Certain traits — otherwise known as coping styles — magnify the risk for illness by increasing the likelihood of chronic stress. Common to them all is a diminished capacity for emotional communication. Emotional experiences are translated into potentially damaging biological events when human beings are prevented from learning how to express their feelings effectively. That learning occurs — or fails to occur — during childhood. The way people grow up shapes their relationship with their own bodies and psyches. The emotional contexts of childhood interact with inborn temperament to give rise to personality traits. Much of what we call personality is not a fixed set of traits, only coping mechanisms a person acquired in childhood. There is an important distinction between an inherent characteristic, rooted in an individual without regard to his environment, and a response to the environment, a pattern of behaviours developed to ensure survival. What we see as indelible traits may be no more than habitual defensive techniques, unconsciously adopted. People often identify with these habituated patterns, believing them to be an indispensable part of the self. They may even harbour self-loathing for certain traits — for example, when a person describes herself as “a control freak.” In reality, there is no innate human inclination to be controlling. What there is in a “controlling” personality is deep anxiety. The infant and child who perceives that his needs are unmet may develop an obsessive coping style, anxious about each detail. When such a person fears that he is unable to control events, he experiences great stress. Unconsciously he believes that only by controlling every aspect of his life and environment will he be able to ensure the satisfaction of his needs. As he grows older, others will resent him and he will come to dislike himself for what was originally a desperate response to emotional deprivation. The drive to control is not an innate trait but a coping style. Emotional repression is also a coping style rather than a personality trait set in stone. Not one of the many adults interviewed for this book could answer in the affirmative when asked the following: When, as a child, you felt sad, upset or angry, was there anyone you could talk to — even when he or she was the one who had triggered your negative emotions? In a quarter century of clinical practice, including a decade of palliative work, I have never heard anyone with cancer or with any chronic illness or condition say yes to that question. Many children are conditioned in this manner not because of any intended harm or abuse, but because the parents themselves are too threatened by the anxiety, anger or sadness they sense in their child — or are simply too busy or too harassed themselves to pay attention. “My mother or father needed me to be happy” is the simple formula that trained many a child — later a stressed and depressed or physically ill adult — into lifelong patterns of repression.

My mother's cell grilled her week after week, month after month, forcing her to produce endless self-criticisms.She had to consent to this agonizing process. Life for a revolutionary was meaningless if they were rejected by the Party. It was like excommunication for a Catholic. Besides, it was standard procedure. My father had gone through it and had accepted it as part of 'joining the revolution." In fact, he was still going through it. The Party had never hidden the fact that it was a painful process. Addictions always originate in pain, whether felt openly or hidden in the unconscious. They are emotional anaesthetics. Heroin and cocaine, both powerful physical painkillers, also ease psychological discomfort. Infant animals separated from their mothers can be soothed readily by low doses of narcotics, just as if it was actual physical pain they were enduring. The pain pathways in humans are no different. The very same brain centres that interpret and “feel” physical pain also become activated during the experience of emotional rejection: on brain scans they “light up” in response to social ostracism just as they would when triggered by physically harmful stimuli. When people speak of feeling “hurt” or of having emotional “pain,” they are not being abstract or poetic but scientifically quite precise. The hard-drug addict’s life has been marked by a surfeit of pain. No wonder she desperately craves relief.

The first publicized exercise of such random (and collective) arrangement produced the following poetic sentence: The exquisite cadavers shall drink the new wine. (Les cadavres exquis boiront le vin nouveau.) Impressive? It sounds even more poetic in the native French. Quite impressive poetry has been produced in such a manner, sometimes with the aid of a computer. But poetry has never been truly taken seriously outside of the beauty of its associations, whether they have been produced by the random ranting of one or more disorganized brains, or the more elaborate constructions of one conscious creator.

