HUNGER AND OBESITY The change in diets around the world is also creating a global obesity epidemic—and in its wake a global diabetes epidemic—even as more than 900 million people in the world still suffer from chronic hunger. In the United States, where many global trends begin, the weight of the average American has increased by approximately twenty pounds in the last forty years. A recent study projects that half the adult population of the United States will be obese by 2030, with one quarter of them “severely obese.” At a time when hunger and malnutrition are continuing at still grossly unacceptable levels in poor countries around the world (and in some pockets within developed countries), few have missed the irony that simultaneously obesity is at record levels in developed countries and growing in many developing countries. How could this be? Well, first of all, it is encouraging to note that the world community has been slowly but steadily decreasing the number of people suffering from chronic hunger. Secondly, on a global basis, obesity has more than doubled in the last thirty years. According to the World Health Organization, almost 1.5 billion adults above the age of twenty are overweight, and more than a third of them are classified as obese. Two thirds of the world’s population now live in countries where more people die from conditions related to being obese and overweight than from conditions related to being underweight. Obesity represents a major risk factor for the world’s leading cause of death—cardiovascular diseases, principally heart disease and stroke—and is the major risk factor for diabetes, which has now become the first global pandemic involving a noncommunicable disease.* Adults with diabetes are two to four times more likely to suffer heart disease or a stroke, and approximately two thirds of those suffering from diabetes die from either stroke or heart disease.† The tragic increase in obesity among children is particularly troubling; almost 17 percent of U.S. children are obese today, as are almost 7 percent of all children in the world. Then, decades later, in the 1970s, a hard-assed U.S. swim coach named James Counsilman rediscovered it. Counsilman was notorious for his “hurt, pain, and agony”–based training techniques, and hypoventilation fit right in. Competitive swimmers usually take two or three strokes before they flip their heads to the side and inhale. Counsilman trained his team to hold their breath for as many as nine strokes. He believed that, over time, the swimmers would utilize oxygen more efficiently and swim faster. In a sense, it was Buteyko’s Voluntary Elimination of Deep Breathing and Zátopek hypoventilation—underwater. Counsilman used it to train the U.S. Men’s Swimming team for the Montreal Olympics. They won 13 gold medals, 14 silver, and 7 bronze, and they set world records in 11 events. It was the greatest performance by a U.S. Olympic swim team in history. Hypoventilation training fell back into obscurity after several studies in the 1980s and 1990s argued that it had little to no impact on performance and endurance. Whatever these athletes were gaining, the researchers reported, must have been based on a strong placebo effect. In the early 2000s, Dr. Xavier Woorons, a French physiologist at Paris 13 University, found a flaw in these studies. The scientists critical of the technique had measured it all wrong. They’d been looking at athletes holding their breath with full lungs, and all that extra air in the lungs made it difficult for the athletes to enter into a deep state of hypoventilation. Woorons repeated the tests, but this time subjects practiced the half-full technique, which is how Buteyko trained his patients, and likely how Counsilman trained his swimmers. Breathing less offered huge benefits. If athletes kept at it for several weeks, their muscles adapted to tolerate more lactate accumulation, which allowed their bodies to pull more energy during states of heavy anaerobic stress, and, as a result, train harder and longer. Other reports showed hypoventilation training provided a boost in red blood cells, allowing athletes to carry more oxygen and produce more energy with each breath. Breathing way less delivered the benefits of high-altitude training at 6,500 feet, but it could be used at sea level, or anywhere. Over the years, this style of breath restriction has been given many names—hypoventilation, hypoxic training, Buteyko technique, and the pointlessly technical “normobaric hypoxia training.” The outcomes were the same: a profound boost in performance.* Not just for elite athletes, but for everyone. Just a few weeks of the training significantly increased endurance, reduced more “trunk fat,” improved cardiovascular function, and boosted muscle mass compared to normal-breathing exercise. This list goes on. The takeaway is that hypoventilation works. It helps train the body to do more with less. But that doesn’t mean it’s pleasant.

COORDINATION: Your ability to combine more than one movement to create a single, distinct movement. For example, performing a simple jump requires that you coordinate several movements. The bend at the waist, knees, and ankles and then the correct extension of those joints must all be combined into a single movement. Your ability to combine these movements, with the proper timing, into one movement determines your coordination, and in turn, how well you can do the exercise. BALANCE: Your ability to maintain control of your body’s center of gravity. FLEXIBILITY: Your range of motion. If Jane, while doing a squat and using good form, can go down until her butt touches her heels, and Tarzan can only go until his thighs are parallel to the ground, then Jane has greater flexibility. The heart is essentially a muscular pump connected to an elaborate network of branching tubes. Although there are several kinds of cardiovascular disease, almost all arise from something going wrong in either the tubes or the pump. Most problems start with the tubes, primarily the arteries that carry blood from the heart to every nook and cranny of the body. Like the pipes in a building, arteries are vulnerable to getting clogged with unwanted deposits. This hardening of the arteries, termed atherosclerosis, starts with the buildup of plaque—a gloppy mixture of fat, cholesterol, and calcium—within the walls of arteries. Plaques, however, don’t simply accumulate in arteries like crud settling in a pipe. Instead, they are dynamic, changing, growing, shifting, and sometimes breaking. They develop when white blood cells in arteries trigger inflammation by reacting to damage usually caused by a combination of high blood pressure and so-called bad cholesterol that irritates the walls of the artery. In an effort to repair the damage, white blood cells produce a foamy mixture that incorporates cholesterol and other stuff and then hardens. As plaque accumulates, arteries stiffen and narrow, sometimes preventing enough blood from flowing to the tissues and organs that need it and further driving up blood pressure. One potentially lethal scenario is when plaques block an artery completely or detach and obstruct a smaller artery elsewhere. When this happens, tissues are starved of blood (also called ischemia) and die. Plaques can also cause the artery wall to dilate, weaken, and bulge (an aneurysm) or to tear apart (a rupture), which can lead to massive bleeding (a hemorrhage). Blocked and ruptured arteries create trouble anywhere in the body, but the most vulnerable locations are the narrow coronary arteries that supply the heart muscle itself. Heart attacks, caused by blocked coronary arteries, may damage the heart’s muscle, leading to less effective pumping of blood or triggering an electrical disturbance that can stop the heart altogether. Other highly vulnerable arteries are in the brain, which cause strokes when blocked by blood clots or when they rupture and bleed. To this list of more susceptible locations we should also add the retinas, kidneys, stomach, and intestines. The most extreme consequence of coronary artery disease is a heart attack, which, if one survives, leaves behind a weakened heart unable to pump blood as effectively as before, leading to heart failure.

