Women are supposed to undergo a second, gynecologist annual exam, and this one has been well defined since its inception in the 1950s: breast and external genitalia exams, a Pap smear to detect cervical cancer, a vaginal and perhaps rectal exam. These exams are not always voluntarily undertaken; they may be required as a condition of obtaining or renewing a prescription for a contraceptive: Recall the searing scene in Mad Men where Peggy undergoes a gyn exam in order to get birth control pills and the (male) doctor cautions her that just because the pills are expensive, she shouldn't become "the town pump just to get [her] money's worth." Out-of-place intimacies, like unwelcome touching by a male coworker, are normally regarded as "sexual harassment," but the entire gyn exam consists of intimate touching, however disguised as a professional scientifically justified procedure. And sometimes this can be a pretty thin disguise.

Low-cost, high-frequency expenditures are those that need to be undertaken for common illnesses that occur periodically or those for the prevention and early treatment of conditions such as chronic kidney disease, cervical cancer, pre-eclampsia, suicide mortality and damage due to glaucoma. These represent small expenditures that are affordable to most people but are often not incurred by individuals either because of the twin phenomena of distorted perception and behaviour, and a gap in understanding about what they need, or because of gaps in health services delivery or access to basic financial services.

After removing these cancers, the electrical polarity shown on the electrometer would normally flip back to a “healthy” positive indicator—but it did not always. When it stayed negative, Burr and Langman suspected that this indicated that they either hadn’t got it all, or the cells had metastasized. Somewhere in the body, a cancerous mass was still sending its nefarious signals. What struck them as especially strange was that the electrode inside the genital tract did not have to be placed directly on, or even particularly near to, the malignant tissue for the anomaly to be detectable. It was like a distress signal was being sent over distances through the body’s healthy tissue.
