participating in the mainstream of society, by exercising their own voice in all the decisions shaping that society. For women to have full identity and freedom, they must have economic independence. Breaking through the barriers that had kept them from the jobs and professions rewarded by society was the first step, but it wasn’t sufficient. It would be necessary to change the rules of the game to restructure professions, marriage, the family, the home. The manner in which offices and hospitals are structured, along the rigid, separate, unequal, unbridgeable lines of secretary/executive, nurse/doctor, embodies and perpetuates the feminine mystique. But the economic part would never be complete unless a dollar value was somehow put on the work done by women in the home, at least in terms of social security, pensions, retirement pay. And housework and child rearing would have to be more equally shared by husband, wife, and society. Equality and human dignity are not possible for women if they are not able to earn. When the young radical kids came into the movement, they said it was “boring” or “reformist” or “capitalist co-option” to place so much emphasis on jobs and education. But very few women can afford to ignore the elementary economic facts of life. Only economic independence can free a woman to marry for love, not for status or financial support, or to leave a loveless, intolerable, humiliating marriage, or to eat, dress, rest, and move if she plans not to marry. But the importance of work for women goes beyond economics. How else can women participate in the action and decisions of an advanced industrial society unless they have the training and opportunity and skills that come from participating in it?
