To exist, a cancer needs a living organism, but it cannot ever become a living organism. Its whole malice—there I go again—lies in the fact that the “best” it can do is to die with its host. Either that or its host will find the measures with which to extirpate and outlive it. But, as I knew before I became ill, there are some people for whom this explanation is unsatisfying. To them, a rodent carcinoma really is a dedicated, conscious agent—a slow–acting suicide–murderer—on a consecrated mission from heaven. The vengeful deity has a sadly depleted arsenal if all he can think of is exactly the cancer that my age and former “lifestyle” would suggest that I got. Fourth, why cancer at all? Almost all men get cancer of the prostate if they live long enough: It’s an undignified thing but quite evenly distributed among saints and sinners, believers and unbelievers. If you maintain that god awards the appropriate cancers, you must also account for the numbers of infants who contract leukemia. Devout persons have died young and in pain. Betrand Russell and Voltaire, by contrast, remained spry until the end, as many psychopathic criminals and tyrants have also done. These visitations, then, seem awfully random. My so far uncancerous throat, let me rush to assure my Christian correspondent above, is not at all the only organ with which I have blasphemed. And even if my voice goes before I do, I shall continue to write polemics against religious delusions, at least until it’s hello darkness my old friend. In which case, why not cancer of the brain? In their book American Grace: How Religion Divides and Unites Us, political scientists Robert Putnam and David Campbell analyzed a variety of data sources to describe how religious and nonreligious Americans differ. Common sense would tell you that the more time and money people give to their religious groups, the less they have left over for everything else. But common sense turns out to be wrong. Putnam and Campbell found that the more frequently people attend religious services, the more generous and charitable they become across the board.58 Of course religious people give a lot to religious charities, but they also give as much as or more than secular folk to secular charities such as the American Cancer Society.59 They spend a lot of time in service to their churches and synagogues, but they also spend more time than secular folk serving in neighborhood and civic associations of all sorts. Putnam and Campbell put their findings bluntly: By many different measures religiously observant Americans are better neighbors and better citizens than secular Americans—they are more generous with their time and money, especially in helping the needy, and they are more active in community life.60 Why are religious people better neighbors and citizens? To find out, Putnam and Campbell included on one of their surveys a long list of questions about religious beliefs (e.g., “Do you believe in hell? Do you agree that we will all be called before God to answer for our sins?”) as well as questions about religious practices (e.g., “How often do you read holy scriptures? How often do you pray?”). These beliefs and practices turned out to matter very little. Whether you believe in hell, whether you pray daily, whether you are a Catholic, Protestant, Jew, or Mormon … none of these things correlated with generosity. The only thing that was reliably and powerfully associated with the moral benefits of religion was how enmeshed people were in relationships with their co-religionists. It’s the friendships and group activities, carried out within a moral matrix that emphasizes selflessness. That’s what brings out the best in people. Putnam and Campbell reject the New Atheist emphasis on belief and reach a conclusion straight out of Durkheim: “It is religious belongingness that matters for neighborliness, not religious believing.”61

Did you know that if you’re a middle-aged woman, you have only a small window of opportunity between the beginning of perimenopause and the start of menopause to start estrogen replacement therapy to protect not only your brain but also your bones and cardiovascular system? I did not, until I dug into the science, because as a woman who was diagnosed with a stage 0 breast lump, I was scared off like so many of us from the results of the Women’s Health Initiative, which got blasted out all over the news and initially showed a link between estrogen replacement therapy and breast cancer, but guess what? That study had so many flaws, its findings are little more than useless and possibly harmful. Worse, women like me without uteri show a decrease in breast cancer with estrogen replacement therapy. But this information never made it either into the headlines or into our gynecologists’ offices. I had to find it in scientific publications such as The Lancet online. In fact, get this: Our medical system barely trains gynecologists in menopausal medicine. A recent study found that only 20 percent of ob-gyn residency programs in the U.S. provide any menopause training. Yes, any. Which means that 80 percent of all gynecological residents in school today are getting no training whatsoever in post-reproductive women’s health. These are people whose job it is to know everything going on in our ladyparts, but they have not been taught the basic tenets of how to care for either us or our plumbing after we stop menstruating. And by “us” I mean 30 percent of all women alive on earth at any given moment. Half of my middle-aged female friends deal with chronic urinary tract infections. Oh, well, we think, throwing up our hands in defeat and consuming far too many antibiotics than are rational or safe or even good for the future safety of humanity. It took Dr. Rachel Rubin, a urologist in Washington, D.C., reaching