We consider it a physical threat, because animals in the natural world are surveilled by predators. Surveillance makes us feel like prey, just as it makes the surveillors act like predators. Psychologists, sociologists, philosophers, novelists, and technologists have all written about the effects of constant surveillance, or even just the perception of constant surveillance. Studies show that we are less healthy, both physically and emotionally. We have feelings of low self-esteem, depression, and anxiety. Surveillance strips us of our dignity. It threatens our very selves as individuals. It’s a dehumanizing tactic employed in prisons and detention camps around the world.

Or perhaps both characters are just facets of a hidden, deeper soul that I have yet to meet. I just don’t know. Sometimes I fear that the drug-free Flynn searingly manic, then catastrophically depressed – is who he really is. But because in that form he is not acceptable to conventional society, he has to be drugged so that his emotions are tempered and his behavior controlled. Perhaps we are blindly living in an Orwellian society where individualism is feared and the biggest pressure is the one to conform. Perhaps Flynn is sane and the rest of the world is mad. The thoughts go round and round in my head.

Predominantly inattentive type Perhaps the majority of girls with AD/HD fall into the primarily inattentive type, and are most likely to go undiagnosed. Generally, these girls are more compliant than disruptive and get by rather passively in the academic arena. They may be hypoactive or lethargic. In the extreme, they may even seem narcoleptic. Because they do not appear to stray from cultural norms, they will rarely come to the attention of their teacher. Early report cards of an inattentive type girl may read, "She is such a sweet little girl. She must try harder to speak up in class." She is often a shy daydreamer who avoids drawing attention to herself. Fearful of expressing herself in class, she is concerned that she will be ridiculed or wrong. She often feels awkward, and may nervously twirl the ends of her hair. Her preferred seating position is in the rear of the classroom. She may appear to be listening to the teacher, even when she has drifted off and her thoughts are far away. These girls avoid challenges, are easily discouraged, and tend to give up quickly. Their lack of confidence in themselves is reflected in their failure excuses, such as, "I can't," "It's too hard," or "I used to know it, but I can't remember it now." The inattentive girl is likely to be disorganized, forgetful, and often anxious about her school work. Teachers may be frustrated because she does not finish class work on time. She may mistakenly be judged as less bright than she really is. These girls are reluctant to volunteer for a project orjoin a group of peers at recess. They worry that other children will humiliate them if they make a mistake, which they are sure they will. Indeed, one of their greatest fears is being called on in class; they may stare down at their book to avoid eye contact with the teacher, hoping that the teacher will forget they exist for the moment. Because interactions with the teacher are often anxiety-ridden, these girls may have trouble expressing themselves, even when they know the answer. Sometimes, it is concluded that they have problems with central auditory processing or expressive language skills. More likely, their anxiety interferes with their concentration, temporarily reducing their capacity to both speak and listen. Generally, these girls don't experience this problem around family or close friends, where they are more relaxed. Inattentive type girls with a high IQ and no learning disabilities will be diagnosed with AD/HD very late, if ever. These