To explore how physical activity helps but doesn’t entirely prevent cardiovascular diseases, let’s return to the trinity of intertwined factors that are the root causes of the problem: high cholesterol, high blood pressure, and inflammation. Cholesterol. A cholesterol test usually measures the levels of three molecules in your blood. The first is low-density lipoprotein (LDL), often termed bad cholesterol. Your liver produces these balloon-like molecules to transport fats and cholesterol throughout your bloodstream, but some LDLs have a harmful tendency to burrow into the walls of arteries, especially when blood pressure is high. These intrusions cause an inflammatory reaction that generates plaques. The second type of cholesterol is high-density lipoprotein (HDL), sometimes called good cholesterol, because these molecules scavenge and return LDLs back to the liver. The third type are triglycerides, fat molecules that are floating freely in the bloodstream and a signpost for metabolic syndrome. To make a long story short, diets rich in sugar and saturated fats contribute to cardiovascular disease because they promote high levels of plaque-forming LDLs. Conversely, physical activity helps prevent cardiovascular disease by lowering triglycerides, raising HDL levels, and to a lesser degree lowering LDL. Blood pressure. A blood pressure test gives you two readings: the higher (systolic) number is the pressure your heart’s main chamber overcomes when it squeezes blood throughout your body; the lower (diastolic) number is the pressure your heart experiences as its main chamber fills with blood. By convention high blood pressure is a reading greater than 130/90 or 140/90. Blood pressures above these values are concerning because, unabated, they damage the walls of arteries, making them vulnerable to invasion by plaque-inducing LDLs. As we already saw, once plaques start to form, blood pressure can rise, potentially stimulating yet more plaques. Chronically high blood pressure also strains the heart, causing it to thicken abnormally and weaken. By forcing more blood to flow more rapidly through arteries, physical activity stimulates the generation of new arteries throughout the body and helps keep existing arteries supple, protecting against high blood pressure. Inflammation. Plaques don’t form out of the blue but instead occur when white blood cells in the bloodstream react to the inflammation caused by LDLs and high blood pressure. Chronic inflammation also increases one’s likelihood of developing plaques from high cholesterol and blood pressure.40 And, as we have previously seen, while inflammation is caused by factors such as obesity, junky diets, excess alcohol, and smoking, it is substantially lowered by physical activity.

Remember that hunger kills brain cells, just like getting drunk. Be careful of caffeine, which causes blood sugar swings, and never crash diet. Food sensitivities are common reactions that are not classic food allergies, so most conventional allergists underestimate the damage they do. They play a major role in mood disruption, much more frequently than most people realize. They cause chemical reactions in the body that destabilize blood sugar and wreak havoc upon hormonal and neurotransmitter balance. This can trigger depression, anxiety, impaired concentration, insomnia, and hyperactivity. The most common sensitivities, unfortunately, are to the foods people most often overconsume: wheat, milk, eggs, corn, soy, and peanuts. The average American gets about 75 percent of her calories from just 10 favorite foodstuffs, and this narrow range of eating disrupts the digestive process and causes abnormal reactions. If a particular food doesn’t agree with you and commonly causes heartburn, gas, bloating, water weight gain, a craving for more, or a burst of nervous energy, you’re probably reactive to it. There are several good books on the subject, and there are many labs that test for sensitivities. Ask a chiropractor, naturopath, or doctor of integrative medicine about them. Don’t expect much help from a conventional allergist. Exercise and Mood Dozens of studies indicate that exercise is often as effective for depression as medication, partly because it increases production of stimulating hormones, such as norepinephrine, and also because it increases oxygen flow to the brain. Exercise can, in addition, help relieve and prevent anxiety, creating a so-called tranquilizer effect that persists for about 4 hours after exercising. Exercise also decreases the biological stress response, which dampens the automatic fear reaction. It is also uniquely effective at causing secretion of Nerve Growth Factor, one of the limited number of substances that cause brain cells to grow. Another benefit of exercise is that it increases endorphin output by about 500 percent and decreases the incidence of major and minor illnesses. For mood, the ideal amount is 30 to 45 minutes of cardiovascular exercise daily. Studies show that exercising less than 30 minutes or more than 1 hour decreases mood benefits.