out to me over Twitter to explain that UTIs in menopausal women do not have to be recurrent. Friendly, funny, chatty – despite the litres of fluid in her abdomen splinting her breathing – we were like two long-lost pals finding themselves next to each other at a bus stop and catching up on all our years apart. Her son has a place at med school, her daughter is at the same school my sister went to, she recognized my socks were Duchamp. I stuck in a Bonanno catheter to take off the fluid and admitted her to the ward for the day team to investigate. And now she’s telling me what they found. She bursts into tears, and out come all the ‘will never’s, the crushing realization that ‘forever’ is just a word on the front of Valentine’s cards. Her son will qualify from medical school – she won’t be there. Her daughter will get married – she won’t be able to help with the table plan or throw confetti. She’ll never meet her grandchildren. Her husband will never get over it. ‘He doesn’t even know how to work the thermostat!’ She laughs, so I laugh. I really don’t know what to say. I want to lie and tell her everything’s going to be fine, but we both know that it won’t. I hug her. I’ve never hugged a patient before – in fact, I think I’ve only hugged a grand total of five people, and one of my parents isn’t on that list – but I don’t know what else to do. We talk about boring practical things, rational concerns, irrational concerns, and I can see from her eyes it’s helping her. It suddenly strikes me that I’m almost certainly the first person she’s opened up to about all this, the only one she’s been totally honest with. It’s a strange privilege, an honour I didn’t ask for. The other thing I realize is that none of her many, many concerns are about herself; it’s all about the kids, her husband, her sister, her friends. Maybe that’s the definition of a good person.

Each tribe’s solution to its central problem is a brilliant, hard-won advance. But the true Master Algorithm must solve all five problems, not just one. For example, to cure cancer we need to understand the metabolic networks in the cell: which genes regulate which others, which chemical reactions the resulting proteins control, and how adding a new molecule to the mix would affect the network. It would be silly to try to learn all of this from scratch, ignoring all the knowledge that biologists have painstakingly accumulated over the decades. Symbolists know how to combine this knowledge with data from DNA sequencers, gene expression microarrays, and so on, to produce results that you couldn’t get with either alone. But the knowledge we obtain by inverse deduction is purely qualitative; we need to learn not just who interacts with whom, but how much, and backpropagation can do that. Nevertheless, both inverse deduction and backpropagation would be lost in space without some basic structure on which to hang the interactions and parameters they find, and genetic programming can discover it. At this point, if we had complete knowledge of the metabolism and all the data relevant to a given patient, we could figure out a treatment for her. But in reality the information we have is always very incomplete, and even incorrect in places; we need to make headway despite that, and that’s what probabilistic inference is for. In the hardest cases, the patient’s cancer looks very different from previous ones, and all our learned knowledge fails. Pretend for a moment that you are in the horrifying situation of watching one of your children being pulled out to sea in a riptide. Would you just go on eating your lunch? No way. The first thing you would do is to scream to get help rescuing your child. You would simultaneously get all other children out of the water as you dive in and try to rescue the missing child, even knowing the danger and that it is probably too late. If you were sensible enough not to swim out or fortunate enough to get back to shore safely, grief would promote endless rumination about what you could have done to prevent the loss. This would help prevent a repetition with other children. Your sobbing would signal your need for help and warn others about the danger. When a child dies of cancer or pneumonia, speculating about what you might have done to prevent it is mostly useless. However, the tendency to blame is built in, so people do it anyway, blaming themselves, doctors, anyone who was involved. Those motives can create marvelous initiatives, Mothers Against Drunk Driving being a spectacular example. Every community has organizations dedicated to preventing the kind of sickness or accident that carried off a loved member of the community. In our ancestral environment, loved ones must often have simply not returned to camp. Searching for them would have been essential. A loss creates mental preoccupation and a search image tuned to detect relevant cues. In the weeks after a loss, bereaved individuals often think that they see or hear the lost loved one. Tiny random sounds or sights are misinterpreted as the person’s voice or form. Visual and auditory hallucinations arise. Such experiences are sometimes interpreted as wish fulfillment, but a more plausible explanation is that they are products of a search image that makes it easier to find the missing person. False alarms in such a system would be normal, useful, and experienced as ghosts. Anniversary reactions are also common and fascinating. Many people occasionally experience sadness that seems unaccountable, until they realize it is the anniversary of a loss. I doubt that anniversary reactions are adaptive in general; however, in ancestral environments many opportunities and dangers recur with seasonal regularity. So smelling overly ripe apples in an orchard may bring back vivid memories of a fall long ago.