bright girls have the ability and the resources to compensate for their cognitive challenges, but it's a mixed blessing. Their psychological distress is internalized, making it less obvious, but no less damaging. Some of these girls will go unnoticed until college or beyond, and many are never diagnosed they are left to live with chronic stress that may develop into anxiety and depression as their exhausting, hidden efforts to succeed take their toll. Just how important a close moment-to-moment connection between mother and infant can be was illustrated by a cleverly designed study, known as the “double TV experiment,” in which infants and mothers interacted via a closed-circuit television system. In separate rooms, infant and mother observed each other and, on “live feed,” communicated by means of the universal infant-mother language: gestures, sounds, smiles, facial expressions. The infants were happy during this phase of the experiment. “When the infants were unknowingly replayed the ‘happy responses’ from the mother recorded from the prior minute,” writes the UCLA child psychiatrist Daniel J. Siegel, “they still became as profoundly distressed as infants do in the classic ‘flat face’ experiments in which mothers-in-person gave no facial emotional response to their infant’s bid for attunement.” Why were the infants distressed despite the sight of their mothers’ happy and friendly faces? Because happy and friendly are not enough. What they needed were signals that the mother is aligned with, responsive to and participating in their mental states from moment to moment. All that was lacking in the instant video replay, during which infants saw their mother’s face unresponsive to the messages they, the infants, were sending out. This sharing of emotional spaces is called attunement. Emotional stress on the mother interferes with infant brain development because it tends to interfere with the attunement contact. Attunement is necessary for the normal development of the brain pathways and neurochemical apparatus of attention and emotional selfregulation. It is a finely calibrated process requiring that the parent remain herself in a relatively nonstressed, non-anxious, nondepressed state of mind. Its clearest expression is the rapturous mutual gaze infant and mother direct at each other, locked in a private and special emotional realm, from which, at that moment, the rest of the world is as completely excluded as from the womb. Attunement does not mean mechanically imitating the infant. It cannot be simulated, even with the best of goodwill. As we all know, there are differences between a real smile and a staged smile. The muscles of smiling are exactly the same in each case, but the signals that set the smile muscles to work do not come from the same centers in the brain. As a consequence, those muscles respond differently to the signals, depending on their origin. This is why only very good actors can mimic a genuine, heartfelt smile. The attunement process is far too subtle to be maintained by a simple act of will on the part of the parent. Infants, particularly sensitive infants, intuit the difference between a parent’s real psychological states and her attempts to soothe and protect the infant by means of feigned emotional expressions. A loving parent who is feeling depressed or anxious may try to hide that fact from the infant, but the effort is futile. In fact, it is much easier to fool an adult with forced emotion than a baby. The emotional sensory radar of the infant has not yet been scrambled. It reads feelings clearly. They cannot be hidden from the infant behind a screen of words, or camouflaged by well-meant but forced gestures. It is unfortunate but true that we grow far more stupid than that by the time we reach adulthood.