GROWTH HORMONES IN MEAT When you eat conventional meat, you’re probably eating hormones, antibiotics, steroids, and chemicals created by the fear and stress suffered by the animal during slaughter and in its inhumane living conditions. In 2009, two Japanese researchers published a startling study in Annals of Oncology. They pointed out that there has been a surge in hormone-dependent cancers that roughly parallels the surge of beef consumption in Japan. Over the last twenty-five years, hormone-dependent cancers such as breast, ovarian, endometrial, and prostate cancer rose fivefold in that country. More than 25 percent of the beef imported to Japan comes from the United States, where livestock growers regularly use the growth hormonal steroid estradiol. The researchers found that US beef had much higher levels of estrogen than Japanese beef because of the added hormones. This finding led them to conclude that eating a lot of estrogen-rich beef could be the reason for the rising incidence of these life-threatening cancers. Injected hormones like estrogen mimic the activity of our natural hormones and prevent those hormones from doing their jobs. This situation creates chaos. Growth hormones may alter the way in which natural hormones are produced, eliminated, or metabolized. And guess what? Hormone impersonators can trigger unnatural cell growth that may develop into cancer. The United States is one of the only industrialized countries that still allows their animals to be injected with growth hormone. Australia, New Zealand, Canada, Japan, and the entire European Union have banned rBGH and rBST because of their dangerous impact on human and bovine health. The binary of illness/wellness is always porous, whether or not we notice. In Illness as Metaphor, Susan Sontag wrote, “Everyone who is born holds dual citizenship, in the kingdom of the well and in the kingdom of the sick.” I used to believe Sontag uncritically. Now I know that the two kingdoms don’t exist; it’s one kingdom, in a quantum state. We are all constantly both sick and well, sick/well, sick and well both, our body and immune system in conversation with a world rich in microbes, viruses, and bacteria, almost all of them either benign or beneficial, countless microbes in me here as I write and in you there as you read. The quantum state of sick/well. Let me take cancer, that disease so long synonymous with death. Cancer is not a binary. We’re willing to acknowledge this, but only when one has already crossed over into the sick category. Cancer has stages. Stage 1 cancer is still small, mostly easily treatable; stage 4 cancer is aggressive, malignant, deadly. Many prostate cancers require no intervention at all. They are subclinical. We live with them until something else kills us. Cancer is not black and white but gray, and further, there is no white to begin with. There is no absence of cancer as long as we have a body. We make cancerous cells every day our cells divide, which is to say every day. In order to keep living, and keep making—for example—new skin to push the outside world out, our cells have to divide. With each cell division, a cell will make mutations. Mutations are the raw material of cancer. Carcinogens and sunlight cause cancer because they cause mutations and damage to our DNA. But without cell division, no life. To risk cancer, to make it day by day, is to live. The absence of cancer is death. So cancerous cells are a normal part of life. In the popular imagination, we see the immune system as mainly protecting us from infectious disease, from bacteria and viruses. But just as importantly, our immune system protects us against cancer, the cells in our own body that mutate to become other, to pose a threat. At almost 40, I’ve certainly had cancerous cells in me, cells that mutate so they can divide and divide and divide. It’s my immune system that finds those cells and kills them so that I, a whole organism, can survive. But because I don’t have cancer doesn’t mean I haven’t had a cancer cell in me. I have, and survived it, so far.

Then in March 1993, everything changed. My one-year-old son, Charlie, had his first seizure. There’s absolutely nothing funny about being the parent of a child with uncontrolled epilepsy. Nothing. After a year of daily seizures, drugs, and a brain surgery, I learned that the cure for Charlie’s epilepsy, the ketogenic diet—a high fat, no sugar, limited protein diet—had been hiding in plain sight for, by then, over seventy years. And despite the diet’s being well documented in medical texts, none of the half-dozen pediatric neurologists we had taken Charlie to see had mentioned a word about it. I found out on my own at a medical library. It was life altering—not just for Charlie and my family, but for tens of thousands like us. The cardiology community and American Heart Association demonize fat based on flawed science. Hospitals profit from tests and procedures, but again no money from diet therapy. There is a world epilepsy population of over sixty million people. Most of those people begin having their seizures as children, and only a minuscule percentage ever find out about ketogenic diet therapies. When I realized that 99 percent of what had happened to Charlie and my family was unnecessary, and that there were millions of families worldwide in the same situation, I needed to try to do something.