Our sensory organs and brains provide the interface through which relationships shape our evolution from infancy to adulthood. Social-emotional interactions decisively influence the development of the human brain. From the moment of birth, they regulate the tone, activity and development of the psychoneuroimmunoendocrine (PNI) super-system. Our characteristic modes of handling psychic and physical stress are set in our earliest years. Neuroscientists at Harvard University studied the cortisol levels of orphans who were raised in the dreadfully neglected child-care institutions established in Romania during the Ceausescu regime. In these facilities the caregiver/child ratio was one to twenty. Except for the rudiments of care, the children were seldom physically picked up or touched. They displayed the self-hugging motions and depressed demeanour typical of abandoned young, human or primate. On saliva tests, their cortisol levels were abnormal, indicating that their hypothalamic-pituitary-adrenal axes were already impaired. As we have seen, disruptions of the HPA axis have been noted in autoimmune disease, cancer and other conditions. It is intuitively easy to understand why abuse, trauma or extreme neglect in childhood would have negative consequences. But why do many people develop stress-related illness without having been abused or traumatized? These persons suffer not because something negative was inflicted on them but because something positive was withheld.

It may seem paradoxical to claim that stress, a physiological mechanism vital to life, is a cause of illness. To resolve this apparent contradiction, we must differentiate between acute stress and chronic stress. Acute stress is the immediate, short-term body response to threat. Chronic stress is activation of the stress mechanisms over long periods of time when a person is exposed to stressors that cannot be escaped either because she does not recognize them or because she has no control over them. Discharges of nervous system, hormonal output and immune changes constitute the flight-or-fight reactions that help us survive immediate danger. These biological responses are adaptive in the emergencies for which nature designed them. But the same stress responses, triggered chronically and without resolution, produce harm and even permanent damage. Chronically high cortisol levels destroy tissue. Chronically elevated adrenalin levels raise the blood pressure and damage the heart. There is extensive documentation of the inhibiting effect of chronic stress on the immune system. In one study, the activity of immune cells called natural killer (NK) cells were compared in two groups: spousal caregivers of people with Alzheimer’s disease, and age- and health-matched controls. NK cells are front-line troops in the fight against infections and against cancer, having the capacity to attack invading micro-organisms and to destroy cells with malignant mutations. The NK cell functioning of the caregivers was significantly suppressed, even in those whose spouses had died as long as three years previously. The caregivers who reported lower levels of social support also showed the greatest depression in immune activity — just as the loneliest medical students had the most impaired immune systems under the stress of examinations. Another study of caregivers assessed the efficacy of immunization against influenza. In this study 80 per cent among the non-stressed control group developed immunity against the virus, but only 20 per cent of the Alzheimer caregivers were able to do so. The stress of unremitting caregiving inhibited the immune system and left people susceptible to influenza. Research has also shown stress-related delays in tissue repair. The wounds of Alzheimer caregivers took an average of nine days longer to heal than those of controls. Higher levels of stress cause higher cortisol output via the HPA axis, and cortisol inhibits the activity of the inflammatory cells involved in wound healing. Dental students had a wound deliberately inflicted on their hard palates while they were facing immunology exams and again during vacation. In all of them the wound healed more quickly in the summer. Under stress, their white blood cells produced less of a substance essential to healing. The oft-observed relationship between stress, impaired immunity and illness has given rise to the concept of “diseases of adaptation,” a phrase of Hans Selye’s. The flight-or-fight response, it is argued, was indispensable in an era when early human beings had to confront a natural world of predators and other dangers. In civilized society, however, the flight-fight reaction is triggered in situations where it is neither necessary nor helpful, since we no longer face the same mortal threats to existence. The body’s physiological stress mechanisms are often triggered inappropriately, leading to disease. There is another way to look at it. The flight-or-fight alarm reaction exists today for the same purpose evolution originally assigned to it: to enable us to survive. What has happened is that we have lost touch with the gut feelings designed to be our warning system. The body mounts a stress response, but the mind is unaware of the threat. We keep ourselves in physiologically stressful situations, with only a dim awareness of distress or no awareness at all.

Adaptiveness is the capacity to respond to external stressors without rigidity, with flexibility and creativity, without excessive anxiety and without being overwhelmed by emotion. People who are not adaptive may seem to function well as long as nothing is disturbing them, but they will react with various levels of frustration and helplessness when confronted by loss or by difficulty. They will blame themselves or blame others. A person’s adaptiveness depends very much on the degree of differentiation and adaptiveness of previous generations in his family and also on what external stressors may have acted on the family. The Great Depression, for example, was a difficult time for millions of people. The multigenerational history of particular families enabled some to adapt and cope, while other families, facing the same economic scarcities, were psychologically devastated. “Highly adaptive people and families, on the average, have fewer physical illnesses, and those illnesses that do occur tend to be mild to moderate in severity,” writes Dr. Michael Kerr. Since one important variable in the development of physical illness is the degree of adaptiveness of an individual, and since the degree of adaptiveness is determined by the multigenerational emotional process, physical illness, like emotional illness, is a symptom of a relationship process that extends beyond the boundaries of the individual “patient.” Physical illness, in other words, is a disorder of the family emotional system [which includes] present and past generations. Children who become their parents’ caregivers are prepared for a lifetime of repression. And these roles children are assigned have to do with the parents’ own unmet childhood needs — and so on down the generations. “Children do not need to be beaten to be compromised,” researchers at McGill University have pointed out. Inappropriate symbiosis between parent and child is the source of much pathology.

He forsakes the fountain of living waters and hews out broken cisterns that can hold no water (Jer. 2:13). He is as a hungry man who dreams that he is eating, and when he awakes finds that his soul is empty; and he is like a thirsty man who dreams that he is drinking, and when he awakes finds that he is faint and that his soul has appetite (Isa. 29:8). Science cannot explain this contradiction in man. It reckons only with his greatness and not with his misery, or only with his misery and not with his greatness. It exalts him too high, or it depresses him too far, for science does not know of his Divine origin, nor of his profound fall. But the Scriptures know of both, and they shed their light over man and over mankind; and the contradictions are reconciled, the mists are cleared, and the hidden things are revealed. Anything short of the attitude of a loving mother toward a charming child is taken as proof of a lack of love. These men usually confuse their affectionate behavior, their wish to please, with genuine love and thus arrive at the conclusion that they are being treated quite unfairly; they imagine themselves to be the great lovers and complain bitterly about the ingratitude of their love partner. In rare cases such a mother-centered person can function without any severe disturbances. If his mother, in fact, 'loved' him in an overprotective manner (perhaps being domineering, but without being destructive), if he finds a wife of the same motherly type, if his special gifts and talents permit him to use his charm and be admired (as is the case sometimes with successful politicians), he is 'well adjusted' in a social sense, without ever reaching a higher level of maturity. But under less favorable conditions -and these are naturally more frequent- his love life, if not his social life, will be a serious disappointment; conflicts, and frequently intense anxiety and depression arise when this type of personality is left alone.

Psychologically, this would have favoured a tendency towards both anxiety about others and also depression-like sickness behaviour during infection.10 Across human history, from generation to generation, there has thus been a leaning towards an ‘inflammatory bias’.

At the beginning of the twenty-first century, physicians such as Iain McInnes were finding out that new biologic drugs designed for inflammatory conditions like rheumatoid arthritis could treat symptoms of depression. A number of researchers have since trawled back through the results of clinical trials that tested anti-inflammatory drugs for various chronic inflammatory conditions such as rheumatoid arthritis, lupus, psoriasis, asthma and inflammatory bowel disease. They were particularly interested in trials that also reported data for symptoms of depression as a secondary outcome. The overwhelming picture is that anti-inflammatory drugs – from older, generalized drugs such as ibuprofen and aspirin to new, targeted monoclonal antibodies – tend to improve depressive symptoms.22 One study, carried out in 2020, was able to look at very detailed patient data from eighteen clinical trials and factored in patients’ physical improvements. These trials had used a range of new anti-inflammatory drugs designed to target the inflammation behind various autoimmune diseases. Two drugs in particular demonstrated an improvement in low mood, regardless of improvement in physical symptoms. These were both antibody drugs that specifically targeted pro-inflammatory cytokines: sirukumab relieves rheumatoid arthritis by targeting IL-6, and ustekinumab treats psoriasis by blocking the cytokines for IL-12 and IL-23. Here we have data to back up Iain McInnes’s observations in his arthritis clinic: anti-inflammatory medication treating the psychological symptoms of depression, regardless of the state of physical symptoms. This doesn’t just help to strengthen an argument for inflammation causing some forms of depression; it offers hope of a treatment.

The results of the first clinical trial of one of the new generation of anti-inflammatory drugs were published in 2013. Eminent American academic psychiatrists Charles Raison and Andrew Miller led a study at Emory University in Atlanta, Georgia, in which patients with depression were given either a drug called infliximab or a placebo.25 Infliximab is a biologic drug that targets the pro-inflammatory cytokine TNF-α and is used in a number of autoimmune conditions. But the results were disappointing: no difference in depression-score reduction between infliximab and the placebo. But when they analysed the data further, they saw that a subgroup of patients did respond well to the medication: those who tended to have a mildly raised background level of inflammatory markers. Specifically, these were patients who had a raised baseline level of C-reactive protein (CRP) – a generic marker of inflammation in the body. A later, larger study assessed the anti-IL-6 drug sirukumab on patients who had not responded to conventional antidepressants and who had raised baseline levels of CRP.26 In this group, the drug improved only the depressive symptom of anhedonia (inability to experience pleasure), though in patients with a very high background CRP, it helped relieve depression better than the placebo.

It is a reality in which a nuanced order, balance, harmony and purpose lay hidden behind every important occurrence. Ours is a cosmos that is ruled by Natural Law.
